The Association Between Risk-Adjusted Wound Healing Rates and Long-Term Outcomes in a Network of U.S. Wound Care Clinics.

OBJECTIVE: Chronic wounds have a high prevalence and poor outcomes in a geriatric population. It is unknown whether higher rates of successful wound healing are associated with better long-term outcomes. We sought to determine the association of risk-adjusted healing rates in wound care clinics with postdischarge outcomes. APPROACH: This observational study compared outcomes of Medicare patients with chronic wounds who received care at clinics with the highest risk-adjusted wound healing rates (n = 91) to those with the lowest rates (n = 88) in a network of 510 specialized U.S. wound care cli

Constipation Faecolith as a Cause for Paediatric Appendicitis - A Retrospective Review from Rural Tertiary Care Centre.

BACKGROUND: Occlusion of the appendiceal lumen by faecolith results in appendicitis and its sequelae, including perforation and gangrenous alterations. A faecolith is believed to be the outcome of constipation. This study assessed the association between constipation faecolith, the occurrence of acute appendicitis and its complications. METHODOLOGY: This study examined the patients in the paediatric surgery department between the ages of 2 and 14 years who were diagnosed with acute appendicitis between January 2022 and January 2024. The study included 79 patients in total. The collected data

Parastomal hernia repair using retromuscular 3D funnel mesh: the "Sugar Funnel" technique.

INTRODUCTION: Parastomal hernias are a common and complex problem for patients and their surgeons. Despite multiple established repair strategies, recurrence rates remain high. We present a novel open retromuscular repair technique using a 3D funnel mesh. METHODS: The technique involves retrorectus dissection, transversus abdominis release (TAR), lateralisation of the stomal conduit, and retromuscular placement of a 3D funnel mesh. A second flat mesh reinforces the remaining retromuscular space. Abdominoplasty was performed where excess skin or fat contributed to the parastomal ‘bulge’. RESULT

Total Pancreatectomy and Islet Autotransplantation: Imaging Findings and Complications.

Total pancreatectomy and islet autotransplantation (TPIAT) is a complex surgical procedure performed in transplant centers throughout the United States, with increasing prevalence over the last 2 decades. The goals of TPIAT are to alleviate pain caused by debilitating chronic pancreatitis or acute recurrent pancreatitis by removing the pancreas and to prevent the development of brittle diabetes by infusion of pancreatic islet cells to replace the function of the surgically absent pancreas. Imaging plays a key role in evaluating patients who have undergone TPIAT. Radiologists must be aware of t

Patch repair versus flap repair for congenital diaphragmatic hernia: A systematic review and meta-analysis.

BACKGROUND: Congenital diaphragmatic hernia (CDH) often requires non-primary repair of large defects using either a prosthetic patch or an autologous muscle flap. However, their comparative effectiveness remains uncertain. We systematically reviewed the existing literature to synthesize outcomes relevant to durability and perioperative safety of patch versus flap repair in neonates. METHODS: PubMed, Embase, and Scopus were systematically searched from inception to May 2025 for pediatric studies directly comparing patch versus flap repair for CDH in neonates. Meta-analyses were performed using

Incisional Hernia after Pancreaticoduodenectomy: A Systematic Review and Meta-Analysis of Incidence and Risk Factors.

BACKGROUND: Pancreaticoduodenectomy (PD) is a high-risk surgical procedure associated with significant postoperative morbidity. While complications such as pancreatic fistula and delayed gastric emptying are well-studied, the incidence and risk factors of incisional hernia (IH) following PD remain poorly characterized. This systematic review and meta-analysis aimed to evaluate the incidence of IH and identify associated risk factors in patients undergoing PD. METHODS: We conducted a systematic search of PubMed, Embase, CENTRAL, and Web of Science databases from inception to December 2024, fol

Short-term outcomes and risk factors for complications in robotic versus open parastomal hernia repair: a retrospective propensity score-matched analysis.

INTRODUCTION: Parastomal hernias (PSHs) significantly impact the quality of life and pose risks including obstruction and stoma dysfunction. Minimally invasive approaches, particularly robotic surgery, are increasingly employed for elective PSH repairs, yet comparative outcomes remain insufficiently studied. This study evaluated short-term outcomes comparing robotic and open PSH repairs. METHODS: A retrospective cohort analysis was conducted using the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database. Patients undergoing elective open, laparoscopi

Laparoscopic Sugarbaker Repair of Parastomal Hernia with Gastric Incarceration: A Case Report and Review of the Literature.

INTRODUCTION: Parastomal hernia is a common complication of stoma creation; however, gastric involvement is extremely rare, with only approximately 2 dozen cases reported. Gastric incarceration in a parastomal hernia can cause severe complications, including gastric outlet obstruction and ischemia, and requires timely surgical management. CASE PRESENTATION: We describe the case of a 57-year-old obese female who underwent transverse colostomy for ischemic colitis and presented with upper abdominal pain and vomiting. She had a history of Buerger's disease, bilateral lower limb amputation, centr

Parastomal hernia prevention and repair in Australasia: A binational CSSANZ survey of contemporary practice.

BACKGROUND: Parastomal hernia (PSH) is a debilitating long-term complication of stoma formation, often required as part of curative or palliative treatment for colorectal cancer. As a common downstream consequence of cancer surgery, PSH contributes significantly to chronic morbidity and impairs quality of life, yet practices surrounding its prevention and repair remain heterogeneous. Despite randomised evidence supporting prophylactic mesh, adoption is limited, and operative approaches to elective and emergency repair vary widely. METHODS: A cross-sectional survey of colorectal surgeons in Au

Preliminary Results of Extraperitoneal Rectus Abdominis Anterior Sigmoid Colostomy (ERASC) for Prevention of Parastomal Hernia Following Abdominoperineal Resection.

INTRODUCTION: Among patients with rectal cancer undergoing abdominoperineal resection (APR), parastomal hernia (PSH) is the most common long-term complication. Although extraperitoneal colostomy (EPC) has been proposed as an effective surgical method to prevent PSH, its efficacy remains controversial. To effectively reduce the incidence of PSH after APR, we report a new surgical technique-extraperitoneal rectus abdominis anterior sigmoid colostomy (ERASC). OBJECTIVE: To preliminarily explore the feasibility of extraperitoneal rectus abdominis anterior sigmoid colostomy (ERASC) and its effecti

Nutritional risk versus established malnutrition in older adults: metabolic considerations for the association with depression.

UNLABELLED: In human cells TORC1 mTOR (target of rapamycin) protein kinase complex renders heat shock transcription factor 1 (Hsf1) competent for stress activation. In such cells, as well as in yeast, the selective TORC1 inhibitor rapamycin blocks this activation in contrast to Hsp90 inhibitors which potently activate Hsf1. Potentially therefore rapamycin could prevent the Hsf1 activation that frequently compromises the efficiency of Hsp90 inhibitor cancer drugs. Little synergy was found between the effects of rapamycin and the Hsp90 inhibitor radicicol on yeast growth. However certain rapamyc

Robot-Assisted Sigmoid Colon Conduit for Urinary Diversion: Avoiding Bowel Anasto-mosis in a Patient with Prior Loop Colostomy and Pelvic Radiation.

PURPOSE: Late radiation-induced genitourinary and gastrointestinal complications after multimodal treatment for pelvic malignancies frequently require complex reconstructive surgery, often necessitating both urinary and fecal diversion (1). In this setting, a colonic segment may provide an effective option for urinary diversion while avoiding bowel anastomosis (2, 3). Minimally invasive robotic approaches have increasingly expanded the feasibility of these complex reconstructions and may reduce morbidity, particularly in frail or previously irradiated patients (4-7). We describe a single-stage

Laparoscopic posterior component separation (transabdominal retromuscular + transversus abdominis release) for obstructed incisional hernia with ventriculoperitoneal shunt entrapment.

Ventriculoperitoneal (VP) shunt placement is the most common treatment for hydrocephalus. Abdominal complications occur in up to a third of the patients, but incisional hernia at the site of catheter entry and presenting as acute intestinal obstruction is a rare occurrence. A 47-year-old female presented with an acute abdomen and on examination was found to have an irreducible L2W1 incisional hernia. Radiology revealed herniation of a loop of small bowel along with the intraperitoneal part of the VP shunt. Emergency diagnostic laparoscopy confirmed the findings. The bowel loop was reduced; the

Ventral hernia repair in emergency settings. A machine learning model to predict post-operative complications.

BACKGROUND: Emergency ventral hernia repair remains a challenging procedure due to patient instability, contaminated surgical fields, and heterogeneity in hernia types and operative techniques. Predicting postoperative complications in this setting is difficult using traditional statistical methods. Machine learning (ML) may offer improved predictive accuracy by recognizing nonlinear patterns among multiple perioperative factors. METHODS: A retrospective multicenter analysis was performed using data from the ACTIVE (Acute Treatment for Incisional Ventral Hernias) study, including 557 adult pa

Comparison of short-term outcomes between open and minimally invasive surgery for emergent parastomal hernia repair: A retrospective propensity score-matched analysis.

BACKGROUND: Parastomal hernia (PSH) is a common complication after stoma creation and may present emergently with obstruction, incarceration, or ischemia. While minimally invasive surgery (MIS, laparoscopic or robotic) shows benefits in elective PSH repair, data in emergencies are limited. This study compared outcomes following open versus MIS emergent PSH repair. METHODS: In this retrospective cohort using ACS-NSQIP, patients undergoing emergent open, laparoscopic, or robotic PSH repair were identified. Surgical approach was categorized as open or MIS. Conversion to laparotomy due to intra-abdominal adhesions during laparoscopic hernia repair: a predictive model based on computed tomography manifestations and clinical indicators.

BACKGROUND: The preoperative assessment of adhesions between the intestine and the abdominal wall in patients with ventral incisional hernias allows surgeons to assess the feasibility of laparoscopic hernia repair. This study aimed to investigate the predictive value of computed tomography (CT) images combined with clinical indicators for conversion to laparotomy due to adhesions in laparoscopic incisional hernia repair, and to develop a nomogram prediction model. METHODS: This study analyzed data of 245 patients with ventral incisional hernias retrospectively. The patients were divided into

Delayed posterior rectus sheath rupture presenting as intestinal obstruction: A case report and review.

Extended-view totally extraperitoneal repair (eTEP) is one of the recently introduced, advanced minimal access approaches for the repair of ventral hernias. Posterior sheath rupture (PRSR), though rare, is a lethal complication of eTEP, exposing the polypropylene mesh to the viscera. Few cases of PRSR have been reported following eTEP ventral hernia; however, delayed presentations as acute small bowel obstruction, has not been reported so far. We present the case of acute intestinal obstruction following eTEP ventral hernia repair after 10 months following the primary surgery. A 67-year-old fe
