Role of leucine-rich α-2-glycoprotein in Taiwanese patients with inflammatory bowel disease as a predictive biomarker for endoscopic activity.

BACKGROUND: Endoscopy is the gold standard for examining inflammatory bowel disease (IBD), and fecal calprotectin (FC) is a widely used surrogate marker for IBD. However, both methods are considered unpleasant by patients because of their invasiveness and inconvenience. Leucine-rich α-2-glycoprotein (LRG) is a novel serum biomarker that has been previously studied only in Japanese populations. AIM: To evaluate the predictive utility of LRG in a Taiwanese cohort. METHODS: Patients with IBD were prospectively enrolled between 2022 and 2024. Serum and stool samples were collected within 1 month

Development and validation of the IBD frailty score in a multicenter prospective cohort of IBD outpatients.

BACKGROUND: Frailty is a multidimensional syndrome associated with poor outcomes and increased vulnerability, yet its assessment in inflammatory bowel diseases (IBD) remains challenging due to the absence of disease-specific tools. AIMS: This study aimed to develop and validate the IBD Frailty Score, a tailored instrument for evaluating frailty in patients with Crohn disease (CD) and ulcerative colitis (UC). METHODS: In the development phase, 28 categorical items were included in the IBD Frailty Score. This tool was tested in an exploratory cohort of 121 IBD outpatients and later validated i

Association of early vedolizumab trough levels with clinical, biochemical, endoscopic response and drug optimization during maintenance therapy in patients with inflammatory bowel diseases.

The exposure-response relationship between vedolizumab (VDZ) trough levels (VTLs) and efficacy outcomes has been extensively studied, but data on early VTLs in Asian populations are limited. We assessed clinical outcomes and biochemical response using fecal calprotectin (BioRES[FC]) or C-reactive protein (BioRES[CRP]) at week 14 (W14) and W54, endoscopic healing (EH) at available follow-up time points, and the need for drug optimization during maintenance therapy among 67 patients treated with VDZ (39 Crohn’s disease [CD], and 28 ulcerative colitis [UC]). Associations between early VTLs and ou

Evaluating treatment to a target of transmural healing in patients with moderately to severely active Crohn's disease: rationale, design and protocol for the randomised controlled VECTORS trial.

INTRODUCTION: The optimal treatment target for Crohn's disease (CD) is unknown. Targeting transmural healing (TMH) may be associated with a lower risk of complications than clinical and endoscopic outcomes. The Study to Evaluate Transmural Healing as a Treatment Target in Crohn's Disease (VECTORS) aims to investigate TMH as a potential treatment target and its long-term benefits in patients with CD. METHODS AND ANALYSIS: VECTORS is a phase 4, interventional, parallel-group, multicentre, randomised controlled trial. Approximately 304 adult patients with moderately to severely active CD from mu

Life's Essential 8, Genetic Susceptibility, and Metabolic Dysfunction-Associated Steatotic Liver Disease in Chinese Adults.

INTRODUCTION: Crohn's disease is a chronic condition of the gastrointestinal tract. It is characterised by transmural, granulomatous inflammation that occurs in a discontinuous pattern, with a tendency to form fistulae. The cause is unknown but may depend on interactions between genetic predisposition, environmental triggers, and mucosal immunity. METHODS AND OUTCOMES: We conducted a systematic review and aimed to answer the following clinical questions: What are the effects of medical treatments to induce remission in adults with Crohn's disease? What are the effects of surgical intervention

Comparing Stapled and Hand-Sutured Colorectal Anastomoses: A Systematic Review and Meta-Analysis of Observational Cohorts Assessing Short- and Long-Term Complications.

This systematic review and meta-analysis was conducted to compare outcomes after stapled and hand-sewn colorectal anastomoses. The study was undertaken because complication patterns, such as anastomotic leak, surgical site infection (SSI), length of stay (LOS), and mortality, remain debated and may vary by technique. The objective was to evaluate whether either method showed a clear advantage across early and late outcomes. A systematic search of PubMed, Scopus, Web of Science, CINAHL (Cumulative Index to Nursing & Allied Health Literature), and Google Scholar was completed for studies publish

Nomenclature of therapies in inflammatory bowel disease: A journey through time and terminology.

The expanding therapeutic landscape of inflammatory bowel disease has highlighted the need for clear and standardized drug nomenclature to support safe prescribing, pharmacovigilance, international communication, and patient understanding. The World Health Organization's international nonproprietary name system, established in 1953, assigns unique and informative names to medicines. However, the increasing number and diversity of monoclonal antibodies used in inflammatory bowel disease and other diseases have outgrown the capacity of the traditional -mab suffix to convey meaningful structural

The interaction between dendritic cells and T follicular helper cells drives inflammatory bowel disease: a review.

Inflammatory bowel disease (IBD), encompassing Crohn's disease (CD) and ulcerative colitis (UC), has an important pathogenesis that lies in the self-amplifying inflammatory circuit formed by bidirectional interactions between dendritic cells (DCs) and T follicular helper (TFH) cells. This review elucidates that specific mature DC subsets in the intestinal inflammatory microenvironment drive TFH cell differentiation through synergistic co-stimulatory signals (CD80/CD86-CD28, OX40L-OX40) and cytokine networks (IL-12/STAT4/BCL-6, TGF-β/c-Maf/CXCR5); conversely, TFH-derived Lymphotoxin alpha 1 bet
