Integration of surgical and Ayurvedic interventions in the treatment of Fournier's Gangrene: A Case Report.

Fournier's gangrene is a rare, life-threatening form of necrotizing fasciitis affecting the perineum and genital region. In Ayurveda, it correlates with Kotharoga, as both share symptoms such as severe tissue necrosis, pus discharge, and a foul odor. This case report discusses a 50-year-old male who presented with fever, severe scrotal pain, blackish discoloration, and purulent discharge two days after undergoing incision and drainage (I&D) of a perianal abscess. The case was diagnosed based on clinical examination and imaging studies. Broad-spectrum antibiotics, along with herbo-mineral medic

Two-stage laparoscopic management of De Garengeot hernia with gangrenous appendicitis: a case report and literature review.

INTRODUCTION AND IMPORTANCE: De Garengeot hernia (DGH), the presence of the appendix within a femoral hernia sac, is exceptionally rare, and acute appendicitis in this setting is even less common. Management is challenging, particularly regarding mesh placement in contaminated fields. This report describes the first two-stage laparoscopic repair with bilateral mesh placement for gangrenous DGH in a young patient. CASE PRESENTATION: A 44-year-old woman presented with right-sided groin pain and swelling. Examination revealed a tender right inguinal mass without systemic signs. Computed tomograp

Surgical Approaches to Perineal Reconstruction: A Narrative Review.

The reconstruction of the perineum remains one of the most demanding fields in reconstructive surgery. The region combines a contaminated operative field, a high prevalence of prior radiotherapy, complex three-dimensional dead space, and functional structures (the anus, urethra, and vagina) whose preservation or restoration directly determines the quality of life. The principal indications for perineal reconstruction are oncologic resection, most commonly abdominoperineal resection and pelvic exenteration for anorectal and gynecologic malignancy, radical vulvectomy, and the soft tissue destruc

Acute mesenteric ischemia caused by adhesive-band-induced mesenteric artery compression with secondary ileal volvulus and internal hernia: a rare case report.

INTRODUCTION: Acute mesenteric ischemia (AMI) is a surgical emergency caused by inadequate blood flow through the mesenteric vessels, resulting in intestinal ischemia and eventual gangrene of the bowel wall. The condition may result from arterial thrombosis, embolism, mesenteric venous thrombosis, or non-occlusive systemic hypoperfusion. CASE PRESENTATION: We report a rare case of AMI in a 73-year-old female with a significant past surgical history of transabdominal hysterectomy with bilateral salpingo-oopherectomy. She presented with sudden, severe abdominal pain that was disproportionate to

[Administration of antibiotics in patients with polytrauma-based urogenital injuries-a narrative review].

BACKGROUND: Urogenital injuries occur in up to 10% of all trauma patients and are associated with increased risk of infectious complications, especially in association with polytrauma. Rational administration of antibiotics is an essential part of care, whereby evidence-based recommendations are lacking for many types of injuries. METHODS: A narrative review on the administration of antibiotics to patients with injuries to the upper and lower urinary tract as well as external genital organs taking into consideration current international guidelines, consensus documents, and review articles is

Neonatal appendicitis with rectal bleeding: an unusual presentation.

BACKGROUND: Neonatal appendicitis is an exceptionally rare condition with a high risk of delayed diagnosis and perforation. Its nonspecific clinical presentation may mimic other causes of acute abdomen and gastrointestinal bleeding in newborns, particularly necrotizing enterocolitis. CASE PRESENTATION: A 36-week, 2500-g male neonate developed abdominal distension, irritability, decreased oral intake, and bright red rectal bleeding on the second day of life. Abdominal radiography demonstrated multiple air-fluid levels, while ultrasonography revealed dilated bowel loops and free intraperitoneal

Assessment of Thoracic Ultrasound as a Diagnostic Tool for Lower Respiratory Tract Disorders in Sheep.

Lower respiratory tract diseases (LRTDs) are a major cause of morbidity and mortality in sheep. Diagnosis is challenging not only because clinical signs are often mild or absent under field conditions, but also because adult sheep may develop a wide range of diseases affecting the lower airways with overlapping clinical presentations. This study evaluated the diagnostic performance of distance observation, comprehensive clinical examination, and thoracic ultrasonography (TUS) in sheep with suspected LRTDs, using post-mortem findings as the reference standard. A total of 103 sheep from differen

Inflammatory and total immunoglobulin biomarker domains in systemic sclerosis vasculopathy: a phenotype-based cross-sectional study.

OBJECTIVES: To characterize inflammatory and total immunoglobulin-domain profiles across clinically defined vasculopathy phenotypes in SSc using a phenotype-oriented framework. METHODS: In this retrospective cross-sectional study, 314 patients with SSc fulfilling the 2013 ACR/EULAR criteria were classified into four vasculopathy phenotypes: no DU/gangrene/clinically diagnosed PAH/SRC-defined overt vasculopathy ("none"), peripheral only, visceral only, and mixed. Inflammatory and total immunoglobulin-domain scores were prespecified as the mean of standardised ESR and hs-CRP, and IgG and IgA, r

Conservative Surgical Management and Delayed Primary Closure in Fournier's Gangrene of Perianal Origin: A Case Report and Literature Review.

Fournier's gangrene (FG) is a fulminant and life-threatening polymicrobial necrotizing fasciitis of the perineal, perianal, and genital regions. It typically affects patients with significant comorbidities, particularly poorly controlled diabetes mellitus, and is associated with high mortality rates. We describe the case of a 57-year-old male patient who presented with a one-month history of severe perineal pain, initially misdiagnosed as uncomplicated hemorrhoidal disease. Upon admission, the patient exhibited systemic inflammatory response syndrome and a large, purulent perianal mass. The pa

Case Report: rectal perforation and Fournier's gangrene after irinotecan in a bevacizumab-exposed, previously irradiated patient.

Bevacizumab-associated intestinal perforation and Fournier's gangrene have been reported separately; however, the potential contribution of residual anti-VEGF exposure prior to subsequent chemotherapy in previously irradiated pelvic tissue remains underrecognized. Radiotherapy causes fibrosis and microvascular injury, bevacizumab impairs healing, and irinotecan-induced diarrhea may exacerbate perforation risk. Deferring DRE, imaging, and surgical consultation in patients with unexplained perineal or buttock pain increases the risk. A 61-year-old man with rectal adenocarcinoma received neoadjuv

Ultra-short bowel syndrome after near-total small bowel resection for gangrenous gut due to superior mesenteric artery thrombosis: a case report.

Ultra-short bowel syndrome (USBS) is a rare and life-threatening condition defined by the retention of ˂30 cm of small intestine following resection. We report the case of a 53-year-old male who presented with gangrenous gut secondary to superior mesenteric artery thrombosis, necessitating near-total small bowel resection. The patient was left with ~15 cm of proximal jejunum and 10 cm of distal ileum, with a jejuno-ileal end-to-end anastomosis. He developed severe postoperative watery diarrhoea and required Total Parenteral Nutrition (TPN) via a peripherally inserted central catheter line. Wit

Bilateral Trapezoid-Like Scrotal Flaps for Reconstruction of Partial Scrotal Defects after Fournier's Gangrene.

Perineal reconstruction in males can be challenging due to the complex anatomy and the potential for complications such as infection, hematoma, and wound dehiscence. In this retrospective case series, we present seven patients who underwent perineal reconstruction between 2020 and 2023 using bilateral scrotal flaps. All defects were secondary to debridement for Fournier's gangrene. In patients with defects involving up to 70% of the total scrotal surface and with sufficient intact scrotal tissue preserved superiorly and inferiorly, reconstruction was performed using bilateral trapezoid-like sc

Paediatric fracture complications following traditional bone setting in low- and middle-income countries: a systematic review and meta-analysis protocol.

INTRODUCTION AND AIMS: Traditional bone setters (TBS)-lay practitioners who manage fractures without formal medical qualification-treat an estimated 10%-85% of fracture patients in low- and middle-income countries (LMICs). Children are disproportionately vulnerable due to physeal immaturity and lower diastolic blood pressure thresholds that increase susceptibility to compartment syndrome at lower absolute intracompartmental pressures than in adults. No prior systematic review has restricted analysis exclusively to the paediatric population or generated pooled meta-analytic estimates of complic

Phlegmasia alba dolens caused by external iliac vein compression from inflatable penile prosthesis reservoir malposition.

Phlegmasia alba dolens (PAD) is a rare manifestation of extensive iliofemoral deep venous thrombosis characterized by near-complete venous outflow obstruction, resulting in severe pain, diffuse limb swelling, and pallor, whereas arterial inflow remains preserved. PAD is a vascular emergency as progression to phlegmasia cerulea dolens and venous gangrene may occur without prompt restoration of venous outflow. A 62-year-old man with a past medical history of prostate cancer status post prostatectomy presented with dyspnea, chest pain, and acute right lower extremity swelling 1 week after inflata

Clostridium septicum Aortitis and Mediastinitis Complicating Perforated Appendicitis in a Patient with Chronic Myelomonocytic Leukemia: A Case Report.

Clostridium septicum is an aerotolerant anaerobe classically linked to occult colorectal malignancy and hematologic disease, and a rare but frequently fatal cause of nontraumatic infective aortitis; it also causes gas gangrene, osteomyelitis and spondylodiscitis. Survival depends on radical surgical debridement, as antibiotics alone are almost uniformly fatal. We describe a 71-year-old man with untreated chronic myelomonocytic leukemia (CMML) who presented with back pain, vomiting and rigors. Computed tomography showed perforated appendicitis with extraluminal air together with mediastinitis a

Reconstruction After Necrotizing Fasciitis: A Timing-Based Algorithm for the Plastic Surgeon.

Survival after necrotizing fasciitis has improved, producing a growing population of survivors whose function and appearance are determined by reconstruction. Guidance on reconstructive timing and technique, however, remains limited. This structured narrative review synthesizes evidence from PubMed/MEDLINE on reconstruction after necrotizing fasciitis from the perspective of the plastic surgeon and proposes a practical, timing-based algorithm. Its central thesis is that reconstruction is planned from the first debridement: wound-bed readiness and patient physiology, rather than a preferred tec
