The Effects of Physical Activity on Functional Dyspepsia: A Mendelian Randomization and Randomized Controlled Study.

BACKGROUND/AIMS: : The relationship between physical activity (PA) and functional dyspepsia (FD) remains unclear due to a lack of robust evidence. We conducted a Mendelian randomization (MR) analysis to assess potential genetic associations, followed by a randomized controlled trial (RCT) assessing the therapeutic effects of moderate-intensity exercise in FD patients. METHODS: : In the MR analysis, genetic variants associated with PA traits from genome-wide association studies were analyzed. For the RCT, 260 FD patients were randomized to moderate-intensity exercise intervention or a control

Short-Course, Low-Dose Metoclopramide as Bridge Therapy for Dysautonomia-Associated Gastrointestinal Dysmotility in Adolescents: A Case Series.

BACKGROUND: Dysautonomia, including postural orthostatic tachycardia syndrome (POTS) and related orthostatic disorders, is frequently associated with debilitating gastrointestinal (GI) symptoms in adolescents, including chronic nausea, early satiety, postprandial fullness, and functional dysmotility. Despite the significant disease burden, pharmacological options for GI dysmotility in this population remain poorly studied. To our knowledge, no prior case series has described metoclopramide as a targeted prokinetic bridge therapy specifically in adolescents with dysautonomia-associated GI dysmo

Quantitative Assessment of Abdominal Physical Features Associated with Cold Pattern.

Background: Abdominal examination (AE) is an important component of clinical assessment in Traditional East Asian Medicine (TEAM), providing information on abdominal shape, tenderness, stiffness, and skin color. However, conventional AE relies largely on subjective judgment and lacks standardized quantitative indicators. This study aimed to quantitatively characterize abdominal physical features associated with the cold pattern (CP) using objective abdominal examination devices and to explore their potential role in supporting standardized pattern-related assessment. Methods: A case-control st

Beyond the Snapshot: Overcoming GES Limitations to Characterize Gastric Phenotypes and Therapeutic Responses Using a 6-Day Ambulatory Wearable Monitor.

BACKGROUND: Gastric emptying scintigraphy (GES) is considered the standard for differentiating gastroparesis from functional dyspepsia and other foregut disorders characterized by nausea and vomiting but provides only a brief physiologic snapshot that may not accurately reflect daily function. We utilized a non-invasive wireless patch system (WPS) to characterize multiday gastric myoelectrical activity, evaluate its relationship with GES, and assess changes following a therapeutic intervention. METHODS: Adults referred for GES for symptoms thought to represent gastroparesis were enrolled in a

Salicifoliosides A and B: A Pair of Furan Glycoside Isomers From Chimonanthus salicifolius and Their Modulation of the Functions of Caco-2 Cells.

Chimonanthus salicifolius has traditionally been used for the treatment of gastrointestinal disorders. In this study, a systematic phytochemical investigation of this plant was conducted, leading to the isolation and identification of two previously undescribed furan glycosides, salicifoliosides A (1) and B (2). Structurally, these compounds feature a rare structural motif consisting of an α,β-unsaturated furanone aglycone esterified with a p-coumaroyl unit, a combination scarcely reported in natural products. Spectroscopic and chemical analyses established that 1 and 2 are cis-trans geometric

Causal associations between human metabolites and functional dyspepsia: Evidence from two sample and multivariable Mendelian randomization.

Evidence for causal associations between functional dyspepsia (FD) and human metabolites remains limited. Two-sample and multivariable Mendelian randomizations to detect the causal relationships between FD and human metabolites. Single nucleotide polymorphisms significantly associated with human metabolites were selected as instrumental variables, the inverse variance weighting method was used as the primary analysis method, and the results were tested for heterogeneity and horizontal pleiotropy. Genetically predicted levels of 26 circulating metabolites and 5 cerebrospinal fluid metabolites w

Circadian regulation of gastrointestinal motility and the effect on disorders of gut-brain interaction.

Gastrointestinal (GI) motility follows a circadian rhythm regulated by a network of central and peripheral clocks, with the suprachiasmatic nucleus serving as the master clock. Disruptions to circadian rhythms are linked to impaired GI motility and disorders of gut-brain interaction, which include irritable bowel syndrome (IBS) and functional dyspepsia (FD). Emerging evidence suggests that circadian misalignment alters gut-brain interactions, exacerbating the symptoms of these disorders. Additionally, sex differences in circadian regulation contribute to variations in susceptibility. Indeed, w

Postprandial attenuation of gastric slow waves in anxiety-depression, with and without functional dyspepsia: associations with heart rate variability and sleep quality.

BACKGROUND AND AIMS: Functional dyspepsia (FD) commonly coexists with anxiety, depression, and sleep disturbances. However, it remains unclear whether the burden of psychological symptoms and sleep disturbances is associated with autonomic regulation and meal-induced gastric myoelectrical responses, particularly in individuals without overt FD. This study aimed to explore the relationships among heart rate variability (HRV), sleep quality, and postprandial electrogastrographic changes in adults with symptoms of anxiety-depression (AnxDep), with or without FD. METHODS: A single-center cross-se

Ganhai Weikang Capsules ameliorate functional dyspepsia in rats by modulating gut microbiota and host metabolic homeostasis.

BACKGROUND: Functional dyspepsia (FD) is a prevalent gastrointestinal disorder with limited long-term efficacy of conventional treatments. Ganhai Weikang Capsules (GHWKCs), a classic traditional Chinese medicine formula, has shown remarkable clinical efficacy in alleviating FD symptoms, but its underlying mechanisms, particularly on the gut microbiota-host metabolism axis, remain unclear. METHODS: We performed chemical profiling of GHWKCs using UPLC-LTQ-Orbitrap-MS and established an FD rat model mimicking the TCM pathogenesis of "spleen deficiency and qi stagnation". We evaluated gastrointes

The genetic association between celiac disease and risk of functional dyspepsia: A Mendelian randomization study.

Celiac disease (CeD) is a common autoimmune disease of the small intestine triggered by ingesting gluten-containing foods. Chronic symptoms are characteristic of functional dyspepsia (FD), including chronic epigastric pain and burning. The clinical and genetic overlap between FD and CeD is currently being investigated in a large-scale comorbidity and genetic predisposition study. In this study, causal connections between CeD and FD were evaluated using Mendelian randomization (MR) methods, including the inverse variance weighted, MR-Egger regression, and the weighted median methods.
