The aim of this study is to compare the results of one stage surgical treatment of combined multifocal aortoiliac (AIOD) and coronary (CA) atherosclerosis (ATS) with two staged surgical approach and isolated aortoiliac reconstructions and to determine adequate surgical strategy for this combination of ATS. Cryofibrinogenemia is an uncommon cause of intravascular coagulation necrosis of the skin and occurs as a result of vascular occlusion from cryoproteins, which reversibly precipitate in cold temperatures. The disease is associated with various conditions, most commonly neoplastic and thromboembolic diseases, and produces cutaneous manifestations such as purpura, ecchymoses, gangrene, and ulcerations. Twenty six patients with Fourniers gangrene seen over an 8 year period at the University College Hospital Ibadan are presented. Their ages ranged from 34 years to 70 years with a mean of 52.5 years. Possible predisposing conditions were present in 17 (65%) patients. The anterior scrotal skin alone was most commonly involved. They were treated with systemic antibiotics, wound debridement and sitz baths. An atypical lactic-acid producing gram positive rod Y6 strain was studied in this report, which was isolated form clinic sample of a patient with multiple bacterial synergistic gangrene, and could not be identified by routine method. The case concerns the occlusion of alimentary canal caused by the incarceration of small intestine in the ring of internal hernia. The ring was formed by pathologically changed Meckel's diverticulum, which had accreted to the internal surface of the abdominal cavity in the umbilical region. Appendicitis occurred as the secondary result of this pnenomenon. This complication referred to the 9 year old boy. He was in good condition after the operation and the wound also healed right.
