A Rare Coexistence of Gastric Fundal False Diverticulum and Sliding Hiatal Hernia: Technical Considerations and Literature Review.

Gastric fundal false diverticula are exceedingly rare, and to our knowledge, this is the first reported case occurring in association with a sliding hiatal hernia. We describe a 49-year-old woman who presented with persistent dysphagia, regurgitation, heartburn, and halitosis refractory to medical therapy. Imaging revealed a 5.0 × 3.6 × 4.7 cm false diverticulum at the posteromedial fundus and a sliding hiatal hernia. Esophageal manometry demonstrated normal motility with ineffective bolus clearance. Laparoscopic management was employed, combining intraoperative endoscopy for diverticulum loca

A Case of Segmental Absence of Intestinal Musculature Associated With a Gastric Diverticulum in an Adult.

A man in his late 50s underwent an abdominal computed tomography (CT) scan as part of a whole-body trauma assessment after a bicycle accident. Incidentally, a mass was detected between the left adrenal gland and the posterior wall of the gastric fundus. Given the differential diagnosis of either an adrenal tumor or a gastric diverticulum, laparoscopic resection was planned. Intraoperatively, the mass was identified as a gastric diverticulum, and a partial gastrectomy was performed. Histopathological examination revealed no abnormalities in the mucosa or the muscularis mucosae. However, the mus

Non-communicating abomasal (gastric) diverticulum in a 7-day-old bull calf: a case report.

A gastric diverticulum (GD) is defined as an outpouching of the gastric wall and is reported rarely in humans, cats and dogs. A congenital GD (true GD) is a full-thickness diverticulum involving all gastric layers. A 7-day-old bull calf with a history of sudden death was presented for post-mortem examination to the Purdue Animal Disease Diagnostic Laboratory. On gross examination, arising from the wall of the caudal aspect of the greater curvature of the abomasum was a soft to firm hollow mass with no gross evidence of communication with the abomasal lumen. Histological sections of the mass re

A giant gastrointestinal stromal tumour mimicking gastric diverticulum: a case report.

Gastrointestinal stromal tumours (GISTs) are the most common mesenchymal tumors of the gastrointestinal tract, arising from the interstitial cells of Cajal. Although typically located in the stomach, they may present atypically, mimicking benign lesions such as gastric diverticula. This is a case of a 40-year-old Nepali female with rheumatoid arthritis and diabetes mellitus presenting with postprandial vomiting, abdominal fullness, and significant weight loss, where radiologic evaluation suggested a large posterior gastric diverticulum. On surgical excision, a gastric diverticulum was isolated

Case Report: Endoscopic full-thickness resection using dual and IT knives for a false gastric diverticulum and systematic review.

Gastric diverticula are the rarest gastrointestinal diverticula, traditionally managed medically or surgically. We report a case of a long-standing gastric diverticulum with postprandial abdominal pain refractory to acid suppressive therapy for the past year, successfully treated by endoscopic full-thickness resection (EFTR). A 56-year-old woman presented with a 20-year history of postprandial left upper quadrant pain that had initially responded to proton pump inhibitors (PPIs) but became refractory over the past year. Repeated endoscopies revealed a gastric diverticulum with food impaction.

Bouveret syndrome: a case report.

Bouveret syndrome is an uncommon form of gallstone ileus that results from a cholecystoenteric fistula. Impaction of gallstone in the proximal duodenum blocks the gastric outlet. We report the first case from a resource-limited setting. A 75-year-old man with calculous cholecystitis arrived with gastric obstruction, melena, coffee-ground emesis, and weight loss. Lab data showed leukocytosis and mild anemia. Computed tomography scan showed gastric distension, pneumobilia, and a gallstone in the duodenum: the characteristic findings in Rigler's triad. Due to its large size and calcification, end

Pylorospasm: a less common functional disorder mimicking hypertrophic pyloric stenosis, leading to persistent vomiting in a preterm male infant.

INTRODUCTION AND IMPORTANCE: Historically, pylorospasm is defined as a condition in which the pyloric canal of the infant has a functional obstruction as opposed to an organic one, compared to hypertrophic pyloric stenosis (HPS), a well-known cause of post-feeding and repetitive vomiting in infancy. CASE PRESENTATION: We have reported a case of a premature male infant born via surrogacy and fed from a milk bank, whose acute after-meal intermittent vomiting led to a pylorospasm diagnosis. He was successfully treated non-surgically by intravenous (IV) atropine and gained weight. CLINICAL DISCU

Functional Lumen Imaging Probe (EndoFLIP) in Upper Gastrointestinal Disorders: Current Evidence, Clinical Applications, and Future Perspectives.

The Functional Lumen Imaging Probe (EndoFLIP®) system is an impedance-based technology providing real-time measurements of cross-sectional area (CSA) and intraballoon pressure across a luminal segment. Initially developed for esophagogastric junction (EGJ) evaluation in achalasia, the application of EndoFLIP has subsequently expanded to the assessment of both sphincteric and non-sphincteric regions throughout the gastrointestinal tract in a variety of clinical conditions, including eosinophilic esophagitis (EoE), post-fundoplication states, and gastroparesis. Its unique ability to be utilized

A Rare Cause of Gastric Outlet Obstruction: A Case Report of Bouveret Syndrome.

Bouveret syndrome (BS) is a rare phenomenon of gallstone ileus causing gastric outlet obstruction that can present significant challenges in management, given its lack of formal treatment guidelines and endoscopic difficulty. We present a case of an 84-year-old woman with vomiting and abdominal pain who was found to have a large gallstone in the duodenum due to a cholecystoduodenal fistula, as well as findings consistent with acute cholecystitis. She was treated with antibiotics, and gastroenterology and general surgery were consulted. Endoscopy was performed due to high suspicion for BS, reve

[Efficacy and Safety of Endoscopic Clousure of Gastrointestinal Fistulas Using a New Endoscopic Suturing System (X-Tack) in Colombia: A Case Series].

INTRODUCTION: Gastrointestinal fistulas are uncommon but potentially severe conditions associated with infectious collections, bleeding, sepsis, malnutrition, and increased morbidity and mortality. Advances in therapeutic endoscopy have expanded the options for minimally invasive treatment. Endoscopic treatment has become a first-line therapeutic strategy for colorectal anastomotic leaks and fistulas. For esophageal and gastric fistulas, esophageal stents remain a widely used alternative; however, their tolerability may be limited by the location of the defect. The X-Tack™ endoscopic suturing

Jejunal perforation caused by transanastomotic pancreaticojejunostomy internal stent following pancreaticoduodenectomy: a case report.

Mortality and morbidity following pancreaticoduodenectomy (PD) have significantly decreased due to multimodal treatment approach and refinement in surgical techniques. Despite lacking proven benefits, surgeons continue using pancreaticojejunostomy (PJ) transanastomotic stent to mitigate post-operative pancreatic fistula (POPF). We share a morbid case of jejunal perforation caused by PJ stent. A 69 years male presented with features of gastric outlet obstruction secondary to carcinoma second part of duodenum. After proper optimization, he underwent pylorus resecting PD with child's reconstructi

Bouveret Syndrome Presenting as Duodenal Bulb Stenosis and Gastric Outlet Obstruction: A Case Report.

Bouveret syndrome, or gallstone obstruction of the duodenum, is a rare cause of gastric outlet obstruction, where gallstones migrate through a bilioenteric fistula and obstruct the pylorus or duodenum. Here, we present a case of a 61-year-old man who presented with intractable nausea and vomiting. Initial imaging, such as computed tomography of the abdomen and pelvis, showed gastric distention without an obvious obstructing lesion, and a subsequent upper endoscopy revealed a cholecystoduodenal fistula filled with gallstones, posterior to a stenotic region in the duodenal bulb. The calculi were

Diagnosis of Aortoenteric Fistula in a 65-Year-Old Male With Recently Discovered Dieulafoy Lesion-A Case Report.

BACKGROUND: Aortoenteric fistulas (AEFs) are an uncommon but often life-threatening cause of gastrointestinal (GI) hemorrhage in patients with a history of abdominal aortic aneurysm (AAA) repair. Dieulafoy lesions are a rare cause of GI bleeding, which can have significant morbidity if not identified promptly. CASE REPORT: In this case, we describe a 65-year-old male with a history of AAA repair, complicated by graft infection, who was admitted for GI bleeding. An initial esophagogastroduodenoscopy (EGD) revealed a Dieulafoy lesion in the gastric fundus, which was treated with endoscopic clip

Successful surgical intervention of Bouveret's syndrome at a district general hospital: a case report and literature review.

INTRODUCTION: Bouveret's syndrome is a rare etiology of gastric outlet obstruction, comprising 3% of gallstone ileus (GI) cases. GI occurs with cholecystoenteric fistulating disease resulting in gallstone impaction within the gastrointestinal tract. There is a lack of unified guidance in GI management for surgeons. This, combined with its unpredictable symptomatology at presentation, results in delayed diagnoses and high mortality rates, despite advancements in surgical care. PRESENTATION OF CASE: A 71-year-old male with a 10-day history of abdominal pain and vomiting. Imaging revealed gallst

Successful Endoscopic Treatment of Walled-Off Necrosis With Complex Quadruple Fistulas Among the Necrotic Cavity, Stomach, Duodenum, and Common Bile Duct.

We report an extremely rare case of duodenal bulb perforation caused by a double-pigtail plastic stent placed for walled-off necrosis (WON), coexisting with a biliary fistula to the WON and a choledochoduodenal fistula, all successfully managed with endoscopic treatment alone. A 61-year-old woman developed severe gallstone pancreatitis and underwent endoscopic ultrasound-guided transmural drainage for WON with placement of a lumen-apposing metal stent and a double-pigtail plastic stent. After repeated endoscopic necrosectomy, the metal stent was removed and the plastic stent remained in place.

Primary gastric EBV-positive cytotoxic-molecule-negative T-cell lymphoma with TET2 multihit mutations: a case report.

Extranodal NK/T-cell lymphoma (ENKTL) is an Epstein-Barr virus (EBV)-positive lymphoma of the NK- or cytotoxic T-cell lineage. According to the World Health Organization Classification of Hematolymphoid Tumors, Fifth Edition (WHO-HAEM5), EBV-positive cases lacking cytotoxic molecules cannot be classified as ENKTL. To the best of our knowledge, this is the first documented case of primary gastric EBV-positive T-cell lymphoma lacking the expression of cytotoxic molecules. A 76-year-old woman initially presented with a disease confined to the stomach, with subsequent contiguous extension into the

The hidden path of mycobacterium: a case series of rare and elusive manifestations of gastrointestinal tuberculosis.

Extrapulmonary tuberculosis continues to challenge clinicians with its protean manifestations, particularly when involving the gastrointestinal tract. While ileocecal TB is well-characterized, isolated involvement of the liver, stomach, and esophagus, Duodenum remains exceptionally rare and diagnostically elusive. We describe four rare presentations of gastrointestinal tuberculosis from a tertiary care center in India. Case 1 involves hepatic tuberculosis mimicking intrahepatic cholestasis, diagnosed via liver biopsy and special staining. Case 2A details isolated gastric TB presenting with non

Characteristics of patients hospitalized for diabetic foot in a national hospital in Peru, 2024.

INTRODUCTION: Expansive processes around the third ventricle have always been a major neurosurgical challenge. Despite all the technological and scientific progress recorded over the last few years, third ventricle tumors are still a very difficult pathology to approach due to their formidable complexity.Treating such a tumor demands a good knowledge of local anatomy, pathophysiology, pathology and a good capacity to integrate all the data gathered from the patient prior to and during surgery. Last but not least, the correct means to approach such a tumor involves using modern neuronavigation

Plasma fractionation in Latin America: a path towards sovereignty and sustainability in the supply of plasma-derived medicines.

PURPOSE: Increased reactive oxygen species (ROS) released by NADPH oxidase and inflammation are associated with arterial hypertension and eye diseases associated with high blood pressure, including glaucoma, retinopathies (e.g., age-related macular degeneration), and choroidopathies affecting ocular function; however, the mechanisms underlying these adverse outcomes remain undefined. The present study was designed to highlight the importance of oxidative stress in severe hypertension-related eye damage. METHODS: Male Wistar rats (n = 7, unless otherwise specified for specific experiments) wer
