Coil embolization of the internal iliac artery (IIA) for proper endovascular treatment of aortoiliac aneurysms is a procedure with an acceptable morbidity rate consisting of buttock claudication in 12% to 55% and erectile dysfunction in 1% to 13%. A case of acquired reactive perforating collagenosis is reported in a 57-year-old Thai woman, with a history of diabetes mellitus, chronic renal insufficiency needing hemodialysis, and dry gangrene of the right fourth toe. Physical examination revealed multiple scattered erythematous hyperkeratotic nodules and plaques and some showed ulceration. Histopathology showed vertical strands of collagen perforating from the ulcerated lesions. The authors also reviewed the literature on this subject.

Non-operative management by pressure reduction is now the preferred treatment for uncomplicated intussusception in children. However, in many developing countries, laparotomy is routinely performed for such cases. This is a retrospective anlaysis of 24 children who had operative reduction of intussusception. A 43 year old female was admitted with features of intestinal obstruction. On laparotomy, a strangulated Richter's hernia with an abscess within an incisional hernial sac through a previous lower segment caesarean section scar was diagnosed. Resection and anastomosis was performed. Strangulated Richter's hernia is a rare entity making up 8.3% of all strangulated hernias. There have been reports of Richter's hernia with strangulation in inguinal hernia, femoral hernia and port hernia after laparoscopic surgery but none so far in an incisional hernia.

A previously healthy 13-year-old boy developed extensive subcutaneous emphysema of the lower limb after a penetrating injury to the knee. Clostridium perfringens was isolated from the wound. Despite surgical debridement and appropriate antibiotics, the emphysema recurred, and prolonged antibiotic treatment was required. This case highlights the distinction between gas gangrene and the lesser known entity of clostridial crepitant cellulitis.
