Causal link between mental disorders and gastrointestinal diseases: a Mendelian randomization study.

BACKGROUND: Observational research suggests that mental diseases may increase the risk of gastrointestinal diseases. However, the causal link between these conditions remains unclear. In this study, we conducted a two-sample Mendelian randomization (MR) analysis to investigate the causal associations between common mental diseases and the risk of gastrointestinal diseases. METHODS: First, a series of parameters were set to select single-nucleotide polymorphisms (SNPs) from genome-wide association studies (GWAS). Second, A two-sample Mendelian randomization analysis was conducted to investigat

Diagnostic Challenges of Upper Gastrointestinal Tract Crohn's Disease.

Crohn's disease (CD) is a chronic inflammatory disorder that commonly affects the distal small bowel and proximal large bowel. However, it can involve any part of the gastrointestinal tract (GIT), including the upper GIT. However, upper GIT involvement is often overlooked due to the limited use of upper gastrointestinal (UGI) endoscopy and biopsy for diagnosis. We present a case of a 30-year-old male with an 11-year history of symptomatic CD. Despite multiple normal findings on various endoscopic and imaging evaluations, his symptoms persisted, leading to numerous treatment modifications from

A Rare Case of Acute Ischemic Pancreatitis and Duodenitis in the Setting of Cardiogenic Shock.

Acute ischemic pancreatitis and duodenitis are uncommon diagnoses that can affect patients with shock. As respiratory, cardiac, and neurological manifestations of shock are prioritized, resultant gastrointestinal (GI) related pathologies can be overlooked, leading to underdiagnosis of these conditions. In this report, we highlight the importance of recognizing acute ischemic pancreatitis and duodenitis in our description of a unique patient presenting with GI related complaints with underlying cardiogenic shock.

Global, regional, and national burden of gastritis and duodenitis from 1990 to 2021 with projections to 2050: a systematic analysis of the Global Burden of Disease Study 2021.

Background: Gastritis and duodenitis are highly prevalent upper gastrointestinal inflammatory conditions. We estimate the most up-to-date global, regional, and national burden in incidence, prevalence, mortality, and disability-adjusted life-years (DALYs) for gastritis and duodenitis from 1990 to 2021 with projections to 2050. Methods: Population-based data in this study was retrieved from the Global Burden of Diseases, Injuries, and Risk Factors Study 2021 (GBD 2021). We evaluated the temporal trends of age-standardized rates of gastritis and duodenitis prevalence (ASPR), incidence (ASIR), mo

Cytomegalovirus Enteritis: A Rare Culprit of Protein Losing Enteropathy in an Immunocompetent Adult.

Protein-losing enteropathy (PLE) is a rare complication of cytomegalovirus (CMV) enteritis in immunocompetent individuals. We present a case of a previously healthy 60-year-old woman with an acute CMV infection, manifesting as intractable vomiting and diarrhea, refractory electrolyte disturbances, and anasarca. The diagnostic evaluation showed low serum protein levels and an increased fecal alpha-1 antitrypsin clearance, confirming PLE. CMV-specific immunohistochemistry on duodenal samples subsequently confirmed that the PLE was caused by CMV enteritis. She completed treatment with valganciclo

Nivolumab-Induced Refractory Hemorrhagic Gastritis and Duodenitis Requiring Multimodal Biologic Therapy.

Rising immune checkpoint inhibitor use in modern cancer therapy has increased the incidence of immune-related adverse events. Although isolated upper gastrointestinal immune-related adverse events including gastritis and duodenitis are uncommon, they may be severe and potentially life-threatening. The heterogeneous presentation of immune-related gastritis and duodenitis causes diagnostic and therapeutic challenges. We present a rare case of severe hemorrhagic gastritis and duodenitis occurring 2 months after completion of nivolumab therapy for stage III melanoma. The disease proved refractory

First description of semiquantitative strain elastography in a dog with chronic inflammatory enteropathy.

A 4-year-old female Shih Tzu presented with intermittent vomiting, diarrhea, and inappetence lasting 6 months. Physical examination revealed a mildly reduced body condition score (2/5), with no other significant abnormalities. Laboratory tests, imaging studies, and endoscopic evaluation confirmed the diagnosis of chronic inflammatory enteropathy. Strain elastography of the duodenum showed a semiquantitative strain ratio (SR) of 1.19 and a heterogeneous color pattern (blue-green with red areas), indicating increased tissue stiffness. Histopathological analysis revealed duodenitis and gastritis.

Associations of digestive diseases exposure and lifestyle factors with Parkinson's disease.

Emerging evidence highlights the importance of gastrointestinal (GI) dysfunction in Parkinson's disease (PD). While inflammatory bowel disease (IBD) has been linked to PD, the association with other GI conditions remains unclear. This study analyzed data from 501,483 participants, including 907 PD cases. Cox models revealed that digestive diseases were significantly associated with an increased PD risk (HR = 1.43). Eleven digestive diseases were linked to PD, with lifestyle factors such as sleep patterns and diet reducing PD risk. Notably, interactions between Non-alcoholic Fatty Liver Disease

Successful Treatment of Refractory Immune-Mediated Colitis and Duodenitis with Tofacitinib.

INTRODUCTION: Immune-mediated colitis (IMC) is a common and potentially severe immune-related adverse event of immune checkpoint inhibitors (ICIs). Although corticosteroids and biologics are standard treatments, some cases remain refractory. Tofacitinib, a Janus kinase inhibitor, has shown promise in refractory IMC; however, evidence regarding its treatment effects remains limited. CASE PRESENTATION: Here, we present a case of pembrolizumab-induced IMC and duodenitis refractory to corticosteroids, infliximab, and vedolizumab. The patient was successfully treated after receiving a 30-day cours

Rare occurrence of immunotherapy-related gastritis and duodenitis in a single tertiary medical center: a diagnostic challenge.

INTRODUCTION: Immune checkpoint inhibitors (ICI) have revolutionized cancer treatment. While generally well-tolerated, some immune-related adverse events (ir-AEs) can impact patient care, highlighting the need for accurate diagnosis. Immunotherapy-related gastritis and duodenitis (ir-GD) is a rare ir-AE. Due to its low incidence and nonspecific symptoms, standardized diagnostic criteria and evidence-based treatment guidelines are lacking. METHODS: We conducted a retrospective analysis of patients undergoing esophagogastroduodenoscopy (EGD) while receiving ICI. Our diagnostic criteria for ir-G

Gastric Outlet Obstruction Caused by Histoplasmosis in a Nonendemic Region.

Histoplasmosis capsulatum is a dimorphic fungus commonly considered endemic in the Mississippi and Ohio River Valley regions. Histoplasmosis can present as asymptomatic in immunocompetent patients or present as a disseminated infection in immunocompromised patients. Diagnosing histoplasmosis can be challenging in nonendemic areas, especially in patients presenting primarily with gastrointestinal symptoms. We describe a case of an immunocompromised 29-year-old man presenting with gastric outlet obstruction caused by disseminated histoplasmosis in a region not commonly considered endemic to Hist

Eosinophilic gastroduodenitis: a pediatric perspective.

Eosinophilic gastroduodenitis (EoGD) is a group of chronic inflammatory disorders characterized by an increased presence of eosinophils in the stomach and duodenum. Although relatively rare, it is an increasingly recognized form of eosinophilic gastrointestinal diseases (EGIDs) with significant clinical implications, particularly in pediatric patients. Despite advancements in diagnostic tools, considerable delays in diagnosis persist due to the nonspecific nature of clinical manifestations, which often resemble more common gastrointestinal disorders. Early referral to a gastroenterologist is c

Impact of Helicobacter pylori Virulence Genotypes cagA, vacA, oipA, and babA2 on Severity of Gastropathies in Brazilian Patients.

Helicobacter pylori (H. pylori) is a Gram-negative, spiral-shaped bacterium that colonizes the human stomach and is linked to various gastroduodenal diseases. The severity of different clinical outcomes may be determined by the combination of virulence genes. The aim of this study was to assess the combinations of the cytotoxin-associated gene A (cagA), the vacuolating cytotoxin A gene (vacA), the outer inflammatory protein A gene (oipA), and the blood group antigen-binding adhesin gene (babA2) genotypes in H. pylori and their associations with the clinical outcomes of infection in patients fr
