BACKGROUND: The optimal preoperative drainage strategy for ureteropelvic junction obstruction remains controversial. This study compares the outcomes between different preoperative urinary drainage methods in robot-assisted pyeloplasty (RAPY).
METHODS: This retrospective study was conducted on 45 patients who underwent RAPY from 2020 to 2024 and categorised by clinical presentation: flank pain with hydronephrosis requiring preoperative urinary drainage double-J ureter catheter (DJ) or percutaneous nephrostomy (PCN) or renal function deterioration (no-drain) scheduled for pyeloplasty. The pati

INTRODUCTION: Children with congenital anomalies often require surgical intervention. Long-term follow-up guidelines are heterogeneous. Our aims were to 1) determine the time to latest complication after index surgery to tailor our follow-up program, and 2) query family attitudes toward long-term follow-up by our clinic. These efforts are intended to plan future quality improvement work at our institution.
METHODS: This is a single center retrospective review of patients treated between January 2015- January 2026 with congenital diaphragmatic hernia, congenital pulmonary airway malformation,

Small bowel herniation through a Caesarean section wound is exceedingly rare and may present with erythema and induration clinically indistinguishable from superficial wound infection, risking dangerous diagnostic delay. A case is reported here of a 39-year-old woman with one prior Caesarean section and a body mass index of 30 kg/m2 who underwent elective lower segment Caesarean section at 39 weeks of gestation, complicated by intraoperative adhesiolysis and decompression of a uterovesical fatty cyst. On postoperative day 3, the patient re-presented with right iliac fossa pain, vomiting, and e

Cryoglobulinemia (CG) is defined by the presence of serum immunoglobulins that precipitate below 37°C and redissolve upon rewarming. It is classified into three types based on immunoglobulin composition. Type I, a rare form, involves monoclonal IgM or IgG and is linked to lymphoproliferative disorders. Types II and III are known as mixed CG (MC): Type II consists of polyclonal IgG and monoclonal IgM with rheumatoid factor (RF) activity, whereas Type III includes polyclonal IgG and polyclonal IgM with RF activity. MC is predominantly associated with hepatitis C virus (HCV) infection and involve

IMPORTANCE: Acute digital ischemia, digital ulcers, and gangrene are debilitating complications of systemic sclerosis and other vasculopathies and are often refractory to standard vasodilator and immunosuppressive therapies. Botulinum toxin (BTX) has emerged as a potential rescue therapy, but its clinical effectiveness and safety remain unclear.
OBJECTIVE: To evaluate the effectiveness and safety of BTX injections for ischemic digital complications and identify predictors of treatment response using individual participant data (IPD).

Trauma and diseases such as gangrene, diabetes mellitus, leprosy, or advanced-stage cancer requiring resections may lead to digit loss due to the limited capacity of tissue regeneration. The increasing global incidence of phalanx fractures necessitates surgical intervention for restoring organ function. Early mobilization post-surgery significantly improves the range of motion and overall functional outcomes, emphasizing the need for mechanically stable and biologically responsive solutions. In this study, a CT-derived, site-specific "personalized" phalanx reconstruction was fabricated using b

Granulomatosis with polyangiitis (GPA) is a small-to-medium vessel vasculitis usually presenting with upper airway, pulmonary, and renal involvement. Critical limb ischaemia (CLI) and arterial thrombosis are rare but severe complications of GPA, often resulting in poor prognosis and limb loss. We describe a 34-year-old woman presenting with right upper limb CLI on a background of GPA, manifesting as fever, purpura with upper airway, pulmonary, and renal involvement. Investigations confirmed proteinase-3 anti-neutrophil cytoplasmic antibody positivity, elevated inflammatory markers, proteinuria

Fibroids are the most common pelvic tumors in women; serious complications are very rare but can be life threatening also. This case report highlights the atypical presentation of spontaneous hemorrhage and gangrene from a uterine myoma that clinicians should consider as a differentials diagnosis of haemoperitonium. It adds to the existing literature in discussing the investigations and management of these rare fibroid complications. We present a case of an atypical presentation of spontaneous hemorrhage from a uterine fibroid with gangrene with no precipitating factors. A 36-year-old lady who

Necrotizing fasciitis (NF) is known to be a potentially life-threatening bacterial infection of the deep fascias showing a tendency to rapid spread. For certain localizations (e.g., the scrotum or labia) of this disease, specific entities have been defined (e.g., Fournier's gangrene). Cellulitis (or phlegmon) represents the most important differential diagnosis of NF. Moreover, septic soft tissue infections with fascial involvement together with eosinophilic and paraneoplastic fasciitis can be diagnostically challenging. The large group of autoimmunologically mediated inflammatory systemic dis

The study aimed to evaluate the incidence and impact of small artery disease (SAD) in persons with ischaemic diabetic foot ulcers (DFUs). The current study is a retrospective research including patients with ischaemic DFUs treated by lower limb revascularization and managed in a specialized diabetic foot service between January 2021 and September 2023. Based on final angiograms, subjects were divided in 2 groups, those with SAD and those without. SAD was defined by the absence or poverty of the plantar arch and the arteries originating from it including tarsal, metatarsal, and digital arteries

A man in his 70s with Parkinson's disease underwent urgent below-elbow amputation for gangrenous sepsis. A right supraclavicular brachial plexus catheter was placed for postoperative analgesia. Shortly after tracheal extubation, he developed acute hypercapnic respiratory failure due to ipsilateral hemidiaphragmatic paresis, requiring urgent tracheal re-intubation and a brief period of mechanical ventilation. Following cessation of the local anaesthetic infusion, diaphragmatic function returned and he was successfully re-extubated. A reduced-dose supraclavicular infusion was later reintroduced

Chronic limb-threatening ischemia (CLTI) compounded by diabetic foot ulceration remains a leading precursor to major lower-extremity amputation. Transverse tibial transport (TTT) can improve perfusion but requires corticotomy and bulky external fixation. We report what is, to our knowledge, the first application of fully automated tibial periosteal distraction (PD) for limb preservation. A 65-year-old man with recurrent forefoot gangrene underwent tibial PD using a miniaturised, programmable motor to gradually elevate the periosteum. Through a 1 cm incision, a low-profile subperiosteal plate w

BACKGROUND: Sodium-glucose co-transporter-2 (SGLT2) inhibitors are widely prescribed to diabetic patients for their cardiorenal benefits. However, concerns over adverse events—specifically amputation risk and gangrene—have risen in recent years. This study aimed to describe the frequency and severity of these events in relation to SGLT2 inhibitor use, using the U.S. FDA Adverse Event Reporting System (FAERS). METHODS: OpenVigil 2.1 was used to extract safety reports submitted to FAERS from 2003 Q4 to 2024 Q4. Amputation and gangrene events linked to SGLT2 inhibitors (canagliflozin, dapaglifloz

Background: Patients with end-stage renal failure (ESRF) are prone to vascular complications that may result in limb ischaemia and gangrene. While lower limb amputation (LLA) outcomes in this population are well-documented, data on upper limb involvement remain limited. This study aimed to characterise the types of hand conditions leading to hand specialist referrals amongst ESRF patients, identify risk factors associated with upper limb amputation (ULA) and assess post-amputation survival outcomes. Methods: ESRF patients referred to a single hand surgery unit were recruited over 5 years. Data

Peripheral vascular disease (PVD) is a rare but severe symptom of CREST syndrome, which itself is a limited cutaneous sclerosis. Even though Raynaud's phenomenon is the gold standard of CREST, the development of critical limb ischemia and self-amputation is rare. We report a case of a 48-year-old female with CREST syndrome who presented with progressive ischemia in both upper and lower limbs, leading to spontaneous auto-amputation of multiple fingers/toes. The patient exhibited sclerodactyly, dry gangrene, delayed capillary refill, and tactile deficits in peripheral pulses. ANA and anti-Scl 70

Hallux rigidus is commonly treated with first metatarsophalangeal (MTP) arthrodesis, a procedure with high success and low complication rates. We present a rare case of postoperative hallux gangrene following first MTP arthrodesis in a 44-year-old woman with celiac disease and no conventional vascular risk factors. Despite normal dorsalis pedis and posterior tibial pulses, the great toe developed ischemic changes on postoperative Day 1, progressing to dry gangrene requiring amputation at the MTP joint. Angiography revealed no proximal obstruction, supporting a microvascular etiology. Potential

BACKGROUND: Prior studies suggest an association between complicated appendicitis and underlying appendiceal neoplasms, but the reported incidence varies widely. This study aimed to determine the incidence and predictors of appendiceal neoplasms among patients with radiographically diagnosed complicated appendicitis.
METHODS: This study analyzed adult patients with radiologic diagnoses of complicated appendicitis (perforation, abscess, phlegmon, gangrene, or mucocele) at a tertiary center (2010-2024). To ensure diagnostic accuracy, an expert surgeon reviewed radiologic images with uncertain f

Congenital gangrene in newborns is a rare condition characterised by tissue gangrene present at birth. It may result from intrauterine vascular compromise, birth trauma or clotting disorders. Early diagnosis and prompt management, including surgical intervention and supportive care, are critical to prevent severe complications and improve survival outcomes. We are reporting a case of congenital gangrene of the gluteal region in the newborn.
