VI Spanish Consensus Conference on the treatment of Helicobacter pylori infection.

Helicobacter pylori (H. pylori) infection is highly prevalent in the Spanish population and is the main cause of chronic gastritis, peptic ulcer disease, and gastric cancer. Five previous consensus meetings on the management of H. pylori infection in Spain have been held, the most recent in 2021. Changes in treatment regimens and the growing body of available evidence justified the organization of this VI Spanish Consensus Conference in June 2026, focused on the treatment of this infection. Sixteen experts in the field participated, performed a systematic literature search, and developed 25 re

IMPACT OF PERCUTANEOUS ENDOSCOPIC GASTROSTOMY ON THE ESOPHAGEAL/GASTRIC MUCOSA AND HELICOBACTER PYLORI IN INDIVIDUALS WITH NEUROGENIC OROPHARYNGEAL DYSPHAGIA.

BACKGROUND: Oropharyngeal dysphagia is frequent among individuals with neurological disorders and may impair nutrition and quality of life. Long-term nasoenteric tube use is associated with esophagitis, gastritis, and increased Helicobacter pylori, highlighting the need for safer nutritional alternatives. Percutaneous endoscopic gastrostomy (PEG) is widely used for long-term enteral feeding. OBJECTIVE: To evaluate esophageal and gastric inflammatory changes and the presence of H. pylori before and 6 months after PEG placement in patients with neurogenic oropharyngeal dysphagia. Association between proton pump inhibitor use and upper gastrointestinal cancer: A matched case-control study accounting for reverse causation and confounding by indication.

BACKGROUND: Proton pump inhibitors (PPIs) are widely used for acid-related disorders, but observational studies have raised concerns about a possible association between long-term PPI use and upper gastrointestinal (GI) cancers. These associations may reflect confounding by indication and reverse causation. We aimed to evaluate the association between PPI use and upper GI cancer while explicitly addressing these biases. METHODS AND FINDINGS: We conducted a matched case-control study using electronic health records from a national health organization in Israel. Cases were 875 adults (age 63.0

Helicobacter pylori biofilm formation and CagA-associated alteration in gastric mucin expression: Implications for bacterial persistence.

BACKGROUND: H. pylori is a major gastric pathogen associated with chronic gastritis, peptic ulcers and gastric malignancies. Two important mechanisms contributing to its persistence are biofilm formation and modulation of host epithelial responses. This study aimed to evaluate the biofilm-forming capacity of clinical H. pylori isolates, assess the effect of proteinase K on biofilm formation and investigate differences in mucin expression following infection with CagA-positive and CagA-negative clinical isolate on mucin expression in gastric epithelial cells. MATERIALS AND METHODS: The biofilm

Beyond glycaemic control: autoimmune polyglandular syndrome type 3 as an emerging distinct entity rather than a mere comorbidity in paediatric type 1 diabetes.

AIM: Autoimmune polyglandular syndrome type 3 (APS-3) is the coexistence of autoimmune thyroid disease (AITD) with type 1 diabetes (T1D) or other autoimmune diseases, after exclusion of adrenal insufficiency. We aimed to evaluate the prevalence, timeline and features of APS-3 in children and adolescents with T1D. METHODS: We studied 439 T1D paediatric patients who were diagnosed and followed up over 16 years using annual biochemical and autoantibody profile evaluations. RESULTS: The prevalence of APS-3a (T1D+AITD) was 23.5%, with significant female predominance (54.4%, P = 0.007). The vast m

Impact of antibiotics on survival outcomes and risk of gastritis/colitis in advanced-stage melanoma patients receiving immune checkpoint inhibitor therapy.

AIM: To determine the impact of antibiotic spectrum of activity and exposure timing on survival outcomes and development of gastritis/colitis. METHODS: We conducted a single-center, retrospective cohort study of 214 patients with advanced, metastatic, or unresectable melanoma treated with immune checkpoint inhibitors. Antibiotic exposure was classified by spectrum of activity (with and without anaerobic coverage) and antibiotic timing. Primary outcomes were the effect of antibiotic administration 30-days prior to starting ICI therapy and during ICI therapy on overall survival (OS) and progres

Commiphora africana Ethanol Extract Attenuates Inflammatory Responses With Reduced NF-κB/AP-1 Activation and Src/TAK1-Related Signaling Changes.

BACKGROUND: The bark, resin, and roots of Commiphora africana (A. Rich.) Engl. have traditionally been used in African medicine to treat fever, stomach disorders, colic, and swelling. However, despite this traditional use, its anti-inflammatory activity and the underlying molecular mechanisms remain poorly characterized. METHODS: We evaluated the anti-inflammatory activity of C. africana ethanol extract (Caf-EE) in lipopolysaccharide (LPS)-stimulated macrophages and in murine models of HCl/EtOH-induced acute gastric mucosal injury and LPS-induced acute lung injury. From commensal to pathogen: recent advances in the pathogenicity and clinical implications of Streptococcus anginosus.

Streptococcus anginosus (S. anginosus, SA), a facultatively anaerobic coccus, functions as both a commensal bacterium and an opportunistic pathogen. It is frequently associated with localized suppurative infections and abscess formation. Increasing evidence implicates it as an emerging pathogen that can trigger gastritis and gastric cancer. This review synthesizes current knowledge with emphasis on pathogenic mechanisms of infections and gastric cancer, as well as clinical management. Pathogenic mechanisms include bacterial adhesion, microbe-host interactions, immune microenvironment remodelin

Multi-omics mapping of TIMP1-associated stromal-myeloid remodeling in autoimmune gastritis and gastric cancer.

BACKGROUND: Autoimmune gastritis (AIG) is a chronic immune-mediated atrophic disease that can induce cancer-related epithelial remodeling, although its association with gastric adenocarcinoma depends on histological and clinical modifiers. Gastric cancer (GC) progression is shaped by the tumor microenvironment (TME), including cancer-associated fibroblasts (CAFs), myeloid cells, and extracellular matrix (ECM) remodeling. Whether AIG-related mucosal remodeling shares stromal programs with GC-associated stromal-myeloid niches remains unclear. METHODS: We integrated one AIG bulk-transcriptomic c

Helicobacter pylori vaccines: from bench to bedside-progress, challenges, and future directions.

Helicobacter pylori (H. pylori) is a major human pathogen that colonizes the gastric mucosa and is a principal etiological agent of gastritis, peptic ulcer disease, and gastric cancer. Although antibiotic-based eradication regimens are available, their cost and the poor prognosis of established gastric cancer make vaccination an economically attractive and potentially more durable alternative. This narrative review synthesizes the literature on H. pylori pathogenesis, host protective immunity, and vaccine development, with particular attention to the human clinical trials reported since 1999 a

Gastric bacterial microbiota and gastric carcinogenesis: from dysbiosis to immune remodeling and therapeutic opportunities.

Gastric cancer remains a leading cause of cancer-related mortality worldwide. The classical Helicobacter pylori-centric paradigm of gastric carcinogenesis is being progressively expanded, as it is now recognized that the stomach harbors a complex and metabolically active microbial ecosystem whose compositional shifts fundamentally influence oncogenic progression. This review proposes a unifying dysbiosis-carcinogenesis-immune remodeling-therapeutic intervention (DCIT) framework to integrate the rapidly growing body of evidence linking gastric microbiota dysbiosis to malignancy. We survey the c

[Molecular mechanisms of chronic gastric mucosal inflammation induced by Helicobacter pylori via immune evasion].

Helicobacter pylori (H. pylori) persistently colonizes the gastric mucosa and induces sustained inflammation, making it a major driver of the development and progression of chronic gastritis, peptic ulcer disease, and gastric cancer. Its pathogenicity does not rely on direct tissue damage inflicted by a single virulence factor, but rather on the coordinated actions of multiple factors, including cytotoxin-associated gene A (CagA), vacuolating cytotoxin A (VacA), lipopolysaccharide, and urease that disrupt the gastric epithelial barrier and perturb mucosal immune homeostasis. Concurrently, H. p

Developmental photoperiod alters the onset and capacity for reproduction in male dark-eyed juncos (Junco hyemalis).

Natural selection favours individuals able to physiologically adjust to seasonal environmental cues, such as photoperiod, and time their reproduction accordingly. Although it is known that differences along the hypothalamic-pituitary-gonadal (HPG) axis can explain differences in reproductive timing across the dark-eyed junco (Junco hyemalis) range, it remains unknown whether these differences arise from local adaptation or early-life environmental effects. We exposed nestlings from a single population to photoperiods mimicking development at the northern and southern extents of the species' br

Overuse of Intravenous Proton-Pump Inhibitors in the Treatment of Low-Risk Upper Gastrointestinal Bleeding.

BACKGROUND: In patients with acute upper GI bleeding (UGIB), current best practice is to discontinue or transition (to oral) intravenous proton pump inhibitors (PPIs) when esophagogastroduodenoscopy (EGD) reveals low risk or no peptic ulcer disease (PUD). STUDY: This retrospective study from a large academic medical center evaluated the use of intravenous PPIs in patients admitted with acute UGIB due to low-risk PUD or non-PUD UGIB. Complete clinical data including endoscopic lesions and the duration of intravenous PPI therapy were recorded. RESULTS: Of 335 patients with acute UGIB, 253 had

Detection of Salivary Helicobacter pylori in Pediatric Patients and Its Association With Gastrointestinal and Oral Health.

Background Helicobacter pylori (H. pylori) infection is one of the most common bacterial infections worldwide and is recognized as a major cause of gastritis. The oral cavity has been suggested as a potential reservoir for this bacterium. Yet, the relationship between its presence in the mouth and stomach, as well as its possible role in dental caries, remains unclear. This study aimed to investigate the prevalence of H. pylori contamination in the saliva of patients aged four to 18 years undergoing endoscopy. Materials and methods This cross-sectional study included 100 children aged four to

The efficacy and safety of indobufen versus aspirin in patients with acute myocardial infarction: a retrospective observational study.

BACKGROUND: Dual antiplatelet therapy with aspirin and P2Y12 inhibitors is the first antiplatelet of choice for acute myocardial infarction (AMI), but alternatives are needed for patients at high bleeding risk or with aspirin intolerance/hypersensitivity. This observational study investigated the efficacy and safety of indobufen, a reversible COX-1 inhibitor, among AMI patients compared to those receiving aspirin. METHODS: We retrospectively enrolled 907 consecutive AMI patients treated between June 2021 and June 2024. The primary endpoints were GUSTO bleeding and MACE between aspirin and ind

Primary (AL) amyloidosis is a rare systemic disorder caused by extracellular deposition of monoclonal immunoglobulin light chains, resulting in multi-organ dysfunction. SARS-CoV-2 infection may induce persistent inflammatory and immune dysregulation, potentially promoting amyloid formation, although clinical evidence is limited, making the recognition of post-COVID-19 amyloidosis clinically relevant and potentially novel. A 39-year-old Caucasian male presented with progressive weight loss (18 kg over 6 months), epigastric pain, early satiety, and hepatosplenomegaly. Ten months prior, he had re
