The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

BACKGROUND: Laparoscopic distal pancreatectomy is a safe and effective surgical method for treating benign and malignant tumors of the pancreatic body and tail. However, laparoscopic surgery requires good intraoperative exposure, and since the pancreas is obstructed by the stomach and duodenum, making surgical operations and the management of intraoperative emergencies challenging. Therefore, gastric traction is crucial in laparoscopic distal pancreatectomy. Common gastric suspension techniques include single and double gastric suspension. Here, we propose a new, simple, and effective gastric

BACKGROUNDS/AIMS: To determine short-term and long-term outcomes after pancreas preserving total duodenectomy (PPTD).
METHODS: A case series and a complementary meta-analysis were conducted. All patients with (pre)neoplastic lesions of duodenum who underwent PPTD in a tertiary center for pancreatic surgery between May 2009 and October 2022 were included for the case series. All studies in the literature with a sample size of 10 or more patients reporting outcomes of PPTD were included for the meta-analysis.
RESULTS: A total of 439 patients (18 from case series and 421 from literature) were a

The majority of previously published meta-analyses compare robotic and laparoscopic pancreaticoduodenectomy (RPD vs. LPD) across both benign and malignant lesions. This meta-analysis aims on focusing exclusively on malignant lesions, providing a detailed and targeted evaluation of operative, oncologic, and post-operative outcomes. This systematic review and meta-analysis adhered to PRISMA 2020 guidelines and the Cochrane Handbook and followed a pre-registered protocol on PROSPERO. A comprehensive search of PubMed, Scopus, and Cochrane Library databases was conducted up to August 1, 2024. Risk

Bouveret syndrome, a rare form of gastric outlet obstruction, occurs when a large gallstone migrates through a cholecystoenteric fistula into the duodenum, causing obstruction. We report a 31-year-old female who initially presented with nausea, vomiting, and abdominal pain. Imaging identified a large, calcified gallstone, a hepatic abscess, and acute cholecystitis. Initial treatment involved drainage of the hepatic abscess and antibiotics, with plans for elective cholecystectomy. Three years later, she returned with recurrent symptoms, including early satiety, significant weight loss, and gast

OBJECTIVE: Acute pancreatitis (AP) is an inflammatory condition associated with a potential torpid evolution comprising multiple organ failure, pancreatic necrosis, infected collections and high mortality. Current management tends use the step-up approach, with endoscopic collection drainage, followed by percutaneous drainage by an interventional radiologist and video-assisted necrosectomy. We present our experience with a new percutaneous technique of establishing an anastomosis of the pancreatic collection with the closest loop, stomach, duodenum or jejunum that uses balloon dilation and dra

INTRODUCTION: A variety of surgical options are available to achieve a complete oncological resection for duodenal gastrointestinal stromal tumours (GISTs). The most common location of such tumours is the second portion (D2) of the duodenum. The key step in assessing the feasibility of localised duodenal resection is the relationship between the GIST and ampulla. We present our surgical approach and results for patients with D2 GISTs undergoing surgical resection at our tertiary oncology centre.
METHODS: Patients diagnosed with duodenal GIST involving D2 who underwent surgical resection at Th

INTRODUCTION: Abdominal tuberculosis is less prevalent than pulmonary tuberculosis. Tuberculous lymphadenitis accounts for fewer than 5% of abdominal cases. An unusual complication is the fistulization of a tuberculous lymph node into the digestive tract, with an even rarer association with upper gastrointestinal bleeding.
CASE REPORT: A 63-year-old male with a history of rectal adenocarcinoma surgery presented with severe gastrointestinal bleeding that required multiple transfusions. Endoscopic attempts failed to locate the bleeding source, but CT angiogram revealed a hemorrhagic necrotic me

BACKGROUNDS/AIMS: Pancreaticoduodenectomy (PD) is the standard treatment for resectable tumors of the pancreatic head, ampulla, distal bile duct, and duodenum. Despite advances, delayed gastric emptying (DGE) remains a common complication. Feeding jejunostomy (FJ) is often used during PD, though its necessity and association with increased morbidity, particularly DGE, remain controversial. This study aimed to evaluate early postoperative outcomes in PD patients with or without FJ, focusing on DGE and related complications.
METHODS: This prospective observational study was conducted from Augus

Fistula formation is a connection between anatomic locations that is intrinsically abnormal. A variety of causative etiologies and involved structures exist for these anomalous developments. Fistulas between vasculature and the enteric system are rare. When present, anatomical proximity is the dominant factor in determining which structures are involved. Aortoenteric fistulas involving the esophagus, duodenum, and small bowel are well-known with the stomach also being involved in rare instances. Fistulas involving the inferior vena cava (IVC) and enteric system have also been seen with the sto

Patients with Roux-en-Y gastric bypass (RYGB) are a significant challenge for endoscopic retrograde cholangiopancreatography (ERCP) due to the altered anatomy. Endoscopic ultrasound (EUS)-directed transgastric ERCP (EDGE) has emerged as a valuable alternative to standard methods like enteroscopy-assisted (EA-ERCP) and laparoscopy-assisted (LA-ERCP) ERCP. EDGE involves creating a temporary fistula between the gastric pouch and the excluded stomach under EUS guidance, typically using a lumen-apposing metal stent (LAMS). This allows a standard ERCP scope to access the second duodenum and the bili

BACKGROUND: Post-pancreaticoduodenectomy (PD) intestinal failure (IF) is rare and associated with poor outcomes. To our knowledge, the role of intestinal transplantation (ITx) as a rescue treatment for this complication has never been reported.
CASE SUMMARY: A 42-year-old female with a benign neurilemmoma of the duodenum underwent PD. Her superior mesenteric vein (SMV) was injured during surgery and required reconstruction. She experienced SMV thrombosis and bowel gangrene requiring massive bowel resection. Consequently, she developed short gut syndrome and an enterocutaneous fistula, leading

BACKGROUND: Minimally invasive pancreaticoduodenectomy (MIPD) is used more commonly, but this surge is mostly based on observational data. This meta-analysis aimed to compare the short-term outcomes between MIPD and open pancreaticoduodenectomy (OPD) using data collected from randomized controlled trials (RCTs).
METHODS: We searched PubMed, Cochrane Library, Embase, and Web of Science databases for RCTs comparing MIPD and OPD published before December 10, 2024. Risk ratios (RRs) and mean differences (MDs) with 95% confidence intervals (CIs) were calculated.
RESULTS: A total of eight studies

INTRODUCTION: Hepatic hydatid disease caused by Echinococcus granulosus remains a significant health burden in endemic regions. Giant cysts may cause compression of adjacent structures, complicating both diagnosis and surgical management. Minimally invasive techniques combined with preoperative endoscopic intervention offer a safe and effective therapeutic option in selected cases.
CASE PRESENTATION: This report describes a 45-year-old female from a rural area presenting with a two-year history of progressive epigastric pain, vomiting, anorexia, and weight loss. Physical examination revealed

BACKGROUND: Bouveret syndrome is a rare cause of gastric outlet obstruction resulting from the passage of a gallstone into the duodenum or stomach through a biliary-enteric fistula. It is a complication of chronic cholelithiasis and accounts for only 1%-3% of all gallstone-related intestinal obstructions.
CASE SUMMARY: A 49-year-old male presented with nausea, vomiting, and upper abdominal pain. Imaging revealed a large gastric stone and a cholecystoduodenal fistula, confirming Bouveret syndrome. An initial endoscopic attempt to remove the stone was unsuccessful, necessitating surgical interv

Background: Bouveret syndrome is a complication of cholelithiasis, characterized by the migration of a large gallstone from the gallbladder to a part of the stomach or intestine through a bilio-enteric fistula. This condition results in a rare form of gallstone ileus, presenting signs and symptoms of gastric outlet obstruction. Methods: This case report aims to present a rare instance of Bouveret syndrome in a 64-year-old woman who presented to our emergency department with recurrent epigastric pain and vomiting for over 2 months. After a CT scan, an esophagogastroscopy was performed following
