IntroductionLaparoscopic appendectomy (LA) is the first-line treatment for acute appendicitis. We compared Hem-O-Lok clips (HOL) and endoscopic staplers to evaluate cost savings without compromising outcomes within the San Bernadino service area Kaiser Permanente Health Maintenance Organization.MethodsThis retrospective case-control study included patients ≥6 years who underwent LA (2016-2022) using HOL clips or ES for stump closure. Of 571 patients, 341 used HOL and 230 ES based on surgeon preference. Propensity score matching (1:1) by age, sex, Body Mass Index (BMI), and American Society of

BACKGROUND: Chronic smoking is a key driver of endothelial dysfunction and microcirculatory impairment, contributing to "mixed-pattern vasculopathy," a condition characterized by coexisting macrovascular stenosis and microvascular tone dysregulation. This pathology significantly increases the risk of refractory distal ulceration, dry gangrene, and amputation. This case report aimed to describe the clinical course and therapeutic outcome of integrative treatment with Chinese herbal medicine (CHM) in a patient with refractory ischemia associated with mixed-pattern vasculopathy.

Raynaud's disease (RD), also referred to as Raynaud's phenomenon (RP) or Raynaud's syndrome, is a vasospastic condition characterized by episodic, reversible constriction of the peripheral digital arteries and cutaneous arterioles in response to cold temperature and/or emotional stress. The condition is classified into 2 forms. The first being primary RD, which occurs in the absence of any identifiable underlying condition, and then secondary RP, which develops in the setting of a systemic disease, most commonly connective tissue disorders such as systemic sclerosis, systemic lupus erythematos

BACKGROUND: Computed tomography (CT) plays a vital role in diagnosing suspected gastrointestinal perforation (GIP), particularly in the presence of pneumoperitoneum and intra-abdominal fluid. Nevertheless, these radiologic findings do not invariably correspond to bowel perforation, and negative surgical exploration may occur. We conducted this study to characterize the clinical features and outcomes of patients who underwent emergency surgery for suspected GIP but had no perforation.
METHODS: We retrospectively reviewed consecutive non-trauma patients who underwent emergency surgical explorat

Background/ObjectivesIn treating Peripheral Artery Disease (PAD), the contralateral femoral artery is commonly used for endovascular access. When antegrade access is not feasible, retrograde access via tibial or pedal vessels may be used. This study evaluates the impact of retrograde access on longitudinal outcomes using propensity score matching to emulate a randomized trial.MethodsThis retrospective analysis included patients who underwent endovascular revascularization at two institutions between 2014 and 2022. Retrograde access was defined as access via the anterior tibial, posterior tibia

BACKGROUND: This study aimed to investigate the factors affecting mortality in Fournier's gangrene (FG) and to establish a simplified scoring system that enables practical bedside assessment for clinicians.
METHODS: The medical records of 130 patients treated for FG between February 2012 and January 2025 were retrospectively reviewed. Survivors (Group 1, n=101) were analyzed separately from non-survivors (Group 2, n=29). The collected data included sex, age, infection spread score, Uludag Fournier's Gangrene Severity Index (UFGSI), Fournier's Gangrene Severity Index (FGSI) scores, source of i

BACKGROUND: This study aimed to compare the prognostic performance of the Fournier's Gangrene Severity Index (FGSI) and the hemoglobin-albumin-lymphocyte-platelet (HALP) score in predicting in-hospital mortality among patients diagnosed with Fournier's gangrene. As Fournier's gangrene remains a life-threatening and rapidly progressive soft tissue infection, early risk stratification is critical for improving patient outcomes. Validating practical scoring systems may support timely clinical decision-making and resource allocation.
METHODS: A retrospective analysis was performed on 52 patients

Although laparoscopic banded Roux-en-Y gastric bypass (B-RYGB) has been adopted in bariatric surgery to enhance long-term weight loss outcomes and reduce pouch dilation, cranial slippage (herniation) of the alimentary limb through a non-slipped MiniMizer gastric ring (Bariatric Solutions International GmbH, Stein am Rhein, Switzerland) following B-RYGB, leading to small bowel ischemia, represents a rare but serious complication. We present a case of a 35-year-old male who had undergone multiple bariatric procedures to manage obesity due to failure to lose weight. The patient presented to our e

A 37-year-old man with hypertension and asthma who injects fentanyl daily presented with rapidly progressive discoloration, swelling, and pain of the hand shortly after fentanyl injection into the radial artery and a reported crush injury. Despite preserved proximal arterial flow on computed tomography angiography and catheter-directed angiography, there was markedly sluggish perfusion in the digital arteries that did not respond to intra-arterial vasodilators. The patient was treated with topical nitroglycerin and supportive measures but subsequently developed dry gangrene of multiple digits

INTRODUCTION: Acute calculous cholecystitis is a common surgical condition with variable clinical severity. Early identification of severe disease remains challenging. Inflammatory biomarkers such as neutrophil-to-lymphocyte ratio (NLR) and C-reactive protein (CRP) have been proposed as simple indicators of systemic inflammation and disease severity. The present study aimed to evaluate the association of preoperative NLR and CRP levels with the severity of calculous cholecystitis and their relationship with clinical outcomes.
MATERIALS AND METHODS: This prospective observational study was con

Clostridium septicum, an anaerobic spore-forming Gram-positive bacillus, is a rare opportunistic pathogen that causes severe infections with rapid progression and high mortality. Trauma with unmanaged contaminated wounds is a major risk factor, and atypical presentations such as tetanus-like neuromuscular symptoms often lead to misdiagnosis. Herein, we report a 72-year-old male farmer with C. septicum and Enterobacter cloacae co-infection following a traumatic toe laceration, presenting with trismus, limited mouth opening, and right toe gangrene. This case highlights the diagnostic challenges

A 68-year-old man with poorly controlled type 2 diabetes and untreated stage 2 hypertension presented with a 7-day history of painful scrotal and perianal swelling. Imaging revealed subcutaneous gas and necrotic sinus tracts, and tissue cultures showed a polymicrobial infection. Fournier's gangrene was diagnosed. Prompt surgical debridement, vacuum sealing drainage (VSD), glycemic control, and antibiotic therapy led to complete recovery, highlighting the importance of early recognition and aggressive management.

BACKGROUND: As life expectancy rises and elderly populations grow, acute appendicitis incidence increases, often manifesting with nonspecific symptoms that challenge diagnosis. This study applied machine learning techniques to build a predictive model for gangrenous perforation, examining clinical features and risk factors in elderly patients with acute appendicitis. METHODS: We conducted a retrospective analysis of elderly patients undergoing laparoscopic appendectomy for acute appendicitis at The Second Affiliated Hospital of Kunming Medical University, China, from June 2021 to January 2024

Strangulated inguinal hernia is a common surgical emergency; however, associated testicular gangrene with concomitant bowel ischemia in adults is exceptionally rare. We report the case of a 69-year-old male who presented with systemic instability and an incarcerated right inguinal hernia. Clinical deterioration necessitated urgent exploratory laparotomy, which revealed a strangulated segment of small bowel extending into the inguinal canal. Bowel resection with primary anastomosis was performed. Further exploration demonstrated complete gangrene of the spermatic cord and testis, requiring righ

INTRODUCTION: This case report describes a 50-year-old man with a 12-year history of long-standing warm-antibody autoimmune haemolytic anaemia (AIHA) who developed peripheral digital gangrene following COVID-19 infection. Although COVID-19-associated AIHA and COVID-19-associated digital ischaemia have each been reported separately, this case is notable for their coexistence with both venous and arterial thrombosis, resulting in tissue loss.
MAIN SYMPTOMS AND IMPORTANT CLINICAL FINDINGS: The patient presented with a 3-day history of fever, cough, and shortness of breath. On examination, his te

Meckel's diverticulum (MD) is the most common congenital anomaly of the gastrointestinal tract, though it rarely presents with complications in adulthood. Axial torsion leading to gangrene, especially due to a mesodiverticular band, is an uncommon presentation and may pose a diagnostic challenge. We report a case of a 58-year-old female who presented with acute intestinal obstruction. Diagnostic laparoscopy revealed a gangrenous MD caused by axial rotation around a mesodiverticular band, without involvement of the adjacent ileum. Laparoscopic diverticulectomy was successfully performed. Isolat

OBJECTIVE: The posterior thigh flap is a fasciocutaneous flap predominantly based on perforating branches of the profunda femoris artery and the medial circumflex femoral artery, with additional vascular supply from perforators of the inferior gluteal artery. It can be used for reconstruction of perineal defects.
METHOD: We describe the case of a 40-year-old man with extensive Fournier's gangrene resulting in complete loss of perineal skin coverage, the entire scrotum, and a large portion of penile skin. After radical necrectomies, intensive resuscitation for septic shock, and supportive ther

Gangrenous cholecystitis is a severe and potentially fatal complication of acute cholecystitis, typically resulting from ischemic necrosis of the gallbladder wall secondary to sustained obstruction and inflammation. We present a rare case of methicillin-sensitive Staphylococcus aureus-associated gangrenous cholecystitis with bacteremia in an elderly female without identifiable risk factors for staphylococcal infection. The patient was treated with antibiotics and emergent percutaneous cholecystostomy. This case highlights the need to consider infrequent pathogens, such as S. aureus, in patient
