Stricturing phenotype is associated with an increased risk of postoperative surgical recurrence in isolated small bowel Crohn's disease.

BACKGROUND: Isolated small bowel Crohn's disease (ISBCD) is often associated with poorer clinical outcomes. This study aims to summarize the clinical characteristics of patients with ISBCD and to investigate the risk factors for surgical recurrence due to disease recurrence. METHODS: A retrospective study was conducted using a prospective database of Crohn's patients. Patients with ISBCD were screened and divided into stricturing and nonstricturing groups according to the Montreal classification behavior definition. The primary endpoint was reoperation due to postoperative recurrence of Crohn

Correlation between patient-determined inflammatory bowel disease severity and the inflammatory bowel disease severity index.

AIM: The inflammatory bowel disease severity index (IBD-DSI) aims to describe inflammatory bowel disease (IBD) severity over the duration of disease. We aimed to correlate the IBD-DSI with a patient-completed disease severity score (PCDSS). We also aimed to investigate whether individual factors such as psychological measures or clinical characteristics moderate this correlation. METHODS: This multicentre trial involved gastroenterologists completing the IBD-DSI and Harvey-Bradshaw Index or Simple Clinical Colitis Activity Index for each participant while patients completed a self-reported IB

Whole-Food Dietary Interventions in Crohn's Disease: A Literature Review.

This narrative review examines advances in diet-based therapy for Crohn's disease with a focus on emerging whole-food interventions that have gained attention in the past five years. The aim is to evaluate the current evidence for the Crohn's disease exclusion diet, specific carbohydrate diet, Mediterranean diet, and newer approaches such as CD-TREAT (Crohn's disease treatment with eating diet), low-FODMAP (fermentable oligosaccharides, disaccharides, monosaccharides and polyols) strategies, and plant-based regimens. Drawing on recent randomised controlled trials and mechanistic studies, this

Efficacy and Safety of Risankizumab in a Patient With Complex Crohn's Disease and a High-Output Ileostomy: A Case Report.

The management of Crohn's disease (CD) in patients with a history of multiple abdominal surgeries and a high-output stoma represents a significant therapeutic challenge. Risankizumab, an interleukin-23 inhibitor, has proven efficacy for moderate-to-severe CD, but its role in complex post-surgical scenarios is not well-documented. We present the case of a 46-year-old male patient with longstanding, penetrating, and stricturing ileocolonic CD. Following multiple surgeries, including the creation of a permanent end ileostomy for an anastomotic leak, he developed a high-output stoma. Risankizumab

Recovery of response and long-term outcomes following loss of response and dose escalation of subcutaneous infliximab: a post hoc analysis of the LIBERTY-CD & LIBERTY-UC trials.

BACKGROUND: Rapidity of onset of efficacy following dose escalation of subcutaneous infliximab after loss of response remains unclear in Crohn's disease (CD) and ulcerative colitis (UC). This post hoc analysis of the LIBERTY-CD and LIBERTY-UC trials evaluated time to response recovery following dose escalation of subcutaneous infliximab after loss of response, and characterized patients who experienced early recovery. METHODS: This analysis included week 10 responders to intravenous infliximab induction who were randomized to receive subcutaneous infliximab 120 mg every other week (Q2W) and u

Baseline Double-Balloon Enteroscopy Findings and Long-Term Outcomes in Pediatric Crohn's Disease.

Background  Pediatric Crohn's disease is frequently characterized by extensive small-bowel involvement and a dynamic disease course. Double-balloon enteroscopy (DBE) allows direct evaluation of proximal small-bowel lesions that may not be detected by conventional upper gastrointestinal endoscopy and ileocolonoscopy; however, the long-term clinical implications of baseline DBE findings remain poorly defined. Methods We conducted a retrospective, single-center longitudinal cohort study including pediatric patients initially diagnosed with Crohn's disease who underwent baseline DBE. Clinical and

Awareness of Inflammatory Bowel Disease in Bahrain: A Survey-Based Study.

BACKGROUND: Limited awareness of inflammatory bowel disease (IBD) can lead to diagnostic delays and increased risk of complications. Although awareness levels may vary across demographic and educational groups, there is a notable paucity of research investigating IBD awareness in Bahrain. To address this gap, the present study aims to evaluate the awareness levels of IBD among the Bahraini population. METHODOLOGY: A cross-sectional survey was conducted at 10 primary healthcare centers in Bahrain using a multistage sampling technique. Adult individuals without a prior history of IBD were invit

Lower Extremity Arterial Occlusive Disease and Abdominal Aortic Aneurysm in Crohn's Disease: A Case Report.

Crohn's disease (CD) is a type of inflammatory bowel disease that primarily affects the small intestine and colon. Patients with CD may develop extra-intestinal manifestations; however, vascular involvement has been rarely reported. Herein, we present the case of a patient with CD who developed multiple arterial complications, including lower extremity arterial occlusions and an abdominal aortic aneurysm (AAA). A 42-year-old man with CD presented with back pain. He also had a history of bilateral iliac artery occlusions that were refractory to revascularization procedures. Enhanced computed to

Pulmonary Nodules and Splenic Lesions as Rare Extraintestinal Manifestations of Crohn's Disease.

Extraintestinal manifestations of Crohn's disease are rare but should be considered in patients with newly discovered lesions in organs not typically affected by inflammatory bowel disease (IBD). Here, we describe a case of a patient who presented with abdominal pain due to splenic lesions, with incidental discovery of pulmonary nodules. The patient was also found to have methicillin-sensitive Staphylococcus aureus (MSSA) bacteremia on presentation, raising initial concern for septic emboli to the lungs and spleen. However, the patient's abdominal pain did not improve, and the splenic lesions

Fatigue in pediatric patients with inflammatory bowel disease: a multicenter study by the SEGHNP.

BACKGROUND: Fatigue is a common and disabling symptom in pediatric inflammatory bowel disease (IBD), often persisting even during clinical remission and reflecting a multifactorial origin. Despite its significant impact on patients' lives, it remains under-recognized. The IMPACT-III and IMPACT-III-P questionnaires facilitate fatigue assessment within a biopsychosocial framework of health-related quality of life (HRQOL). METHODS: In this multicenter study supported by the Spanish Society of Pediatric Gastroenterology, Hepatology and Nutrition (SEGHNP), 382 patients aged 10-17 years and their c

Analysis of possible baseline and treatment-course factors associated with non-remission in patients with Crohn's disease treated with ustekinumab: a retrospective real-life analysis.

BACKGROUND: Factors predicting persistent failure to achieve long-term clinical remission with ustekinumab (UST) in Crohn’s disease (CD) are poorly defined, limiting early identification of patients unlikely to benefit from continuation of an ineffective treatment. METHODS: A multicentre retrospective registry was established comprising adult patients with CD treated with UST (induction as per guidelines and maintenance 90 mg every 8 weeks), including clinico-demographic variables and disease activity parameters. These variables were retrospectively collected at baseline (T0) and at 2 (T1), 3

Two-Dimensional PCR for Simultaneous Detection of Fecal Bacterial Markers in the Diagnosis and Management of Inflammatory Bowel Disease.

BACKGROUND: Inflammatory bowel disease (IBD) is showing an increasing incidence in China, posing a significant threat to public health. IBD patients exhibit gut microbiota dysbiosis, and detection of IBD-associated gut microbes can aid in diagnosis and management. METHODS: We developed a 2D-PCR method based on base-quenching probe technology, which allowed closed-tube, single-tube simultaneous detection of four IBD-associated microbes: Ruminococcus gnavus, Veillonella spp. Ruminococcus torques, and Clostridioides difficile. Clinical fecal samples were tested including 224 IBD patients, 112 pa

Advanced Medical Therapies for Perianal Fistulizing Crohn's Disease: A Systematic Review of Clinical, Radiological, Surgical, and Composite Outcomes.

Background: Perianal fistulizing Crohn's disease (CD) is associated with significant morbidity and remains difficult to treat. Although advanced medical therapies are widely used, much of the available evidence derives from heterogeneous fistula populations or luminal CD trials, with limited perianal-specific synthesis and inconsistent outcome definitions. We conducted a systematic review focusing exclusively on perianal-specific clinical, radiologic, and composite outcomes in adults with perianal fistula (PAF) CD. Methods: We performed a systematic review in accordance with PRISMA 2020. Elect
