Changing Epidemiology and Etiology of Upper Gastrointestinal Bleeding.

Upper gastrointestinal bleeding (UGIB) is defined as bleeding from the esophagus, stomach, and duodenum, whereas lower gastrointestinal bleeding originates from below the ligament of Treitz, including the small bowel and colon. The incidence of UGIB has decreased globally over the past 20 years, reaching approximately 50-150 and 47 cases per 100,000 of the global population per year for variceal and non-variceal bleeding, respectively. The eradication of Helicobacter pylori and the widespread introduction of proton pump inhibitors have contributed to the current improvement in epidemiological

Prevalence, Risk Factors, and Endoscopic Findings of Helicobacter pylori Infection Among Lebanese Patients Undergoing Gastroscopy: A Retrospective Study from a Single Tertiary Center.

(1) Background: Gastric cancer continues to pose a significant public health challenge, with its incidence influenced by various factors, including Helicobacter pylori (H. pylori) infection. In Lebanon, data on H. pylori prevalence and its associated risk factors remain limited. This study aimed to determine the prevalence of H. pylori infection among Lebanese outpatients presenting with gastrointestinal symptoms undergoing gastroscopy, to explore correlations between the infection and demographic and clinical variables, and to evaluate the prevalence of associated conditions such as gastritis

Dupilumab in Eosinophilic Gastrointestinal Disorders With Extraesophageal Involvement: A Pediatric Case Series and Systematic Review.

Dupilumab has proven effective in the treatment of eosinophilic esophagitis (EoE) in randomized controlled trials. Its efficacy in other, less common and generally more severe eosinophilic gastrointestinal disorders (EGIDs) is scientifically plausible, although it has not been studied to date. Our objective was to describe published and our experience on the empirical use of dupilumab in patients affected by EGIDs with extraesophageal involvement. We retrospectively analyzed the medical charts of children diagnosed with extraesophageal EGIDs treated with dupilumab at our tertiary medical cente

Limited Diagnostic Yield of Routine Gastroscopy in FIT-Positive Patients.

Background and aim: The necessity and diagnostic yield of routine gastroscopy in Fecal Immunochemical Test (FIT)-positive patients with normal colonoscopy findings remains controversial and poorly defined. Here, we aimed to investigate the prevalence and clinical significance of upper gastrointestinal lesions detected by gastroscopy in FIT-positive patients, stratified by normal and abnormal colonoscopy findings. Methods: This retrospective study included 38,392 adults (≥18 years) who tested positive for FIT between 2016 and 2022 across eight medical centers in Israel. Of them, 1560 patients u

[Upper-gastrointestinal tract toxicities associated with Immune Checkpoint Inhibitors (ICI)].

Although immune-mediated colitis is well known and is one of the most common toxicities of Immune Checkpoint Inhibitors (ICIs), the toxicity of these agents to the upper digestive tract is largely unknown and its incidence is probably underestimated. It can affect the stomach and/or the duodenum, and much more rarely the esophagus. Involvement of several segments is common, as is the association with colitis and possibly extra-digestive toxicity(ies). Severity is extremely variable, but severe forms are possible, particularly due to hydro-electrolytic and nutritional repercussions, possible ha

Coffee Consumption and Risk of Incident Gastrointestinal Disease: A Large Prospective Cohort Study.

Coffee's health effects are well studied, but its relationship with gastrointestinal diseases (GIDs) remains unclear, particularly with the frequent use of sweeteners. A prospective cohort of 147,263 participants without pre-existing GIDs was analyzed. Coffee intake, assessed via a 24-h dietary recall, was categorized as unsweetened, naturally sweetened, or artificially sweetened. Incident GIDs were identified through medical records. Cox proportional hazards models estimated hazard ratios (HRs) and 95% confidence intervals (CIs). Genetic predisposition was assessed using polygenic risk scores

Gastric mucosal change after the eradication of Helicobacter pylori in patients at high risk for gastric cancers.

BACKGROUND: Eradication of Helicobacter pylori alone will not eliminate the risk of gastric cancer. There was no conclusive evidence that eradication therapy reduced the incidence of gastric cancer in patients with atrophic gastritis or intestinal metaplasia. MATERIALS AND METHODS: This study was conducted from March 2010 to September 2022, and enrolled 1,789 consecutive patients who underwent endoscopic submucosal dissection for gastric mucosal dysplasia. Patients who underwent two or more procedures for simultaneous or metachronous gastric epithelial dysplasia as high-risk groups for gastri

Fatal black esophagus and duodenum.

Black esophagus (BE) is a rare finding characterized by a black appearance on the distal esophageal mucosa. In the forensic field, BE is often reported in cases of diabetic ketoacidosis and hypothermia. Coexistence of acute duodenitis (black duodenum) has also been reported. Serious complications, such as esophageal perforation and hemorrhagic shock from bleeding from the esophageal mucosa, can be life-threatening. We report a forensic autopsy case of a relatively young man who died of BE and black duodenum with no underlying conditions such as diabetes or alcohol/drug abuse. The patient was a

Clinical, Endoscopic, and Upper Gastrointestinal Histological Manifestations in Pediatric Inflammatory Bowel Disease: Insights from a Large Cohort.

BACKGROUND: Upper GI involvement is common in pediatric IBD, but the relative frequencies of specific endoscopic and histologic abnormalities at diagnosis remain poorly defined. OBJECTIVE: To determine whether specific upper GI endoscopic and histologic features differ among pediatric Crohn disease (CD), ulcerative colitis (UC), and indeterminate colitis (IC) at diagnosis. METHODS: We retrospectively analyzed 243 pediatric patients (<18 years) with newly diagnosed IBD (CD 64.6%, UC 31.2%, IC 4.1%). Clinical parameters, upper GI endoscopic findings, and histologic features were compared using

A Rare Case of Congenital Glucose Galactose Malabsorption Due to SLC5A1 Mutation.

Protracted neonatal diarrhea is severe and potentially life-threatening if not promptly diagnosed and treated. Causes include congenital defects in sodium, chloride, glucose/galactose, bile acid transport, enterokinase deficiency, and villous atrophy. Glucose-galactose malabsorption (GGM) is a rare autosomal recessive disorder caused by mutations in the SGLT1 gene, impairing glucose and galactose absorption and leading to osmotic diarrhea. This case report highlights the rarity of GGM and emphasizes the importance of early recognition to enable prompt dietary intervention, preventing failure t

Prescription trends of gastric acid suppressants and association with potential adverse events in Korea: a real-world cross-sectional study.

BACKGROUND: Recent changes in the acid-related disease market, particularly the emergence of potassium-competitive acid blockers (P-CABs), require an updated analysis of trends in gastric acid suppressant utilization. OBJECTIVES: As the use of P-CAB expands globally, a cross-sectional study is needed to assess its association with potential adverse events from a pharmacovigilance perspective. DESIGN: Cross-sectional study. METHODS: We analyzed gastric acid suppressant use at prescription and patient levels using the Health Insurance Review and Assessment National Patient Sample database (20

Muscular and serosal eosinophilic gastritis and enteritis with eosinophilic ascites successfully treated with dupilumab.

Primary eosinophilic gastrointestinal disorders beyond the esophagus (EGIDs) are rare, predominantly Th2-mediated inflammatory conditions, affecting various segments of the gastrointestinal tract. We report the case of a 12-year-old boy with recurrent abdominal pain, peripheral eosinophilia, and progressive gastritis, duodenitis, and enteritis with eosinophilic ascites. He was diagnosed with extensive EGID with muscular and serosal involvement. He was treated with oral corticosteroids and milk elimination diet, which provided temporary relief, but relapses prompted the addition of dupilumab, w

Dupilumab in an adolescent with eosinophilic esophagitis and eosinophilic duodenitis, a valuable treatment option.

The authors report the first adolescent (to our knowledge) in Belgium to receive duplimumab for eosinophilic esophagitis (EoE) and eosinophilic duodenitis (EoD). EoE was diagnosed in 2020 in a 13-year-old boy by endoscopy with biopsies following complaints of abdominal pain. Several treatments (omeprazole, topical steroids, and elemental diet) have been tried without adequate and sustained success. In 2023, in addition to the diagnosis of EoE, a diagnosis of mucosal EoD with significant ulceration was established. In January 2023, the EMA approved the use of dupilumab for EoE in children above

Case Report: Clinical and histopathological findings of porcelain gallbladder in a dog.

Porcelain Gallbladder (PGB) is a condition marked by extensive calcification and thickening of the gallbladder wall. PGB is extremely rare in dogs. The exact mechanism of PGB remains unclear. However, chronic cholecystitis and factors such as biliary hyperplasia, mucin hypersecretion, and cystic duct obstruction can lead to fibrosis and calcification of the gallbladder wall, potentially resulting in PGB. A 13-year-old spayed female Shih Tzu presented with anorexia, lethargy, vomiting, and weight loss. Physical exams showed mild epigastric pain. Blood tests indicated mild alanine aminotransfera

Novel presentation and pathophysiology of heavy parasitic burdens in Weddell seals (Leptonychotes weddellii) during sedation.

BACKGROUND: Marine mammals act as sentinel species, with top predators' overall health reflecting their ecosystem, integrated across multiple trophic levels. Yet apparently healthy wild animals may have significant subclinical pathology that goes undetected due to unknown medical histories. Marine mammals, particularly phocid seals, often suffer from heavy parasite burdens. While there are documented cases of severe respiratory infections resulting in complications during sedation, there have been no reports of gastrointestinal parasites contributing to poor outcomes during examinations requir

Olmesartan-associated gastroduodenitis that was detected on endoscopic follow-up.

We present a rare case of olmesartan-associated gastroduodenitis in a 65-year-old female with predominant upper gastrointestinal symptoms and significant changes in endoscopic findings over a short period. The patient presented with epigastralgia and nausea but no diarrhea. Esophagogastroduodenoscopy (EGD) performed 26 months earlier showed a duodenal ulcer and partial mucosal atrophy in the lower gastric body. EGD conducted at our hospital revealed diffuse atrophic mucosa throughout the stomach and fragile mucosa from the middle gastric body to the antrum, with white purulent adhesions. Diffu

Dissecting the bidirectional associations between the progression of gastrointestinal and endocrine diseases.

BACKGROUNDS: The widespread intrinsic link between gastrointestinal and endocrine diseases was poorly understood. We aimed to dissect the bidirectional association in the progression of gastrointestinal with endocrine diseases, either the overall, individual, or comorbidities with each other. METHODS: A bidirectional-designed prospective cohort included 481841 and 452858 participants free of gastrointestinal and endocrine diseases at baseline in the UK Biobank. Multivariable Cox proportional hazard models were used to estimate the hazard ratios (HRs) and 95% confidence interval (CI) for incid
