High-Mortality Outbreak of Staphylococcus aureus Mastitis Associated with Poor Milking Practices in a Goat Dairy.

An outbreak of mastitis caused by Staphylococcus aureus occurred in a commercial dairy goat herd during kidding season, resulting in fatal gangrenous mastitis in approximately 30% of the herd. S. aureus was recovered from milk, mammary tissue, and other organs in does subjected to necropsy. The S. aureus milk culture-positive rate among does in the hospital pen was 58.3%, while whole-herd milk cultures of clinically normal mature does identified S. aureus in 15.0% with an additional 15.0% positive for coagulase-negative Staphylococcus (CNS), yielding a total culture-positive rate of 30.0%. Pyogenic Hepatic Abscess as a Complication of Appendicitis: A Case Report.

Pyogenic liver abscess is a rare and under-recognized complication of gangrenous or perforated appendicitis in children. Management typically requires a combination of antibiotic therapy and abscess drainage. Although uncommon in the pediatric population, pyogenic liver abscess is more frequently observed in the elderly people. With an aging population, it is increasingly important for young physicians to recognize its clinical presentation and treatment strategies. This report describes an 11-year-old boy admitted with fever, impaired consciousness, nausea, and vomiting-initially raising conc

The long-term impact of early HbA1c control on nephropathy, neuropathy, and retinopathy in type 2 diabetes: Findings from a large UK observational study.

INTRODUCTION: The evidence remains contradictory regarding the optimal glycaemic targets needed to address the long-term effects of hyperglycaemia in people with diabetes mellitus (T2DM). We examined the association between HbA1c levels and the risk of individual microvascular complications among people with T2DM. METHODS: We used the Clinical Practice Research Datalink (CPRD) GOLD database for a prospective cohort study, following patients ≥18 years old from diagnosis of T2DM between January 2007 and December 2017. Neuropathy included foot ulcers, peripheral arterial disease, gangrene, and a

Prognostic impact of Lesion Subset with or without below the Knee Lesion in Chronic Limb-Threatening Ischemia following Endovascular Revascularization.

AIMS: The aim of this study is to evaluate the impact of each level atherosclerotic lesion subset on clinical outcomes following EVT in CLTI patients. METHODS: We identified 705 consecutive CLTI patients undergone successful EVT between January 2010 and December 2019 at our hospital in Japan. The technical success was defined as the acquisition of one straight line flow to the ulcerated or gangrenous region. We excluded 15 patients with follow up <30days. The primary endpoint was major adverse limb event (MALE) at 2 years after EVT. MALE was defined as composite of all cause death and major a

De Garengeot Hernia Presenting as a Rare Cause of Incarcerated Femoral Hernia.

De Garengeot hernia is defined as the presence of the appendix within a femoral hernia sac and represents a rare surgical condition. Its clinical presentation closely resembles that of an incarcerated or strangulated femoral hernia, often leading to delayed or intraoperative diagnosis. We report the case of an elderly woman who presented with acute right-groin pain and an irreducible groin mass concerning for an incarcerated femoral hernia. Emergent open exploration through an inguinal approach revealed a gangrenous appendix within the femoral canal. Appendectomy and primary tissue repair were

Jejunojejunal Intussusception Following Roux-en-Y Gastrectomy for Gastric Adenocarcinoma.

Jejunojejunal intussusception is an exceptionally rare but potentially serious complication following Roux-en-Y gastrectomy, with an estimated incidence of 0.4%-0.64%. It most often occurs at the jejunojejunal anastomosis, typically in a retrograde fashion, due to postoperative motility disturbances. Although most cases occur within 2 years postoperatively, delayed presentations several years after surgery can pose diagnostic challenges. We report the case of an 81-year-old male who presented 6 years after D2 subtotal gastrectomy with Roux-en-Y reconstruction for gastric adenocarcinoma and hig

Right Atrial Mass With Pulmonary Embolism and Tricuspid Dysfunction: Features Favoring Right-Sided Infective Endocarditis.

Right-sided infective endocarditis (RSIE) represents a minority of infective endocarditis cases but carries substantial morbidity due to embolic and hemodynamic complications, particularly in patients with intravenous drug use. We describe a 45-year-old man with intravenous drug use who presented in septic shock complicated by disseminated intravascular coagulation and peripheral gangrene. Blood cultures grew Streptococcus agalactiae. Multimodal cardiac imaging revealed a large, mobile right atrial mass prolapsing through the tricuspid valve, resulting in severe tricuspid regurgitation and ass

Appendico-Ileal Knotting (Appendiceal Tourniquet Syndrome): A Systematic Review and Clinicopathological Classification.

Appendico-ileal knotting (AIK), also known as appendiceal tourniquet syndrome, is an exceptionally rare and frequently overlooked cause of small bowel obstruction in which the appendix encircles the ileum, creating a closed-loop obstruction that rapidly progresses to ischemia and bowel gangrene. Despite advancements in cross-sectional imaging, AIK remains a formidable diagnostic enigma. Its clinical masquerade as common small bowel obstruction, coupled with the absence of pathognomonic radiographic markers, results in a persistently low preoperative diagnostic rate that necessitates a high ind

Fimbria torsion as a rare cause of lower abdominal pain: A case report and literature review.

BACKGROUND: Isolated fimbrial torsion is a rare gynecological emergency that can lead to severe complications if not promptly diagnosed. Its clinical presentation often mimics more common causes of abdominal pain, creating a significant diagnostic challenge. CASE PRESENTATION: This case report describes a 47-yr-old woman presenting with right lower quadrant abdominal pain. The initial clinical and ultrasound findings were suggestive of acute appendicitis. However, diagnostic laparoscopy revealed isolated gangrenous torsion of the right fallopian tube fimbria. CONCLUSION: This case underscore

Prognostic role of systemic inflammatory indices in predicting the severity of acute calculous cholecystitis.

BACKGROUND: Acute calculous cholecystitis (ACC) is one of the most common conditions encountered in emergency medicine and surgical practice. Delayed recognition of severe cases can lead to complications such as empyema, gangrene, or perforation, resulting in high morbidity and mortality. While the Tokyo Guidelines provide standardized diagnostic and severity grading criteria, the availability and reliability of imaging may be limited in certain settings. Therefore, there is growing interest in simple and cost-effective biomarkers. This study aimed to evaluate the diagnostic and prognostic val

Nonocclusive mesenteric ischemia secondary to organophosphorus poisoning: A case report.

Nonocclusive mesenteric ischemia (NOMI), a rare subtype of mesenteric ischemia, carries high morbidity and mortality due to its delayed recognition. Organophosphorus poisoning (OPP) manifests with varied gastrointestinal symptoms, but ischemia has been described only in a few case reports. Imaging, particularly contrast-enhanced computed tomography (CECT), plays a pivotal role in early diagnosis. We report the case of a 23-year-old male who intentionally ingested an organophosphorus (OP) compound and was admitted to the intensive care unit (ICU). During the course of treatment, he developed si

Large-Volume Pneumoperitoneum in a Neonate: A Rare Case of Necrotizing Gastritis.

Neonatal spontaneous gastric perforations are rare but are associated with high mortality rates. They usually occur within the first week of life, with a sudden onset of abdominal distension. Imaging often shows pneumoperitoneum. Gastric perforations require emergent surgical management. We report a case of a two-day-old male with spontaneous patchy gastric gangrene and perforation of the greater curvature. The patient presented with abdominal distension after initiation of feeds. An X-ray of the abdomen revealed a large-volume pneumoperitoneum. The patient underwent emergent exploratory lapar

Acute mesenteric ischemia in a child: A rare case report and diagnostic challenge in the emergency department.

Acute mesenteric ischemia (AMI) is a rare but life-threatening vascular emergency that typically affects elderly individuals with cardiovascular risk factors. We report a unique case of AMI in a previously healthy 15-year-old boy who presented with acute abdominal pain, hypotension, and metabolic acidosis. Point-of-care ultrasound revealed ascites without clear signs of ischemia, and ascitic fluid analysis was suggestive of secondary peritonitis. Due to clinical deterioration and rising lactate levels, the patient underwent emergency explorative laparotomy, which revealed extensive gangrenous

Ochrobactrum anthropi Causing Fournier's Gangrene: A Report of a Rare Case.

Fournier's gangrene is a form of necrotizing fasciitis that affects the penis, scrotum, or vulva. Novel bacteria for this type of infection include both aerobic and anaerobic bacteria. Rarely, other microorganisms can be the causative agent of the infection. Ochrobactrum anthropi is an aerobic, gram-negative, opportunistic infection that often affects orthopedic hardware and has been described as a pathogen within the cornea, as well. This case report describes a patient presenting with Fournier's gangrene in which the causative pathogen was identified via tissue culture as O. anthropi that wa

Negative pressure wound therapy for perianal necrotizing fasciitis: case report and literature review.

Perianal necrotizing fasciitis (PNF) is a life-threatening soft tissue infection with high mortality. Despite the established role of radical debridement, outcomes are often hindered by late diagnosis and suboptimal postoperative care. While negative pressure wound therapy (NPWT) promotes complex wound healing, its synergistic application with radical debridement in PNF requires further validation. We report a case of extensive PNF that demonstrated the limitations of traditional interventions. By utilizing an integrated strategy of extended-incision radical debridement and early NPWT, we achi
