Comparison of Different Sites of Microbiota Shows Increased Pseudomonas Species in the Gut Mucosa in Inflammatory Bowel Disease.

BACKGROUND: Inflammatory bowel disease (IBD), including ulcerative colitis (UC) and Crohn's disease (CD), is associated with changes in the gut microbiome. Studies comparing fecal, gut mucosal, and salivary microbiomes are rare, and questions remain regarding the interaction of these compartments. METHODS: In this case-control study, 16S rRNA gene amplicon sequencing was performed on samples from stool, intestinal mucosa, and saliva of 120 patients with IBD. Patients with signs of non-IBD colonic inflammation (N = 28) and healthy subjects (N = 67) served as controls. A total of 480 16S profil

Differences in Anti-αvβ6 Integrin Antibody Expression between U.S. and Japanese Cohorts in Inflammatory Bowel Disease.

BACKGROUND: Inflammatory bowel diseases (IBDs), including ulcerative colitis (UC) and Crohn's disease (CD), have complex pathologies requiring precise diagnostic tools. We evaluated the clinical utility of anti-integrin αvβ6 antibodies in diagnosing UC, focusing on differences between a U.S. cohort (self-reported White) and a Japanese cohort, and additionally assessed whether combining anti-αvβ6 with anti-EPCR improved diagnostic performance. METHODS: Serum anti-αvβ6 antibody levels were measured in 1138 participants (514 in the U.S. cohort, 624 in the Japanese cohort), including 1093 IBD cas

Clinical Significance of Mucin Signatures in Inflammatory Bowel Diseases: A Systematic Review of Their Expression Patterns, Polymorphisms, and Post-translational Modifications.

BACKGROUND: The intestinal mucosal barrier plays an important role in the pathophysiology of inflammatory bowel disease (IBD), with mucins being key components of this barrier. Classified as either transmembrane or secreted, these highly glycosylated proteins protect the mucosal barrier while also influencing barrier integrity. Given their indispensable role in maintaining the intestinal mucosal barrier, mucins are compelling candidates for the evaluation of barrier dysfunction. Numerous studies have investigated mucins in the context of IBD, but a clear consensus regarding their expression, p

The Chicago Mesenteric Fat Index: A Novel Metric for Point-of-Care Intestinal Ultrasound Evaluation of Mesenteric Fat Wrapping in Crohn's Disease.

BACKGROUND: Mesenteric fat (MF) wrapping is a characteristic feature of Crohn's disease (CD). Current assessment methods are limited to binary classification (presence/absence), restricting their utility in assessing disease progression and treatment response. This study aimed to develop and validate a novel semi-quantitative metric for grading MF wrapping using intestinal ultrasound (IUS), and to evaluate its reliability and clinical relevance. METHODS: This cross-sectional study included 95 ultrasound scans from 63 patients with CD. The novel index described here categorized MF wrapping int

Microbial signature of pediatric Crohn's disease: Differentiation from functional gastrointestinal disorders and relationship with increased disease activity.

The prevalence and incidence of Crohn's disease (CD) in pediatric populations have been steadily increasing. Growing evidence suggests that gut microbiomal community differences play a critical role in the pathogenesis of CD. Additionally, the clinical course of patients with CD is unpredictable, making treatment decisions challenging. We investigated the fecal microbiome of newly diagnosed, treatment-naïve pediatric CD patients (n = 43) compared to age- and sex-matched controls with other functional gastrointestinal disorders (n = 139). We also correlated microbial changes with CD disease act

Appendicectomy versus switching to a JAK inhibitor in inducing remission in patients with active ulcerative colitis after biologic therapy failure (COSTA): 1-year results of a multicentre, prospective, cohort study.

BACKGROUND: Although retrospective and limited prospective observational studies have suggested a potential benefit of appendicectomy in therapy-refractory ulcerative colitis, its effectiveness compared with advanced medical therapy in active, biologic-exposed patients has not been evaluated prospectively. We evaluated the efficacy of laparoscopic appendicectomy in inducing remission, compared with JAK inhibitor therapy, in patients with active ulcerative colitis who had persistent disease activity despite previous advanced therapy exposure (a small molecule or a biologic). Multi-centered T cell repertoire profiling identifies alterations in the immune repertoire of individuals with inflammatory bowel disease across different disease stages.

BACKGROUND: Inflammatory bowel disease (IBD) is an incurable immune-mediated inflammatory disease, affecting the gut with a high rate of primary- and secondary- loss-of-response to therapy. By investigating the T cell receptor repertoire of individuals with IBD, novel therapeutic and preventive strategies can be identified, and a better understanding of IBD can be obtained. METHODS: To identify and validate T cell clonotypes implicated in the pathogenesis of IBD, we profiled the T cell receptor alpha (TRA) repertoire of three cohorts containing treatment-naive, treated individuals, and indivi

Genotype subtyping approach to identify unnoticed variants in diseases from GWAS data.

Genome-wide association studies (GWAS) are vital for investigating single-nucleotide polymorphisms (SNPs) in diseases. Comparisons of detected SNPs for the same disease between populations often reveal gaps, which are attributed to factors such as population, sample size, and rare variants, among others. We propose a method referred to as ‘Genotype Subtyping’ that is based on a classical GWAS to identify seed SNPs; each SNP stratifies cases and controls by its genotype, followed by a sub-GWAS per genotype while keeping the additional number of statistical tests low. We evaluated four databases

Linking complex microbial interactions and dysbiosis through a disordered Lotka-Volterra model.

The rapid advancement of environmental sequencing technologies, such as metagenomics, has significantly enhanced our ability to study microbial communities. The eubiotic composition of these communities is crucial for maintaining ecological functions and host health. Species diversity is only one facet of a healthy community's organization; together with abundance distributions and interaction structures, it shapes reproducible macroecological states, that is, joint statistical fingerprints that summarize whole-community behavior. Despite recent developments, a theoretical framework connecting

Type 1 fimbrial protein FimH of Enterobacteriaceae-a promising target to mitigate different infections.

Gram-negative bacterial infections are increasingly becoming resistant to available antibiotic treatment options. The World Health Organization attributed over 1 million deaths to bacterial antimicrobial resistance (AMR) in 2021. While there is a crisis in terms of the available effective antibiotic repertoire, there is also a simultaneous decline in novel drug discovery. In this scenario, the search for alternative or complementary therapeutic options is not only relevant, but also urgently needed. Bacterial virulence factors have been proposed as alternative therapeutic targets since there i

Annexin A1 and A2 in inflammatory bowel disease pathogenesis: exploring new avenues for diagnosis and treatment.

Inflammatory bowel disease (IBD), which includes Crohn's disease and ulcerative colitis, is a significant global health burden with gastrointestinal inflammation resulting from dysfunctional immune responses and chronic inflammation. This review synthesizes the roles of annexin A1 (AnxA1) and annexin A2 (AnxA2) in the pathophysiology of IBD, their diagnostic biomarkers, and therapeutic potential. AnxA1 interacts with FPR2/ALX receptors, inhibiting the release of pro-inflammatory cytokines and promoting epithelial cell repair. AnxA2 exhibits dual roles by interacting with S100A10. AnxA2 can ind

Gut microbiome remodeling induced by microplastic exposure in humans.

The impact of microplastics (MPs) on the diversity and composition of the gut microbiome has been extensively documented in animal studies, but evidence in humans remains limited. Recognizing the potential differences in MP effects between animal and human gut microbiomes, this review synthesizes current evidence concerning their impact on the human gut microbiota. Furthermore, the potential links between microplastic-induced dysbiosis and the pathogenesis of human diseases were analyzed. Cross-sectional studies have been conducted to explore microplastic exposures (such as in humans who consu

Vedolizumab in inflammatory bowel disease: pharmacokinetics and the role of immunomodulator co-therapy.

Vedolizumab (VED) is a gut-selective monoclonal antibody and an effective biological agent for many patients with inflammatory bowel disease (IBD). Therapeutic drug monitoring has been used to optimise treatment with other biological agents; however, the clinical applicability of measuring vedolizumab trough concentrations is unclear. Initial registration trial data demonstrated a positive exposure-efficacy relationship of vedolizumab in IBD. However, there is conflicting data in more recent studies, with no definitive benefit of vedolizumab dose-escalation and no consistent target vedolizumab

Research engagement and patient-reported quality of care: a cross-sectional study of UK inflammatory bowel disease services.

OBJECTIVE: Across several clinical specialties, research-active hospitals deliver higher-quality care and achieve better patient outcomes. To date, these associations have primarily been demonstrated using hospital-reported metrics, with limited insight from the patient perspective. This study evaluated, for the first time, the association between research activity within UK inflammatory bowel disease (IBD) hospital services and adult patient-reported quality of care. METHODS: A cross-sectional analysis was conducted using 2023 national IBD UK benchmarking Patient and Service Survey data link

Anastomotic configurations and early endoscopic recurrence following ileocolonic resection in Crohn's disease: systematic review and meta-analysis.

PURPOSE: Crohn's disease (CD) frequently requires surgery, with ileocolonic resection being the most common procedure. Postoperative endoscopic recurrence remains a major concern, and the role of anastomotic configuration is still debated. This systematic review and meta-analysis aimed to evaluate the impact of different anastomotic techniques on early endoscopic recurrence following ileocolonic resection for CD. METHODS: A systematic search of PubMed, Embase, and Web of Science was performed up to July 2025. Studies comparing stapled side-to-side anastomosis (SSA), handsewn end-to-end anasto

Longitudinal Evidence of Sustained Taurine Deficiency in Inflammatory Bowel Disease.

Inflammatory Bowel Diseases (IBD), most notably ulcerative colitis (UC) and Crohn's disease (CD), are long-standing disorders driven by dysregulated immune responses within the gastrointestinal tract and characterized by several metabolic disturbances, which are believed to influence disease progression. We have recently shown that a systemic deficiency of taurine, a semi-essential amino acid with anti-inflammatory properties, marks IBD. To characterize the temporal dynamics and determinants of taurine deficiency in IBD, we conducted a prospective longitudinal study assessing serum taurine lev

Perianal Crohn's Disease in Inflammatory Bowel Disease: Diagnosis, Assessment and Treatment.

Perianal fistulizing Crohn's disease (pfCD) represents one of the most challenging manifestations of CD, often associated with severe phenotypes, refractory luminal inflammation, and a substantial reduction in quality of life. Its pathogenesis is multifactorial and incompletely understood, involving genetic susceptibility, epithelial and stromal dysfunction, and microbiome-related mechanisms. Diagnosis and monitoring rely on advanced imaging, while management requires coordinated medical-surgical strategies. Significant unmet needs persist regarding standardized treatment targets, optimal imag

Treat-to-Target in Ulcerative Colitis: How Soon Is Now?

Ulcerative colitis (UC) is a chronic progressive inflammatory bowel disease, with evolutive potential for extension to the entire colon, development of complications and need for colectomy. Therapeutic goals in UC have moved from symptom control to more stringent outcomes such as endoscopic and histologic remission, which have been observed to correlate with improved long-term outcomes. Disease clearance, a composite endpoint simultaneously including clinical remission, endoscopic and histologic healing, has been recently proposed as the ultimate target. A treat-to-target approach, as endorsed
