Endoscopic Examination of Mitrofanoff Channel Findings and Subsequent Surgical Change of Management-Observations From a 2-Year Patient Cohort.

INTRODUCTION: A Mitrofanoff channel is a continent catheterisable channel, most commonly constructed from the appendix or small bowel (Monti channel) to provide an alternative to the urethra for intermittent catheterization. It is imperative for patients to be counselled on the risks of channel complications, including stomal stenosis, recurrent urinary tract infections (rUTI), and, less commonly, incontinence. Endoscopy of the Mitrofanoff (Mitrofanoscopy), is a diagnostic tool employed in these scenarios to visualize the channel and bladder. Herein, we aim to characterize the findings of Mitr

An unusual colo-ovarian fistula of advanced colorectal carcinoma: A case report and literature review.

In colorectal cancer, the invasion of the colon is not unusual, but the perforation of all layers of the colon or rectum and the presentation of the patient with generalized peritonitis is rare. Colo-ovarian fistulas have as their starting point either in the sigmoid colon, rectum, appendix or, more frequently, as a benign or malignant ovarian tumor. The present study reports an extremely rare case of colorectal cancer with perforation at the rectosigmoid junction into a metastatic ovarian tumor. Intraoperatively, a colo-ovarian fistula with diffuse peritonitis was diagnosed. The treatment con

Comparing Preoperative Healthcare Utilization Between Immigrants and Non-Immigrants Undergoing Emergency General Surgery in British Columbia.

Immigrants to Canada face unique barriers that result in them accessing publicly funded healthcare services differently than non-immigrants. These barriers are particularly critical in the context of Emergency General Surgery (EGS) where timely access to healthcare is essential for better health outcomes. This study compares preoperative healthcare utilization between immigrants undergoing EGS with non-immigrants. EGS procedures in British Columbia between 2013 and 2021 were analyzed using population-based linked immigration, physician billing, and hospital discharge summary data. Preoperative

Appendicovesical fistula in a child with acute myeloid leukaemia.

A paediatric patient was diagnosed with acute myeloid leukaemia (AML), with biallelic mutations in the transcription factor CCAAT/enhancer binding protein α (C/EBPα). During her treatment, she developed recurrent episodes of febrile neutropenia with neutropenic colitis, leading to the development of an appendicovesical fistula (AVF) with drainage of feculent urine. She underwent laparotomy, division of the fistula with a wide margin of the bladder with repair, with appendicectomy which essentially cured the condition. This case report illustrated the unusual occurrence of AVF in a paediatric p

Entero-Appendiceal Fistula Presenting with Overt Gastrointestinal Bleeding: A Case Report.

INTRODUCTION: Entero-appendiceal fistula is an extremely rare entity, with very few cases reported in the literature. Most previously described cases presented with abdominal pain or were diagnosed incidentally. To our knowledge, there have been no reports of such a fistula presenting with overt gastrointestinal bleeding. CASE PRESENTATION: A 56-year-old man with a medical history of human immunodeficiency virus infection, alcoholic cirrhosis, and paroxysmal atrial fibrillation, who was under regular follow-up at our hospital, presented with a 4-day history of hematochezia that was occurring

Appendico-Sigmoid Fistula as a Rare Complication of Chronic or Recurrent Appendicitis: A Case Report.

BACKGROUND Appendico-colic fistula is a rare condition characterized by an abnormal communication between the appendix and colon, most commonly involving the sigmoid colon (ie, appendico-sigmoid fistula) because of their close anatomical proximity. It is usually associated with chronic or recurrent appendicitis, diverticular disease, inflammatory bowel disease, or malignancy. Preoperative diagnosis is challenging due to nonspecific clinical and radiologic findings. CASE REPORT A 46-year-old woman presented with a 3-month history of lower abdominal pain, which had worsened over the preceding 15

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Primary retroperitoneal pseudomyxoma masquerading as a benign retroperitoneal abscess from an ascending colon subacute perforation: a case report.

INTRODUCTION AND IMPORTANCE: Pseudomyxoma peritonei is a neoplastic process characterized by diffuse mucinous implants on peritoneal surfaces arising from mucin-producing tumors. Rarely, pseudomyxoma from a ruptured mucin-producing tumor can be confined to the retroperitoneum, termed "retroperitoneal pseudomyxoma" or "extraperitoneal pseudomyxoma." This entity often masquerades as a retroperitoneal abscess of benign etiology. CASE PRESENTATION: A 58-year-old man presented with a symptomatic retroperitoneal cystic mass with a fistulous communication to the ascending colon seen at colonoscopy,

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Uncommon Presentation of Perforated Appendicitis: Abdominal Wall Abscess and Fistula Formation.

BACKGROUND Acute appendicitis is a common surgical emergency, and perforated appendix is one potential complication. Acute appendicitis can be complicated by perforation and peritonitis, but chronic abscess formation is less common. This report presents the case of a 45-year-old woman with a 7-day history of right lower-abdominal pain and swelling due to perforated acute appendicitis and abdominal wall abscess that required laparotomy and drainage. The presentation of this particular case is unique in that a fistulous tract formed subsequent to perforation of the appendix, with the resultant a

Delayed malignant appendiceal fistula with ileal conduit following radical cystectomy: A case report.

Fistula formation after radical cystectomy is rare, often presenting early and involving the small bowel. Herein we describe a case of an appendiceal fistula in a patient who underwent chemoradiotherapy and pelvic exenteration over a decade prior to the presentation of their fistula. Initially suspected to be radiation-induced, final pathology revealed a mucinous neoplasm of the appendix, which had fistulized with the ileal conduit. This case emphasizes the importance of considering alternative causes, such as malignancy, in delayed fistulas, especially in patients with prior radiation. Clinic

Using panoramic radiographs to assess the probability of causing oroantral communication following tooth removal. A retrospective cross-sectional study.

OBJECTIVES: To preoperatively assess the probability of oroantral communication (OAC) following the removal of maxillary (pre)molars using panoramic radiographs (PAN) and to assess the OAC-rate. MATERIALS AND METHODS: During a 4,5-year period, patient characteristics of removals of maxillary (pre-)molars were recorded: 'sex', age, 'sidedness', 'type of tooth', 'reason for removal'' and 'occurrence of OAC'. On the PAN of OAC-cases and of 100 control cases, the 'Fraction of the Root Overlapping the Maxillary Sinus' (FROMS) was calculated. The OAC-rate was reported overall, per tooth type and fo

Rare case of non-Hodgkin lymphoma of the appendix and ileocecal valve presenting as acute perforated appendicitis.

Non-Hodgkin lymphoma of the gastrointestinal tract is one of the least common primary malignancies, with diffuse large B-cell lymphoma being the most common histological subtype. Primary appendiceal lymphoma is extremely rare, with symptoms usually mimicking those of acute appendicitis. Here, we present a rare case of primary appendiceal non-Hodgkin lymphoma with ileocecal valve involvement and coexistence of Schistosoma eggs. The patient was initially diagnosed with acute perforation appendicitis. Following laparoscopic appendectomy, the patient developed an enterocutaneous fistula, which was

A rare case of congenital appendiceal duplication complicated by an appendiceal-sigmoid fistula: A case report.

INTRODUCTION: Duplication of the appendix is a rare congenital anomaly, occurring in approximately 0.004 % to 0.009 % of the population. In contrast, appendiceal-sigmoid fistulas are uncommon acquired conditions, usually associated with chronic inflammation or neoplastic processes. To our knowledge, the coexistence of these two entities has not been previously reported. CASE PRESENTATION: A 26-year-old female presented with a one-month history of right lower quadrant abdominal pain, loss of appetite, and low-grade fever. Imaging findings initially suggested chronic appendicitis. During explor

Follow-up of 35 appendiceal orifice neoplasms resected by endoscopic full-thickness resection.

BACKGROUND AND AIMS: Endoscopic full-thickness resection (EFTR) is an established, safe technique for the resection of appendiceal orifice (AO) neoplasms. Post-EFTR appendicitis is a recognized adverse event. There are no systematic reviews, and there is a paucity of literature with assessed outcomes, particularly with respect to delayed appendicitis, mucocele, or fistula formation. We aimed to evaluate the efficacy of EFTR for AO lesions. METHODS: Consecutive AO lesions referred for consideration of EFTR were prospectively studied. Multiple data points were recorded including technical succe

Acquired appendiceal-cecal fistula as a complication of untreated acute appendicitis in a noncompliant patient: a case report.

BACKGROUND: Appendiceal fistula is a primary appendix perforation into the surrounding hollow viscera or skin and can rarely occur as a complication of appendicitis. To date, only nine cases of appendiceal fistula to the cecum have been reported. CASE PRESENTATION: We present the case of a 30-year-old Persian male who presented with a low-grade fever and lower abdominal pain for 3 days. He had a history of untreated acute appendicitis 3 years prior. During the appendectomy, a fistula between the appendix and cecum, along with a thick cecal wall, was found. The patient underwent a right hemico

Crohn's disease presenting as acute appendicitis: Case series.

INTRODUCTION AND IMPORTANCE: Crohn's disease (CD) is a chronic, transmural inflammatory bowel disease (IBD) that can affect any part of the gastrointestinal tract. Its rare involvement of the appendix can mimic acute appendicitis, often leading to misdiagnosis and inappropriate initial management. CASE PRESENTATION: We report three patients who presented with right lower quadrant abdominal pain and were initially diagnosed and surgically treated for acute appendicitis. However, all were subsequently found to have ileocecal CD. Each case involved complications such as fistula formation or pers

Robot-assisted laparoscopic treatment of appendicovesical fistula in a child: a case report and literature review.

Appendicovesical fistula (AVF) is a rare condition that occurs in children characterized by an abnormal communication between the appendix and the bladder, caused by diseases such as inflammation or malignancy. Due to the lack of specific clinical symptoms and the difficulty in confirming the diagnosis with traditional imaging examinations such as ultrasound and CT, the disease often has a protracted course. This paper reports a case of a pediatric case of AVF presenting with dysuria, urinary frequency, and diarrhea. The diagnosis was confirmed by contrast-enhanced ultrasound (CEUS), and the p

Late-Onset Mesh Infection 10 Years After Right Inguinal Hernia Repair: A Case Report.

BACKGROUND Late-onset mesh infection after inguinal hernia repair is uncommon, and onset occurring years later is rare. Risk factors include field or implant contamination, host factors, such as diabetes and chronic steroids, and secondary fistulae with adjacent organs. CASE REPORT An 86-year-old man underwent right inguinal hernia repair with the Prolene Hernia System about 10 years earlier. He presented with right lower quadrant pain. Computed tomography (CT) showed an abscess tracking from the appendiceal tip to the right lower abdominal wall, suggesting mesh infection. Symptoms improved wi
