Concomitant Meckel's Diverticulum With Focal Paneth Cell Metaplasia and Acute Appendicitis Presenting as Chronic Periumbilical Pain in a Child.

Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract but remains diagnostically challenging in children because of its varied and nonspecific clinical presentation. Concomitant Meckel's diverticulum and acute appendicitis are uncommon and may further obscure diagnosis. We report a 9-year-old boy who presented with chronic periumbilical abdominal pain of 3 months' duration, with acute worsening over 3 days accompanied by vomiting. Physical examination revealed localized right periumbilical tenderness without signs of peritonitis. Laboratory investigations de

Acute Appendicitis as an Incidental Diagnosis of Situs Inversus Totalis: A Case Report and Literature Review.

Acute appendicitis in patients with situs inversus totalis (SIT) represents a diagnostic challenge because the clinical presentation frequently differs from the classic right lower quadrant pain pattern. We present the case of a 37-year-old male with acute appendicitis associated with previously undiagnosed SIT, identified incidentally during diagnostic imaging. The patient presented with left lower quadrant abdominal pain, leukocytosis, and localized tenderness. Computed tomography revealed dextrocardia and left-sided inflammatory changes compatible with acute appendicitis in the setting of S

Abdominal screening with ultrasound in children with suspected acute appendicitis.

OBJECTIVES: Ultrasound (US) is the first-line modality for the assessment of acute appendicitis (AA) in children. Because alternative diagnoses in these patients are common, some centers have expanded US to include screening of the entire abdomen and pelvis. However, insufficient data prevent consensus on whether this practice improves the detection of abdominal pathologies in the differential diagnosis for AA. To determine whether comprehensive US screening of the abdomen/pelvis improves the detection rate of abdominal pathologies among children presenting with symptoms consistent with AA, an

Acquisition of Knowledge and Clinical Skills in Diagnosing Acute Appendicitis Among Medical Students: Contributing Factors and Overall Assessment.

Background and aim Acute appendicitis (AA) is one of the most common surgical emergencies and serves as a key indicator of diagnostic proficiency among medical trainees. Variability in clinical presentations, the occurrence of atypical cases, and the ongoing evolution of diagnostic and management strategies can pose significant challenges for undergraduate medical students, particularly those with limited clinical exposure. This study aimed to assess the knowledge and applied clinical reasoning of fifth- and sixth-year medical students regarding the diagnosis and management of AA. The primary

Delayed residual intra-abdominal abscess with threatened preterm labor after laparoscopic appendectomy in the second trimester.

Acute appendicitis is the most common non-obstetric surgical emergency in pregnancy. We report a 30-year-old primigravida at 25 weeks' gestation who underwent laparoscopic appendectomy for appendicitis with localized peritonitis. Thirteen days later, she represented with worsening lower abdominal pain, vomiting, leukocytosis, vaginal bleeding, and uterine contractions. Ultrasound showed a right lower abdominal collection and a viable 27-week fetus. Magnetic resonance imaging demonstrated a peri-cecal abscess adherent to the uterus with associated intraperitoneal fluid suspicious for purulent p

Postoperative pain after low-pressure versus standard-pressure laparoscopic surgery in children. A randomized clinical trial.

BACKGROUND: Children still suffer from substantial pain after laparoscopic surgery. It has been hypothesized that low-pressure pneumoperitoneum reduces postoperative pain after laparoscopic surgery. However, this has not yet been studied in school-aged children. Our objective was to determine whether low-pressure pneumoperitoneum (LPP) reduces postoperative abdominal pain compared to standard-pressure pneumoperitoneum (SPP) in children undergoing laparoscopic appendectomy for acute appendicitis. METHODS: Single-center, parallel-group, superiority randomized controlled trial including children

Lateral cutaneous nerve entrapment syndrome associated with appendicitis.

A man in his 60s presented with localized right flank pain. Physical examination revealed a discrete tender point and a positive pinch sign. A trigger-point block led to immediate pain relief. Based on these findings, he was clinically diagnosed with lateral cutaneous nerve entrapment syndrome (LACNES). His Alvarado score was 7 and abdominal computed tomography revealed appendiceal swelling and surrounding fat stranding, indicating underlying appendicitis. The patient underwent emergency laparoscopic appendectomy. He made a full recovery with no recurrence of the pain. In this case, we specula

Diagnostic Challenge of Sequential Appendicitis in a Patient With Appendiceal Duplication: A Case Report.

BACKGROUND Appendiceal duplication is an exceptionally rare congenital anomaly, with a reported prevalence of less than 0.003%, and remains poorly represented in the contemporary evidence-based surgical literature. Recent Cochrane reviews and PubMed-indexed meta-analyses (2024 onward) have refined diagnostic pathways and management strategies for acute appendicitis; however, congenital anatomical variants such as appendiceal duplication are largely excluded from these analyses. Among the described subtypes, type B2 duplication according to the Cave-Wallbridge classification is clinically signi

Appendiceal abscess mimicking a painless abdominal wall mass in an adolescent: a case report.

BACKGROUND: Acute appendicitis is the most common surgical abdominal emergency in children. Its atypical presentations remain a significant source of diagnostic error. The presentation of an appendiceal abscess as an isolated abdominal wall mass, in the complete absence of abdominal pain, represents a misleading manifestation that directly contradicts the classic teaching that peritoneal signs are essential for diagnosis. CASE PRESENTATION: A previously healthy 13-year-old boy presented with a 10-day history of a painful right lower abdominal wall swelling, explicitly denying any abdominal pa

Pelvic osseous foreign body identified during laparoscopic appendectomy in a child with acute appendicitis: a case report.

BACKGROUND: Acute appendicitis (AA) is among the most frequent surgical emergencies in children, and laparoscopic appendectomy is widely accepted as the standard treatment. During imaging evaluation for pediatric acute abdomen, incidental pelvic abnormalities may occasionally be identified and can complicate perioperative decision-making. We report a rare case of acute suppurative appendicitis in a child with an unexpected pelvic osseous foreign body detected on preoperative computed tomography (CT). [Open abdomen as a method of source control in peritonitis: indications and risks].

OBJECTIVE: To compare the results of staged surgical treatment of patients with disseminated secondary peritonitis and to clarify the indications for open abdomen technique. MATERIAL AND METHODS: We analyzed treatment outcomes in 978 patients with disseminated peritonitis. The main causes of peritonitis were destructive appendicitis (430 patients, 43.9%) and gastroduodenal ulcer perforation (217 patients, 22.2%). RESULTS: Staged debridement in patients with high risk of death (WSES SSS score ≥ 9) significantly reduces in mortality in this group of patients (from 82.5% with traditional tactic

Incidental Visceral Peritoneal and Hepatic Sarcoidosis During Routine Laparoscopic Appendectomy: A Case Report and Implications for Laparoscopic Entry.

A 50-year-old male with a history of pulmonary sarcoidosis presented to the DC Veterans Affairs Medical Center with acute appendicitis. Laparoscopic view of the liver and peritoneum revealed studding lesions with biopsy confirmation of non-necrotizing granulomas consistent with intra-abdominal sarcoidosis. Operative and postoperative hospital courses were uncomplicated. In our case, avoidance of peritoneal sarcoid disease segments during laparoscopic entry into the abdomen may have helped reduce the chances of postoperative complications such as surgical site infection, wound dehiscence, or de

Low-Grade Appendiceal Mucinous Neoplasm Presenting as Small Intestine Obstruction Due to Internal Hernia: A Case Report.

BACKGROUND: Appendiceal mucinous neoplasms (AMNs) are rare tumors that are often discovered incidentally and may present with non-specific symptoms such as abdominal pain, ascites, or features mimicking appendicitis. CASE PRESENTATION: A 72-year-old male presented with small intestine obstruction secondary to an internal hernia. CT scan imaging initially suggested adhesive bands; however, surgical exploration revealed an inflamed, tumor-bearing appendix tip adherent to the mesentery, forming a hernia orifice that trapped a loop of small intestine. The patient underwent appendectomy, mesenteri

A novel approach to peritonitis.

Peritonitis is a deadly abdominal condition seen across many surgical disciplines, with causes including appendicitis, perforated peptic ulcers, and abdominal trauma. In the South African context, peritonitis is largely influenced by tuberculosis (TB), human immunodeficiency virus (HIV), and widespread malnutrition linked to socioeconomic challenges. Although peritonitis is traditionally classified into primary, secondary, and tertiary types, the addition of quaternary peritonitis could reflect the severe complication risks associated with recalcitrant, systemic, or locally advanced disease. Case Report: Midgut malrotation presenting as left-sided appendicitis with secondary omental torsion: a diagnostic challenge in a child.

BACKGROUND: Omental torsion is an uncommon cause of acute abdomen in children and is usually secondary to other intra-abdominal pathology. Congenital midgut malrotation can alter anatomic relationships and lead to atypical presentations. This case report aims to highlight that congenital malrotation should be considered when left-sided appendicitis is suspected, especially when omental abnormalities are present. CASE PRESENTATION: We report a 12-year-old child who presented to a tertiary children's hospital in Southwest China with more than 48 h of abdominal pain and 24 h of fever after heavy

Does age matter? Acute appendicitis in octogenarians vs. younger patients - results of a German multicenter analysis.

BACKGROUND: Acute appendicitis is a common indication for emergency abdominal surgery. Octogenarians (≥ 80 years) are considered high risk due to comorbidities and reduced physiological reserve. This study examined age-related differences in symptoms, surgical management, and outcomes in patients undergoing surgery for acute appendicitis. METHODS: This multicenter retrospective cohort study includes 1,582 surgically treated patients from 41 German hospitals, comprising 1,533 non-octogenarians (18-79 years) and 49 octogenarians (≥ 80 years). RESULTS: Octogenarians had higher comorbidity burde

Isolated fallopian tube torsion (IFTT) associated with hydrosalpinx in an adolescent: a case report.

INTRODUCTION: Isolated fallopian tube torsion (IFTT) is a rare and often under-recognized cause of acute lower abdominal pain, particularly in adolescents. Clinical features are nonspecific and frequently mimic more common conditions, such as appendicitis or ovarian torsion, leading to delayed diagnosis and an increased risk of tubal necrosis. Association with hydrosalpinx in young patients without traditional risk factors is uncommon. CASE PRESENTATION: A 15-year-old nulligravid girl with regular menses and no history of sexual activity or prior pelvic surgery presented with a 2-day history

Perforation of appendiceal diverticulum causing local peritonitis: a case report and literature review.

BACKGROUND: Diverticulosis of the appendix is a rare pathological entity and there is association with the presence of neoplasms. Peritonitis caused by ruptured appendiceal diverticulum, as a result of inflammation, is an even rarer medical condition. The inflammation of appendiceal diverticula should be part of the differential diagnosis of right iliac fossa pain, especially in patients with signs of acute appendicitis, and computed tomography of the abdomen is the most useful imaging modality. An early accurate diagnosis is difficult and surgical approach remains the treatment of choice. CA

Trauma-Associated Perforation of a Duodenal Ulcer With Concomitant Acute Appendicitis Presenting As Generalized Peritonitis: A Case Report.

A perforated duodenal ulcer is a potentially life-threatening surgical emergency that may present with generalized peritonitis and require prompt operative management. Although most cases are related to peptic ulcer disease, clinical presentation following blunt abdominal trauma can create diagnostic uncertainty, particularly when imaging findings are non-specific or suggest an alternative intra-abdominal source. We report the case of a 68-year-old man with no significant medical history who presented with progressive abdominal pain four days after a ground-level fall. Physical examination rev
