Amputation and gangrene associated with SGLT2 inhibitors: pharmacovigilance analysis of the FDA adverse event reporting system (FAERS) database.

BACKGROUND: Sodium-glucose co-transporter-2 (SGLT2) inhibitors are widely prescribed to diabetic patients for their cardiorenal benefits. However, concerns over adverse events—specifically amputation risk and gangrene—have risen in recent years. This study aimed to describe the frequency and severity of these events in relation to SGLT2 inhibitor use, using the U.S. FDA Adverse Event Reporting System (FAERS). METHODS: OpenVigil 2.1 was used to extract safety reports submitted to FAERS from 2003 Q4 to 2024 Q4. Amputation and gangrene events linked to SGLT2 inhibitors (canagliflozin, dapaglifloz

Upper Limb Amputations in End-Stage Renal Failure Patients: A Single Institution's Prospective Cohort Study on Risk Factors and Survival Analysis.

Background: Patients with end-stage renal failure (ESRF) are prone to vascular complications that may result in limb ischaemia and gangrene. While lower limb amputation (LLA) outcomes in this population are well-documented, data on upper limb involvement remain limited. This study aimed to characterise the types of hand conditions leading to hand specialist referrals amongst ESRF patients, identify risk factors associated with upper limb amputation (ULA) and assess post-amputation survival outcomes. Methods: ESRF patients referred to a single hand surgery unit were recruited over 5 years. Data

Beyond Raynaud's: Atypical Peripheral Vascular Manifestations in a Case of CREST Syndrome.

Peripheral vascular disease (PVD) is a rare but severe symptom of CREST syndrome, which itself is a limited cutaneous sclerosis. Even though Raynaud's phenomenon is the gold standard of CREST, the development of critical limb ischemia and self-amputation is rare. We report a case of a 48-year-old female with CREST syndrome who presented with progressive ischemia in both upper and lower limbs, leading to spontaneous auto-amputation of multiple fingers/toes. The patient exhibited sclerodactyly, dry gangrene, delayed capillary refill, and tactile deficits in peripheral pulses. ANA and anti-Scl 70

Hallux gangrene following first metatarsophalangeal arthrodesis.

Hallux rigidus is commonly treated with first metatarsophalangeal (MTP) arthrodesis, a procedure with high success and low complication rates. We present a rare case of postoperative hallux gangrene following first MTP arthrodesis in a 44-year-old woman with celiac disease and no conventional vascular risk factors. Despite normal dorsalis pedis and posterior tibial pulses, the great toe developed ischemic changes on postoperative Day 1, progressing to dry gangrene requiring amputation at the MTP joint. Angiography revealed no proximal obstruction, supporting a microvascular etiology. Potential

True Incidence of Appendiceal Neoplasms in Patients Presenting with Complicated Appendicitis: A Call for a Revised Management Algorithm.

BACKGROUND: Prior studies suggest an association between complicated appendicitis and underlying appendiceal neoplasms, but the reported incidence varies widely. This study aimed to determine the incidence and predictors of appendiceal neoplasms among patients with radiographically diagnosed complicated appendicitis. METHODS: This study analyzed adult patients with radiologic diagnoses of complicated appendicitis (perforation, abscess, phlegmon, gangrene, or mucocele) at a tertiary center (2010-2024). To ensure diagnostic accuracy, an expert surgeon reviewed radiologic images with uncertain f

Congenital gangrene of the perineum in a newborn.

Congenital gangrene in newborns is a rare condition characterised by tissue gangrene present at birth. It may result from intrauterine vascular compromise, birth trauma or clotting disorders. Early diagnosis and prompt management, including surgical intervention and supportive care, are critical to prevent severe complications and improve survival outcomes. We are reporting a case of congenital gangrene of the gluteal region in the newborn.

Extensive Lower Extremity Venous Thrombosis and Phlegmasia From Heparin-Induced Thrombocytopenia Following Percutaneous Coronary Intervention (PCI) Successfully Managed With Limb-Salvage Surgery.

We report the case of a 64-year-old woman recently treated with unfractionated heparin during percutaneous coronary intervention who developed progressive left lower extremity edema after hospital discharge. Over the next several days, she experienced worsening swelling, sensory loss, and subsequent cyanosis. She presented to the emergency department with marked limb swelling, mottling, leukocytosis, thrombocytopenia, and elevated D-dimer. Duplex ultrasonography revealed extensive iliofemoral and infrapopliteal thrombosis. Given recent heparin exposure and new thrombocytopenia, her 4Ts score i

Management of Panniculus Gangrene After Cesarean Hysterectomy for Placenta Accreta: A Case Report.

BACKGROUND Placenta accreta spectrum (PAS) disorders are more likely with repeated cesarean sections and can result in life-threatening postpartum hemorrhage (PPH). Consequences can include ischemia, septic shock, and rare complications such as necrosis of abdominal fatty tissue after a cesarean hysterectomy. There are several reported cases of panniculus fat necrosis following a Pfannenstiel incision after cesarean sections or hysterectomies. CASE REPORT A 40-year-old woman, pregnant with twins and with a history of 5 pregnancies and 4 previous cesarean sections, was diagnosed with placenta p

A Novel Single-stage Technique for Ventral Penile and Urethral Reconstruction Using a Deep External Pudendal Artery Propeller Flap.

Reconstruction of the penile shaft represents a complex surgical challenge, especially when both cutaneous and urethral components are involved. Although total phallic reconstruction is well established, partial ventral penile defects lack standardized treatment strategies, particularly in older or multimorbid patients unfit for microsurgical procedures. We present the case of a 78-year-old man with a 2.5 × 3.5 cm full-thickness ventral shaft defect and associated urethral involvement, secondary to Fournier gangrene. A 3.0 × 5.0 cm freestyle propeller flap based on the deep external pudendal a

Perforated sigmoid diverticulitis presenting as Fournier's gangrene.

Fournier's gangrene (FG) is a fulminant necrotising fasciitis of the perineum and genital region. We report a rare case of FG secondary to retroperitoneal perforation of sigmoid diverticulitis in a male patient in his late 60s. The patient presented with septic shock, scrotal necrosis and faecal discharge. Cross-sectional imaging revealed retroperitoneal air and oral contrast tracking from a perforated sigmoid colon into the scrotum. He underwent emergency scrotal debridement followed by sigmoidectomy with end colostomy. Early surgical source control, broad-spectrum antibiotics and vacuum-assi

The role of prognostic scoring systems in determining mortality and morbidity in patients with Fournier's gangrene.

BACKGROUND: Fournier’s gangrene (FG) is a rare but severe form of necrotizing fasciitis characterized by high mortality and morbidity rates. In this study, the predictive power of the classical Fournier’s Gangrene Severity Index (FGSI) was compared with new hematological-inflammatory and nutritional scoring systems, including the Hemoglobin, Albumin, Lymphocyte, and Platelet (HALP) score, Global Immune-Nutrition-Inflammation Index (GINI), and Prognostic Nutritional Index (PNI), in predicting the prognosis of FG. MATERIALS AND METHODS: This retrospective cohort study included the clinical and l

Hyperbaric oxygen therapy and Fournier's gangrene: a systematic review and meta-analysis.

FactsA meta-analysis demonstrated hyperbaric oxygen therapy reduces the relative risk of death in patients suffering from Fournier's gangrene. The present study has revealed no statistically significant differences in terms of hospital length of stay or the number of surgical debridements in patients with Fournier's gangrene between the hyperbaric oxygen therapy and control groups. The extant evidence supports the use of hyperbaric oxygen therapy as an ancillary measure of Fournier's gangrene following prompt surgical debridement and antimicrobial therapy, rather than as a primary treatment moda

Appendico-Ileal Knotting: A Rare, Dangerous, and Often Overlooked Complication of Appendicitis: A Case Report.

Appendicitis and small bowel obstruction (SBO) are common causes of acute abdomen encountered in clinical practice. However, appendicitis as a cause of SBO is very rare and often overlooked, commonly resulting in delayed diagnosis and management. Appendico-ileal knotting, a rare but dangerous way appendicitis could lead to mechanical SBO, occurs when an inflamed appendix forms a ring-like structure through which a small bowel loop herniates, resulting in closed-loop obstruction that could be complicated by small bowel strangulation and gangrene if not identified and intervened early. Assessment of Blood-Count-Derived Biomarkers, Homocysteine Levels, MTHFR Mutation, and Clinical Manifestations in Severe Peripheral Artery Disease.

Background: Infection and consequent limb amputations are complications of severe peripheral artery disease, especially in diabetic patients. Risk factors and prognostic markers are of particular importance in defining patient care. Methods: This study included 99 peripheral artery disease (PAD) patients admitted for surgical intervention in the 2020-2021 time interval. The included subjects were stratified by type 2 diabetes mellitus (T2DM) diagnosis (present/absent). Protein, albumin concentrations, blood-count-derived inflammatory markers, and cultures from gangrenous wounds were assessed.

Characterization of Th2 Serum Immune Response in Acute Appendicitis.

Acute Appendicitis (AA) is the commonest abdominal digestive surgical emergency, but its etiology is not clarified. Based on histologic observations, an allergic cause has been proposed. In a type I hypersensitivity allergic reaction, there is a Th2 immune response characterized by Th2 cells, eosinophils, basophils, IgE, IL-4, IL-5, and IL-13 serum elevation. Recent studies showed a local appendicular endoluminal and parietal Th2 immune response in acute phlegmonous appendicitis. We performed a prospective single-center study where we evaluated the Th2 blood immune response in 38 patients with
