Small Bowel Lymphangiomatosis Presenting With Amyloidosis: A Case Report.

Small bowel lymphangiomatosis is a rare benign lymphatic malformation with nonspecific clinical manifestations that may complicate preoperative diagnosis. We present the case of a 57-year-old male with a history of recurrent umbilical hernias who complained of abdominal pain, nausea, vomiting, and an unreducible ventral hernia, all of which suggested a small bowel obstruction. Imaging revealed a presumed strangulated hernia with bowel obstruction and abnormal thickening of various loops of small bowel. Exploratory laparotomy disclosed incarcerated and gangrenous small bowel requiring resection

A spontaneous paracecal herniation: a rare form of an uncommon case.

Paracecal hernias are a rare form of internal hernias. This is a case of a spontaneous paracecal retroperitoneal hernia in an elderly patient. A 73-year-old male, a known patient of adenocarcinoma of the rectum, who had undergone neoadjuvant radiotherapy and chemotherapy a year ago, presented with a 2-day-history of exacerbated colicky peri-umbilical and right-sided non-radiating abdominal pain, with associated progressive abdominal distension, which were aggravated by feeding. A contrasted abdominal computed tomography scan showed features of small bowel obstruction. An exploratory laparotomy

Chances of Mortality Are 3.5-Times Greater in Elderly Patients with Umbilical Hernia Than in Adult Patients: An Analysis of 21,242 Patients.

The goal of this study was to identify risk factors that are associated with mortality in adult and elderly patients who were hospitalized for umbilical hernia. A total of 14,752 adult patients (ages 18−64 years) and 6490 elderly patients (ages 65+), who were admitted emergently for umbilical hernia, were included in this retrospective cohort study. The data were gathered from the National Inpatient Sample (NIS) 2005−2014 database. Predictors of mortality were identified via a multivariable logistic regression, in patients who underwent surgery and those who did not for adult and elderly age g

It's not just an ileus: disparities associated with ileus following ventral hernia repair.

PURPOSE: We sought to identify risk factors associated with postoperative ileus following ventral hernia repair. METHODS: Utilizing the Nationwide Inpatient Sample (NIS) from 2008 to 2012, we identified adult patients that underwent either open or laparoscopic hernia repair for umbilical and ventral hernias with a diagnosis of umbilical/ventral hernia. We excluded cases with diagnosis of obstruction and bowel gangrene that underwent bowel resection, or with missing data. Risk variables of interest were age, sex, race, income status, insurance status, elective admission, comorbidity status (us

Delay in emergency hernia surgery is associated with worse outcomes.

BACKGROUND: Patients requiring emergent surgery for hernia vary widely in presentation and management. The purpose of this study was to determine if the variation in timing of urgent surgery impacts surgical outcomes. METHODS: The national NSQIP database for years 2011-2016 was queried for emergent surgeries for abdominal hernia resulting in obstruction or gangrene by primary post-op diagnosis. Diaphragmatic hernias were excluded. Patients were grouped by surgical timing from admission to day of surgery: same day, next day, and longer delay. Multinomial propensity score weighting was used to

National variation in opioid prescribing after pediatric umbilical hernia repair.

BACKGROUND: Pediatric umbilical hernia repair is a common procedure that requires minimal tissue disruption. We examined variation in opioid prescription fills after repair of uncomplicated umbilical hernias to characterize the types and doses of medication used and persistent postsurgical use. METHODS: Using the Truven Health Analytics MarketScan© Research Database for June 2012-September 2015, we identified pediatric patients undergoing umbilical hernia repair. We excluded patients with obstruction, gangrene, an earlier repair or a concurrent surgical procedure, and those without available

Indications and outcome of childhood preventable bowel resections in a developing country.

BACKGROUND: While many bowel resections in developed countries are due to congenital anomalies, indications for bowel resections in developing countries are mainly from preventable causes. The aim of the following study was to assess the indications for, morbidity and mortality following preventable bowel resection in our centre. PATIENTS AND METHODS: Retrospective analysis of all cases of bowel resection deemed preventable in children from birth to 18 years from June 2005 to June 2012. RESULTS: There were 22 preventable bowel resections with an age range of 7 days to 17 years (median 6 mont

Gangrenous meckel's diverticulum in a strangulated umbilical hernia in a 42 year-old woman: a case report.

INTRODUCTION: Meckel's diverticulum affects 1 - 3% of general population and is known as the most common anomaly of gastrointestinal tract. However, its estimated lifetime complication rate is approximately 4%. Intestinal obstruction is most common complication of Meckel's diverticulum in adult population. CASE PRESENTATION: In the present study, we reported a 42-year-old female patient with a gangrenous Meckel's diverticulum in a strangulated umbilical hernia sac treated by dissection of diverticulomesenteric bands and diverticulectomy. In 36 months follow-up, there was neither any complicat

Lack of Insurance is Associated With Increased Risk for Hernia Complications.

OBJECTIVE: We sought to determine whether lack of insurance is associated with an increased likelihood of presenting to a hospital with a complicated hernia, and whether insurance status might be associated with clinical outcomes. CONTEXT: Delays in elective repair of hernias appear to increase the likelihood of emergency presentation, morbidity, and mortality. Lack of access due to insurance status is a plausible contributor to such delays. METHODS: This retrospective study evaluated ambulatory surgical and inpatient hospitalization data from January 1, 2005 through December 31, 2006 in Cal

Complicated umbilical hernias in children.

Umbilical hernia is a common problem in children, particularly in Africans, but complications in these hernias are thought to be rare. In a retrospective study of 47 children presenting for umbilical hernia repair in 14 years, 30 had complications. The complications included acute incarceration 15, recurrent incarceration 10 and spontaneous evisceration 5. Of the 15 with acute incarceration, 2 required bowel resection for gangrene, and an abscess formed in the hernia sac in one. The age of patients with acute incarceration was 2 months-8 years (median 5 years). The 10 with recurrent incarcerat

A five-year U.S. Army experience with 36,250 abdominal hernia repairs.

Thirty-six thousand two hundred fifty abdominal hernia repairs were performed in U.S. Army medical treatment facilities during a five-year period. This study presents data about the type of hernia, incidence of complications by obstruction or strangulation, age, sex, and mortality. Hernias occurring with intestinal obstruction or gangrene (strangulation) are referred to as complicated hernias. Inguinal hernias in children less than two years of age, femoral hernias, and unusual (such as internal or obturator) hernias were found to have an increased incidence of complications. Surgical repair o

Obstructed umbilical hernias in childhood in Kuwait - Richter - and other types.

Obstruction of umbilical hernia in children is uncommon, but seven out of 51 operations for umbilical hernia were done as emergencies for obstruction, in 38 months, in Kuwait. Unusual conditions like Richter type obstruction and gangrenous omentum were noted in two children. Solid faeces is considered to be the precipitating factor for obstruction.

Pattern of intestinal obstruction in Khartoum.

We operated on 138 patients for intestinal obstruction. The different causes are discussed and their pattern of occurrence is compared with that reported from England and other African countries. The 27.6% mortality rate was closely associated with the 28.2% incidence of gangrene. Factors which may lower this high mortality rate are enumerated.

A current appraisal of problems with gangrenous bowel.

Gangrenous bowel most often results from hernia, adhesions and mesenteric insufficiency. The overall mortality rate for 151 cases was 37%. This figure was 20% for hernia, 23% for adhesions and 74% for mesenteric insufficiency. In the latter category where bowel resection was feasable the mortality rate was 40%. Other causes of bowel gangrene had a mortality rate of 28%. In many instances the pathophysiologic processes were of such a nature that current medical expertise has not reached a level of development to effectively cope with the situation. There were, however, a significant number of c

Malrotation of the bowel in infants and children: a 15 year review.

One hundred and fifty-nine patients with malrotation treated in the Children's Hospital Medical Center between 1951 and 1967 are presented and analyzed with regard to the factors contributing to the success or failure of their management. There were 28 deaths, the majority of which occurred in patients who were less than one month old, had multiple congenital anomalies, or had volvulus of the small bowel with gangrene and perforation. Some of the points we have found most helpful in making the diagnosis are reviewed and the disease as it presents in older children is considered briefly.
