Mechanistic insight into eosinophilic enteritis and related disorders.

PURPOSE OF REVIEW: Eosinophilic gastrointestinal diseases (EGIDs) affecting the gastrointestinal tract outside of the esophagus are categorized as non-EoE EGIDs, and knowledge regarding their natural history and pathophysiology is rapidly advancing. This review synthesizes evolving mechanistic insights in non-EoE EGID immunopathogenesis, emerging paradigms of disease development, and how these insights influence potential therapeutic options for these conditions. RECENT FINDINGS: The prevalence of non-EoE EGIDs is rising, though overall prevalence remains much lower than that of eosinophilic

Coded Gastrointestinal Diagnoses in Women Aged 18-44 Years with Iron Deficiency Anemia Without Selected Coded Alarm Features Who Underwent Same-Day Bidirectional Endoscopy: A Multicenter U.S. Electronic Health Record Study.

BACKGROUND: The role of bidirectional endoscopy in younger women with iron deficiency anemia (IDA) and no alarm features remains uncertain. AIMS: To describe gastrointestinal (GI) codes recorded through 3 months after same-day bidirectional endoscopy in women aged 18-44 years with biochemically supported IDA and without selected coded alarm features. METHODS: We conducted a retrospective descriptive TriNetX study. The first colonoscopy defined index; esophagogastroduodenoscopy was required the same day. An IDA code, hemoglobin ≤12 g/dL, and ferritin ≤45 ng/mL were required within 3 months on

Hypofractionated Proton Beam Therapy for Localized Residual or Recurrent Unresectable Hepatoblastoma.

BACKGROUND: We evaluated hypofractionated proton beam therapy (PBT) feasibility for unresectable hepatoblastoma (HBL). OBSERVATIONS: Six patients with residual/relapsed localized HBL received PBT (50 to 60 Gy (RBE) in 10 fractions) between April 2019 and September 2024, with follow-up through September 2025. After 55-month follow-up, 5 patients remained alive without disease. One marginal recurrence was successfully salvaged by re-irradiation, and another bridged to hepatectomy (10% viable tumor). Toxicities were minimal, with one grade 2 duodenitis. One patient died of treatment-unrelated my

Ischemic Gastroduodenitis: Navigating the Challenges of Enteral Nutrition Resumption.

Ischemic gastritis (IG) and ischemic duodenitis (ID) are rare due to the extensive anastomotic vascular supply arising from the celiac trunk. Profound splanchnic hypoperfusion is required to induce a clinically significant ischemic insult. Etiologies typically include systemic hypotension, vasculitis, or thromboembolism. Patients with IG/ID often face high morbidity and mortality given the severity of ischemia required to induce extensive mucosal damage, leaving clinicians to balance prolonged bowel rest and early enteral feeding. We present the case of an 18-year-old man with spastic quadripl

Finding of celiac disease during the study of Kimura disease in a Caucasian male.

We present the case of a 27-year-old man, with no previous diseases, who was referred to the Department of Otorhinolaryngology in May 2023 for a 2-cm right submandibular lymphadenopathy of two month's evolution. He did not report fever or night sweats, and no oral or genital ulcers were observed. He presented eosinophilia in the analysis performed and a maculopapular exanthema on the left leg and malleolus areas, which was initially treated with topical antibiotic and corticosteroids. A soft tissue ultrasound revealed two right laterocervical lymphadenopathies at levels I and II. Given the sus

Trends in the burden and quality of care for upper gastrointestinal diseases from 1990 to 2021: An analysis of the global burden of disease study 2021.

BACKGROUND: Upper gastrointestinal (GI) diseases, primarily gastroesophageal reflux disease (GERD), peptic ulcer disease (PUD), and gastritis and duodenitis (GD), are highly prevalent, chronic, and recurrent conditions worldwide, imposing a substantial burden on population health and quality of life. However, long-term trends in their quality of care and the disease-specific burden remain poorly characterized. This study aimed to analyze global trends in the disease burden and quality of care for upper-GI diseases, from 1990 to 2021. METHODS: Based on data from the Global Burden of Disease St

GLP-1 receptor agonist-associated eosinophilic duodenitis presenting as a bowel obstruction : a case report and literature review.

We report a rare drugs adverse effect of a GLP1-RA, presenting as severe gastric stasis, eosinophilic duodenitis, and high bowel obstruction. The patient, treated with oral semaglutide for type 2 diabetes and obesity, developed progressive upper gastrointestinal symptoms culminating in faecal vomiting. Endoscopic and histological findings revealed marked eosinophilic infiltration of the duodenum. To our knowledge, this is the first reported case linking semaglutide to eosinophil-driven duodenitis causing mechanical obstruction. As the use of GLP1-RAs expands, clinicians should be aware of rare

Metagenome-assembled genomes from a population-based cohort uncover novel gut species and within-species diversity, revealing prevalent disease associations.

UNLABELLED: Metagenomic profiling has advanced the understanding of microbe-host interactions. However, widely used read-based approaches are limited by incomplete reference databases and the inability to resolve strain-level variation. Here, we present a scalable, genome-resolved framework that integrates population-specific metagenome-assembled genomes (MAGs) to discover novel species, within-species diversity, and disease associations. From 1,878 deeply sequenced samples in the Estonian Microbiome Cohort (EstMB-deep), we reconstructed 84,762 MAGs representing 2,257 species, including 353 (1

Isolated immune-mediated enteritis in patients treated with immune checkpoint inhibitor therapy.

BACKGROUND: Immune checkpoint inhibitors (ICIs) commonly cause colitis, but isolated immune-mediated enteritis (IMEN) is poorly characterized. This study describes the clinical features, diagnostic findings, and outcomes of IMEN. METHOD: We retrospectively identified adults with cancer who developed IMEN within one year of ICI initiation, confirmed as duodenitis, ileitis, or both. RESULTS: Among 20,991 ICI-treated patients, 30 (0.143%) developed isolated IMEN. Median age was 69.5 years; 73.3% were male, and 80% white. The most common malignancies were gastrointestinal/hepatobiliary (26.7%) a

[Haploinsufficiency of A20 in a family caused by a heterozygous deletion in the TNFAIP3 gene].

The proband was a 3-year-4-month-old boy who had experienced recurrent diarrhea, fever, joint swelling and pain for more than a year accompanied by oral and perianal ulcers. Gastrointestinal endoscopy revealed multiple colorectal ulcers. The proband's 36-year-old father had a three-year history of oral ulcers and a one-year history of recurrent diarrhea; gastroscopy showed chronic non-atrophic gastritis with erosion and duodenitis, while colonoscopy was unremarkable. Whole exome sequencing revealed that the proband and his parents had no pathogenic single nucleotide variants or small insertion

A case of immune-mediated adverse events enterocolitis and duodenitis with gastrointestinal bleeding successfully treated with ustekinumab.

Gastrointestinal immune-mediated adverse events (imAEs) complicated by bleeding may result in a severe clinical course. We report the case of a 64-year-old man with immune-mediated colitis who relapsed after initial treatment with prednisolone, improved following treatment with infliximab, but subsequently developed immune-mediated enterocolitis. Infliximab was reintroduced after hospitalization; however, his condition worsened and was complicated by bloody stools, then his condition progressed to hemorrhagic shock. A proton pump inhibitor was initiated, and upper gastrointestinal endoscopy re

[Chronic gastroduodenitis, associated and not associated with Helicobacter pylori infection, and its relationship with functional intestinal pathology depending on the content of fecal calprotectin].

BACKGROUND: In recent years, the role of chronic gastroduodenitis in the occurrence of functional intestinal diseases within the framework of the crossover syndrome, which occurs against the background of minimal inflammation in the stomach and intestines associated with Helicobacter pylori infection, has become increasingly relevant. AIM: To determine the relationship of chronic gastroduodenitis, associated and not associated with H. pylori infection, with the level of fecal calprotectin (FC) and clinical symptoms of functional intestinal disorders. MATERIALS AND METHODS: The study included

Depressive symptoms and their gastrointestinal correlates: A nationwide observational study in the Chinese middle-aged and elderly population with nonmalignant gastrointestinal disorders.

The pathophysiological interplay between depressive symptoms and benign upper gastrointestinal (UGI) disorders encompassing peptic ulcer disease, gastritis, duodenitis, and gastroesophageal reflux disease remains incompletely defined. These chronic inflammatory conditions impose a substantial health burden on Chinese middle-aged and elderly adults, yet the temporal relationship linking depressive symptomatology to disease onset remains underexplored. This nationally representative cohort study seeks to address this critical knowledge gap through longitudinal analysis. This national population-

Histopathologic Features of Chimeric Antigen Receptor (CAR) T-cell Therapy-associated Gastrointestinal Mucosal Injury: A Case Series and Review of Literature.

Chimeric antigen receptor (CAR) T-cell therapy is an effective treatment for refractory hematologic malignancies; however, gastrointestinal adverse events (GI-AEs) are increasingly recognized as its use expands. We identified 6 patients with GI-AEs following CAR-T therapy over a 5-year period at our institution and conducted a comprehensive PubMed literature review to characterize histopathologic patterns and highlight differential diagnostic considerations. In the institutional cohort, the colon was most frequently involved (83%), followed by the small bowel (67%) and stomach (33%). Most pati

Upper Gastrointestinal Biopsy Findings in Pediatric Inflammatory Bowel Disease: Histologic and Clinicopathologic Correlations.

OBJECTIVES: We aimed to evaluate histologic findings in upper gastrointestinal (UGI) biopsies of pediatric inflammatory bowel disease (IBD) patients, their correlation with endoscopic features, and association with disease activity. METHODS: UGI biopsies from 50 pediatric IBD patients (31 Crohn's disease [CD] and 19 ulcerative colitis [UC]) were retrospectively reviewed. Esophageal, gastric, and duodenal samples were categorized as highly suggestive, suspicious, or unlikely for UGI involvement. Associations with endoscopic abnormalities, colonoscopic disease extent, and simplified Geboes scor

Essential minerals and risk of pancreatic diseases: a large-scale prospective cohort study.

OBJECTIVE: Pancreatic disorders are characterised by high mortality rates and limited therapeutic options and pose a global health challenge. As a dual-function organ with endocrine and exocrine roles, the pancreas exhibits a heightened sensitivity to the disruption of mineral homeostasis. However, the pathophysiological associations between mineral imbalances and pancreatic diseases remain controversial, and large-scale population studies validating these relationships are lacking. METHODS: This prospective cohort study used data from the UK Biobank and enrolled 191,875 participants with a m
