'Malignant granular cell tumor of the Appendiceal orifice: a rare case and review of literature'.

Granular cell tumors (GCTs) are rare, mostly benign soft-tissue neoplasms, with only 8% occurring in the gastrointestinal tract, making the colon an uncommon location. We present the case of a 37-year-old Hispanic woman who underwent a screening colonoscopy due to a family history of colon cancer. A submucosal nodule at the appendiceal orifice was resected, and a biopsy revealed a malignant GCT, confirmed by S100-positive staining. The patient underwent a right hemicolectomy, with the resection showing a 2 mm focus of granular cell tumor with clear margins. Histopathology confirmed malignancy,

The Systemic Inflammatory Response Index as a Novel Diagnostic Tool for Acute Appendicitis.

OBJECTIVE: This study aimed to assess the potential of the Systemic Inflammatory Response Index (SIRI) for diagnosing AA, its possible advantages over current methods, and its utility in distinguishing uncomplicated from complicated appendicitis. METHODS: In this cross-sectional study, 240 patients scheduled for appendectomy with a diagnosis of AA were enrolled. Demographic information, clinical and paraclinical findings, including complete blood count (CBC), Alvarado score, SIRI, sonography findings, and pathology results, were documented. Data were analyzed using SPSS software version 26.

Barium Meal and Follow-up in the Diagnosis of Chronic Appendicitis in Children with Recurrent Abdominal Pain: Is the Old War Horse Still Effective?

BACKGROUND: Recurrent abdominal pain (RAP) in children presents a complex diagnostic challenge, with causes ranging from organic to functional and psychological. While most cases are benign, chronic appendicitis remains a rare but important consideration. Its atypical presentation and overlap with other gastrointestinal disorders make diagnosis difficult. Among the various diagnostic tools, barium meal studies, though not primarily designed to assess the appendix, can provide useful insights when combined with small bowel follow-through, potentially revealing subtle indicators of appendiceal p

Incidentally-detected nonspecific and unusual histopathological findings in childhood appendectomy specimens: a retrospective analysis of 2633 cases.

BACKGRUND: Acute appendicitis is an important clinical condition that usually occurs as a result of obstruction of the appendix lumen due to fecaloma or reactive lymphoid hyperplasia. However, rarely, various nonspecific and unusual pathologies can cause acute appendicitis or mimic this clinical condition. The aim of this study was to present patients who underwent surgery with a preliminary diagnosis of acute appendicitis and to determine the incidence of incidentally diagnosed nonspecific and unusual pathologies found during histopathological examination of the specimens. METHODS: Data from

Metallic Pin Causing Appendicitis: A Case From Syria.

Acute appendicitis is the most common surgical emergency, typically resulting from luminal obstruction caused by inspissated stool, lymphoid hyperplasia, vegetable seeds, parasites, or, rarely, foreign bodies. We report the case of a 10-year-old girl presenting with acute abdominal pain, nausea, and loss of appetite. Ultrasound and laboratory findings were diagnostic for acute appendicitis. Her previous urinary tract infection initially obscured the clinical picture, delaying suspicion of an alternative cause. The discovery of a metallic pin within the inflamed appendix highlights the importan

Interval appendicitis.

There are a few studies that have focused on histopathologic findings in interval appendectomies compared to acute appendicitis and negative appendectomies. Our aim is to compare histopathologic features in interval appendectomies to a similar control group of stump appendectomies. A retrospective review study was conducted over a 10-year period. We found 21 (0.3 %) patients [age range: 10-70 years, average age: 40 years, male to female ratio: 1.3 to 1.0, average time interval: 6 weeks] who had interval appendectomies. Seventeen patients (81 %) showed three main patterns: xanthogranulomatous i

Broad Ligament Hernia Presenting as Acute Appendicitis: A Case Report.

Internal hernias, although rare, can present as surgical emergencies in certain instances, depending on the patient's symptomatology and clinical features. Broad ligament hernia is considered a rare type of internal hernia. Acute appendicitis is a condition that is caused by the inflammation of the vermiform appendix in response to a fecalith or lymphoid aggregation. Although stemming from differing etiologies and pathophysiological mechanisms, the conditions mentioned above can present similar signs and symptoms in clinical practice. In this case, we present a 37-year-old previously healthy w

The Great Mimicker: Extragonadal Endometriosis Presenting as an Appendiceal Mass and Acute Appendicitis.

A 45-year-old female presented to the Emergency Department with acute right lower quadrant (RLQ) pain. Her past medical history was significant for heavy menstrual bleeding and moderate dysmenorrhea, though a diagnosis of endometriosis had never been established previously. Initial work-up revealed anemia and leucocytosis. A CT of the abdomen and pelvis showed an enlarged structure in RLQ along the medial border of the cecum, suspicious for acute appendicitis with a retained rupture. CA19-9, CA-125, and CEA were sent, of which only CA-125 was elevated at 92.7 U/mL (reference range: 0-35 U/mL).

Changing trends in appendiceal pathology: a 16-year retrospective extension study of surgically removed appendices in a tropical teaching hospital.

BACKGROUND: The vermiform appendix is prone to various diseases, with acute appendicitis (AA) being the most common. The increasing incidence in Nigeria complicates diagnosis, leading to negative appendectomies. Accurate and timely diagnosis, along with routine histopathological examination, is essential for effective patient care. AIM AND OBJECTIVES: To investigate changes in the pathology of surgically removed appendix tissue from our center over 16 years, establish temporal trends in the incidence of appendicitis, and determine if findings regarding appendiceal pathology hold over time. Pseudo-submucosal tumor in the colon: seeing is believing.

A 65-year-old woman was admitted to our hospital with complaints of lower abdominal pain. Her physical examination was unremarkable. The results of routine laboratory testing were within the normal limits. In addition, abdominal CT was normal. Colonoscopy showed a cecum submucosal tumor with a pale yellow surface. Endoscopic ultrasound revealed homogeneous hypoechoic lesions originated from submucosal layer. ESD was subsequently performed to remove the submucosal lesion. During the ESD procedure, fecal outflowed from appendix opening . Yellow fecal-like material was visible after submucosal in

Appendiceal mucocele pathologically classified as appendiceal epithelial hyperplasia and preoperatively diagnosed by contrast-enhanced ultrasonography: A case report.

We report a patient with a mucocele with diffuse wall thickening diagnosed by transabdominal ultrasonography and contrast-enhanced ultrasonography. Transabdominal ultrasonography showed diffuse thickening of the entire appendix wall and an anechoic area that appeared to be fluid collected throughout the appendix lumen. However, the "onion skin sign" was not detected. Contrast-enhanced ultrasonography combined with superb microvascular imaging revealed abundant mucosal blood flow and no abnormal vascular network within the mucosa of the appendix wall. We preoperatively diagnosed a mucocele comp

Unexpected Culprit: Unveiling a Unique Case of Appendicitis Triggered by a Foreign Object.

Appendicitis is a common surgical emergency marked by inflammation of the appendix, often due to blockage of the appendix lumen by fecoliths, lymphoid hyperplasia, or neoplasms. While various causes are known, appendicitis triggered by a foreign body (FB) is exceptionally rare. This case report highlights a rare presentation of appendicitis in a 32-year-old male with no significant medical history, who presented with acute lower right abdominal pain, fever, and vomiting. Initial evaluation suggested appendicitis, further supported by laboratory findings and diagnostic imaging revealing a retro

A practical guide to serrated appendiceal lesions.

Several neoplastic and non-neoplastic proliferations of the appendix can show varying degrees of serrated epithelial architecture. Of these, diffuse mucosal hyperplasia is most common, followed in frequency by low-grade mucinous and serrated neoplasms. It is important to distinguish serrated appendiceal neoplasms from their potential mimics because these entities may be managed differently. Diffuse mucosal hyperplasia is a non-neoplastic change that usually develops in the setting of resolving appendicitis and requires no further therapy or surveillance, and serrated neoplasms confined to the

Is appendoscope a new option for the treatment of acute appendicitis?

Acute appendicitis is a common surgical emergency. It is commonly caused by obstruction of the appendiceal lumen due to fecaliths, tumors, or lymphoid hyperplasia. For over a century, appendectomy has been the primary treatment for acute appendicitis. Abraham Groves performed the first open appendectomy in 1883. In 1983, Kurt Semm completed the first laparoscopic appendectomy, heralding a new era in appendectomy. However, appendectomy is associated with certain complications and a rate of negative appendectomies. Studies have suggested controversy over the impact of appendectomy on the develop

Acute appendicitis induced by bone fragment ingestion: A pediatrics case report.

Acute appendicitis is an ordinary surgical emergency, typically attributed to luminal obstruction by fecaliths or lymphoid hyperplasia. However, ingested foreign bodies as an etiology are rare but increasingly recognized, particularly in pediatric patients. We present the case of a 9-year-old male patient who presented to the emergency department with symptoms consistent with acute appendicitis. Further investigation revealed the presence of a bone fragment within the appendix, leading to acute inflammation. Foreign body ingestion should be considered in pediatric patients with acute appendici

Enterobius vermicularis in the Appendiceal Lumen: A Case Report.

Enterobius vermicularis is a common parasitic infection worldwide. Acute appendicitis (AA) is a frequently encountered condition in general surgery; however, its association with E. vermicularis remains controversial. AA caused by E. vermicularis is a relatively uncommon infection that primarily affects children. We reported a 21-year-old female who was admitted to our hospital due to right lower quadrant abdominal tenderness. Laboratory tests and imaging were not consistent with AA. She underwent a diagnostic laparoscopy and appendectomy. Histopathological examinations revealed the presence o

Acute appendicitis due to appendiceal endometriosis: Two case report and literature review.

INTRODUCTION: Appendiceal endometriosis (AE) is a rare condition, with a prevalence ranging from 0.05 % to 1.7 % in patients with endometriosis. It represents <1 % of cases of acute appendicitis (AA). CASES PRESENTATION: We present two cases of AA where the histological cause was endometriosis. Both cases involved patients around 40 years old who presented with abdominal pain in the right iliac fossa. AA was diagnosed through abdominal computed tomography (CT), which in the first case showed acute appendicitis, successfully treated with laparoscopic appendectomy. In the second case, the CT sh
