[Postmortem findings in peritonitis (author's transl)].

About 11 000 autopsies were done in Darmstadt from 1955 to 1977; 879 (8%) of the cases had had peritonitis. The incidence of acute peritonitis was highest (56,8%), followed by acute local peritonitis (26,6%), peritoneal abscess and covered perforation (11,4%) and strangulation ileus (3,8%). Peritonitis had been the cause of death in 56%; in 37% of the cases peritonitis was an essential finding, in 6,3% a minor finding. A comparison was made with statistics of peritonitis at postmortem in Berlin in 1908; total incidence of peritonitis was about the same, however peritonitis originating from app

[Pelvis abscess as a complication of acute appendicitis].

The work analyzes case histories of 19 patients with pelvis abscesses after appendectomy. The overwhelming majority of them had destructive forms of appendicitis. The authors make a conclusion that the main cause of these complications is defects of operation (insufficient drainage of the abdominal cavity), groundless operation under local anesthesia, refusal of drainage of the abdominal cavity despite indications for it or errors in the drainage technique. The main method of the treatment of pelvic abscesses is operation.

Fixed combination mezlocillin/oxacillin injected intramuscularly. Comparative study of local tolerability compared with ampicillin and determination of effectiveness and tolerability in the treatment of surgical and urinary tract infections.

The results of two studies on tolerability and efficacy of mezlocillin/oxacillin are reported. The pain caused by i.m. injection of 1 g (20% solution) of the fixed antibiotics combination mezlocillin/oxacillin is compared with that caused by 1 g ampicillin. The injection volume was 5 ml. The statistical analysis of the patients' statements indicates a better tolerance of the fixed antibiotics combination mezlocillin/oxacillin than of ampicillin. The second study was determined the clinical efficacy and tolerability of mezlocillin/oxacillin in the treatment of wound infections, biliary tract in

Perforation of the appendix in the neonatal period.

Perforation of the appendix in the neonatal period may be a complication of neonatal necrotizing enterocolitis and should be differentiated from perforating appendicitis in later life. A patient is presented together with a review of the literature to illustrate this concept. Perforation of the appendix occurred in a 12-day-old preterm baby. Th cause of this perforation is assumed to be localized full thickness necrosis of the appendiceal wall, a form of neonatal necrotizing enterocolitis. The similarity between the clinical histories of neonates with so called "appendicitis" and those with ne

[Prevention of suppurative complications of appendiceal peritonitis (author's transl)].

The high incidence and severity of suppurative complications consecutive to appendiceal rupture in children can justify this review. Mixed colonic flora, with a predominance of "E. coli" among and "B. fragilis" among anaerobes, is constantly recovered from the original purulent exudate as well as from that of suppurative complications. It seems therefore wise to complete surgical treatment with an adequate early antibiotic association direction to the particular flora. Gentamycin and Clindamycin seem to fulfill satisfactorily these requirement. On the contrary, peritoneal irrigation either wit

[Differential diagnosis of torsion of the hydatid of Morgagni and of acute appendicitis in children].

Ninety four children were operated for torsion of testicle hydatids. A conclusion is made that in certain patients differential diagnosis of acute appendicitis is necessary. Irradiation of pain into the abdomen due to close connection of testicle hydatid nerves with nervous plexuses of the retroperitoneal space is one of the causes of possible errors. Thorough palpation of the scrotum organs, novocain blockade of the spermatic cord in doubtful cases help to make correct diagnosis and avoid useless laparotomy. Torsion of the uterine tube hydatid in girls can be a cause of the local aseptic infl

[Appendicular peritonitis: comparative study of antibiotic therapy and a topical bactericidal agent].

Fifty four patients operated on for appendicular peritonitis where randomly divided. In the first group intravenous Dalacin C and Spectacilline were given during five days postoperatively; in the second, Taurolin, a bactericid, was administered by intraperitoneal route. There was a statistical difference in either treatment, neither on intraperitoneal nor parietal septic complications. The use and way of prophylactic antibiotic administration are discussed.

[Results of therapy combining peritoneal lavage and local application of antibiotics in 250 patients with acute perforating appendicitis].

Since 1967, 2480 appendectomies have been performed at the St. Lucas Hospital (Amsterdam) and the St. Annadal Hospital (Maastricht), of which 250 were for perforated appendicitis. A so-called 'combined method' of treatment was used in all these cases: during the operation saline was used to lavage the peritoneal cavity with the patients in the reverse-Trendelenburg position. Antibiotics were left in the peritoneal cavity. No intraperitoneal drainage was performed, but the wound was drained for 5 days. Postoperative treatment consisted of nasogastric suction and parenteral antibiotics for 5 day

[The place of lavage-irrigation of the peritonium in peritonitis from the appendix in children. A report of 10 cases (author's transl)].

Confronted with the difficult problems presented in acute appendicular peritonitis, whether generalized or multiple focused, the authors used the "lavage irrigation" method in ten cases. They analysed the local and general conditions that led them to employ this method, in light of the evolution of this technic which they had observed in their experiences whit it. They have attempted to elucidate, using information previously available in the literature, the indications, the steps, and the eventual complications of the "lavage irrigation" of the peritonemum in the child.

Carcinoid tumors of the appendix. Mesoappendiceal extension and nodal metastases.

Carcinoid of the appendix occurred in 92 of 400,000 surgical pathology specimens covering 70 years, and followed previously reported incidences by age, sex, symptoms, size, localization and histology. Invasion of the mesoappendix occurred in 13 cases, and in two of these, a reoperative ileocolectomy demonstrated regional node metastases. No distant metastases or fatalities appeared. Tumor size correlated with mesoappendiceal invasion. Carcinoid invasion of the mesoappendix may provide a valuable marker for more distant tumor spread, and justify an aggressive surgical approach in selected patie

[Leiomyo-fibro-angiomatous hamartoma of the myocardium].

Casuistic report on an intramural hamartoma of the size of a walnut in the left ventricle consisting of fibrous connective tissue, vessels and smooth musculature which was found as secondary findings in the autopsy of a 76-year-old man. With the help of statistic material from literature is referred to the rarity of primary cardiac tumours. On the basis of other casuistic contributions of newer literature is referred to the clinical significance which cardiac tumors may have in adequate localisation as well as sufficient size.

The localization of multiple intra-abdominal abscesses by gallium scanning.

A septic course suggestive of intra-abdominal abscess followed surgery in a small child with a perforated appendix. Usual diagnostic studies, fluoroscopy, and rectal and wound exploration failed to localized an abscess. The use of gallium 67 scanning accurately localized multiple abscesses which were successfully drained.

[Treatment of intestinal paresis in children].

In the paper, a clinical experience with treatment of intestinal paresis in 545 children, aged from 1 day to 13 years, including 225 children operated upon for peritonitis is summarized. The authors differentiate 3 stages in the development of intestinal paresis depending on the degree of intensity of systemic and local disturbances. The employed methods of treatment in intestinal paresis were classified by the principle of their effect as 3 groups. Application of some or other method of the paresis therapy is determined by the stage of its development. In treatment of postoperative intestinal

[Complications of acute appendicitis in children].

Among 976 children with acute appendicitis 966 were operated upon, 169 of them (17.4%)-for common appendicitis, 401 (41.2%)-for phlegmonous appendicitis, 396 (40.4%)-for gangrenous appendicitis; 10 children were subjected to surgery due to the presence of appendicular infiltration. Preoperatively, in 137 (13.7%) patients peritonitis was observed: local-in 90, diffuse and progressive-in 47. There was noted a dependence of complications on the terms of the disease and patients' stay at the hospital prior to surgery. Following the operation in 98 (10.1%) patients different complications were obse

[Causes of development of peritonitis after appendectomy].

According to the authors' data peritonitis developed after appendectomy in 122 of 18347 patients, operated upon for acute appendicitis, was an extremely grave complication of this lesion and the related surgical intervention. The main causes for development of postoperative peritonitis were local and diffuse peritonitis (101 cases), more rarely--technical and tactical errors made during the operation. Due to diagnostic difficulties a purposeful treatment for postoperative peritonitis was undertaken with a delay, reoperations were performed in late terms and not in every case either. The mortal
