Post Fournier gangrene raw areas in the penis or scrotum: the role of intense pulsed light and radiofrequency in conservative treatment.

BACKGROUND: For infected wounds, including clostridial myonecrosis, necrotizing soft tissue infections, and Fournier's gangrene, hyperbaric oxygen therapy (HBOT) is advised; however, tension pneumothorax and gas emboli may occur during the process. Other instances of relative contraindications include illnesses like asthma and chronic obstructive lung disease. We can use radio frequency and strong pulsed light in our research without having to worry about these kinds of issues and could have a significant impact on improving treatment outcomes. OBJECTIVE: To assess how well intensely pulsed l

How to manage pelvic-perineal necrotic bacterial dermohypodermitis.

Pelvic-perineal necrotic bacterial dermohypodermitis, also known as Fournier's gangrene, is a rare infection, at 1.6/100,000, affecting males in more than 95% of cases, frequently overwhelming and with very poor prognosis. It is polymicrobial in more than 80% of cases, spreading rapidly in the subcutaneous perineal tissue due to bacterial enzymes. Recent mortality estimates were between 7.5 and 20%. The main risk factors, related to impairment of immunity and microcirculation, include diabetes, smoking and alcohol abuse, obesity, HIV infection, cardiac, renal and/or hepatic comorbidity, and ca

[Negative Pressure Wound Therapy for Fournier's Gangrene in the Inguinal Region : A Case Report].

A 47-year-old male had been under treatment with debridement and antibacterial therapy for Fournier's gangrene from the right inguinal region to the scrotum. Local negative pressure wound therapy (NPWT) was started simultaneously during the second debridement. After confirming that good granulation was achieved without any signs of infection, a segmental skin grafting procedure was performed 3 weeks after the start of NPWT. NPWT was continued for fixation of the graft. One week after the implantation, the grafts were confirmed to have grown and the patient was discharged.

A growing crisis of neonatal hyperosmolar-hypernatremic dehydration during heat waves: clinical insights from a semi-arid region.

Hyperosmolar-hypernatremic dehydration (HHND) is a life-threatening yet preventable neonatal condition, often due to inadequate breastfeeding. The recent North Indian heat wave heightened dehydration risks, necessitating an evaluation of extreme temperatures' impact on neonatal hydration. This retrospective study analysed neonates admitted to a tertiary care level 3 neonatal intensive care unit (NICU) at AIIMS Jodhpur between April and June 2024. Case records were reviewed, and details on maternal age, feeding practices, presenting complaints, biochemical profile, and outcome were studied. The

Preoperative Inflammatory Markers (NLR, MLR, PLR) in Evaluating Acute Cholecystitis Severity and Operative Difficulty.

BACKGROUND: Acute cholecystitis, a leading cause of urgent surgical intervention, poses challenges in predicting severity and operative complexity. This study characterized the immuno-inflammatory profile distinguishing acute from chronic cholecystitis and assessed whether blood-derived ratios-neutrophil-to-lymphocyte (NLR), monocyte-to-lymphocyte (MLR), and platelet-to-lymphocyte (PLR)-correlate with histologic severity and surgical difficulty. METHODS: The study retrospectively analyzed 759 patients undergoing cholecystectomy from 2016 to 2024. Inflammatory indices from preoperative bloodwo

When acute cholecystitis is not what it seems: torsion of a free-floating gallbladder.

Free-floating gallbladder is a rare anatomic variant characterized by excessive mobility due to an elongated mesentery, predisposing it to torsion and vascular compromise. When torsion occurs, obstruction of the cystic duct and artery may rapidly progress to ischemia, necrosis, and gangrene, constituting a surgical emergency. Preoperative diagnosis is challenging because clinical, laboratory, and imaging findings often mimic uncomplicated acute cholecystitis. We report the case of an 87-year-old male with liver cirrhosis presenting with diffuse abdominal pain and elevated inflammatory markers.

Laparoscopic endostapler partial cecal resection in complicated appendicitis with appendiceal base involvement: a single-center retrospective study.

BACKGROUND: Acute appendicitis (AA) is a common cause of acute abdomen. Appendiceal base gangrene or perforation represents a severe form of complicated appendicitis, in which secure closure of the appendiceal stump is crucial for surgical management, and no standardized optimal strategy has yet been established. OBJECTIVE: This study aimed to evaluate the safety and feasibility of laparoscopic endostapler partial cecal resection in patients with complicated appendicitis involving appendiceal base gangrene or perforation. METHODS: A retrospective study was conducted on consecutive patients w

The Hidden Enemy: A Case Report of Early-Onset Rheumatoid Vasculitis Masquerading as Peripheral Artery Disease.

Rheumatoid vasculitis (RV) is a rare, severe extra-articular manifestation of rheumatoid arthritis (RA), typically occurring after long-standing erosive disease. Advances in RA therapy have reduced its incidence; however, RV can occasionally present early in high-risk patients. We report a man in his thirties from Central India with a 3-year history of inadequately treated seropositive RA, who presented with progressive polyarthritis, neuropathy, and distal ischemic changes. Over five months, he developed worsening joint pain and swelling, followed by tingling, numbness, and subsequently dry g

Functional Limb Preservation after Surgical Distal Venous Arterialization with Free Flap in Chronic Limb-Threatening Ischemia with Severe Foot Arterial Disease.

INTRODUCTION: In chronic limb-threatening ischemia (CLTI), especially with diabetes and dialysis dependence, below-the-ankle arterial lesions and severe calcification often limit distal bypass options. While surgical distal venous arterialization (sDVA) can restore perfusion, achieving wound healing requires additional surgical strategies such as free flap transfer to provide adequate soft tissue coverage. CASE PRESENTATION: A male patient in his 70s with diabetes and end-stage renal disease on hemodialysis presented with a right toe ulcer and rest pain. He had severe intradialytic hypotensio

Clinical Outcomes of Open Repair for Small Popliteal Artery Aneurysms.

BACKGROUND: Indications for surgical repair of <20 mm popliteal artery aneurysms (PAAs) remain uncertain. Although the SVS recommends intervention for symptomatic small PAAs (sPAAs), clinical outcomes are poorly characterized. We evaluated the perioperative and long-term results of open repair for sPAAs. METHODS: This multicenter retrospective cohort study included 34 patients (40 limbs) who underwent open surgery for PAAs between January 2009 and December 2024 at 2 Japanese vascular surgery centers. The sPAA group (maximal diameter <20 mm) was compared with the large PAA group (LPAA; maximal

A Case Report of Acute Appendicitis Complicated by Appendicoliths.

UNLABELLED: This case report highlights the clinical complexities of diagnosing and managing appendicitis complicated by appendicoliths in a 44-year-old female patient. Appendicitis is a common cause of acute abdominal pain, with imaging playing a crucial role in diagnosis. The patient presented with symptoms of right lower quadrant pain, nausea, vomiting, and chills that were initially misattributed to menstrual cramps. Diagnostic evaluation revealed multiple appendicoliths via computed tomography (CT), and the decision for laparoscopic appendectomy was based on the imaging results and clinic

Outcomes of endovascular revascularization of upper limb thromboangiitis obliterans via palmar arch angioplasty.

BACKGROUND: Thromboangiitis obliterans (TAO), or Buerger's disease, is a rare, nonatherosclerotic segmental vasculitis affecting small- and medium-sized arteries, most commonly in the extremities. Upper extremity involvement presents unique therapeutic challenges, particularly in preserving hand function and preventing amputation. METHODS: We retrospectively reviewed nine male patients with angiographically confirmed upper extremity TAO. Demographics, angiographic findings, procedural details, and clinical outcomes - including revascularization success and amputation rates - were analyzed. Pa

Ocular complications among Noma survivors: A cross-sectional study.

BACKGROUND: Noma (cancrum oris) is a swiftly advancing gangrenous condition that predominantly impacts malnourished children in low-resource environments. Survivors often endure extensive facial disfigurement, which may involve the periorbital and orbital regions, leading to considerable ocular morbidity. Despite this, the range and distributon of ocular sequelae in Noma survivors remain inadequately documented in the literature. This study was conducted to ascertain the pattern and frequency of ocular complications among Noma survivors. OBJECTIVES: To describe the pattern and distribution of

The Diagnostic Accuracy of Neutrophil-to-Lymphocyte Ratio (NLR) Compared to C-Reactive Protein (CRP) in Patients with Acute Cholecystitis: A Systematic Review and Meta-Analysis.

Background/Objectives: Acute cholecystitis is associated with an increased risk of morbidity and mortality. Thus, early diagnosis and detection of complications are essential. Neutrophil-to-lymphocyte ratio (NLR) and C-reactive protein (CRP) are inflammatory biomarkers that could predict the diagnosis and complications of acute cholecystitis. Yet, the results of which biomarker has higher accuracy are inconsistent. Objective: To compare the accuracy of NLR and CRP in the diagnosis and prediction of complications in patients with acute cholecystitis. Methods: We searched PubMed, Scopus, and Web

A proof of concept case series of botulinum toxin a treatment for ischemic feet due to Raynaud's phenomenon.

Botulinum Toxin A (BTX-A) has shown benefit in treating Raynaud's phenomenon (RP) of the hands, but its use in the feet remains largely unreported. We present a retrospective case series of seven patients undergoing 11 BTX-A treatment sessions for refractory ischemic RP of the feet. Injections (100-150 units per foot) were administered under ultrasound guidance. At six weeks, 81.8% of treatment episodes demonstrated resolution of ulcers and/or gangrene. Pain significantly improved (p=0.013), with the proportion of pain-free cases increasing from 9.1% to 63.6%. One transient adverse event was o

Child Opportunity and Severity of Pediatric Acute Appendicitis: A Statewide Analysis.

INTRODUCTION: Socioeconomic disadvantage is associated with increased disease severity and worse clinical outcomes among children with acute appendicitis. We sought to evaluate the association between neighborhood-level child opportunity and complicated appendicitis (CA) among a retrospective pediatric cohort. We hypothesized that lower neighborhood-level opportunity, measured by the Child Opportunity Index (COI), is associated with higher incidence of CA and hospital length of stay (LOS). METHODS: We performed a retrospective review of children (<18 y) with appendicitis in the North Carolina

Acute intestinal intussusception revealing Peutz-Jeghers Syndrome in a 16-year-old male: a case report.

INTRODUCTION AND IMPORTANCE: Peutz-Jeghers Syndrome (PJS) is a rare autosomal dominant disorder characterized by hamartomatous polyps throughout the gastrointestinal tract and mucocutaneous pigmentation. Patients are at high risk for complications such as bleeding, obstruction, and especially small-bowel intussusception, which often occurs in childhood or adolescence. Early recognition is essential, as intussusception can be the first life-threatening presentation of PJS. This case highlights the importance of considering PJS in young patients presenting with acute intestinal obstruction. PRE
