Low-volume bowel preparation provides safe and effective pan-enteric capsule endoscopy in suspected or established Crohn's disease.

BACKGROUND: Effective pan-enteric capsule endoscopy (PCE) visualization requires thorough bowel preparation. We hypothesized that a novel low-volume protocol (LVP) provides comparable safety and effectiveness to the standard high-volume protocol (HVP). AIM: To evaluate the safety and effectiveness of a novel low-volume vs standard high-volume preparation protocol for PCE. METHODS: We conducted an observational study including all prospectively documented PCEs at our tertiary center (2018-2021), comparing preparation adequacy and completion rates between patients receiving LVP vs HVP. Prepara

Systematic review and meta-analysis of childhood exposure to antibiotics and the subsequent risk of IBD.

BACKGROUND: Antibiotic use in early childhood may alter the developing microbiome and has been proposed as a risk factor for inflammatory bowel disease (IBD). We conducted a systematic review to examine the association between childhood antibiotic use and subsequent risk of IBD. METHODS: In a systematic literature search, we identified cohort and case-control studies reporting the association between antibiotic use (exposure age <1 to 17 years) and development of IBD. MEDLINE and EMBASE databases were searched from inception through December 31, 2024. Studies reporting a hazard ratio, odds ra

Malassezia globosa lipidome: The dynamics of uptake and secreted lipids.

Malassezia globosa plays a crucial role as part of the human skin's mycobiome. However, this yeast has been detected in other niches, such as the gut. Despite being commensal, the pathogenic link in several dermatological conditions, but recently, chronic diseases such as cancer, Crohn's disease, and Parkinson's disease, among others, have been explored. Lipids can be involved in fungal pathogenesis, and this yeast is characterized by a significant lipid metabolic versatility, with a lack of the complex fatty acid synthase (FAS) required for the de novo synthesis of fatty acids, as it relies o

Serum phosphatidylinositol depletion associates with fecal calprotectin and disease severity in female and male IBD patients.

BACKGROUND: Phosphatidylinositol (PI) is a phospholipid that exerts anti-inflammatory effects when injected during experimental colitis. The levels of PI species in the serum of patients with inflammatory bowel disease (IBD) and their association with disease activity have not yet been determined. This exploratory study investigates whether the levels of PI species in the serum are associated with the severity of the disease. METHODS: Serum concentrations of 14 PI species were assessed using direct flow injection analysis with a triple quadrupole mass spectrometer. The study involved 16 health

Patients' attitudes to disease prevention in inflammatory bowel disease: a US-based survey.

BACKGROUND: Recent advances in biomarkers have identified at-risk cohorts for inflammatory bowel disease (IBD), and potential interception strategies to prevent disease onset are in progress. We sought to understand patient and family members' views on IBD prevention, as they are key stakeholders in future adoption of prevention recommendations. METHODS: A workgroup of patient advocacy organizations and researchers adapted a survey for completion by the IBD community residing in the United States. All responses were anonymous. Descriptive results, and comparisons, were undertaken of pooled re

Type I and III interferons synergize with TNF to promote virally-triggered damage to the intestinal epithelium.

Excessive cell death in the epithelium due to prolonged immune activation is associated with intestinal diseases such as Crohn's disease. Mice with mutations in the Crohn's disease susceptibility gene Atg16l1 are susceptible to inflammation associated with intestinal epithelial cell (IEC) death, including loss of antimicrobial Paneth cells triggered by infection with murine norovirus (MNV). Here, we show that intestinal disease downstream of MNV depends on IFN-α/β and IFN-λ signaling in IECs. In mouse organoids, IFNs synergize with TNF to induce RIPK1-dependent cell death, amplified by ATG16L1

BACKGROUND: Network meta-analysis (NMA) can compare several interventions at once by combining head-to-head and indirect trial evidence. However, identifying, extracting, and modelling these often takes months, delaying updates in many therapeutic areas. OBJECTIVE: To develop and validate MetaMind, an end-to-end, transformer-driven framework that automates NMA processes-including study retrieval, structured data extraction, and meta-analysis execution-while minimizing human input. METHODS: MetaMind integrates Promptriever, a fine-tuned retrieval model, to semantically retrieve high-impact cl

Quorum-Sensing Molecules Are Elevated in Long-Standing Ulcerative Colitis and Are Linked to the Development of Colitis-Associated Cancer.

BACKGROUND & AIMS: Chronic colonic inflammation can lead to colitis-associated cancer (CAC) in ulcerative colitis (UC) patients. The host-microbiome interface plays a critical role in CAC development. Quorum-sensing molecules (QSMs) are bacterial products that regulate bacterial processes. We investigated whether QSMs are related to risk factors for CAC in UC patients and drive CAC development in mouse models. METHODS: Blood samples from UC patients and mouse models of CAC (azoxymethane-dextran sodium sulfate) were analyzed for 3 bacterial QSM classes: short-chain n-acyl homoserine lactones (

Inequities in inflammatory bowel disease care: a Tasmanian case study with proposed solutions.

There is well-recognised inequity of care for people treated with inflammatory bowel disease (IBD) between metropolitan and regional areas. We aim to describe these barriers to care using Tasmania as a case example and to suggest solutions for addressing these barriers that can be used in Tasmania and beyond. Tasmania has some of the highest rates of IBD in the world, with high rates of advanced therapy use reflecting a more complex population. The landscape of IBD care in Tasmania is marked by diverse challenges, some of which are unique to the state and others shared across regional Australi

A novel diagnostic serum protein signature for pediatric inflammatory bowel disease.

OBJECTIVES: Diagnostic delay is common in pediatric inflammatory bowel disease (PIBD), and fecal calprotectin (FCP) is often limited by challenges with sample collection. Therefore, we aimed to identify and validate a blood-based diagnostic protein signature of PIBD. METHODS: Proteins were analyzed using proximity extension assay in plasma samples from treatment-naïve pediatric patients in a Swedish inception cohort referred for suspected inflammatory bowel disease (IBD) and validated in an independent Norwegian population-based pediatric inception cohort. Diagnostic performance was estimated

Intestinal epithelial Smad7 drives purine metabolic dysregulation and ileal inflammation.

BACKGROUND AND AIMS: Defects of the gut epithelial barrier and counter-regulatory mechanisms are a prerequisite for the onset of intestinal inflammation in patients with inflammatory bowel diseases (IBD). Elevated levels of Smad7, an inhibitor of TGF-β1, in both epithelial and immune cells have been associated with the pathological processes observed in IBD. We aim to determine the relevance of Smad7 expression in the epithelial compartment. METHODS: We generated mice overexpressing Smad7 in the gut epithelium (Smad7TgCre+) and monitored pathology development. Inflammatory cell infiltration w

Combining Cytokine-Related Biomarkers to Better Define Tumor Necrosis Factor-α Antagonist Response in Inflammatory Bowel Disease: An Observational Cohort Study.

INTRODUCTION: Interleukin-13 receptor alpha 2 (IL13RA2), triggering receptor expressed on myeloid cells-1 (TREM-1), and oncostatin M (OSM) may be associated with response to tumor necrosis factor-α antagonists (TNFAs) in inflammatory bowel disease. We aimed to assess the direction of association between TNFA-induced clinical remission and IL13RA2 and TREM-1, respectively, and assess the value of combining biomarkers for identifying nonresponders. METHODS: Plasma samples from a retrospective inflammatory bowel disease cohort were collected before TNFA start. Clinical remission at 1-year, surge
