MRI-Based Gastric Function Assessment in Patients With Functional Dyspepsia.

BACKGROUND: Magnetic resonance imaging (MRI) enables non-invasive, radiation-free assessment of gastric emptying, intragastric liquid distribution, and motility. However, characterization of the stomach function in patients with functional dyspepsia (FD) remains limited. We aimed to apply the Virtual Gaster (VIGA) MRI platform to characterize gastric geometry, emptying dynamics, and motility patterns in patients with FD compared with healthy controls (HCs). METHODS: Twenty-six adults with Rome IV-defined FD and twenty HCs underwent static and dynamic gastric MRI during fasting, gastric fillin

I make the world the way I like it.

This review critically analyses a systematic review with a network meta-analysis (NMA) on the effectiveness of endoscopic interventions for gastroparesis by Eckhardt et al. (Eckhardt D, et al. BJS 2025;112. DOI: 10.1093/bjs/znaf183) is critically analysed in the light of the literature. The results show that, by restricting the network exclusively to interventional procedures - G-POEM, pyloroplasty, GES and botulinum toxin injection - drug-based and dietary therapies were not taken into account, even though these constitute first-line treatment in most consensus guidelines; furthermore, the ag

Functional dyspepsia and risk of Barrett's esophagus and dysplasia: A propensity-matched multicenter retrospective cohort study.

BACKGROUND: Functional dyspepsia (FD) and gastroesophageal reflux disease (GERD) exhibit substantial symptom overlap (40%-50%), and up to one-third of patients diagnosed with FD demonstrate pathologic acid exposure on objective testing. While GERD is a well-established risk factor for Barrett's esophagus (BE), the association between FD diagnosis and BE risk remains unexplored. Understanding this relationship is critical because FD is typically diagnosed symptomatically without routine objective reflux testing in clinical practice, potentially masking undiagnosed GERD or identifying shared pat

Exploratory assessment of the potential protective effects of Kampo treatment against low dose medical radiation: a single arm prospective observational study.

INTRODUCTION: Low-dose computed tomography (CT) scans can induce DNA damage, including chromosomal aberrations (CAs) and DNA double-strand breaks, which may contribute to long-term cancer risk. D-limonene, a bioactive component present in several Kampo formulas, has shown radioprotective effects in preclinical studies; however, clinical evidence remains limited, and the gastrointestinal side effects associated with isolated D-limonene may restrict its clinical application. This single-arm observational pilot study aimed to evaluate whether Kampo formulas containing D-limonene could mitigate CT

Composite fiber remodels gut microbiota and shows a trend of alleviating dyspepsia in adolescents with functional dyspepsia: a 16S rRNA pilot study.

BACKGROUND: Adolescent functional dyspepsia (FD) is highly prevalent and closely linked to intestinal microbiota imbalance. Dietary fiber exerts prebiotic effects by regulating gut microbiota, but targeted studies investigating whether composite fiber can modulate gut microbiota and alleviate dyspeptic symptoms in adolescent FD patients remain scarce. This pilot trial aimed to explore the gut microbial regulatory characteristics and symptomatic improvement trend of standardized composite fiber supplementation. METHODS: Thirty-two adolescents with FD were enrolled in a 28-day double-blind, pla

Lacticaseibacillus paracasei HP7 Attenuates Helicobacter pylori Activity and Improves Upper Gastrointestinal Symptoms: A Double-Blind, Randomized, Placebo-Controlled, Clinical Trial.

Helicobacter pylori remains a major cause of chronic gastritis and dyspeptic symptoms, and the effectiveness of standard eradication therapy continues to decline with increasing antibiotic resistance. Lacticaseibacillus paracasei HP7 has shown inhibitory and anti-inflammatory effects on H. pylori in preclinical studies. This study investigated whether 8-week oral HP7 supplementation could improve H. pylori-related outcomes in adults with confirmed infection. Seventy-eight adults with positive 13C-urea breath test (13C-UBT) results and gastric discomfort were randomized to receive low-dose HP7

Insurance Authorizations in Pediatric Disorders of Gut-Brain Interaction Treated With Auricular Neuromodulation: A Tertiary Center Experience.

OBJECTIVES: Auricular percutaneous electrical nerve field stimulation (PENFS) is a Food and Drug Administration-approved treatment for functional abdominal pain associated with irritable bowel syndrome and functional dyspepsia. Because PENFS is relatively new, insurance coverage has been inconsistent, with patients experiencing denials or delays in authorization. Thus, we aimed to evaluate insurance authorizations and their influence on outcomes in children with disorders of gut-brain interaction (DGBI) who undergo PENFS. MATERIALS AND METHODS: We reviewed charts of patients with DGBI aged se

Sleep and Disorders of Gut-Brain Interaction.

Sleep plays a critical role in maintaining good physical and emotional health. Sleep and circadian rhythms are fundamental regulators of human physiology, influencing most biological processes from the cellular to the organ-system level. Sleep disorders are common with a prevalence of 30% in adults. Impaired sleep reduces patients' quality of life and has a significant negative economic impact on the health care system. Sleep deprivation can affect the brain and alter normal circadian rhythms. These disturbances influence the brain-gut axis and subsequently affect both symptom development and

Mirtazapine in Treating Functional Dyspepsia: A Systematic Review and Meta-Analysis.

BACKGROUND: Functional dyspepsia (FD) is a disease of gut-brain interaction that impacts the quality of life of many patients with limited effective pharmacologic options. Neuromodulators are increasingly used for refractory symptoms, but the efficacy of mirtazapine remains poorly defined. Although multiple randomized trials have evaluated mirtazapine for FD, its effects have not previously been pooled in a focused meta-analysis. The objective of our study was to synthesize results of randomized controlled trials (RCTs) to clarify their efficacy in treating this condition. METHODS: A systemat

Plant-Derived Bioactives and PPAR Signaling in Functional Dyspepsia: Mechanistic Insights and Therapeutic Perspectives.

Functional dyspepsia (FD) is a common disorder of gut-brain interaction in which gastric sensorimotor abnormalities may coexist with impaired duodenal barrier function, low-grade immune activation, and altered neuroimmune signaling. Peroxisome proliferator-activated receptors (PPARs) are relevant to this emerging model because they regulate epithelial differentiation, inflammatory responses, mitochondrial metabolism, redox balance, and lipid-mediator signaling. Impaired duodenal release of palmitoylethanolamide (PEA) provides direct translational evidence that defective PPARα-dependent lipid s

Comparison of Ultrasound and Scintigraphy in Gastric Emptying: A Comprehensive Review.

Gastric emptying is the process by which gastric contents are transferred from the stomach to the small intestine for further digestion and nutrient absorption. It is the result of multiple gastrointestinal mechanisms and is essential for understanding the kinetics of digestion in humans. Delayed gastric emptying is clinically relevant as it is commonly observed in conditions such as gastroparesis and functional dyspepsia. A wide range of diagnostic modalities are available to assess gastric emptying, including scintigraphy, ultrasonography, magnetic resonance imaging, gastric aspiration, and
