INTRODUCTION: There is no consistent definition of burnout. It is neither a defined diagnosis in ICD-10 nor in DSM-IV. Yet it is diagnosed by office-based doctors and clinicians. Mainly due to reimbursement reasons, diagnoses like depression are used instead of burnout diagnoses. Therefore burnout has a very high individual, social and economic impact.
OBJECTIVES: How is burnout diagnosed? Which criteria are relevant? How valid and reliable are the used tools? What kind of disorders in case of burnout are relevant for a differential diagnosis? What is the economic effect of a differential diagn

INTRODUCTION AND IMPORTANCE: Appendiceal diverticulitis (AD) is an uncommon and often overlooked pathology of the vermiform appendix. It closely mimics acute appendicitis but carries a significantly higher risk of perforation and is frequently associated with appendiceal neoplasms, including neuroendocrine tumors and mucinous adenomas. Preoperative diagnosis remains challenging.
CASE PRESENTATION: We report the case of a 59-year-old man presenting with a 2-day history of right iliac fossa pain, nausea, and vomiting. Ultrasonography identified a dilated, thick-walled appendix, suggestive of ac

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

PURPOSE: This study evaluates the effectiveness of machine learning (ML) algorithms for improving the preoperative diagnosis of acute appendicitis in children, focusing on the accurate prediction of the severity of disease.
METHODS: An anonymized clinical and operative dataset was retrieved from the medical records of children undergoing emergency appendectomy between 2014 and 2021. We developed an ML pipeline that pre-processed the dataset and developed algorithms to predict 5 appendicitis grades (1 - non-perforated, 2 - localized perforation, 3 - abscess, 4 - generalized peritonitis, and 5

Tumours of the appendix - a vestigial digestive organ attached to the colon - are rare. Although we estimate that around 3,000 new appendiceal cancer cases are diagnosed annually in the USA, the challenges of accurately diagnosing and identifying this tumour type suggest that this number may underestimate true population incidence. In the current absence of disease-specific screening and diagnostic imaging modalities, or well-established risk factors, the incidental discovery of appendix tumours is often prompted by acute presentations mimicking appendicitis or when the tumour has already spre

RATIONALE: This is the third update of a Cochrane review first published in 2015 and last updated in 2021. Appendectomy, the surgical removal of the appendix, is performed primarily for acute appendicitis. People who undergo appendectomy for complicated appendicitis, defined as gangrenous or perforated appendicitis, are more likely to suffer postoperative complications in comparison to uncomplicated appendicitis. The routine use of abdominal drainage to reduce postoperative complications after appendectomy for complicated appendicitis is controversial.
OBJECTIVES: To evaluate the benefits and

INTRODUCTION: Acute appendicitis is one of the most common surgical emergencies (8.6 % in men and 6.4 % in women), with varying presentations, including complications such as appendicular abscess. While the typical clinical course involves right lower quadrant pain and fever, rare complications can present with atypical symptoms, particularly in high-risk patients such as those with diabetes.
PRESENTATION OF CASE: A 55-year-old diabetic female presented with a 10-day history of worsening abdominal pain, foul-smelling discharge at the anterior abdominal wall below the umbilicus, and fever. Ima

BACKGROUND: Appendiceal Mucinous Neoplasms (AMNs) are a rare disease characterized by the accumulation of mucus within the vermiform appendix and are frequently misdiagnosed as appendicitis. Hence, it is crucial to consider AMNs because they have the potential to progress into peritoneal pseudomyxoma (PP), a clinical syndrome distinguished by mucus buildup in the peritoneum leading to progressive abdominal pathology.
CASE REPORT: We present a case involving a 13-year-old male patient who was initially suspected of having acute purulent appendicitis prior to surgery, a formal laparoscopic appe

Acute appendicitis is one of the most common causes of emergency surgery worldwide. Its diagnosis is primarily based on clinical presentation, supported by laboratory tests such as a complete blood count and imaging studies, with a non-contrast CT scan considered the gold standard. Although appendectomy is generally safe, it carries the risk of immediate and delayed complications. One such complication, stump appendicitis, is rare and often underrecognized, which can delay diagnosis and treatment. The objective of this report is to highlight the importance of considering stump appendicitis in

INTRODUCTION: Pediatric complicated appendicitis carries significant risk of abscess formation. Current surgical techniques for reduction of postoperative abscess include suction, irrigation with saline or dilute povidone-iodine, or drain placement. However, these approaches are not supported by high-quality evidence. This study aims to investigate the feasibility of administering low-volume atomized povidone-iodine (LVAP) as an antiseptic technique for complicated appendicitis.
MATERIALS AND METHODS: A retrospective case-control study of pediatric complicated appendectomies was performed at

IMPORTANCE: Perforated appendicitis is a common and morbid pediatric surgical condition frequently requiring prolonged antibiotic therapy. Adjunctive intraoperative strategies for local antimicrobial therapy are limited, and photodynamic therapy (PDT) may offer a targeted, resistance-independent approach.
OBJECTIVE: To evaluate the feasibility, safety, and preliminary efficacy of laparoscopic methylene blue photodynamic therapy (MB-PDT) for intra-abdominal disinfection in a rabbit model of perforated appendicitis.
DESIGN: Experimental preclinical animal study conducted between 2023 and 2025.

The early diagnosis of complicated appendicitis in pregnancy remains challenging. We report the case of a 25-year-old multiparous woman with complicated appendicitis masquerading as septic abortion in the first trimester. She was referred to us on the second post-abortal day with persistent abdominal pain. Two days earlier, at 12 weeks of gestation, she had presented to her local hospital with lower abdominal pain and vaginal bleeding. She was diagnosed with inevitable abortion, and a suction and evacuation procedure was performed. On presentation to our center, she was septicemic with feature

INTRODUCTION: Complicated appendicitis is defined as perforated appendicitis, periappendicular abscess, or peritonitis (acute inflammation of the peritoneum secondary to infection of the appendix). The use of pelvic drains in patients undergoing laparoscopic appendicectomy for complicated appendicitis emerged as a topic of considerable debate.
AIM: This audit aimed to explore local practice and the outcomes of patients undergoing laparoscopic appendicectomy for complicated appendicitis with and without the use of pelvic drains.
METHODS: This retrospective audit was conducted at a district ge

INTRODUCTION: Appendicitis represents a prevalent surgical emergency globally, posing a significant risk of severe complications. These complications encompass a spectrum of conditions, including ileus, peritonitis, abscess formation, and, in extreme cases, mortality.
AIM: The present study aims to assess the association of existing risk scores for the diagnosis of acute appendicitis and patient characteristics with the diagnosis of acute appendicitis, the odds of operative vs non-operative management, and postoperative outcomes.
MATERIAL AND METHODS: In total, 109 patients assessed for acut

While small bowel diverticulosis, particularly, ileal diverticulitis is rare and often asymptomatic, it can lead to chronic vague symptoms or acute complications. Acute presentations may mimic appendicitis, small bowel obstruction, or colonic diverticulitis but can also appear with atypical symptoms, making diagnosis challenging. In many cases, definitive diagnosis relies on imaging such as abdominal CT, which can identify features like extraluminal air or mass lesions, though surgical exploration may be required in complex cases. Small bowel diverticula are structural outpouchings of the inte

BACKGROUND & OBJECTIVES: Differentiation of histologic subtypes of appendiceal mucoceles may prove to be difficult on computed tomography (CT). The main objective of this study was to identify the CT features of mucocele of the appendix and correlate the imaging findings with histopathology in inflammatory, benign, and malignant neoplastic lesions, and whether these entities can be accurately differentiated on CT imaging.
MATERIALS AND METHODS: CT scans of 31 patients with diagnosis of appendiceal mucocele were retrospectively reviewed and compared with histopathology. The appendix was evalua

INTRODUCTION: Acute appendicitis is the acute inflammation of the vermiform appendix. Several etiologic factors, including intestinal helminths, can cause it. Ascaris lumbricoides, a common intestinal worm with several manifestations, can cause acute appendicitis.
CASE PRESENTATION: The case presents a 10-year-old male child with signs and symptoms of acute appendicitis which was confirmed by ultrasound. During appendectomy, adult Ascaris lumbricoides was removed from the acutely inflamed appendix and was found to be the cause of the appendicitis. About 100 mL of thin pus was also evacuated.

RATIONALE: Low-grade appendiceal mucinous neoplasm (LAMN) is a rare subtype of appendiceal pathology characterized by epithelial hyperplasia, cellular atypia, and mucinous accumulation within the appendiceal lumen, leading to obstructive expansion of the organ. Representing a mere 0.2% to 0.3% of all appendectomies and approximately 0.5% of gastrointestinal tumors, LAMN poses diagnostic challenges in surgical practice.
PATIENT CONCERNS: We conducted a retrospective analysis of a patient with the appendiceal neoplasm. A 39-year-old female presented to Jiaozhou Hospital, East Hospital Affiliate

Non-typhoidal Salmonella (NTS) infection typically causes gastroenteritis but can sometimes lead to persistent or recurrent abdominal pain due to mesenteric lymphadenitis. Here, we present a rare case of a 10-year-old Japanese boy who experienced persistent abdominal pain caused by appendicitis in addition to mesenteric lymphadenitis due to NTS. Initially, he exhibited symptoms of gastroenteritis, including transient fever, persistent lower abdominal pain, and loose stools. Stool cultures revealed Salmonella O7, leading to treatment with fosfomycin. However, his abdominal pain persisted and wa
