Vitamin B12 replacement strategies: a Brazilian delphi consensus.

BACKGROUND: Vitamin B12 deficiency is common in older adults, atrophic gastritis, and after bariatric surgery, and can cause irreversible neurological injury. Oral, sublingual, and intramuscular routes all correct serum concentrations, yet whether any route is superior for clinically meaningful outcomes is unclear, and international guidelines diverge. This consensus was convened to define where evidence is adequate, where it is lacking, and the priority research questions. METHODS: This was a modified Delphi consensus study. The RAND/UCLA Appropriateness Method was applied through a two-stag

Prevalence of non-Helicobacter pylori Helicobacter (NHPH) species in H. pylori-associated gastritis.

Helicobacter pylori is a well-established cause of gastritis and gastric malignancy, but other Helicobacter species-collectively termed non-Helicobacter pylori Helicobacter (NHPH)-also contribute to gastric disease. This study retrospectively analysed the prevalence of NHPH in 1115 routine gastric biopsies from a large academic medical centre submitted for H. pylori drug susceptibility genotyping using a next-generation sequencing (NGS) assay targeting 16S and 23S rRNA genes. NGS results of identified pathogens were compared against those identified on histology. NHPH species were detected in

Clinical Characteristics, Diagnostic Challenges, and Therapeutic Outcomes in Non-eosinophilic Esophagitis Eosinophilic Gastrointestinal Diseases in Comparison with Eosinophilic Esophagitis.

Eosinophilic gastrointestinal diseases (EGIDs) are chronic, TH2-mediated conditions. Eosinophilic esophagitis (EoE) is the most common type, while non-EoE EGIDs affect other gut segments. This retrospective study of 111 patients (50 EoE and 61 non-EoE) from 2011 to 2022 compared the clinical aspects of these 2 types. Dysphagia and food impaction dominated in EoE, while abdominal pain, nausea, and diarrhea were more common in non-EoE EGIDs. Atopic comorbidities were frequent. Diagnostic delays>1 year were more common in EoE (72% vs 47.5%). The overall clinical response rate was 88.29%, with mos

Helicobacter pylori multiplex serology in patients with autoimmune atrophic gastritis negative for Helicobacter pylori at histology: A case-control study.

BACKGROUND: Autoimmune atrophic gastritis (AAG) is an immune-mediated disorder affecting the gastric oxyntic mucosa. Two pathogenetic models are proposed: a pure autoimmune disorder or gastric autoimmunity triggered by Helicobacter pylori (Hp)-infection. In AAG, histological diagnosis of Hp may be challenging and serology can help assess exposure to Hp-infection. This study aimed to determine seroreactivity to Hp-antigens in AAG patients by using Hp-multiplex serology assay. METHODS: A single-centre case-control study on 178 adults: 75 patients with serological and histological AAG diagnosis,

MAIT cells accumulated via CXCL12-CXCR4 chemotaxis are involved in pathologic gastric inflammation among Helicobacter pylori-infected patients.

BACKGROUND: Helicobacter pylori can cause gastritis, peptic ulcers, and even gastric cancer. In addition to conventional T cells, mucosal-associated invariant T (MAIT) cells may have an important immunologic role in mucosal infectious diseases. However, the phenotypic and effector characteristics of gastric MAIT cells and the relationship between mucosal MAIT cells and H. pylori-related diseases have not been established. METHODS: We analyzed the phenotypes and cytokines of gastric and peripheral MAIT cells from H. pylori-infected patients by flow cytometry. CXCL12-mediated chemotaxis of MAIT

Characterization of neuroendocrine cell hyperplasia in autoimmune gastritis: improving H&E-based diagnosis through systematic training.

BACKGROUND: The incidence of autoimmune gastritis (AIG) is low, and our understanding of the proliferation of neuroendocrine cells, specifically enterochromaffin-like (ECL) cells, is insufficient because of the reliance on chromogranin A (CgA) staining for diagnostic assistance. The purpose of this study was to analyze the morphological characteristics of neuroendocrine cell hyperplasia in H&E-stained sections and improve pathologists' diagnostic accuracy in its identification. METHODS: A total of 73 patients with AIG were reviewed in a single center-the Department of Pathology of Chuiyangliu

Characterization and Validation of the Antibacterial Activity of Heyndrickxia coagulans BHE26 Against Helicobacter pylori.

Helicobacter pylori infection is a primary cause of gastritis and gastric ulcers. It is crucial to find alternative therapies for H. pylori infection due to the significant side effects of current antibiotics. Heyndrickxia coagulans is an ideal probiotic due to its functionality and stability in production and storage. This study explored the anti-bacterial effects of H. coagulans BHE26 in vitro and in vivo. H. coagulans BHE26 showed notable tolerance to simulated gastric juice (pH 3.0) and 1% bile salts, highlighting its potential suitability for gastrointestinal survival. H. coagulans BHE26

Antioxidant, Antibacterial, and Antidiabetic Activities of Aleuritopteris bicolor From Nepal: A LC-MS, In Vitro, and In Silico Investigations to Establish Its Potential as a Therapeutic Candidate.

Aleuritopteris bicolor (Family: Pteridaceae; Nepalese name: Raani Sinka) is an edible fern native to Nepal, India, China, Bangladesh, Sri Lanka, and Pakistan. Ethnomedicinal practitioners from various ethnic tribes in Nepal have traditionally prescribed it to heal wounds, diarrhea, dysentery, and gastritis. However, scientific evidence supporting these efficacies remains limited until January 2025. In the present study, we aimed to validate these traditional uses through chemical, in vitro, and in silico analyses of the plant's hydroalcoholic extract. The 70% methanolic extract of the plant ex

Schaftoside as a functional marker for gastroprotection in wheatgrass and lotus seed embryos.

Wheatgrass (Triticum aestivum) and lotus seed embryos (Plumula Nelumbinis, PN) are widely consumed functional foods, yet efficacy-relevant quality control markers for gastroprotection remain unclear. We integrated HPLC-UV/LC-MS profiling with a function-guided cellular screen to identify marker candidates from PN and wheatgrass. In AGS cells, the C-glycosyl flavonoid schaftoside evoked Ca²⁺ signaling coupled to mucin-related responses, which were selectively attenuated by inhibition of the TRPM5/Na⁺/Ca²⁺ exchanger-linked pathway. In mice, PN extract (300 mg/kg), wheatgrass extract (300 mg/kg),

Characterization using ultra-deep sequencing of the intra-host distribution of the mutations associated with H. pylori antibiotic resistance.

INTRODUCTION: Helicobacter pylori is a slow-growing, gram-negative strictly pathogenic bacterium, which colonizes the stomachs of half the global population and is responsible for gastritis, peptic ulcer and even adenocarcinoma, Treatment of choice for eradication is a combination of PPIs and multiple antibiotic therapy. Recently, therapeutic failures began to be attributable to increased antibiotic resistance due to mutations in identified genes (rpoB, 16S rRNA coding gene, gyrA, 23S rRNA coding gene, pbp1A, frxA, rdxA). OBJECTIVES: This study aimed to determine, using ultra-deep sequencing,

Hypothesizing the Biotherapeutic Potential of Nitrosotalea devanaterra: Targeting Ammonia Dependency to Disrupt Helicobacter pylori Survival Strategies in Gastritis.

The increasing antibiotic resistance of Helicobacter pylori (H. pylori) underscores the urgent need for alternative, nonantibiotic therapeutic strategies. This conceptual framework hypothesizes that Nitrosotalea devanaterra (N. devanaterra), an ammonia-oxidizing acidophile, could function as a biological competitor to H. pylori by reducing local ammonia availability, a critical factor for its survival and colonization in the gastric environment. To explore this hypothesis, a stepwise experimental framework is proposed. Initially, in vitro coculture models using gastric epithelial cells under m

Research progress on animal models of autoimmune gastritis.

Autoimmune gastritis (AIG) is an autoimmune disease characterized primarily by the destruction of gastric parietal cell structure and atrophy of the gastric fundic and body mucosa. The global prevalence of AIG is approximately 3.85%. Its main complications include pernicious anemia, gastric neuroendocrine tumors, and gastric cancer, which pose significant health risks. Currently, targeted treatment options for AIG are lacking worldwide. Animal models of AIG are crucial for investigating its pathogenesis and for developing drug therapies. However, reproducible methods for establishing AIG anima
