[Subdiaphragmatic abscesses in children].

At the clinic of pediatric surgery of the Sverdlovsk Medical Institute 30 children aged from 3 to 14 years were treated for subdiaphragmatic abscesses in 1979-1989. In 27 of them the abscesses occurred in 6 days to 6 months after an emergency operation. The largest group (19 children) was formed of patients who underwent operation for destructive appendicitis and peritonitis. The subdiaphragmatic abscess was found on the left side in 16 (53.3%) and on the right side in 14 (46.6%) cases. In 11 (36.6%) patients in was combined with abscesses of other localization. X-ray and ultrasonic studies an

[Evaluation of methods for local treatment of generalized purulent peritonitis of appendicular origin in children].

The work is based on the analysis of treatment of 212 children with generalized purulent peritonitis of appendicular origin at 7 different pediatric surgical clinics of Russia. The total mortality rate was 1.9%. It is shown that the removal of pus from the abdominal cavity by aspiration has no advantages over its removal by means of moistened tampons. Irrigation of the abdominal cavity during the operation does not affect essentially the results of treatment of patients with generalized purulent peritonitis. Drainage of the abdominal cavity by means of an aspiration drain installed properly an

[Endolymphatic drug infusion in the treatment of complicated acute appendicitis].

The work is based on lymphogenous methods of treatment, i.e. direct endolymphatic therapy with antibiotics and other agents in complications of acute appendicitis. On the basis of information in the literature, among the antibiotics we chose gentamicin, claforan, and a new antibiotic fortum. In addition to antibiotics, for the correction of disturbed blood rheological properties we gave endolymphatic infusions of trasylol, aspisol, and trental. The purpose of our study was the development of methods and treatment of complications of acute appendicitis. In view of this, we chose the patients ac

[Discitis in children. Report of two cases].

Two cases of intervertebral disk infection in 2 3-year-old boys are presented. The study reported on the anamnesis, clinical, laboratory and instrumental findings as well as therapy in both cases. Diagnosis was suspected on the basis of pain, refusal to assume any position that flexes the spine and irritability. Low-grade fever, the moderate increment of inflammatory indexes and the characteristic changes seen in 1 case on Magnetic Resonance Imaging (MRI) only and in the other one on both Technetium 99m polyphosphate bone-scanning and MRI verified the clinical diagnosis. Furthermore the author

[Appendicular peritonitis].

Peritonitis (i.e. acute inflammation of the peritoneum) from appendicular origin, can be due to acute appendicitis or be observed after appendicectomy. Diagnosis is based on physical examination. Imaging procedures (ultrasound and computed tomography) are particularly useful in localized and postoperative peritonitis. Management includes surgery, antibiotics and intensive care. Early surgical treatment by appendicectomy represents the best way to improve the prognosis of the disease. Mortality is mainly observed in case of delayed diagnosis and in aged patients.

Sonography of acute appendicitis in childhood: perforation versus nonperforation.

We evaluated the sonographic findings in 133 consecutive children referred for suspected appendicitis. Fifty-eight of these patients (44%) ultimately underwent surgery, with 54 of these proved to have acute appendicitis. Thirty-one (58%) of the 54 had nonperforated appendicitis, and 23 (43%) had evidence of perforation. Previously described sonographic findings that have been employed in the diagnosis of appendicitis were evaluated, with the presence or absence of these findings being compared in patients with non-perforated and perforated appendicitis. In those patients who did not undergo su

Appendicitis in children: color Doppler sonography.

The authors used color Doppler ultrasonography (US) to evaluate 33 children with suspected appendicitis and found locally increased blood flow in all of 10 patients with appendicitis or periappendiceal abscess; the studies were normal in 16 patients without appendicitis. The gray-scale sonographic results were concordant in all 26 of these patients. In two other patients with presumptive mesenteric adenitis and in one patient with a hemorrhagic ovarian cyst at gray-scale US, color Doppler imaging showed no increased perfusion and aided in confirming the absence of a significant inflammatory pr

[Acute appendicitis in children with atypical location of the vermiform process].

The analysis of case records of 19,966 children operated on for acute appendicitis at the Belorussian centre of pediatric surgery for 15 years (from 1970 till 1984) is presented. In 2796 (14%) children, the atypical location of the vermiform process was diagnosed at operation. In atypical location of the vermiform process, the clinical manifestations of acute appendicitis differ from those in its common location: abdominal pain is most frequently (47.8% of cases) localized outside the inguinal region, less often, tensity of the anterior abdominal wall muscles, symptoms of peritoneal irritation

[Appendicitis in the aged].

From 1984 to 1989 78 patients over the age of 60 years were operated on acute appendicitis. Compared to appendicitis in younger people these older patients showed 3 times longer an interval between the first appearance of symptoms and their contact to the doctor, or surgical treatment. As result of this longer period appeared the high perforation rate 53.8%. In this group of patients with perforation the complication rate was 4 times higher than the intraoperative and histologically confirmed acute appendicitis. From these facts results a mortality rate of 4%. In the retrospective evaluation w

[Rheological indicators of blood in patients with suppurative processes in the abdominal cavity].

Rheological properties of blood were studied in dynamics in 33 patients with different localization of purulent processes in the abdominal cavity. It was found that in patients with a limited purulent process the alterations of blood rheology corresponded to the clinical status of the patients and their parameters has a tendency to increase. In patients with generalized purulent processes the alterations of blood rheology did not correspond to the degree of the inflammatory process and pointed to manifestation of signs of coagulopathy of consumption requiring special many-component infusion-tr

Salmonella typhimurium appendicitis.

A child with signs and symptoms of acute gastroenteritis developed localization of her pain to the right lower quadrant. A clinical diagnosis of appendicitis was made and an inflamed appendix was found at surgery. The postoperative period was marked by high spiking fevers and profuse nonbloody diarrhea. Cultures of the appendix and the stool revealed Salmonella typhimurium. Nontyphoidal Salmonella organisms are a rare cause of acute suppurative appendicitis. Intraoperative cultures of the appendix and peritoneal fluid as well as postoperative cultures of the diarrheal fluid were crucial in elu

Laparoscopic appendectomy in children: report of 465 cases.

The authors present a retrospective analysis of 465 pediatric laparoscopic appendectomies. The ages of these patients ranged from 3 to 16 years, with a mean age of 10 years. The diagnosis of acute appendicitis was based on one or more of the following: the initial or repeated physical examination, abdominal radiographs, leukocyte blood count, and ultrasonography. On gross examination, 90% of appendices appeared inflamed, while on microscopic examination, 93% showed evidence of acute inflammation. There was a 3.6% incidence of minor intraoperative incidents and a 3.0% postoperative complication

Primary closure of contaminated wounds in perforated appendicitis.

We studied the clinical course of 506 children consecutively admitted with appendicitis at The Hospital for Sick Children from 1985 to 1989. One hundred eighty-one children (35%), ranging in age from 1 to 17 years, presented with perforation verified by histological examination. Ninety-six of them (53%) had generalized peritonitis, 47 (26%) had localized peritonitis, and 38 (21%) had abscess formation. Usually, triple antibiotics were begun preoperatively if perforation was suspected; otherwise, cefoxitin was started. Triple antibiotics were used postoperatively for 5 to 7 days in almost all c

[Treatment of peritonitis of appendicular etiology].

The authors had 475 patients with appendicular peritonitis under observation. All of them underwent emergency operation and were given antibacterial therapy with metronidazole and agents of the aminoglycoside series. Among 223 patients, the operation on whom was completed by traditional drainage of the abdominal cavity and complete closure of the operative wound, 148 patients developed postoperative complications. Five patients died. In 252 patients, primarily delayed sutures were applied to the wound and drainage of the abdominal cavity was limited. The abdominal cavity was inspected repeated

Appendicitis in children.

A study of 406 consecutive children operated upon for appendicitis from July 1982 to July 1987 was compared with a previously published study of 657 children with the same diagnosis operated upon between 1972 and 1982. This was done to determine if the methods of therapy continue to yield low complication rates and zero mortality rates. The routine use of antibiotic coverage for both aerobic and anaerobic bacteria in perforated appendicitis resulted in low complication rates, 3.2 per cent for major and 2.5 per cent for minor complications. Major complications included small intestinal obstruct

[Intra-abdominal infections].

50% of the patients admitted for acute abdominal pain have an infectious intra-abdominal disease, most of them an appendicitis, a cholecystitis or a diverticulitis. These infectious diseases are due to a noninfectious lesion of the wall of an intestinal organ. The defense mechanism and the bacterial synergism limit the number, nature and local extension of the multiple micro-organism, producing a strong selection. For the treatment, antibiotics play an important adjuvant role. The main question in all cases is to determine if the cure of the wall lesion is necessary. Furthermore, it is importa

Pelvic pain: lessons from anatomy and physiology.

Pelvic pain is often a difficult differential diagnosis in the emergency department. For physiologic reasons, pain in the pelvis is difficult to localize to a specific organ, and pelvic peritonitis is hard to recognize. On the other hand, differences in types of pain can be very useful in arriving at a correct diagnosis. The clinician must learn to recognize superficial and deep somatic pain, and differentiate between various types of visceral pain which originate from inflammation, ischemia, or colic. A review of the anatomy and physiology of pelvic pain helps identify some of the problems as

[Acute appendicitis in children. Experience at a general hospital].

In order to establish a guide for the diagnosis of acute appendicitis, we reviewed the charts of patients with appendectomy. In a 7-year period, 385 patients were studied. The age range was 3 to 15 years. In 53% there was an administration of medications prior to surgery. Perforated appendicitis was found in the majority (53%) of the cases. We could not find any association between age and perforation. Only localized, persistent abdominal pain, peritoneal irritation, anorexia, and vomiting were useful for differential diagnosis. In patients with acute appendicitis (p greater than 0.05), leukoc

Systemic plus local metronidazole and cephazolin in complicated appendicitis: a prospective controlled trial.

Appendicectomy was performed on 100 patients with complicated appendicitis through a grid-iron incision. All patients received systemic metronidazole and cephazolin sodium which started preoperatively and continued postoperatively for 5 days. At operation, patients were allocated randomly to receive either local instillation of metronidazole and cephazolin intraperitoneally and interparietally (group A) or no local antibiotic therapy (group B). All wounds were closed primarily without drainage. Postoperative wound sepsis occurred in four (8%) of the 50 patients in group A and in 17 (34%) of th
