A unique case of appendiceal diverticulum presenting with positive faecal immunochemical test.

Appendiceal diverticulum is a rare condition that usually presents with symptoms similar to acute appendicitis. Although imaging can be used to aid the diagnosis of this condition, it is usually confirmed postoperatively on the basis of histology. Because of an increased risk of appendiceal neoplasms, the usual management is prophylactic appendicectomy. We report the case of a 70-year-old lady with no symptoms referred from her GP surgery for a positive faecal immunochemical test as part of the bowel screening programme. Colonoscopy showed a mass at the appendiceal orifice with normal histolog

Recurrent Ileocolic Intussusception With the Appendix as the Pathologic Lead Point in Children: A Report of Two Cases and Review of Literature.

Ileocolic intussusception is a consideration in young pediatric patients with acute abdominal pain. Meckel's diverticulum is the most common pathologic lead point for intussusception in children and the appendix acting as the lead point is rare. In addition, management guidelines for recurrent ileocolic intussusception (RICI) are lacking. We present two cases of RICI in which the pathological lead point was the appendix. The first patient, a two-year-old with no medical history, had intermittent abdominal pain and non-bloody vomiting for a month. Ultrasound revealed ileocolic intussusception,

Perforation of Meckel's diverticulum by an unusual foreign body: A case report and a review of literature.

KEY CLINICAL MESSAGE: Perforation of Meckel's diverticulum (MD) is rare, particularly by foreign body. High index of suspicion and thorough intraoperative assessment is needed in patients undergoing surgery for acute appendicitis, specifically when appendix appears normal. ABSTRACT: Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract. While often asymptomatic, it can present with several complications. Perforation due to foreign body ingestion is rare but can have severe consequences if late diagnosis occurs. A 13-year-old male, initially suspected of acu

Concurrent Appendicitis and Meckel's Diverticulitis Presenting With Small Bowel Obstruction in an Adult: A Case Report.

We present a case of multiple abdominal pathologies occurring simultaneously, which emphasizes the importance of keeping a broad differential and evaluating each diagnosis. A 33-year-old female presented with abdominal pain, nausea, and vomiting. Her workup included computerized tomography which demonstrated acute appendicitis with concern for a closed-loop bowel obstruction. She was offered diagnostic laparoscopy with anticipation of laparoscopic appendectomy and further evaluation for the source of the bowel obstruction. At the time of surgery, a Meckel's diverticulum with acute diverticulit

Differentiating appendiceal neoplasm from perforated appendiceal diverticulum in chronic appendicitis: a case report.

Low mucinous neoplasm of the appendix (LAMN) and appendiceal diverticulum are both uncommon pathologies that may pose a diagnostic challenge. Both may present as either appendicitis or asymptomatically and have a risk of perforation. LAMN, carries the additional risk of pseudomyxoma pertitonei and metastasis. Ensuring correct histopathology is crucial, as computed tomography (CT) abdomen/pelvis may only demonstrate a mildly dilated appendix, delaying diagnostic laparoscopy and appendicectomy. Here, we describe the case report of a 56-year-old woman who presented with chronic intermittent right

Subhepatic Appendicitis With Appendicular Diverticulitis: A Rare Combination for Acute Abdomen.

Subhepatic appendicitis is an unusual presentation of acute appendicitis (AA). Similarly, another uncommon condition that resembles AA is appendiceal diverticulitis (AD), which is a rare form of vermiform appendix pathology. It is exceedingly uncommon for the two to occur simultaneously. We present the case of a 41-year-old male presented with a one-day history of sudden onset of right iliac fossa (RIF) pain associated with a two-day history of nausea and fevers. The only notable lab finding was elevated C-reactive protein (CRP). Clinical examination revealed right abdominal and renal angle te

Ruptured Appendiceal Diverticulum Leading to Tubo-Ovarian Abscess in a Non-Sexually Active Woman: A Case Study.

BACKGROUND Pelvic inflammatory diseases and tubo-ovarian abscesses (TOAs) are rarely seen in non-sexually active (NSA) women. While the pathogenesis of TOA remains unclear, its risk factors include ascending infection of the genital tract, gastrointestinal tract translocation, congenital genitourinary anomalies, as well as virulence of the causative agents, with preceding bacteremia and septicemia. CASE REPORT Herein, we present the case of a 25-year-old female patient who was initially diagnosed with ovarian torsion and underwent diagnostic laparoscopy. Her intraoperative findings included bi

Perforated Meckel's Diverticulum as a Rare Cause of Pneumoperitoneum in a Post-pubescent Adolescent: A Case Report.

Meckel's diverticulum (MD) is known to cause surgical emergencies including intussusception, obstruction, and bleeding, but rarely results in perforation with pneumoperitoneum. Symptomatic MD is rare but most commonly presents in early childhood. We report a case of a 17-year-old male who presented with peritonitis and radiographic imaging demonstrating pneumoperitoneum and inflammation near the appendix and terminal ileum. Intraoperative findings revealed a perforated MD. A perforated MD may mimic a diagnosis of complicated appendicitis in the adolescent and adult population and should be con

Small Bowel Obstruction Secondary to Meckel's Diverticulum: A Rare Complication.

Appendicitis is defined as an inflammation of the appendix and is one of the most common presentations to the general surgical team. The presence of right iliac fossa pain associated with a rise in inflammatory markers raises the suspicion of appendicitis in young, healthy patients. Colitis and bowel obstruction would be considered lower down on the list of differentials. Other common differentials to take into consideration include ovarian pathology and pelvic inflammatory disease in females and diverticulitis in the other population. Investigation for these patients includes ultrasound scans

Appendiceal diverticulitis: A rare case report highlighting diagnostic challenges and complication risks.

INTRODUCTION AND IMPORTANCE: Diverticular disease of the appendix is a rare condition often mistaken for acute appendicitis, with most cases diagnosed incidentally during histopathology, underscoring the importance of comprehensive pathological evaluation for accurate diagnosis. CASE PRESENTATION: A 34-year-old male presented with one day of periumbilical pain migrating to the right iliac fossa, without vomiting or bowel changes. Clinical examination revealed tenderness in the RIF. Laboratory tests indicated a syndrome inflammatoire biologique. Ultrasound and CT scans suggested appendicular d

Meckel's diverticulum mimicking acute appendicitis in children: a retrospective cohort study.

BACKGROUND: The aims of this study were to summarize the clinical presentation and histological results of 20 cases of complicated Meckel diverticulum (MD) who were presumed to have acute appendicitis before surgery, as well as to improve the diagnosis and treatment of complicated MD in children. MATERIALS AND METHODS: We retrospectively reviewed the records of 20 complicated MD admitted to our institution who were preoperatively diagnosed with acute appendicitis from January 2012 to January 2019. Patients were divided into the perforated MD group and the Meckel's diverticulitis group. Patien

Complicated Meckel's Diverticulum Presenting as Pneumoperitoneum in an Adolescent.

BACKGROUND The finding of pneumoperitoneum frequently leads to operative management for diagnosis and treatment. The etiology of pneumoperitoneum includes perforated viscus, such as perforated peptic ulcers, small or large intestinal perforations, appendicitis, and complicated sigmoid diverticulitis. We describe the preoperative, intraoperative, and postoperative course of a patient with perforated Meckel’s diverticulitis presenting with pneumoperitoneum. This unusual presenting finding highlights that Meckel’s diverticulum should be included in the differential diagnosis in adoles

Congenital diverticulum of the vermiform appendix: a rare finding of a routine operation.

Congenital diverticulum of the appendix as well as diverticulitis of the vermiform appendix remain a rare, mainly retrospective diagnosis, reported by the pathologist, post-appendicectomy. In clinical practice, however, it is important to be aware of this entity, the different types, diagnosis, association with malignancy and treatment options. We are presenting a young female case of pathologically proved true diverticulum of the appendix with associated uncomplicated appendicitis and an overview of the literature.

Low incidence of occult colorectal carcinoma in a cohort of 150 consecutive patients with diverticular strictures.

BACKGROUND: The symptoms of stricturing diverticulitis can overlap with those of colorectal cancer. Additionally, the stricture itself may mask a "hidden" colorectal cancer. We aimed to describe the demographics, operative details and outcomes, including occult colorectal cancer, in a cohort of consecutive resections for presumed diverticular strictures. METHODS: In this single-center, retrospective cohort study, all patients undergoing resection for a presumed diverticular stricture between January 2010 and December 2015 were included. Preoperative imaging and colonoscopies were individually

Complicated appendiceal diverticulitis a extremely rare cause of acute abdomen: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Appendiceal diverticulitis (AD) represents a rare cause of acute abdomen. Diagnosis of AD is a challenge because of its rarity and resemblance to other ileocecal diseases like as cecal diverticulitis (CD) and acute appendicitis (AA). Preoperative imaging can be useful to aid diagnosis. Surgery represents the correct treatment of AD. CASE PRESENTATION: A 48-year-old Caucasian male presented to the Emergency Department with a two-day history of right lower quadrant (RLQ) abdominal pain and fever. Physical examination revealed RLQ abdominal pain and rebound tendernes

A case of laparoscopic appendectomy for appendiceal bleeding.

BACKGROUND: Appendiceal bleeding is very rare, accounting for about 0.4% of all lower gastrointestinal bleeding. We present a case of laparoscopic appendectomy in a patient with a diagnosis of appendiceal bleeding. CASE PRESENTATION: A 71-year-old man came to our hospital with a complaint of bloody stools. He had progressive anemia and persistent fresh bloody stools, so he underwent lower gastrointestinal endoscopy. Active bleeding was confirmed from the orifice of the appendix, but the bleeding could not be stopped even with clips, so an emergency laparoscopic appendectomy was performed. His

Amyand's hernia in a patient with acute complicated diverticulitis. A case report and review of the literature.

INTRODUCTION: Amyand's hernia with an inflamed or perforated appendix is rare with incidence of 0.1 % and 0.01 % of cases respectively. It has been described in conjunction with cecal perforation, cholecystitis and ureterolithiasis. Yet, its association with acute sigmoid diverticulitis has never been reported before. PRESENTATION OF CASE: A 57-year-old male presented to the emergency department with acute abdomen and an indurated right inguinal mass. A preoperative computed tomography (CT) scan reported a giant inflamed sigmoid diverticulum and an Amyand's hernia with a complicated appendici

Acute Abdomen Secondary to Ileal Diverticulum: A Case Report.

Small-bowel diverticulosis is rare. We report the case of a male with an acute abdomen secondary to an ileal diverticulum. A 46-year-old male complained of progressive abdominal pain over 24 hours of evolution in the left flank. On physical examination, we found abdominal pain in the left flank and mesogastrium, tenderness, and signs of peritonitis. The simple abdominal CT showed a heterogeneous tubular image in the small bowel. We performed a diagnostic laparoscopy and found a normal cecal appendix. There was no free abdominal fluid or adhesions, and the colon was without diverticula. We foun

Is Appendiceal Diverticulitis Mimicking Acute Appendicitis?

Appendicular diverticulitis (AD) is a rare entity characterized by the inflammation of the arising diverticulum of the appendix. It has been reported to carry a high risk of perioperative complications, such as bleeding and perforation. Furthermore, multiple articles have highlighted the importance of diagnosing AD early due to its strong association with malignancies. Limited published cases concerning AD in our country and globally are available in the literature. Hence, we present in this article a case series of five exciting cases of incidental findings of AD that were initially diagnosed

The Frontiers of Appendiceal Controversies: Mucinous Neoplasms and Pseudomyxoma Peritonei.

Appendiceal mucinous neoplasms show a range of morphologic features and biological risk. At one end of the spectrum, high-grade adenocarcinomas are cytologically malignant with infiltrative invasion, lymph node metastases, and behavior similar to that of extra-appendiceal mucinous adenocarcinomas. At the other end, mucinous neoplasms confined to the mucosa are uniformly benign. Some cases lying between these extremes have potential risk to metastasize within the abdomen despite a lack of malignant histologic features. They show "diverticulum-like," pushing invasion of mostly low-grade epitheli
