Esophagogastric Dysfunction and Recurrent Symptoms in Neurologically Impaired Children with an Intact Nissen Fundoplication.

BACKGROUND: Neurologically impaired children frequently experience recurrent gastroesophageal reflux symptoms after laparoscopic Nissen fundoplication, even when the fundoplication remains anatomically intact. Esophagogastric dysfunction may contribute to these recurrences but is often underrecognized. AIM: To analyze the causes of recurrent gastroesophageal reflux symptoms in neurologically impaired children with an intact Nissen fundoplication attributable to esophagogastric dysfunction and to describe their management and outcomes. METHODS: We conducted a retrospective descriptive study o

Global burden and trends of stomach cancer, Colon and rectum cancer, peptic ulcer disease, and gastritis and duodenitis from 1990 to 2021: a systematic analysis for the global burden of disease study 2021.

BACKGROUND: Geographical disparities are evident in the burden of digestive system diseases. To date, however, a comparative analysis that simultaneously examines the burden of common digestive cancers alongside non-malignant conditions using the most recent GBD 2021 data is lacking. A comprehensive global assessment of the burden of digestive system diseases is therefore warranted. METHODS: This study analyzed incidence, prevalence, mortality, and DALYs of four digestive system diseases from the GBD 2021 database. The incidence, prevalence, mortality, and DALYs were further examined across a

Gastric duplication cyst causing hypergastrinemia in an infant.

Gastric duplication cysts are rare congenital anomalies, with fewer than 10% occurring in the stomach. We report a 14-month-old girl presenting with failure to thrive, recurrent emesis, hematemesis, and severe duodenitis with ulceration. Laboratory evaluation revealed marked hypergastrinemia (1781 pg/mL), and initial imaging was unrevealing. Persistent symptoms prompted repeat imaging, which identified a 3.7 × 5.2 cm gastric duplication cyst causing gastric outlet obstruction. Surgical resection confirmed gastric mucosa within the cyst, and postoperative gastrin levels normalized. This case re

A Striking Greenish-Yellow Endoscopic Appearance of Opportunistic Fungal Duodenitis in a Non-Human Immunodeficiency Virus (HIV) Immunocompromised Patient.

Opportunistic gastrointestinal infections in immunocompromised patients can present with atypical and misleading findings. We report a renal transplant recipient on chronic immunosuppression who presented with persistent nausea, vomiting, and abdominal pain. Endoscopy revealed a striking diffuse greenish-yellow discoloration of the duodenal mucosa with friability and superficial sloughing. Histopathology demonstrated Candida species and encapsulated yeast forms suspicious for Cryptococcus despite negative serum cryptococcal antigen testing. This case highlights a potentially underrecognized en

Severe Diffuse Ulcerative Esophagitis Following Treatment with Enfortumab Vedotin and Pembrolizumab in Metastatic Urothelial Carcinoma: A Case Report.

Background and Clinical Significance: Enfortumab vedotin combined with pembrolizumab has emerged as an effective first-line therapy for advanced urothelial carcinoma. While immune checkpoint inhibitors are associated with digestive tract toxicities, upper gastrointestinal involvement such as esophagitis remains rare, and its presentation in combination with enfortumab vedotin is not well characterized. Case Presentation: A 72-year-old man with metastatic urothelial carcinoma presented with generalized weakness, poor oral intake, odynophagia, dysphagia, anemia, and systemic symptoms following t

Indications and Adverse Events of Toradol: Based on FDA Adverse Event Reporting System (FAERS).

BACKGROUND: Toradol, a potent nonsteroidal anti-inflammatory drug, is widely used for perioperative analgesia, especially in orthopedic surgeries. Postmarketing surveillance is crucial due to its extensive clinical application. This study aimed to investigate adverse events (AEs) associated with Toradol using the FDA Adverse Event Reporting System (FAERS), analyze the association strengths of key AEs, and provide clinical references. METHOD: FAERS data from 2004 Q1 to 2017 Q4 were analyzed, including reports where Toradol was the primary suspect drug. Disproportionality analyses were conducte

Duodenal Strongyloidiasis Mimicking Inflammatory Bowel Disease: A Case Report of a 52-Year-Old Immunocompetent Woman.

BACKGROUND: Strongyloides stercoralis is an intestinal nematode with a unique autoinfective life cycle that enables decades-long persistence in the human host. Its gastrointestinal manifestations may resemble inflammatory bowel disease (IBD), creating a clinically important diagnostic pitfall. Misdiagnosis is particularly hazardous because corticosteroids or other immunosuppressive therapies may precipitate Strongyloides hyperinfection syndrome or disseminated disease. CASE PRESENTATION: We report a 52-year-old immunocompetent woman from northern Iran who presented with a 20-day history of pe

Diffuse duodenitis in a patient with ulcerative colitis who was successfully treated with upadacitinib.

Ulcerative colitis is a chronic inflammatory bowel disease usually confined to the colon and rectum. Although extraintestinal manifestations are common, ulcerative colitis-like lesions of the upper gastrointestinal tract, referred to as gastroduodenal ulcerative colitis, are rare and typically observed in patients with pancolitis or after total proctocolectomy. We report a 43-year-old woman with steroid-dependent pancolitis-type ulcerative colitis who developed recurrent duodenal lesions without prior total proctocolectomy. She subsequently presented with nausea and epigastric pain; endoscopy

Strongyloides stercoralis Presenting as Duodenitis and Colitis: A Case Report.

We report a case of Strongyloides stercoralis infection in an Ethiopian female refugee with advanced, untreated HIV who presented with weight loss, dysphagia, gastrointestinal bleeding, and pancytopenia. Duodenal and colonic biopsies confirmed the diagnosis, and the patient was successfully treated with oral ivermectin and initiation of antiretroviral therapy (ART), with full symptom resolution at follow-up. This case underscores the importance of maintaining a high clinical suspicion for strongyloidiasis in immunocompromised refugees from endemic regions, even when classic markers such as eos

Spectrum of Upper Gastrointestinal Endoscopic Findings in Patients With End-Stage Renal Disease Undergoing Maintenance Hemodialysis: A Single-Center Experience From Pakistan.

INTRODUCTION: Symptoms related to the upper gastrointestinal (GI) tract and bleeding complications are common presentations in patients with end-stage renal disease (ESRD) receiving maintenance hemodialysis. Various factors, including uremia, platelet dysfunction, use of anticoagulation during hemodialysis, anemia, impaired GI motility, diabetes mellitus, and polypharmacy, can lead to a wide range of abnormalities. Local literature from Pakistan is scant. Therefore, the aim was to assess the spectrum of upper GI endoscopic findings in hemodialysis patients with ESRD. METHODS: After approval f

Immune Checkpoint Inhibitor-Related Gastrointestinal Toxicity: Pathogenesis, Diagnosis, and Management.

Immune checkpoint inhibitors (ICIs) have transformed the treatment landscape across multiple malignancies. However, their expanding use has been accompanied by an increasing spectrum of immune-related adverse events (irAEs), particularly those affecting the gastrointestinal (GI) tract. GI toxicities may involve both the upper and lower GI tract and range from mild, nonspecific symptoms to severe inflammatory complications requiring treatment interruption, hospitalization, or selective immunosuppressive therapy. This review summarizes the current evidence regarding the pathogenesis, clinical pr

Association of depression and gastrointestinal diseases: a three-stage study.

BACKGROUND: Depression as a mental illness is commonly observed to co-occur with various somatic diseases, such as gastrointestinal diseases. However, previous studies have primarily focused on the risk of mental disorders following physical illnesses. Our study took depression as a risk factor, attempting to explore its relationship with gastrointestinal diseases. METHODS: A total of 457,940 participants (aged 37-73 years) in the UK Biobank were included. The Cox proportional hazards model was used to assess the relationship between depression and gastrointestinal diseases. Mendelian randomi

Perforated duodenal tuberculosis with pancreaticobiliary disruption requiring salvage pancreaticoduodenectomy: a case report.

Gastroduodenal tuberculosis is uncommon, and duodenal perforation is exceptional. A 25-year-old man from rural Tanzania presented with generalized peritonitis after prolonged epigastric symptoms, weight loss, night sweats, and household tuberculosis exposure. Emergency laparotomy showed destructive pyloroduodenal perforation with insufficient viable tissue for local repair; distal gastrectomy and Roux-en-Y gastrojejunostomy were performed. Persistent high-output pancreatic-type drainage and progressive jaundice prompted relaparotomy, which demonstrated pancreaticoduodenal destruction and loss

Additional Role of Transabdominal Ultrasonography on Esophagogastroduodenoscopy for Uninvestigated Dyspepsia: A Prospective Study.

BACKGROUND AND AIM: Dyspepsia is a common issue worldwide, primarily caused by gastroduodenal disorders. Esophagogastroduodenoscopy (EGD) is the standard investigation of uninvestigated dyspepsia (UID), whereas transabdominal ultrasonography (TUS) is used to evaluate various hepatopancreatobiliary diseases that may mimic dyspeptic symptoms. TUS is widely used among physicians, even though the diagnostic yield remains unclear. This study is aimed at assessing the additional benefit of TUS on EGD in investigating the etiologies of dyspepsia. METHODS: This prospective study included patients wit
