Second-look operations after massive intestinal resections secondary to mesenterovascular occlusion are a frequent practice. In about one half of patients who undergo second-look procedures, no intraabdominal intervention is necessary. Clostridium perfiringens may cause myonecrosis (i.e. gas gangrene), acute food poisoning or necrotic enteritis (e.g. enteritis necroticans or Pig Bel). We describe a case of enteritis necroticans in a 33 year old man with acute myeloid leukaemia. He presented with an acute abdomen, diarrhoea and pancytopaenia and extensive accumulation of gas in the intrahepatic and extrahepatic portal veins. A retrospective analysis of nine consecutive cases of ileosigmoid knotting (ISK) that were seen at Gondar hospital from 1993 to 1995 is presented. Ileosigmoid knotting is a condition in which the ileum and the sigmoid entangle each other to form a knot and become gangrenous. It was found that the clinical features of ISK were the results of combination of symptoms and signs of small and large bowel obstruction. Products containing terbutaline, formoterol, budesonide, and the combination budesonide/formoterol are widely used by patients with asthma and COPD. Use of prostheses in emergency surgery is a controversial issue due to the risk of infection. We report our experience with a series of 31 strangled hernias treated in emergency situations with a polypropylene (n = 29) or a vinyl (n = 2) sheath placed in a pre-peritoneal or retro-muscular position. BACKGROUND: Necrobiosis lipoidica (NL) is a rare skin disease, mostly seen on the legs and often occurring in patients with diabetes mellitus. Obstruction of umbilical hernia in children is uncommon, but seven out of 51 operations for umbilical hernia were done as emergencies for obstruction, in 38 months, in Kuwait. Unusual conditions like Richter type obstruction and gangrenous omentum were noted in two children. Solid faeces is considered to be the precipitating factor for obstruction.

We operated on 138 patients for intestinal obstruction. The different causes are discussed and their pattern of occurrence is compared with that reported from England and other African countries. The 27.6% mortality rate was closely associated with the 28.2% incidence of gangrene. Factors which may lower this high mortality rate are enumerated.

Gangrenous bowel most often results from hernia, adhesions and mesenteric insufficiency. The overall mortality rate for 151 cases was 37%. This figure was 20% for hernia, 23% for adhesions and 74% for mesenteric insufficiency. In the latter category where bowel resection was feasable the mortality rate was 40%. One hundred and fifty-nine patients with malrotation treated in the Children's Hospital Medical Center between 1951 and 1967 are presented and analyzed with regard to the factors contributing to the success or failure of their management. There were 28 deaths, the majority of which occurred in patients who were less than one month old, had multiple congenital anomalies, or had volvulus of the small bowel with gangrene and perforation. Some of the points we have found most helpful in making the diagnosis are reviewed and the disease as it presents in older children is considered briefly.
