Neglected Incisional Hernia Becomes a Life-Threatening Emergency: A Case Report.

Although complications are common following abdominal surgeries, the nontraumatic spontaneous rupture and obstruction with skin gangrene of an incisional hernia leading to bowel evisceration is unusual. This report describes the case of a 40-year-old female with a long-standing incisional hernia that presented as a spontaneous evisceration of an obstructed bowel with evidence of skin rupture and gangrene. She was managed with an emergency laparotomy with myofascial anterior component separation and onlay mesh repair.

The advantages and intraoperative management of the laparoscopic approach in perforated appendicitis.

INTRODUCTION: Gangrene/perforated appendicitis entails high rates of morbidity and mortality. The purpose of this study is to discuss the reliability, surgical management and post-operative complications of laparoscopic interventions in the treatment of perforated appendicitis. PATIENTS AND METHODS: One hundred and ten patients (50 men and 60 women) diagnosed with perforated appendicitis based on history, physical examinations, laboratory and computed tomography findings between January 2016 and May 2022 were analysed retrospectively. Preoperative laboratory and radiological findings, surgica

Acute appendicitis within an incisional hernia: a type of Amyand's hernia.

A man in his 60s presented to the emergency department with generalised abdominal pain and distention associated with the inability to pass stool or gas. The patient had undergone a laparoscopic partial right nephrectomy due to renal cell carcinoma a year ago. The workup diagnosis suggested an incarcerated incisional hernia. Surgical treatment was proposed. Intraoperatively, we found an ileocecal appendix with a macroscopically gangrenous appearance inside the hernia sac. An appendectomy was performed, and the hernia defect was closed using a tension repair. Amyand's hernia, a rare condition,

Spontaneous Evisceration of Small Bowel in Incisional Hernia.

Spontaneous rupture of an incisional hernia leading to the evisceration of the intra-abdominal organs is one of the malefic complications seen in these patients. In addition to its rarity, it gets accompanied by possible lethality in the form of incarceration, sequential strangulation, necrosis, and eventual gangrene. If not treated aptly, the clinical scenario could lead to a life-threatening condition with a delay in timely intervention. With less than 20 documented cases, herein, we report a 48-year-old female with a previous history of a midline laparotomy who presented to us with an acute

An incisional hernia containing a gangrenous gallbladder: a case report and review of the literature.

We present a 76-year-old male who presented to the emergency department with 24 hours of sudden onset, severe abdominal pain. Physical exam and laboratory analysis indicated acute cholecystitis, and a CT scan demonstrated a ventral hernia containing an inflamed gallbladder. This patient was managed operatively with an open cholecystectomy. The ventral hernia was not repaired at the index operation in the setting of frank gallbladder necrosis. The patient recovered well after a short post-operative stay. This report is intended to illustrate an unusual presentation of acute, gangrenous cholecys

Acute appendicitis in supraumbilical hernia.

Clinical presentation of acute appendicitis, the most common cause of acute abdomen, is influenced by its location. The usual clinical signs of appendicitis may be completely changed or even absent in cases of atypical appendicular positions. Incisional hernia is the most common late complication of intra-abdominal operations. Organs most likely to be localized in the hernia sac are the omentum and small and large intestines. Occasionally other organs may also herniate, for example, the appendix. Incarcerations of these organs can lead to infarsation or even to gangrene. We present the case of

Emergency laparoscopic and open repair of incarcerated ventral hernias: a multi-institutional comparative analysis with coarsened exact matching.

BACKGROUND: The safety of emergent laparoscopic repair of incarcerated ventral hernias is not well established. The objective of this study was to determine if emergent laparoscopic repair of incarcerated ventral hernias is comparable to open repair with respect to short-term clinical outcomes. METHODS: Patients undergoing emergency repair of an incarcerated ventral hernia with associated obstruction and/or gangrene were identified using the ACS-NSQIP 2012-2016 dataset. One-to-one coarsened exact matching (CEM) was conducted between patients undergoing laparoscopic and open repair. Matched co

Intestinal perforation after surgical treatment for incisional hernia: iatrogenic or idiopathic?

Intestinal perforation (IP) is a life-threatening gastroenterological condition requiring urgent surgical care, which may present itself as an uncommon complication following incisional hernia repair surgery, most often because of iatrogenic traumatism occurring during the procedure. However, we report a case where a spontaneous onset can be hypothesised. A 60-years-old patient underwent repair of an abdominal laparocele, through rectus abdominis muscle plasty, 5 years after development of an incisional hernia due to exploratory laparotomy for the treatment of acute appendicitis. Use of polypropylene mesh in contaminated and dirty strangulated hernias: short-term results.

PURPOSE: Synthetic non-absorbable meshes are routinely used in hernia surgery for clean cases but are avoided in the case of clean-contaminated or contaminated hernia or dirty cases to minimize the risk of infection. The present study was conducted to evaluate the outcome of polypropylene mesh in the settings of bowel resection performed for strangulated hernias of the abdominal wall. METHODS: This prospective observational study was conducted in the Department of Surgery of a teaching hospital in central India between 1st March 2015 and 31st Aug 2017. Adult patients > 18 years age, with abdo

Outcome of stoma closure in babies with necrotising enterocolitis: early vs late closure.

UNLABELLED: Newborns undergoing surgery for necrotizing enterocolitis (NEC) often require a stoma. Currently, there is no consensus regarding the best time for stoma closure (SC). Our aim was to determine the outcomes of early versus late closure. METHODS: Retrospective analysis of patients who underwent SC following stoma formation for NEC between Jan 2009 and July 2015 was done. Early (EC) versus late closure (LC) was defined as less than 10 weeks versus at or after 10 weeks of stoma formation. RESULTS: Of 36 patients, M:F was 23:13. Indications for laparotomy were pneumoperitoneum (30) an

A validated, risk assessment tool for predicting readmission after open ventral hernia repair.

BACKGROUND/PURPOSE: To present a validated model that reliably predicts unplanned readmission after open ventral hernia repair (open-VHR). STUDY DESIGN: A total of 17,789 open-VHR patients were identified using the 2011-2012 ACS-NSQIP databases. This cohort was subdivided into 70 and 30% random testing and validation samples, respectively. Thirty-day unplanned readmission was defined as unexpected readmission for a postoperative occurrence related to the open-VHR procedure. Independent predictors of 30-day unplanned readmission were identified using multivariable logistic regression on the te

Robotic single-site cholecystectomy in the obese: outcomes from a single institution.

BACKGROUND: Robotic single-site cholecystectomy (RSSC) has been shown to be a safe alternative to the laparoscopic approach in selected patients. Patient exclusion criteria have prevented RSSC as a surgical option in many obese patients. This study reports the feasibility of performing RSSC in obese patients (body mass index [BMI] ≥ 30). METHODS: Between November 2012 and February 2014, a total of 200 patients underwent RSSC at our institution. All patients were offered the robotic procedure regardless of their BMI, age, previous surgery, and acuity of their disease with no exclusion criteria

[Incisional hernia following laparoscopy, complicated by perforated gangrenic appendicitis -- a case review].

Acute appendicitis is the commonest cause of acute abdomen. Early indication for surgery -- appendectomy, plays the key role in its therapy. The rate of incisional hernias (of all operated hernias) is high and they are, to a certain extent, caused by technical, mechanical factors and the patient himself. The authors present a case review of a female patient, presenting with atypical urgent abdomen, who was hospitalized with a diagnosis of advanced absces of the abdominal wall, resp. strangulated incisional hernia. Surgical revision confirmed that the condition was caused by perforated gangreno

An incarcerated appendix: report of three cases and a review of the literature.

We came across three rare cases of incarcerated hernia, with different presentations. The first case was an elderly female, who presented with an incarcerated incisional hernia on the right lower iliac region diagnosed on contrast enhanced computed tomography (CT); the contents were the small bowel and the perforated tip of the appendix. In the second case of inguinal incarcerated hernia, ultrasonography showed the inflammed appendix in the subcutaneous plane of the hernial sac, which is very rarely diagnosed pre-operatively and was confirmed during surgery. Inflammed appendix with gangrenous

Predictors of failure of the laparoscopic approach for the management of small bowel obstruction.

Small bowel obstruction (SBO) is a common cause of hospital admission. Our objective is to determine variables that correlate with failure of the laparoscopic approach for SBO. Twenty-three consecutive patients underwent diagnostic laparoscopy with curative intent for treatment of SBO by a single surgeon over a 3-year period. The laparoscopic approach was successful in 18 patients (78%); there were five (22%) conversions to laparotomy. The causes of obstruction included adhesive band in 16 patients; and small bowel lymphoma, metastatic esophageal cancer, small bowel gangrene, Meckel diverticul

Gas gangrene of the abdominal wall due to late-onset enteric fistula after polyester mesh repair of an incisional hernia.

The occurrence of enteric fistulae after wall repair using a prosthetic mesh is a serious but, fortunately, rare complication. We report the case of a 66-year-old diabetic man who presented with gas gangrene of the abdominal wall due to an intra-abdominal abscess caused by intestinal erosion six years after an incisional hernia repair using a polyester mesh. The aim of this case report is to illustrate the seriousness of enteric fistula after parietal repair using a synthetic material.
