Persistent electrical storm following strong acid descaling agent ingestion successfully rescued with venoarterial extracorporeal membrane oxygenation: a case report.

BACKGROUND: Ingestion of industrial strong acids is typically associated with corrosive injury to the gastrointestinal tract and severe metabolic disturbances. While cardiac complications are uncommon, the occurrence of a persistent electrical storm following strong acid ingestion is rarely documented. This case highlights a life-threatening manifestation of non-hydrofluoric acid poisoning, characterized by refractory ventricular arrhythmias and cardiogenic shock, which necessitated advanced extracorporeal life support. CASE PRESENTATION: A 62-year-old man accidentally ingested approximately 1

No significant association between Helicobacter pylori infection and Barrett's esophagus: results from a large screening cohort in Central Europe.

BACKGROUND: Barrett's esophagus (BE) is a premalignant condition associated with gastroesophageal reflux disease (GERD) and an increased risk of esophageal adenocarcinoma (EAC). While Helicobacter pylori (H. pylori) has been proposed as a potential protective factor against BE, evidence remains conflicting, and clinical implications are uncertain. We aimed to investigate the association between H. pylori infection and BE in a largescreening cohort from Central Europe. METHODS: We analyzed data from 4,074 asymptomatic participants who underwent upper endoscopy at a single center as part of a c

Epithelial cell-derived exosomes carry NamiRNA-143-5p and promote ASPN expression in fibroblasts to induce Helicobacter pylori infected gastritis progression.

In Helicobacter pylori (H. pylori) associated gastritis, fibroblasts are recruited to inflammatory sites and secrete multiple pro-inflammatory cytokines, but the underlying mechanism remains unclear. Here, immunohistochemical staining revealed that ASPN expression was significantly upregulated in fibroblasts from H. pylori positive gastritis tissues, whereas its level remained unchanged in fibroblasts directly infected with H. pylori. Co-culture assays demonstrated that exosomes released from H. pylori infected epithelial cells induced the upregulation of ASPN and its downstream cytokines (IL-

Phlegmonous Gastritis in Chemotherapy-Related Neutropenia with Endoscopic Findings That Prompted Early Therapeutic Intervention.

Phlegmonous gastritis is a rare and potentially fatal infection of the gastric wall that is often difficult to diagnose due to non-specific symptoms. We report the case of a 56-year-old man with chemotherapy-related neutropenia and immunosuppression, who presented with shock and epigastric pain. Computed tomography revealed diffuse gastric wall thickening, and esophagogastroduodenoscopy revealed extensive mucosal injury, prompting the administration of empiric antimicrobial therapy. He responded well to conservative treatment, with clinical and endoscopic improvements. This case underscores th

Stomach at the crossroads: nuclear receptor signaling at the interface between what we are and what we eat.

The stomach is home to numerous nuclear receptor transcription factors (NRs) that can respond to food, toxins, and other ingested agents. Conversely, signals secreted from other organs (e.g., hormones) can engage gastric NRs to modulate gastric physiology. Thus, there is a rich potential interface between external and internal signals that gastric NRs might respond to and interpret. Here, we seek to comprehensively review the role of NRs in gastric homeostasis and disease pathogenesis. NRs are evolutionarily conserved proteins that regulate gene transcription by interpreting hormonal and envir

Candida serves as a reservoir associating with facilitating the survival and pathogenesis of Helicobacter pylori.

Helicobacter pylori (H. pylori) is a key pathogen in gastritis and gastric ulcers, noted for its high recurrence rates. H. pylori internalization into Candida vacuoles may enhance its survival and pathogenicity. In this study, we investigated the intrinsic association between H. pylori and Candida by performing H. pylori-related assays on Candida isolates obtained from clinical fecal samples, gastric mucosal tissues, and vaginal secretions. About 399 isolates of Candida were isolated from fecal samples of patients with digestive diseases. H. pylori antigens in feces and 16S rDNA within Candida

Optimizing endoscopic detection of early gastric cancer: stratification and preventive strategies for Peri-ESD diagnostic oversights.

BACKGROUND: Peri-endoscopic submucosal dissection missed early gastric cancers (peri-ESD MEGCs), defined as ESD-indicated lesions overlooked during pre-ESD diagnostic workup or post-ESD surveillance, may arise from deficiencies at any procedural phase (preparation, observation, diagnosis, or sampling). To address this, we developed a temporal-procedural bidirectional assessment protocol specifically targeting peri-ESD MEGCs, aiming to optimize endoscopic quality and prevent diagnostic omissions. METHODS: In this retrospective cohort study, 1,011 EGC lesions treated with endoscopic submucosal

Helicobacter pylori and Non-Helicobacter pylori Helicobacter (NHPH) Zoonotic Infections: A Survey Among Greek Veterinarians Aiming to Enhance Communication.

Helicobacter species affect humans and animals, mainly causing gastrointestinal but also extra-gastrointestinal pathologies. Besides Helicobacter pylori, which is the main human pathogen, Non-Helicobacter pylori Helicobacters (NHPH) are also associated with human diseases, thus raising concern about their zoonotic potential. Veterinarians are considered a risk group for NHPH infections and act as first-line communicators to animal owners about their prophylaxis. Therefore, we aimed to assess the knowledge and perception of veterinarians working in Greece about Helicobacter pylori and NHPH by a

Prolonged Radiation-Induced Gastritis Following Yttrium-90 Radioembolization: A Case of Persistent Dyspepsia and Endoscopic Surveillance.

Yttrium-90 (Y-90) transarterial radioembolization (TARE) is an established therapy for unresectable hepatic malignancies. Although generally safe, Y-90 therapy can occasionally lead to extrahepatic complications, including gastrointestinal injury. Here, we present a case of delayed-onset Y-90-induced gastritis in a 72-year-old woman with intrahepatic cholangiocarcinoma. Following Y-90 radioembolization, the patient developed delayed gastrointestinal symptoms. Subsequent endoscopy revealed erythematous gastric mucosa with erosions, and biopsy showed intravascular microspheres consistent with ra

Dietary Factors Associated With the Progression of Gastric Intestinal Metaplasia: A Multicenter, Prospective Cohort Study in a Western Population.

INTRODUCTION: Although several dietary factors such as high salt intake are linked to progression of gastric intestinal metaplasia (GIM) to gastric cancer (GC) in high-risk countries, their effect on GIM progression in Western populations remains less clear. This study investigates the influence of dietary factors on GIM progression in a Western population. METHODS: The PROgression and REgression of precancerous GAstric Lesions (PRORGAL) study (2009-2024) is a prospective cohort study of GIM patients undergoing surveillance. The operative link on GIM criteria determined GIM stage, with an inc
