Perforated Meckel's Diverticulum in an Adult that Resembles Acute Appendicitis: A Case Report and Review of the Literature.

BACKGROUND: Perforation is one of the rarest effects of Meckel's diverticulum and may clinically resemble acute appendicitis. CASE REPORT: A 34-year-old woman with pain in the right iliac fossa, nausea, and vomiting for three days was brought to the emergency department. An abdominal examination indicated rebound tenderness in the area of the right iliac fossa. Abdominal ultrasound showed a heterogeneous lesion in the left iliac fossa measuring 5 cm × 3.5 cm × 4 cm with no internal vascularity. Abdominal Computed Tomography (CT) demonstrated a hypodense lesion located left of the midline of t

Torsion of Meckel's diverticulum misdiagnosed as acute appendicitis in a resource-constrained care setting: a case report.

BACKGROUND: Meckel's diverticulum is the most common congenital small-bowel anomaly and is usually asymptomatic. However, torsion of Meckel's diverticulum is an extremely rare complication that can mimic other acute abdominal conditions such as appendicitis or small-bowel obstruction, posing a clinical challenge to surgeons that may result in misdiagnosis, especially in the absence of advanced imaging. CASE PRESENTATION: A 16-year-old Syrian male patient presented to the emergency department with 24-hour duration of severe right lower quadrant pain, vomiting, nausea, and 2-day history of obst

Cytomegalovirus Lymphadenitis After Anti-CD19 Chimeric Antigen Receptor T-cell (CAR-T): An Underappreciated Etiology of Hypermetabolic Lymphadenopathy in the Post-CAR-T Setting.

Chimeric antigen receptor T-cell (CAR-T) is a revolutionary type of immunotherapy that genetically engineers a patient's own immune T-cells to recognize and attack cancer cells. This type of therapy has transformed the treatment of relapsed/refractory large B-cell lymphoma (LBCL). However, profound immunosuppression may lead to opportunistic infections that could resemble relapse in rare situations. Cytomegalovirus (CMV) reactivation and clinical infection are well recognized after hematopoietic stem cell transplantation but are less studied in the post-CAR-T setting. We describe a 56-year-old

Randomized adaptive assessment of post COVID syndrome treatments (RAPID): a study protocol for a multicenter, randomized, controlled adaptive platform trial of treatment options for Post Covid Syndrome (PCS) on patients physical function including the first intervention specific appendix RAPID_REVIVE (reducing inflammatory activity in patients with PCS).

BACKGROUND: The majority of patients recovers from severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) coronavirus disease 2019 (COVID-19) without obvious sequelae, but a significant proportion suffers long-term consequences which have been termed post COVID syndrome (PCS). Despite a wide range of considerations on treatment options in PCS and a significant number of trials initiated, only very few results from randomized controlled trials are currently available. In conclusion, there is an evident medical need to identify treatments for patients with PCS. METHODS: The primary o

Fecal Fistula Following Appendectomy as an Unusual Late Complication: A Rare Case Report.

Post-appendectomy fecal fistula is a rare complication, but it is associated with significant morbidity. Etiological factors of post-appendectomy fecal fistula formation include severe peri-appendicitis involving the base of the appendix as well as adjoining caecal wall; neoplasia of the appendix and caecum; infective bowel conditions especially intestinal tuberculosis, actinomycosis and Crohn's disease; distal obstruction and gossypiboma. Diagnosis of post-appendectomy fecal fistula is usually made on CT-scan but Fistulography is also useful in the late stages of the disease. Treatment of pos

Gastrointestinal Neuroendocrine Neoplasms in Children and Adolescents: Data from the German MET Studies (1997-2024).

INTRODUCTION: Gastrointestinal neuroendocrine neoplasms (GI-NENs) outside the appendix and pancreas are exceptionally rare in children and adolescents. Limited data on presentation, treatment, and outcomes hinder clinical decision-making. METHODS: We retrospectively analyzed 16 patients under 18 years with histologically confirmed NENs of gastrointestinal origin, enrolled in the German Malignant Endocrine Tumor (MET) Registry from 1997 to 2024. Neuroendocrine carcinomas were eligible for inclusion but were not observed. Findings were compared with a cohort (age 0-20 years) from the SEER datab

Changing trends in appendiceal pathology: a 16-year retrospective extension study of surgically removed appendices in a tropical teaching hospital.

BACKGROUND: The vermiform appendix is prone to various diseases, with acute appendicitis (AA) being the most common. The increasing incidence in Nigeria complicates diagnosis, leading to negative appendectomies. Accurate and timely diagnosis, along with routine histopathological examination, is essential for effective patient care. AIM AND OBJECTIVES: To investigate changes in the pathology of surgically removed appendix tissue from our center over 16 years, establish temporal trends in the incidence of appendicitis, and determine if findings regarding appendiceal pathology hold over time. Organic electrolyte gated field effect transistor for detecting alpha synuclein longitudinally in Parkinsonism mouse model.

BACKGROUND: Enthesopathy is an evolving area for applied clinical research. MRI is the gold standard in the diagnosis of elbow joint pathology, but recent reports indicate that ultrasound imaging is more sensitive and accurate than MRI in detecting enthesopathy of the heels and knees. Too many patients are under-diagnosed and/or misdiagnosed because the early pathological changes of enthesitis in the different types of seronegative arthropathies are not detected. OBJECTIVES: This study was undertaken to describe the ultrasound features of elbow enthesitis in patients with seronegative arthrop

Incidental Appendico-Ileal Fistula in a Patient With Adhesive Intestinal Obstruction: A Case Report.

A fistulous communication between the appendix and any viscus is rare. Such fistula is most often acquired due to recurrent appendicitis, cystic fibrosis, Crohn's disease, tuberculosis, and malignancy. Here in, we report a rare case of an appendico-ileal fistula incidentally detected during laparotomy for adhesive small bowel obstruction. The fistula was divided, the ileal opening was sutured, and appendectomy was performed. Postoperative recovery was uneventful, with no evidence of malignancy, tuberculosis, or inflammatory bowel disease on the histopathological examination of the appendix.

Amyand's Hernia: A Rare Case Study of Perforated Appendicitis in an Inguinal Hernia.

Amyand's hernia is defined as the presence of an appendix contained within an inguinal hernia. An inguinal hernia is the protrusion of a small or large bowel through a peritoneal defect of the groin. In rare cases, the appendix can become incarcerated or strangulated within the hernia, cutting off the blood supply to the organ. If incarcerated, the appendix is at risk for strangulation, which can lead to rupture and cause worsening of symptoms and/or collapse. We report a case of a 76-year-old male with a history of inguinal hernia repair 30 years prior, who presented with 30 days of intermitt

Diagnosis of Pediatric Appendicovesical Fistula by Contrast-enhanced Ultrasound: A Case Report.

Appendicovesical fistula is a rare complication associated with appendicitis, Crohn's disease, or appendiceal tumors, posing significant diagnostic challenges. We reported a pediatric case of appendicovesical fistula that remained undiagnosed by non-contrast computed tomography, fluoroscopic voiding cystourethrography (VCUG). Although identified during cystoscopy, its connection to the fistula site could not be determined. However, the transvesical contrast-enhanced ultrasound clearly demonstrated the presence of fistula. Subsequently, laparoscopic appendectomy and bladder repair were performe

Early versus delayed appendicectomy for appendiceal phlegmon or abscess.

BACKGROUND: This is an update of a Cochrane review first published in 2017. Acute appendicitis (inflammation of the appendix) can be simple or complicated. Appendiceal phlegmon and appendiceal abscess are examples of complicated appendicitis. Appendiceal phlegmon is a diffuse inflammation in the bottom right of the appendix, while appendiceal abscess is a discrete inflamed mass in the abdomen that contains pus. Appendiceal phlegmon and abscess account for 2% to 10% of acute appendicitis. People with appendiceal phlegmon or abscess usually need an appendicectomy to relieve their symptoms (e.g.

Uncommon Presentation of a Perforated Appendicitis Leading to Duodenal Fistula: Case Report and Literature Review.

Multiple types of fistulas associated with the appendix have been reported; however, duodenal fistula resulting from perforated acute appendicitis has only been documented in one previous case. In this report, we present the case of an 18-year-old male patient who was diagnosed to have a complicated appendicitis in its normal position with abscess formation. He was started on IV antibiotics and underwent a CT-guided drainage of the abscess with drain placement. Two days later due to biliary output from the drain, CT fistulography and diagnostic laparoscopy were performed that revealed the pres

An International Consensus Survey among Pediatric Surgeons on the Role of Appendectomy in Malrotation.

INTRODUCTION: Ladd's procedure, originally described in 1936 for the treatment of malrotation, does not traditionally include appendectomy as a standard step. We conducted a multinational survey to investigate the current consensus on the role of appendectomy in Ladd's procedure. METHODOLOGY: An anonymous online survey was distributed to pediatric surgeons worldwide. The survey collected demographic data and explored surgical preferences related to the management of malrotation. Open-ended questions were used to assess the opinions regarding the necessity of appendectomy, decision-making fact

Low-grade appendiceal mucinous neoplasm penetrating sigmoid colon: A case report.

Low-grade appendiceal mucinous neoplasm (LAMN) is principally characterized by low-grade cytology without evidence of invasion to other organs. We report a LAMN surgical case whose appendiceal tumor penetrated the sigmoid colon wall. An 87-year-old man was referred for endoscopic resection (ER) of a colon polyp. Despite four ERs over 5 years, the polyp recurred at the same site. Laparoscopic surgery revealed a dilated appendix firmly attached to the sigmoid colon. We performed en bloc resection of both the sigmoid colon and appendix without tumor exposure. The histopathological evaluation show

Abscesses in different body sites among children: a single-center prospective study.

OBJECTIVES: An abscess is a localized collection of pus contained within a fibrous capsule. In this study, we aimed to determine the demographic pattern, common anatomical sites, risk factors, and the microbial profile of abscesses in different body sites among children. METHODS: We conducted a prospective study in our pediatric surgical department among children with abscesses in different body sites between January 2019 and December 2022. RESULTS: During the study period, 85 children were included. The participant age range was 0 to 14 years old, and 66% of participants were boys. The most

New classification of Amyand's hernia, our experience: a retrospective observational study with a literature review.

PURPOSE: Amyand's hernia (AH) is the name given to the type of hernia in which the appendix is found in a hernial sac. We aimed to share our clinical experience with a literature review for AH. METHODS: A total of 1,774 inguinal hernias and 13 AH cases were repaired in our clinic between 2009 and 2020. In addition, detailed clinical features about AH were extracted by including unpublished data of 165 cases, which were gathered from the extensive literature on childhood AHs using PubMed, Web of Science, and Cochrane databases. RESULTS: The rate of AH was 0.73% in all inguinal hernias; this r

Pseudo-submucosal tumor in the colon: seeing is believing.

A 65-year-old woman was admitted to our hospital with complaints of lower abdominal pain. Her physical examination was unremarkable. The results of routine laboratory testing were within the normal limits. In addition, abdominal CT was normal. Colonoscopy showed a cecum submucosal tumor with a pale yellow surface. Endoscopic ultrasound revealed homogeneous hypoechoic lesions originated from submucosal layer. ESD was subsequently performed to remove the submucosal lesion. During the ESD procedure, fecal outflowed from appendix opening . Yellow fecal-like material was visible after submucosal in

Appendiceal mucocele pathologically classified as appendiceal epithelial hyperplasia and preoperatively diagnosed by contrast-enhanced ultrasonography: A case report.

We report a patient with a mucocele with diffuse wall thickening diagnosed by transabdominal ultrasonography and contrast-enhanced ultrasonography. Transabdominal ultrasonography showed diffuse thickening of the entire appendix wall and an anechoic area that appeared to be fluid collected throughout the appendix lumen. However, the "onion skin sign" was not detected. Contrast-enhanced ultrasonography combined with superb microvascular imaging revealed abundant mucosal blood flow and no abnormal vascular network within the mucosa of the appendix wall. We preoperatively diagnosed a mucocele comp
