Decoding the human CD4+ T cell epitope repertoire for Lassa fever virus reveals T cell-based pan-mammarenavirus vaccine candidates.

Mammarenaviruses are classified into Old World and New World viruses (Old World arenaviruses [OWAs] and New World arenaviruses [NWAs]). Characterization of antigens recognized by human T cells is essential for identifying immunodominant targets, informing vaccine design, and performing immunological assessments. Here, we select the Lassa virus (LASV) as a prototype OWA to map the human CD4 T cell epitope repertoire. We then calculate conservation in different LASV lineages and other representative OWAs and NWAs and define conserved T cell epitope regions (CTERs) using Lassa as the OWA prototyp

Adjuvanted inactivated rabies virus-vectored Lassa virus vaccine in healthy adults: a phase 1 trial.

Lassa fever causes substantial morbidity and mortality in West Africa, and no licensed vaccine is available. We evaluated LASSARAB, an inactivated rabies virus-vectored Lassa virus (Josiah strain) glycoprotein complex vaccine. We conducted a randomized, controlled, dose-escalation phase 1 trial. Participants (total n = 54) received two intramuscular doses of LASSARAB containing 700 (n = 15), 1,400 (n = 15) or 2,800 (n = 14) relative units of antigen formulated with the TLR-4 agonist 3D-6-acyl PHAD-SE adjuvant, or licensed rabies vaccine control (n = 10), administered 28 days apart. Myocardial FAPI Uptake Post-myocardial Infarction in a Systemic Vasculitis Patient: Insights from 68Ga-FAPI PET/CT.

BACKGROUND: The rarity of atopy in traditional societies has been attributed to high parasite-driven blocking IgE concentrations. Information is lacking on the relationship between atopy, IgE and intestinal helminth infection in African populations. OBJECTIVE: To determine the prevalence of atopy and intestinal helminth infection and to relate these to wheeze history and serum total IgE in a community sample of adults from an urban (Banjul) and a rural (Farafenni) area of the Gambia. METHODS: Six hundred and ninety-three adults were interviewed about respiratory symptoms using a modified ver

Bacterial Infection in Ebola Virus Disease, Marburg Virus Disease, Crimean-Congo Hemorrhagic Fever, and Lassa Fever.

BACKGROUND: Concern for bacterial coinfection in patients with high consequence viral infections (HCVIs) has made empiric antibiotic administration the standard of care. However, the incidence, microbiology, and clinical significance of bacterial coinfection in HCVIs remain unclear. METHODS: We conducted a systematic review of case reports, cohort studies, and cross-sectional analyses describing bacterial coinfection in Ebola virus disease (EVD), Marburg virus disease (MVD), Crimean-Congo hemorrhagic fever (CCHF), and Lassa fever (LF). Data were extracted on study design, setting, diagnostic

Mechanisms of neuroimmune crosstalk in vestibular disorders: Linking inflammation with neural circuit dysfunction (Review).

BACKGROUND: Foot orthoses (FOs) are prescribed as an important conservative treatment option in patients with foot problems related to rheumatoid arthritis. However, a broad variation in FOs is used, both in clinical practice and in research. To date, there is no overview on the outcomes of the treatment with different kinds of FOs in patients with rheumatoid arthritis and a specific foot problem. The objectives of the present study were to summarize the comparative effectiveness of FOs in the treatment of various foot problems in patients with rheumatoid arthritis, on the primary outcomes foo

Infectious medical waste (IMW) and disease transmission: A critical public health challenge in Africa and the global implications - A systematic review.

BACKGROUND: Infectious Medical Waste (IMW) from both human and animal healthcare sources have the potential to transmit diseases in human populations due to its pathogenic composition. Conversely, a paucity of data regarding cause-and-effect relationships inhibits our understanding of the extent to which disease transmission occurs. In parts of Africa where the management of IMW is not prioritized, the risks to public health, along with its global implications cannot be ignored. METHODOLOGY: A systematic literature search following the PRISMA guidelines was conducted across five electronic da

UNLABELLED: The sirolimus-eluting stent (CYPHER( trade mark )) is a metal stent coated with 140 micro g/cm(2) of sirolimus blended with synthetic polymers. After stent implantation, sirolimus is slowly released causing localized cytostatic inhibition of proliferation of vascular smooth muscle cells in the peri-stent arterial wall over a period of about 1 month. Only minimal amounts of sirolimus enter the bloodstream and these appear to be insufficient to be of clinical relevance. In clinical trials that evaluated single de novo lesions and in-stent restenosis in patients with coronary artery d

Parabens and estrogen-mediated cancers: unraveling coincidence, co-occurrence, and causality.

Trabeculectomy remains the 'gold standard' intraocular pressure (IOP)-lowering procedure for moderate-to-severe glaucoma; however, this approach is associated with the need for substantial post-operative management. Micro-invasive glaucoma surgery (MIGS) procedures aim to reduce the need for intra- and post-operative management and provide a less invasive means of lowering IOP. Generally, MIGS procedures are associated with only modest reductions in IOP and are targeted at patients with mild-to-moderate glaucoma, highlighting an unmet need for a less invasive treatment of advanced and refracto

Etiologies of community-acquired febrile illness identified by TaqMan Array Card qPCR on blood samples: a systematic review and meta-analysis.

UNLABELLED: Community-acquired febrile illness presents a significant diagnostic challenge. The TaqMan Array Card (TAC) enables simultaneous detection of many bacterial, viral, parasitic, and fungal pathogens. This systematic review aimed to characterize the etiologies of community-acquired fever diagnosed by TAC directly on blood specimens. A systematic search of PubMed, Scopus, Embase, and Web of Science (from inception to January 2026 in English) identified studies using TAC to detect pathogens directly in human blood to identify the etiology of community-acquired febrile illness. The Plague of Athens: Investigating the Enigmatic Epidemic of Ancient Greece.

The ancient Greek historian Thucydides described in his book "History of the Peloponnesian War" one of the earliest epidemics in known human history, the "Plague of Athens". The plague, which lasted from 430-426 B.C., had a death toll estimated at around 75,000 - 100,000. The importance of the plague was immense. Pericles, the historical leader of Athens, was among its victims, and his death was just the beginning of the fall of the Golden Age of Athens. The enormous death toll from the plague weakened Athens, leading to its eventual defeat in the war against Sparta. So far, the cause of this

Pathways to health resilience: A cross-country analysis of health system response to re-emerging infectious diseases in Sub-Saharan Africa.

Sub-Saharan Africa (SSA) faces recurrent threats from re-emerging infectious diseases, including Ebola virus disease, cholera, measles, Lassa fever, and Marburg virus disease. This study assessed the resilience of health systems in 25 SSA countries from 2000 to 2023 using a multidimensional resilience index and a first-order Markov state-transition framework. Country-year observations were classified as Low Resilience, Moderate Resilience, or High Resilience based on epidemiological performance, health financing, surveillance, response capacity, and recovery indicators. Resilience was heteroge

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Teaching Caregivers to Implement the Picture Exchange Communication System: An Evaluation of Video Modeling.

BACKGROUND: Around 60% to 80% of patients with periampullary carcinoma are unresectable either due to distant metastasis or local vascular invasion. With the advancement of endoscopic interventional procedures, the role of surgical bypass has diminished. However, surgical bypass is still appropriate in patients with unresectable disease discovered at the time of surgery. This study was conducted to assess the results of palliative surgical bypass for patients with unresectable periampullary carcinoma at our hospital, a tertiary referral center of Northern India. METHOD: The study group compri

Pathological Insights into Lassa Virus-Induced Vestibular Dysfunction in Mice: Histopathological Analysis of the Inner Ear Vestibular Apparatus.

Lassa fever (LF), caused by Lassa virus (LASV) infection, typically leads to mild symptoms in humans, but some survivors experience audiovestibular problems. Here we present vestibular histopathological insights in our LF model mice. We observed (1) hemorrhage within the vestibular ganglion and stroma beneath the sensory epithelium, (2) preserved hair cells and supporting cells, (3) LASV antigen presence in the vestibular ganglion cells and the stroma beneath the sensory epithelium, and (4) CD3-positive T-lymphocyte infiltration in the vestibular ganglion and the stroma underlying the sensory

Analysis of synonymous codon usage bias of Lassa virus.

Lassa virus genome consists of two single-stranded, negative-sense RNA segments that lie in the genus Arenavirus. The disease associated with the Lassa virus is distributed all over the world, with approximately 3,000,000-5,000,000 infections diagnosed annually in West Africa. It shows high health risks to the human being. Previous research used the evolutionary time scale and adaptive evolution to describe the Lassa virus population pattern. However, it is still unclear how the Lassa virus takes advantage of synonymous codons. In this study, we analyzed the codon usage bias in 162 Lassa virus

Flunarizine as a Candidate for Drug Repurposing Against Human Pathogenic Mammarenaviruses.

Lassa fever (LF), a viral hemorrhagic fever disease with a case fatality rate that can be over 20% among hospitalized LF patients, is endemic to many West African countries. Currently, no vaccines or therapies are specifically licensed to prevent or treat LF, hence the significance of developing therapeutics against the mammarenavirus Lassa virus (LASV), the causative agent of LF. We used in silico docking approaches to investigate the binding affinities of 2015 existing drugs to LASV proteins known to play critical roles in the formation and activity of the virus ribonucleoprotein complex (vR

Enhancing surveillance for dengue fever in Oyo State, Nigeria - a one health approach.

BACKGROUND: Dengue fever (DF) poses a growing global threat, necessitating a comprehensive one-health approach to address its complex interplay between human, animal, and environmental factors. In Oyo State, Nigeria, the true burden of DF remains unknown due to underdiagnosis and misdiagnosis as malaria, exacerbated by poor health-seeking behavior, weak surveillance systems, and inadequate health infrastructure. Adopting a one-health approach is crucial to understanding the dynamics of DF transmission. METHODS: A cross-sectional study was conducted from January 2022 to April 2023 in 10 high-r
