Endothelial activation in thromboangiitis obliterans: mechanisms and therapeutic horizons.

Thromboangiitis obliterans (TAO) is a non-atherosclerotic, inflammatory vasculopathy characterized by thrombotic occlusion of small- and medium-sized vessels, leading to tissue ischemia and gangrene. Emerging evidence underscores endothelial cell (EC) activation as a central driver of disease progression, mediated by immune dysregulation, oxidative stress (Nrf2/ROS imbalance), impaired nitric oxide signaling (eNOS/iNOS dysregulation), endoplasmic reticulum and mitochondrial dysfunction, and disrupted copper/iron homeostasis. These pathways collectively promote a prothrombotic, proinflammatory

Case Report: Integrated traditional Chinese and Western medicine in the outpatient management of diabetic foot gangrene complicating uremia.

Diabetic foot gangrene (DFG) in uremic patients presents profound management challenges due to immune dysfunction and impaired tissue repair. We report on a 65-year-old male patient with end-stage renal disease undergoing peritoneal dialysis, presenting with progressive right foot gangrene that was unresponsive to conventional treatment. Clinical evaluation indicated extensive gangrene affecting the first and second toes, elevated inflammatory markers (C-reactive protein at 95.57 mg/L and fibrinogen at 8.18 g/L), and cardiovascular compromise (brain natriuretic peptide at 4869 pg/mL). Lower li

Right Hepatic Lobe Hypoplasia and Gangrenous Cholecystitis: Surgical Implications and Case Report.

BACKGROUND Hypoplasia of the right hepatic lobe is a rare congenital anomaly often discovered incidentally during imaging or surgery. This anomaly can be challenging during surgery, leading to intraoperative complications. Patient with right hepatic hypoplasia usually live normally without symptoms, but it can be associated with liver cirrhosis, portal hypertension, and gallstones. While the exact pathophysiological relationship between congenital hepatic hypoplasia and cholecystitis remains unclear, a few contributing factors have been proposed. These include mutations in the hepatocyte nucle

The Neglected Bands: Untethering their Effects.

Elastic bands such as forgotten thread ties on limbs or as accidental hair tied around genitalia, are mostly found in India. These are uncommon presentations, commonly seen in children <6 years old. Due to a variable period of presentation, they lead to various complications such as infection, edema, gangrene, ulceration, amputation, and functional loss. The longer the band is left in situ, the greater is the damage caused by them. Early diagnosis and surgical management are of utmost importance to save the involved extremity/body part. Surgical treatment involves exploration, identification,

A Rare Presentation of Fournier's Gangrene: Necrotizing Infection Traveling Through a Fistula From the Rectum to the Corpus Cavernosum.

A corpus cavernosum filled with air is a rare finding during imaging, and it should raise suspicion for a serious urologic injury or infection. We present the case of a patient with air in their corpus cavernosum seen on emergency computed tomography. He was diagnosed with Fournier's gangrene, secondary to an infectious spread through a fistula from the rectum to the corpus cavernosum. Radiologists and clinicians should consider rare presentations of necrotizing infections when encountering an air-filled corpus cavernosum on imaging.

Continuous low-frequency pelvic nerve stimulation for therapy of intractable gluteal/heel pressure ulcers in persons with spinal cord injury.

STUDY DESIGN: Case report. OBJECTIVES: Pressure ulcers (PUs) affect many spinal cord-injured individuals and can result in serious life-threatening complications including infections and gangrene. Treatment is lengthy and extremely expensive due to recurrent and prolonged hospitalization and multiple surgeries. Consequently, for both medical and economic reasons, intensive efforts in prophylaxis and new effective therapeutic options are mandatory. This report presents the positive effects of pelvic nerve stimulation on the healing of gluteal and heel PUs. SETTING: Hospital in Zürich/CH. MET

A Surgicel™ Surprise: Retained Oxidized Regenerated Cellulose Posing as a Biloma With Retained Gallstones After Subtotal Cholecystectomy.

Surgicel™ is a bioabsorbable local hemostatic agent frequently used to control bleeding during laparoscopic cholecystectomy. Although rare, residual Surgicel™ can cause a local inflammatory response and subsequent calcification. This may complicate the interpretation of imaging studies, leading to misdiagnosis and unnecessary workup. Sixteen months after subtotal cholecystectomy for gangrenous cholecystitis, a 64-year-old man presented to the emergency department with epigastric pain. A CT of the abdomen demonstrated mild inflammation and a localized hyperdensity within the residual gallbladde

Clinical and angiographic characteristics of below-the-knee arterial disease: A case series study from Vietnam.

BACKGROUND: Below-the-knee peripheral arterial disease (BTKPAD) is a major cause of chronic limb-threatening ischemia (CLTI), yet data on its clinical and angiographic presentation in resource-limited settings remain scarce. This study aimed to characterize the clinical staging and angiographic features of patients with BTKPAD at the time of hospital admission. METHODS: A retrospective consecutive case series study was conducted at the University Medical Center Ho Chi Minh City from January 2022 to September 2024. A total of 127 patients diagnosed with BTKPAD were included. A standardized dia

Conversion Rates and Outcomes of Laparoscopic Appendectomy in Complicated Appendicitis: A Retrospective Study.

Background Laparoscopic appendectomy is a minimally invasive technique widely preferred for acute appendicitis due to less postoperative pain, faster recovery, and better cosmesis compared to open surgery. However, in complicated appendicitis, such as perforation, gangrene, abscess, or peritonitis-distorted anatomy, friable tissues, dense adhesions, and contamination can make the procedure technically demanding. These challenges may require conversion to open surgery, potentially increasing operative time, complications, and hospital stay. Objective To evaluate the conversion rate from laparos

Diabetic foot ulcer with overlap syndrome: A case report and review of literature.

BACKGROUND: Diabetic foot ulcer (DFU) is a common and serious complication among individuals with diabetes. However, the co-occurrence of DFU with overlap syndrome, particularly involving anti-synthetase syndrome (ASS) and systemic sclerosis (SSc), is exceptionally rare. CASE SUMMARY: We report a case of overlap syndrome (ASS and SSc) with diabetes manifesting as biped gangrene. The patient, a 60-year-old female, presented with painful ulcers on both heels that had persisted for nine months. She was diagnosed with DFU, which was likely attributable to SSc, suboptimal long-term glycemic contro

Radiomic signatures from postprocedural MRI thalamotomy lesion can predict long-term clinical outcome in patients with tremor after MRgFUS: a pilot study.

OBJECTIVE: Magnetic resonance-guided focused ultrasound (MRgFUS) thalamotomy is an effective treatment for essential tremor (ET) and tremor-dominant Parkinson's disease (PD), yet a substantial proportion of patients experience tremor recurrence over time. Reliable imaging biomarkers to predict long-term outcomes are lacking. The purpose of the study was to evaluate whether radiomic features extracted from 24-h post-treatment MRI can predict clinically relevant tremor recurrence at 12 months after MRgFUS thalamotomy, using a machine learning (ML) approach. MATERIALS AND METHODS: Retrospective,

Transcranial static magnetic field stimulation of the primary motor cortex in essential tremor: a randomized pilot study.

Alterations in the motor cortex may contribute to essential tremor (ET) pathophysiology. Transcranial static magnetic field stimulation (tSMS), a non-invasive and portable technique, reduces corticospinal excitability, but its therapeutic potential for ET remains untested. This randomized, double-blind pilot study (clinicaltrials.gov#: NCT03780426, registered on December 19, 2018) investigated the acute effects of tSMS applied to the primary motor cortex in 27 ET patients (mean age: 66.9 ± 12.1 years). Patients received a 30-min tSMS session on either the left (n = 13) or right (n = 14) hemisp

Simultaneous Carpal Tunnel Syndrome and Guyon's Canal Syndrome Caused by a Lipoma Arising From the Flexor Tenosynovium: A Case Report.

Carpal tunnel syndrome (CTS) is the most common peripheral entrapment neuropathy. While most cases are idiopathic, secondary CTS due to space-occupying lesions such as tumors is relatively uncommon. We report a rare case of simultaneous compression of the median nerve and the dorsal cutaneous branch of the ulnar nerve by a small lipoma arising from the flexor tenosynovium. Magnetic resonance imaging clearly demonstrated the lesion within the carpal tunnel and near Guyon's canal. Surgical excision led to resolution of median nerve symptoms, whereas ulnar sensory disturbance persisted due to inv

Unilateral discoid lupus erythematosus: two atypical cases with distinct presentations.

Discoid lupus erythematosus can present in various atypical forms, posing significant diagnostic challenges as it can mimic other infectious or neoplastic dermatoses. Herein, we detail two cases of strictly unilateral, multifocal discoid lupus erythematosus and compare them with previously reported cases. This report contributes to the limited literature on this unusual presentation, particularly in individuals with skin of color.

Foster Kennedy Syndrome, Euthanasia, and the Purpose of Eponyms.

Robert Foster Kennedy (1884-1952) was an Irish-born neurologist who in 1911 described the association of frontal lobe tumor with unilateral optic atrophy and contralateral papilledema. After immigrating to America he became a respected neurologist who at the height of his career became President of the American Neurological Association. He is less known for his views on eugenics and advocacy of euthanasia for children with birth defects. In 1939 he initially proposed his support for state-sanctioned euthanasia in Collier's magazine, a popular family periodical. Two years later he discussed his
