Oral Lichenoid Reactions Caused by Mesalazine for Treatment of Ulcerative Colitis: A Case Report and Review of the Literature.

BACKGROUND: Mesalazine, also known as 5-aminosalicylic acid (5-ASA), is commonly used to treat ulcerative colitis and Crohn's disease. While various adverse effects of mesalazine have been reported, oral lichenoid reactions (OLRs) are rarely associated with its use. OBJECTIVE: The aim of this study is to present a rare case of mesalazine-induced OLR and a brief review of the relevant literature. CASE PRESENTATION: A 45-year-old female with ulcerative colitis, treated with mesalazine for 2 years, presented with persistent roughness and burning sensation on the bilateral buccal mucosa for 6 mo

Revolutionizing Crohn's disease detection: integrating AI with intestinal ultrasound for superior diagnosis.

BACKGROUND: Intestinal ultrasound (IUS) is increasingly utilized by healthcare providers for the diagnosis of Crohn's disease (CD), which has highlighted new challenges related to standardized image interpretation and its limitations as a research instrument. The application of artificial intelligence methodologies may offer solutions to these challenges. This study aimed to assess the feasibility of integrating IUS with clinical features to enhance the diagnostic accuracy of CD. We compared the performance of five machine learning (ML) models to diagnose the most effective model for this purp

Fecal microbiome of patients with ulcerative colitis reflects their phenotype and inflammatory level.

Inflammatory bowel diseases affect ever-increasing numbers of individuals worldwide. Alterations of the intestinal microbiome were reported for Crohn's disease and at relapse in Ulcerative Colitis (UC); they were not clearly detected in UC at remission. Here we report the characterization of the microbiome by quantitative metagenomics in a cohort of 121 individuals, composed of 65 UC adult patients in remission and 56 healthy controls. A cross-sectional comparison revealed substantial microbiome differences, patients in remission having lower microbiome richness and paucity of the Ruminococcus

The Longer-Term Impact of Biosimilar Switching Policies in Patients With Inflammatory Bowel Disease in British Columbia, Canada: A Retrospective, Population-Based Study.

BACKGROUND AND AIMS: In 2019, British Columbia (BC) became the first jurisdiction in North America to introduce a mandatory non-medical switching (NMS) policy requiring patients with Crohn's disease (CD) and ulcerative colitis (UC) to be switched from originator biologics to biosimilar equivalents. This study examines the influence of the BC Biosimilars Initiative NMS policy on the longer-term infliximab and adalimumab biosimilar utilization and patterns of continuation/discontinuation of biosimilars. METHODS: We used health administrative data from BC to study drug utilization trends between

High-fat diet exacerbates experimental colitis by inhibiting lysosomal function via the STAT3-TFEB Axis.

An elevated risk for inflammatory bowel disease (IBD) has been linked to the intake of high-fat diet (HFD), yet the underlying molecular mechanisms remain unclear. The lysosome and the macroautophagy/autophagy-lysosome pathway (ALP) are critical for maintaining the intestinal epithelial barrier. By employing both an in vivo model of dextran sulfate sodium (DSS)-induced colitis in mice and an in vitro model using lipopolysaccharide (LPS)-treated NCM460 cells, we established that HFD in vivo and palmitic acid (PA) in vitro profoundly impair epithelial barrier function and amplify inflammation, w

Magnetic resonance imaging-based simplified MaRIA scores are associated with future surgery in Crohn's disease, but modest correlation with ileo-colonoscopic inflammation limits their utility in clinical trials: results from the PROFILE trial.

BACKGROUND: Ileo-colonoscopy provides accurate assessment of mucosal inflammation in Crohn's disease. Magnetic resonance imaging (MRI) offers a non-invasive alternative. MRI-based scores such as the simplified MaRIA (sMaRIA) have been proposed for measuring treatment response in clinical trials, but systematic comparisons with endoscopic scoring systems in prospective datasets are lacking. METHODS: PROFILE trial participants underwent ileo-colonoscopy and small bowel MRI at baseline and week 48 (end-of-trial). Using this dataset our primary objective was to assess the correlation between Simp

Reference-guided genome assembly of long non-coding rna transcripts reveals target genes associated with Crohn's disease.

Crohn's disease (CD) is highly heterogeneous in presentation and progression with no cure. Molecular phenotyping has been used to elucidate cellular and tissue-based alterations to characterize drivers and effects of disease. One currently understudied class of functional molecules is long non-coding RNAs (lncRNAs). Studying the full lncRNA landscape in CD is challenging due in part to an incomplete lncRNA annotation and a lack of their functional characterization in tissues of interest. We used a genome-guided alignment strategy to assemble predicted lncRNA transcripts using short RNA-sequenc

The Role of FOSL1 in Inflammatory Bowel Disease (IBD) Pathogenesis and IBD-Associated Tumorigenesis.

Inflammatory bowel diseases (IBD), including ulcerative colitis and Crohn's disease, are chronic inflammatory disorders of the gastrointestinal tract associated with epithelial barrier dysfunction, dysregulated immune responses, and an increased risk of cancer. Persistent inflammation is a key driver of IBD-associated tumorigenesis, yet the transcriptional regulators linking inflammatory signaling to epithelial transformation remain incompletely defined. FOSL1 (FOS-like antigen 1), a member of the activator protein-1 (AP-1) transcription factor family, has emerged as a critical mediator at the

Role of creatine supplementation in intestinal health: a narrative review of its antioxidant and anti-inflammatory potential.

Inflammatory bowel diseases (IBD) are chronic, idiopathic disorders of the gastrointestinal tract, classified as ulcerative colitis and Crohn's disease. The most common pharmacological therapies include aminosalicylates, glucocorticoids, immunosuppressants, and targeted biological therapies. However, due to their many potential adverse effects, natural compounds such as creatine have also gained attention. Creatine is a compound formed from the amino acids arginine, glycine, and methionine, and can be synthesized endogenously or obtained through food or supplements. Because of its ability to i

A decade of clinical data with vedolizumab: the past, present, and future.

Inflammatory bowel disease (IBD) is a chronic inflammatory disorder of the gastrointestinal tract, typically classified as either ulcerative colitis (UC) or Crohn's disease (CD). The treatment approaches for both UC and CD have changed substantially over the last 10 years, going from step-up to top-down approaches and culminating in treat-to-target algorithms. In addition to the evolution of treatment strategies for IBD, disease endpoints have also changed from symptom-based outcomes to more objective measures, including endoscopy, with histology and transmural healing currently under investig

A Rare Pseudotumoural Bladder Presentation Revealing Crohn's Disease in the Absence of an Enterovesical Fistula: A Case Report.

Urological involvement in Crohn's disease is rare and most often associated with enterovesical fistulas. However, reactive thickening of the bladder wall without true fistulous communication is rarely described and may suggest a malignant bladder tumour. We report the case of a 63-year-old man presenting with lower urinary tract symptoms and gross haematuria. The results of imaging and cystoscopy initially suggested a bladder tumour. Further examinations revealed fistulous Crohn's disease with an inflammatory collection above the bladder causing marked reactive thickening of the bladder dome,

Incidence of Clostridium difficile infection and enteric pathogens in children with inflammatory bowel disease presenting with disease exacerbation.

OBJECTIVE: Pediatric patients with inflammatory bowel disease (IBD) are at increased risk of gastrointestinal infections including Clostridioides difficile infection (CDI), which can trigger disease exacerbations. Since treatment for disease flares and infections differs, identifying the infection and causative microorganisms is crucial. There is limited data on the frequency of CDI in pediatric patients with IBD in our region. This study aimed to determine the incidence of CDI and other enteric infections in pediatric patients with IBD presenting with symptoms suggestive of a flare, and to id

Serum proteomics in paediatric inflammatory bowel disease from a case-control study: biomarker discovery and ulcerative colitis-Crohn's disease differentiation.

BACKGROUND: The diagnosis of inflammatory bowel disease (IBD), ulcerative colitis (UC), and Crohn's disease (CD), relies on clinical and pathological criteria. Non-invasive precision medicine tools to diagnose IBD and discriminate between UC and CD are needed to personalise management. Serum proteomics identified protein biomarkers capable of diagnosing IBD and differentiating CD from UC subtypes. METHODS: We obtained serum samples from a retrospective study of 47 patients with IBD and non-IBD patients seen in a tertiary care paediatric gastroenterology clinic and applied SomaScan proteomics

Predicting Inflammatory Bowel Disease Flares Using Artificial Intelligence and Remote Monitoring: Toward Proactive Disease Management.

Inflammatory bowel disease (IBD), including Crohn's disease and ulcerative colitis, is characterized by a chronic relapsing-remitting course with unpredictable disease flares that significantly contribute to morbidity, impaired quality of life, and increased healthcare utilization. Despite advances in biologic therapies and treat-to-target strategies, disease management remains largely reactive, relying on clinical symptoms and biomarkers that often reflect established inflammation rather than anticipate disease exacerbation. Recent advances in artificial intelligence (AI) and remote monitorin
