Interobserver Variability in Intraoperative Classification of Acute Appendicitis and Postoperative Antibiotic Management.

BACKGROUND: During laparoscopy for suspected acute appendicitis (AA), surgeons classify the appendix as normal, phlegmonous, gangrenous, or perforated, with or without abscess. The surgeon's assessment directly influences postoperative antibiotic treatment and length of hospitalization. There is currently no objective gold standard for this classification. This study aimed to evaluate interobserver variability in the laparoscopic classification of AA. METHODS: Surgeons and surgical trainees viewed video recordings of laparoscopies from 30 patients suspected of AA. Each participant anonymously

Preventing Penile Retraction After Fournier's Gangrene: Progressive-Tension Soft-Tissue Fixation for Penile Reconstruction.

Fournier's gangrene (FG) is a rapidly progressive necrotizing soft-tissue infection associated with substantial morbidity and mortality. Extensive genital debridement may result in complex defects requiring reconstruction to restore function, body image, and quality of life. Penile reconstruction is particularly challenging because successful skin grafting requires both graft immobilization and maintenance of penile projection. We report a 58-year-old man with type II diabetes mellitus and obesity who presented with fever, genital pain, and severe systemic inflammation (C-reactive protein: 248

Clinicogenetic Characterization of H Syndrome in a South African Child.

H syndrome is a rare autosomal recessive genodermatosis caused by loss-of-function SLC29A3 variants, combining hyperpigmented indurated plaques and hypertrichosis with life-threatening autoinflammation. We report a 5-year-old South African girl of first-cousin parents who presented with hyperinflammation mimicking macrophage activation syndrome, progressing to purpura fulminans with digital gangrene and autoamputation of fingertips and toes, myocarditis, flexion contractures, flat feet, and enlarged cheeks. Next-generation sequencing identified a homozygous SLC29A3 variant, c.7G>A (p.Val3Ile),

Acute Gangrenous Cholecystitis During Pregnancy: A Case Report.

Acute cholecystitis is one of the most common nonobstetric surgical emergencies during pregnancy. Progression to gangrenous cholecystitis is rare but is associated with significant maternal and fetal morbidity. We report the case of a 26-year-old primigravida at 25 weeks' gestation who presented with severe right upper quadrant pain, fever, and vomiting. Laboratory evaluation revealed elevated serum inflammatory markers. Transabdominal ultrasonography demonstrated gallbladder wall thickening, pericholecystic fluid, and biliary sludge. Despite 24 h of conservative management, her clinical condi

Beyond Deep Vein Thrombosis: Early Recognition of Phlegmasia Cerulea Dolens With Arterial Inflow Compromise and Concomitant Pulmonary Embolism.

Phlegmasia cerulea dolens (PCD) is a rare and severe complication of extensive deep vein thrombosis (DVT) that can rapidly progress to limb ischemia, venous gangrene, and death. Early recognition and restoration of venous outflow are, therefore, critical. We report the case of a 48-year-old man who presented with a three-day history of progressive left lower-limb swelling, pain, and bluish discoloration. Examination revealed tense swelling of the entire limb, delayed capillary refill, and a diminished dorsalis pedis pulse. Duplex ultrasonography demonstrated extensive left iliofemoral and popl

Caring for a patient with Fournier gangrene.

Fournier gangrene (FG) is a form of necrotizing fasciitis that affects the genital and perineal tissues. This life-threatening condition is most often caused by a polymicrobial bacterial infection of the tissues. Predisposing factors include obesity, advanced age, chronic alcohol abuse, and immunosuppression. Although rare, FG can be life-threatening and can significantly impact the patient's quality of life. Adept nursing care can improve patient outcomes. This article discusses the pathophysiology and common complications of FG and outlines expected nursing considerations for patients with t

Spermatic cord tunnel Penrose drainage for cephalad-tracking Fournier's gangrene: a case report.

Fournier's gangrene tracking cephalad along spermatic cords complicates source control and soft tissue reconstruction. We describe a 61-year-old male with poorly controlled diabetes who presented with extensive scrotal necrotizing fasciitis tracking superiorly to the suprapubic region. Following wide excisional debridement, bilateral Penrose drains were passed through the spermatic cord tunnels and brought out through dependent perineal counter incisions. This technique provided effective through-and-through drainage of the cephalad tracts while limiting abdominal tissue loss. Deep wound cultu

A last resort that works: Distal deep vein arterialization in no-option diabetic critical limb-threatening ischemia: A prospective interventional study.

BACKGROUND: Chronic limb-threatening ischemia (CLTI) is the most advanced stage of peripheral arterial disease characterized by ischemic rest pain, non-healing ulcers, and gangrene. In patients with "no-option" CLTI, distal deep vein arterialization (DDVA) offers a potential limb salvage strategy. We prospectively studied the outcomes of open DDVA in patients with diabetes with CLTI. METHODS: This prospective interventional case series included 10 patients with diabetes with CLTI (rest pain, ulcers, or gangrene) confirmed using duplex ultrasonography and CT angiography underwent open DDVA. Radiographic Findings Concerning for Gangrenous Cholecystitis in an Asymptomatic Elderly Patient: A Case for Conservative Management.

Gallbladder necrosis is a rare but life-threatening complication of acute cholecystitis, likely resulting from chronic blood flow, inflammation, and intraluminal pressure. It most commonly occurs in elderly or critically ill patients with significant comorbidities and is often underreported due to its nonspecific presentation. This diagnosis is usually associated with right upper quadrant pain, leukocytosis, and fever. Here, we present an 80-year-old woman with metastatic lung cancer, COPD, and an extensive surgical and oncologic history who was incidentally found to have radiographic findings

Recent advances in pretreatment and detection technologies for ergot alkaloids.

Ergot alkaloids (EAs) are a class of mycotoxins widely present in complex matrices such as grains, pharmaceuticals, and biological samples. EAs possess vasoconstrictive and neurotoxic properties, which can lead to severe health issues such as gangrene and hallucinations. However, significant matrix effects, numerous co-existing interferents, and tedious sample pretreatment procedures in complex matrices pose substantial challenges for high-sensitivity and high-selectivity analysis. Accurate analysis of EAs is crucial for food safety and public health. This paper systematically reviews the rese

Rapidly Progressive Acute Limb Ischemia With Irreversible Gangrene and Major Amputation in a Resource-Limited Setting: A Case Report.

Acute limb ischemia (ALI) is a vascular emergency in which delayed recognition and treatment can result in irreversible tissue necrosis, gangrene, limb loss, and potentially life-threatening systemic complications. We present the case of a 63-year-old woman with atrial fibrillation and heart failure who developed acute lower-limb ischemia with progressive discoloration and tissue necrosis. Lower-extremity arterial Doppler ultrasonography demonstrated extensive arterial occlusive disease with absent flow across multiple major arterial segments, confirming severe limb ischemia. Gallbladder Involvement in Pediatric Hepatitis A: Findings From an Italian Outbreak Cohort.

BACKGROUND: Gallbladder involvement during acute hepatitis A virus (HAV) infection is generally considered an uncommon extrahepatic manifestation, although recent reports suggest it may be underrecognized. We evaluated the prevalence, clinical characteristics and outcomes of gallbladder abnormalities in a multicenter cohort of children hospitalized with acute HAV infection during an Italian outbreak. METHODS: We conducted a multicenter observational study, including consecutive children with laboratory-confirmed acute HAV infection admitted to 4 tertiary pediatric hospitals between January an

Association of Referring Specialty With Limb Loss in Chronic Limb-Threatening Ischemia.

ObjectiveChronic limb-threatening ischemia (CLTI) is associated with high morbidity and mortality. While delayed referral is known to worsen outcomes, the impact of referring specialty remains unclear. This study evaluates whether different referral sources to vascular surgery impact mortality and major amputation rates.MethodsA retrospective review was conducted of patients referred to vascular surgery for CLTI at a single institution (2021-2022). The outcomes investigated were mortality, amputation and composite freedom from amputation or mortality (amputation free survival). Univariable ana

Urogenital abscess with a complex etiology: a case report.

BACKGROUND: Scrotal abscesses requiring hospitalization are uncommon, with progression to sepsis being even rarer. A feared complication of a scrotal abscess is Fournier gangrene, an uncommon type of necrotizing fasciitis that starts in the scrotum or perineum. Fournier gangrene is associated with a mortality rate of about 40% and accounts for less than 0.02% of all hospital admissions. The patient in this report was hospitalized for a complex scrotal abscess and, during repeat incision and drainage (I&D), was found to have Fournier gangrene. CASE DESCRIPTION: The patient was a 49-year-old ma

Surgical Approaches to Perineal Reconstruction: A Narrative Review.

The reconstruction of the perineum remains one of the most demanding fields in reconstructive surgery. The region combines a contaminated operative field, a high prevalence of prior radiotherapy, complex three-dimensional dead space, and functional structures (the anus, urethra, and vagina) whose preservation or restoration directly determines the quality of life. The principal indications for perineal reconstruction are oncologic resection, most commonly abdominoperineal resection and pelvic exenteration for anorectal and gynecologic malignancy, radical vulvectomy, and the soft tissue destruc
