Appendicitis occurring within an umbilical or paraumbilical hernia is an extremely rare clinical entity and poses a significant diagnostic challenge. Because umbilical hernia sacs usually contain omentum or bowel, the presence of the appendix may obscure classical features of appendicitis and mimic a strangulated hernia. We report the case of a 63-year-old woman with a 29-year history of umbilical hernia who presented with acute pain, erythema, and an irreducible umbilical mass. She underwent emergency surgical exploration, which revealed gangrenous omentum and a gangrenous appendix measuring

Colon perforation is a rare but life-threatening emergency that may result from colonoscopy procedures, particularly in elderly patients with comorbidities. It often presents with non-specific abdominal pain that can mimic other acute abdominal conditions, including appendicitis. Early recognition and emergent repair are critical interventions needed to avoid adverse outcomes. We present an 83-year-old man with a medical history of diabetes, chronic kidney disease, hypertension, and prior colonic polyps who presented to the emergency department with severe right lower quadrant pain two days po

BACKGROUND: Umbilical hernia is a common surgical condition that imposes significant economic burdens on healthcare systems. Despite its prevalence, factors influencing hospitalization costs associated with umbilical hernia repairs remain inadequately understood, particularly in China. This study aimed to analyze trends in hospitalization costs and identify key determinants influencing these costs over a 13-year period.
METHODS: We retrospectively analyzed data from 1,578 adult patients who underwent umbilical hernia repair surgery at Beijing Chaoyang Hospital from January 2012 to December 20

Small bowel lymphangiomatosis is a rare benign lymphatic malformation with nonspecific clinical manifestations that may complicate preoperative diagnosis. We present the case of a 57-year-old male with a history of recurrent umbilical hernias who complained of abdominal pain, nausea, vomiting, and an unreducible ventral hernia, all of which suggested a small bowel obstruction. Imaging revealed a presumed strangulated hernia with bowel obstruction and abnormal thickening of various loops of small bowel. Exploratory laparotomy disclosed incarcerated and gangrenous small bowel requiring resection

INTRODUCTION: One of the most prevalent congenital wall abnormalities in children, umbilical hernias are often linked to premature or small-for-gestational-age babies. In cases of intestinal malrotation or if the cecum is very movable, generalized peritonitis may facilitate the imprisonment of these hernias.
CASE REPORT: We described a case of a 4-month-old baby who had a prior reducible umbilical hernia with a history of fever, vomiting, poor appetite, and constipation for around 48 h. The patient experienced significant intestinal bloating, vomiting, irreducibility of the umbilical hernia,

BACKGROUND: Congenital hernia of the umbilical cord (CHUC) is the rarest type of anterior abdominal wall defect, in which an intact umbilical ring is always present and viscera pass through the base of normal-looking umbilicus.
OBJECTIVES: This study was conducted to document the intraoperative findings and postoperative outcomes of patients with congenital hernia of the umbilical cord up to discharge from a tertiary care center.
METHODS: This study was a retrospective observational study conducted for two years (August 2020 to July 2022) in the Department of Pediatric Surgery, at the tertia

INTRODUCTION AND IMPORTANCE: Infantile umbilical hernia is common in children. It has a regressive course in most cases. Conservative management is the standard in most cases before the age of 3 years unless there are complications such as incarceration, rupture with evisceration which are extremely rare and warrants emergency surgery.
CASE PRESENTATION: Our case was a full term 6-month-old male of normal birth weight with history of umbilical hernia but with no obvious risk factors to develop complications. The loops evisceration was spontaneous with a small umbilical skin damage. The poor p

Paracecal hernias are a rare form of internal hernias. This is a case of a spontaneous paracecal retroperitoneal hernia in an elderly patient. A 73-year-old male, a known patient of adenocarcinoma of the rectum, who had undergone neoadjuvant radiotherapy and chemotherapy a year ago, presented with a 2-day-history of exacerbated colicky peri-umbilical and right-sided non-radiating abdominal pain, with associated progressive abdominal distension, which were aggravated by feeding. A contrasted abdominal computed tomography scan showed features of small bowel obstruction. An exploratory laparotomy

INTRODUCTION AND IMPORTANCE: Richter's hernia is a protrusion of a portion of the circumference of anti-mesenteric side of intestine through the fascial defect. It is a rare type of hernia and typically occurs in elderly patients. Richter's hernia could result in grave complications. The objective of the study was to describe a rare case of incarcerated Richter's congenital umbilical hernia in a 20-year-old male patient.

This case describes an 80-year-old obese woman who presented with a giant, incarcerated umbilical hernia. The hernia was present for over 15 years, continuously increasing in size. The only symptom was the pain which lasted about two hours before arrival at the hospital. In an emergency laparotomy, gangrenous colon ascendens, transversum, and lienal flexure have been found. A subtotal colectomy with the creation of terminal ileostomy without hernia repair has been done. The recovery was uneventful, and the patient was discharged on the fifth postoperative day. After one month, the reconstructi

The goal of this study was to identify risk factors that are associated with mortality in adult and elderly patients who were hospitalized for umbilical hernia. A total of 14,752 adult patients (ages 18−64 years) and 6490 elderly patients (ages 65+), who were admitted emergently for umbilical hernia, were included in this retrospective cohort study. The data were gathered from the National Inpatient Sample (NIS) 2005−2014 database. Predictors of mortality were identified via a multivariable logistic regression, in patients who underwent surgery and those who did not for adult and elderly age g

OBJECTIVES: To characterize regional variation in the age of patients undergoing umbilical hernia repair to determine costs and subsequent care.
STUDY DESIGN: We performed a cross-sectional descriptive study using a large convenience sample of US employer-based insurance claims from July 2012 to December 2015. We identified children younger than 18 years of age undergoing uncomplicated (not strangulated, incarcerated, or gangrenous) umbilical hernia repair as an isolated procedure (International Classification of Diseases, Ninth Revision procedure codes 53.41, 53.42, 53.43, or 53.49, Internat

PURPOSE: We sought to identify risk factors associated with postoperative ileus following ventral hernia repair.
METHODS: Utilizing the Nationwide Inpatient Sample (NIS) from 2008 to 2012, we identified adult patients that underwent either open or laparoscopic hernia repair for umbilical and ventral hernias with a diagnosis of umbilical/ventral hernia. We excluded cases with diagnosis of obstruction and bowel gangrene that underwent bowel resection, or with missing data. Risk variables of interest were age, sex, race, income status, insurance status, elective admission, comorbidity status (us

BACKGROUND Hepatorenal syndrome (HRS), which is a type of functional renal impairment, is one of the most serious complications in patients with liver cirrhosis. Terlipressin can induce splanchnic vasoconstriction, which increases the renal blood flow and has beneficial effects on HRS. However, terlipressin administration may cause serious ischemic complications such as skin ischemia, peripheral gangrene, and ischemic bowel necrosis. Here, we report a case of peripheral cyanosis following terlipressin administration in a cirrhotic patient with HRS. CASE REPORT The patient was a 65-year-old mal

A 62-year-old man with no pertinent medical history presented with lower extremity weakness and worsening distal fingertips and toe cyanosis/gangrene. In the outpatient setting, he was initially being treated for Raynaud's phenomenon with a calcium channel blocker. On presentation, the patient had elevated inflammatory markers and white blood cell count. Serum vasculitis panel (proteinase-3 antibody) supported the diagnosis of granulomatosis with polyangiitis. His hospital course was complicated by ischaemic stroke and a diagnosis of mononeuritis multiplex in his lower extremities. After initi

BACKGROUND: Patients requiring emergent surgery for hernia vary widely in presentation and management. The purpose of this study was to determine if the variation in timing of urgent surgery impacts surgical outcomes.
METHODS: The national NSQIP database for years 2011-2016 was queried for emergent surgeries for abdominal hernia resulting in obstruction or gangrene by primary post-op diagnosis. Diaphragmatic hernias were excluded. Patients were grouped by surgical timing from admission to day of surgery: same day, next day, and longer delay. Multinomial propensity score weighting was used to

Intestinal perforation (IP) is a life-threatening gastroenterological condition requiring urgent surgical care, which may present itself as an uncommon complication following incisional hernia repair surgery, most often because of iatrogenic traumatism occurring during the procedure. However, we report a case where a spontaneous onset can be hypothesised. A 60-years-old patient underwent repair of an abdominal laparocele, through rectus abdominis muscle plasty, 5 years after development of an incisional hernia due to exploratory laparotomy for the treatment of acute appendicitis. PURPOSE: Synthetic non-absorbable meshes are routinely used in hernia surgery for clean cases but are avoided in the case of clean-contaminated or contaminated hernia or dirty cases to minimize the risk of infection. The present study was conducted to evaluate the outcome of polypropylene mesh in the settings of bowel resection performed for strangulated hernias of the abdominal wall.
METHODS: This prospective observational study was conducted in the Department of Surgery of a teaching hospital in central India between 1st March 2015 and 31st Aug 2017. Adult patients > 18 years age, with abdo
