Successful pharmacologic treatment of lower extremity ulcerations in 5 patients with chronic critical limb ischemia.

BACKGROUND: Ischemic ulcerations of the distal lower extremities are a manifestation of chronic critical limb ischemia. Without restoration of arterial flow, subsequent gangrene and limb loss can ensue. Unfortunately, revascularization is not always possible. METHODS: A literature search of MEDLINE was performed and a case series of 5 patients with lower extremity ischemic ulcerations is described. RESULTS AND CONCLUSION: Five patients with severe peripheral artery disease had nonhealing lower extremity ischemic ulcerations. Because 3 patients were not ideal candidates for percutaneous or su

The management of cecal volvulus.

Cecal volvulus is second only to sigmoid volvulus in its frequency of occurrence. Diagnostic doubt is not uncommon in cecal volvulus; nonoperative decompression is rarely achievable; and if gangrene supervenes, mortality rises appreciably. Resection is mandatory for gangrene and a grossly distended, thin-walled cecum. Cecopexy and cecostomy seem less-effective and more morbid options than resection and anastomosis for viable bowel. However, their role needs reappraisal in the light of advances in minimally invasive techniques.

Complications of gallstone disease: Mirizzi syndrome, cholecystocholedochal fistula, and gallstone ileus.

Gallstone is a common disease with a 10% prevalence in the United States and Western Europe. However, it is only symptomatic in 20-30% of patients, with biliary pain "colic" being the most common symptom. Complications of asymptomatic gallstone disease are generally rare, with an incidence of <1 %/yr. The most common complications of gallstone disease are acute cholecystitis, acute pancreatitis, ascending cholangitis, and gangrenous gallbladder. Less frequent complications include Mirizzi syndrome, cholecystocholedochal fistula, and gallstone ileus. Mirizzi syndrome and cholecystocholedochal f

Acute hemorrhagic pellagra in an Albanian refugee.

We report a peculiar case of hemorrhagic pellagra in an exhausted Albanian refugee who had walked for 3 days under sunny skies on his way from his country to Greece. The peculiarities of the case are the fulminant course of the disorder; the "terrifying" appearance of the patient (initially he was admitted to an emergency unit); the gangrenous appearance of the hemorrhagic lesions of the palms and fingernails; the disturbed hepatic function that gradually returned to normal; and the absence of a history of alcohol consumption, alcohol malabsorption, or drug intake.

Laparoscopic versus open surgery for suspected appendicitis.

BACKGROUND: Laparoscopic surgery has been proposed to have diagnostic and therapeutic advantages over conventional surgery. OBJECTIVES: To compare the diagnostic and therapeutic effects of laparoscopic and conventional 'open' surgery in the treatment of suspected acute appendicitis. SEARCH STRATEGY: We searched for original articles and abstracts published until end of 2000. As main search tools we employed the Cochrane Controlled Trials Register (CCTR), MEDLINE, EMBASE and SciSearch. CCTR and MEDLINE searches were repeated until 10 October 2001, all other databases were searched 10 October

Arsenic poisoning in groundwater: health risk and geochemical sources in Bangladesh.

Of the 2508 water samples analyzed in 10 districts of Bangladesh, 51%, on an average, contained arsenic levels of 0.05 to 2.50 mg/l. 95% of nail, 96% of hair, and 94% of urine samples contained arsenic above the normal level. Approximately 3.58 million people out of a total of 17.92 million who are drinking water containing arsenic levels >0.20 mg/l are potentially exposed to high risk of health hazard. Eight thousand and five hundred arsenic patients are identified; they are suffering from various skin lesions, gangrene in leg, skin, lung, bladder, liver, and renal cancer. A big portion of th

Diabetes-related morbidity and mortality in a national sample of U.S. elders.

OBJECTIVE: Although the number of elders with diabetes has increased dramatically, there are few data on rates of mortality and serious complications in older populations with diabetes. To determine such rates, we conducted a population-based, nonconcurrent cohort study using claims data from the 1994-1996 Medicare 5% Standard Analytical File. RESEARCH DESIGN AND METHODS: Codes from the ICD-9 were used to identify diabetes and the following complications: amputation, lower extremity infection, gangrene, blindness, acute myocardial infarction, ischemic heart disease, stroke, and metabolic diso

Free-tissue transfer in patients with peripheral vascular disease: a 10-year experience.

Advances in free-tissue transfer have allowed for lower limb salvage in patients with significant peripheral vascular disease and limb-threatening soft-tissue wounds. The authors retrospectively reviewed their 10-year experience with free flaps for limb salvage in patients with peripheral vascular disease to assess postoperative complication rates and long-term functional outcome. They identified all patients undergoing free-tissue transfer with significant peripheral vascular disease and otherwise unreconstructible soft-tissue defects. Charts were reviewed for perioperative and long-term outc

Managing foot ulcers in patients with diabetes.

Over 1 million people in the UK have diabetes mellitus, of whom around 7-10% will, at some point, develop a foot ulcer. These ulcers, which in severe cases extend to muscle and bone, are prone to infection and can lead to gangrene and may necessitate amputation. Around 85% of amputations in people with diabetes are preceded by persistent foot ulcers. Here, we review the management of such ulcers.

Pedal gangrene secondary to disseminated intravascular coagulation with gastric carcinoma.

Consumptive coagulopathy resulting in a disseminated intravascular coagulation is most often seen in infectious diseases and hematologic malignancies. Solid tumors may be associated with disseminated intravascular coagulation that results in gangrene of the upper extremity. A case report of lower-extremity gangrene as the pathology for gastric carcinoma is presented. The need for a multidisciplinary approach to this clinical presentation is noted.

Fibrin sealant for the reconstruction of fournier's gangrene sequelae.

PURPOSE: We describe the use of fibrin tissue adhesive as an adjunct for reconstructing genital skin loss due to Fournier's gangrene. MATERIALS AND METHODS: We treated 2 patients with Fournier's gangrene with repeat surgical débridement and antibiotics. Delayed primary closure was enhanced by using liquid fibrin sealant. In 1 case the sealant was used to obliterate a large testicular thigh pouch that had become infected. In the other case it was used to anchor the under surface of a thigh flap for scrotal reconstruction. RESULTS: In each patient the fibrin tissue adhesive prevented further c

[Fournier's gangrene: early diagnosis is the best remedy].

Despite the broadening knowledge about Fournier's gangrene, its mortality rate is still considered very high. The authors describe the etiology, pathomechanism, the possible pathways of progression, and the causes and predisposing factors of the disease. Besides the traditional--surgical and antibiotic--therapy, complementary ways of treatment are described. Between 1994 and 2000, ten patients were treated for Fournier's gangrene in our department. We present our experience, the early signs and the way clinical symptoms develop. It is important to observe the early severe general condition and

The relative roles of aggressive wound care versus revascularization in salvage of the threatened lower extremity in the renal failure diabetic patient.

Current literature indicates poor survival and limb salvage rates in renal failure diabetic patients who present with ulcerated or gangrenous lower extremities. Even in those limbs that were successfully revascularized, the amputation rate was as high as 37 percent. This has led some to advocate immediate amputation when treating the threatened limb of a renal failure diabetic patient. The authors reviewed all renal failure diabetic patients in their wound registry to determine whether such pessimism was warranted. The authors then analyzed the relative roles of revascularization and aggressiv

Ischaemic colitis.

The term colonic ischaemia was first coined approximately 40 years ago. Up until that point, the disease was only diagnosed in cases where the colon was completely gangrenous. In the 1960s reversible non-transmural ischaemia of the colon was described and much has been written about the disease since. Ischaemic colitis is usually a disease process that is seen in the elderly. However, the true incidence of the disease remains unknown since many cases are probably never diagnosed. We now know that the disease can occur both with occlusive and non-occlusive vascular disease, the latter being muc

The causes and management of foot ulceration.

Managing foot ulceration is a challenge for all concerned. This article argues that the key to successful management of ulceration lies with the accurate identification of the underlying cause of the ulcer and consideration of the additional factors that are likely to impair healing. The common causes of foot ulceration are discussed, the means of evaluation and assessment are outlined and appropriate management strategies are identified. As with all care pathways, patient concordance is essential in reaching successful clinical outcomes and foot ulceration is no exception.

Laparoscopic appendectomy in children: A favorable alternative in simple and complicated appendicitis.

BACKGROUND/PURPOSE: The laparoscopic treatment of pediatric appendicitis remains controversial, particularly in complicated cases (gangrene and perforation). This study evaluates outcomes of open (OA) and laparoscopic appendectomy (LA). METHODS: The 391 cases of pediatric appendectomy performed between January 1998 and January 2001 were reviewed for age, sex, weight, type and length of intervention, operative description, antimicrobial therapy, analgesia, complications, length of hospitalization, and histopathology. RESULTS: A total of 126 patients were operated on by laparoscopy, 262 by lap

Peripheral vascular diseases resulting from chronic arsenical poisoning.

Drinking water contaminated by arsenic remains a major public health problem. Long-term arsenic exposure has been found to be associated with peripheral vascular diseases in a variety of studies. Reports of vascular effects of arsenic in drinking water, which span almost 100 years, have been published in Taiwan, Chile, Mexico, and China. This paper reviewed the association of peripheral vascular diseases resulting from arsenic exposure to drinking water from the clinical and pathological points of view. An endemic peripheral vascular disorder called "blackfoot disease" has been noticed in a li
