Antimicrobial susceptibility-guided treatment is superior to empiric therapy for Helicobacter pylori infection in children.

OBJECTIVES: Helicobacter pylori (HP) clinical guidelines encourage obtaining gastric biopsy culture and antimicrobial susceptibilities to guide therapy. Susceptibility-guided treatment (SGT) reduces unnecessary exposure to ineffective antibiotics, minimizes adverse events, and promotes antimicrobial stewardship. However, real-world data on its effectiveness remains limited. We aim to compare the efficacy of SGT to empirical therapy (ET). METHODS: Retrospective cohort study, single pediatric tertiary care center, Boston Children's Hospital, January/2019 to June/2024. First-time H. pylori-histo

From duodenitis to stricture: Decoding the impact of partial annular pancreas over years.

Partial annular pancreas (PAP) is the rarer variant of annular pancreas with unknown prevalence, and is characterised by ring of pancreatic tissue partially surrounding the duodenum. PAP are frequently asymptomatic, however, symptoms may only arise in adulthood, manifesting as pancreatitis, duodenal ulceration, or, less commonly, biliary obstruction or intermittent bowel obstruction. It is often overlooked on radiological examinations due to its subtle manifestations. Even asymptomatic individuals are at risk of intestinal obstruction and pancreatitis, underscoring the importance of regular fo

Upper gastrointestinal endoscopy features and Helicobacter pylori infection among patients with type 2 diabetes mellitus presenting with dyspepsia: A cross-sectional observational study.

BACKGROUND: Dyspepsia is a common gastrointestinal disorder frequently associated with Helicobacter pylori (H.Pylori) infection and may be exacerbated by Type 2 Diabetes Mellitus (T2DM). Chronic hyperglycaemia and immune dysfunction in T2DM can increase susceptibility to H. pylori and worsen upper gastrointestinal (GI) pathology. This research aimed to compare endoscopic findings, histological changes, and H. pylori infection in dyspeptic patients with and without T2DM. METHODS: This is a cross-sectional study conducted in a tertiary care centre in South India. Total of 58 diabetics and 58 no

Vision and Quality of Life in Fuchs' Endothelial Dystrophy Using a Prototype Aberrometer: A Cross-Sectional Study.

BACKGROUND: Fuchs' endothelial corneal dystrophy (FECD) is a progressive disease that affects vision and quality of life (QoL). While best distance-corrected visual acuity (BDCVA) is commonly used to assess visual function, the role of optical aberrations and high-resolution imaging in explaining patient-reported outcomes remains unclear. This study aims to evaluate how clinical, optical, and QoL parameters interrelate in patients with FECD. METHODS: Prospective, cross-sectional study. Patients underwent ophthalmic evaluation including best distance-corrected BDCVA, refraction, central cornea

Multiple Endocrine Neoplasia Type 1 Presenting as Recurrent Overt Gastrointestinal Bleeding and Ulceration: A Diagnostic Challenge.

Multiple endocrine neoplasia type 1 (MEN1) is a multisystem endocrine disorder marked by tumors of the parathyroids, pancreatic islet cells, and pituitary gland. Although most cases are inherited through an autosomal dominant pattern, a proportion arises through sporadic mutation. As the clinical presentation is highly variable, it often leads to diagnostic delay in patients with de novo mutations. We report a 48-year-old man with no family history and a past medical history of renal calculi with intermittent hematuria. Over several months, he had multiple hospitalizations due to progressive e

Disease network analysis to reveal comorbidity patterns in hospitalized patients with COPD using large-scale administrative health data.

Chronic obstructive pulmonary disease (COPD) is a common respiratory condition with a high comorbidity burden. This study aims to utilize regional administrative health data and employ network analysis to systematically investigate COPD comorbidity patterns across the entire spectrum of chronic diseases. The hospitalization discharge records from all secondary and tertiary hospitals in Sichuan Province, China, from 2015 to 2019 were collected, including 2,004,891 COPD inpatients. We constructed comorbidity networks using the Salton Cosine Index, applied centrality measures to identify central

Pembrolizumab-Induced Duodenitis Mimicking Celiac Disease.

Pembrolizumab is a programmed cell death-1 (PD-1) inhibitor frequently used as primary immunotherapy in many cancers, including melanoma, lung, renal cell, cervical, urothelial, and gastric cancers. Given its immune-checkpoint inhibition and upregulation of the immune response, pembrolizumab has been associated with many immune system-related adverse effects including stomatitis, conjunctivitis, pneumonitis, myositis, and even type 1 diabetes mellitus. Colitis is the most frequent gastrointestinal adverse reaction, with upper gastrointestinal reactions like esophagitis, gastritis, and duodenit

Management of cytomegalovirus reactivation in a patient with immunotherapy-induced gastritis.

We present the case of a 52-year-old woman diagnosed with stage IV clear cell renal cell carcinoma who received combination of surgery and systemic therapy with nivolumab (anti-PD1) and ipilimumab (anti-CTLA-4). During treatment, patient presented oral intolerance, vomiting and abdominal pain. Computed tomography (CT) and gastroscopy (EGD) were performed, identifying findings suggestive of severe gastro-duodenitis with friability and diffuse oedema of the mucosa and deep ulcers. A gastrointestinal immunotherapy-induced toxicity was suspected so patient was managed with proton pump inhibitors (

Hyperacute reactivation of cytomegalovirus-induced gastroduodenitis during remission induction in a young male patient with granulomatosis with polyangiitis: a case report and review of literature.

BACKGROUND: Cytomegalovirus is a pathogen known to aggravate the inflammatory response in autoimmune diseases via molecular mimicry. Although it is recognized that cytomegalovirus activation can happen during extended but variable periods of immunosuppression (14-90 days), it is rarely reported in conjunction with an acute flare-up of an autoimmune disease. Currently, there is no consensus on cytomegalovirus prophylaxis for patients initiating remission induction. CASE PRESENTATION: Here, we present the case of a 31-year-old male patient of South Indian ethnicity, presenting with a 2-month hi

Sex and age-specific multimorbidity profiles among working-age inpatients in China: a comparative network analysis.

BACKGROUND: Multimorbidity substantially increases health and economic burdens, and reduces productivity in working-age individuals. We conducted a study using data from 393,170 working-age inpatients (18-59 years) in Shaanxi, China, collected from 1998 to 2018. We aimed to identify multimorbidity profiles by sex and age and explore multimorbidity patterns using network analysis. METHODS: The sampling technique in our study involved forming two cohorts-blood donor and non-blood donor groups-from Shaanxi Province, China, based on age, sex, and region using a 1:1 matching method, resulting in 3

A case of systemic sarcoidosis with pituitary adrenal insufficiency complicated by eosinophilic gastroenteritis.

This paper reports a rare case of coexisting systemic sarcoidosis and eosinophilic gastroenteritis. Multiple inflammatory lesions were detected by computed tomography and (18)F-fluorodeoxyglucose positron emission tomography, and a diagnosis of systemic sarcoidosis was established by biopsies from the hilar lymph node and palpable purpura. The patient exhibited peripheral blood eosinophilia. Upper endoscopy revealed duodenitis, and histological examination confirmed marked eosinophilic infiltration of the duodenal mucosa, consistent with eosinophilic gastroenteritis (EGE). The patient also had

Dupilumab Can Induce Remission of Eosinophilic Gastritis and Duodenitis: A Retrospective Case Series.

INTRODUCTION: Noneosinophilic esophagitis eosinophilic gastrointestinal disorders (non-EoE-EGIDs) have limited treatment options to induce histologic and clinical remission. Dupilumab is a human monoclonal antibody against the interleukin-4 receptor ɑ subunit, which has been reported to induce improvement in pediatric patients with non-EoE-EGIDs. METHODS: We conducted a retrospective chart review to identify if patients with eosinophilic gastritis (EoG) and/or eosinophilic duodenitis (EoD) experience clinical and histologic remission with dupilumab. RESULTS: Twelve patients were included (2

Determination of Optimal Eosinophil Thresholds for Diagnosis of Eosinophilic Gastritis and Duodenitis: A Pooled Analysis of 4 Prospective Studies.

INTRODUCTION: Consensus is lacking regarding the number of eosinophils (eos) required for the diagnosis of eosinophilic gastritis (EoG) and eosinophilic duodenitis (EoD). In addition, thresholds that require multiple high-power fields (HPFs) may not be practical for clinical use, resulting in delayed or missed diagnoses. This pooled analysis of 4 prospective studies assessed thresholds for multiple and single HPFs used to diagnose EoG and EoD. METHODS: Studies included the phase 2 ENIGMA1, the phase 3 ENIGMA2, an EoG/EoD prevalence study and a healthy volunteer study. Eos were quantified in t

Acute necrotizing duodenitis in diabetic ketoacidosis.

Acute necrotizing esophagitis (ANE), acute necrotising duodenitis (AND), and Wishnesky's lesions (WLs) are three peculiar upper gastrointestinal pathologies that can be seen in death from diabetic ketoacidosis (DKA). Amongst these three, AND has only been recently described. Morphologically, ANE and AND present as generalized black discoloration of the intestinal tract, and florid necrosis and inflammation. Whereas WLs are discrete black lesions in the stomach with necrosis and muted inflammation. We report a case of isolated AND with an unusual morphology not previously reported. A man in his
