The Role of Microbiota and Fecal Transplantation in Inflammatory Bowel Disease.

Inflammatory bowel diseases (IBDs), including ulcerative colitis (UC) and Crohn's disease (CD), are consistently associated with alterations in gut microbial communities, although the extent and characteristics of these alterations vary across studies, supporting a potential role of the microbiota in disease pathogenesis and therapeutic modulation. We conducted a systematic review to synthesize current evidence on microbiota alterations in IBD and the clinical application of fecal microbiota transplantation (FMT). A total of 118 studies were included (76 focused on microbiota profiling and 42

Diagnostic Challenges and Management of Crohn's Disease: The Value of Capsule Endoscopy.

Crohn's disease (CD) can be difficult to diagnose, particularly in its early stages when symptoms lack chronicity. This report describes a male patient with persistent right lower quadrant (RLQ) pain and weight loss despite an appendectomy. Despite normal biopsies and imaging, symptoms continued, prompting further evaluation with capsule endoscopy (CE), which revealed distal small bowel inflammation supporting the diagnosis of CD. Treatment with mesalamine and a specific carbohydrate diet (SCD) led to symptom resolution. However, after discontinuing treatment, the patient developed disease pro

High-Grade Small Bowel Dysplasia Following Prior Adenocarcinoma in Small Bowel Crohn's Disease.

Patients with Crohn's disease are at increased risk of small bowel adenocarcinoma due to chronic inflammation, though absolute incidence is low and surveillance guidelines are unclear. We report a patient with long-standing small bowel Crohn's disease and prior adenocarcinoma who developed high-grade dysplasia despite clinical, biochemical, and endoscopic remission on biologic therapy. An asymptomatic rise in fecal calprotectin prompted imaging and device-assisted enteroscopy, revealing a non-traversable stricture. Surgical resection confirmed high-grade dysplasia with negative margins. This c

Development and validation of a predictive nomogram for response to biologics and targeted therapy in Crohn's disease: a retrospective cohort study.

BACKGROUND: Biologics and targeted therapies are a first-line treatment option for individuals with Crohn's disease (CD). Nevertheless, clinical remission rates vary considerably among different biologics and targeted therapies. This study aims to develop and validate a predictive nomogram for the response to biologics and targeted therapy in individuals with CD. METHODS: This study included individuals with CD admitted to the First People's Hospital of Wuhu from January 2020 to June 2025. Clinical remission rates at 14 and 26 weeks were observed. The clinical remission rates of different bio

Dual Pathology: A Case of Concurrent Lymphocytic Esophagitis and Crohn's Disease.

Crohn's disease (CD) is a chronic, systemic disease characterized by transmural inflammation of the gastrointestinal (GI) tract. Patients can present with a range of GI and extraintestinal features depending on the areas involved. Esophageal involvement can be a direct result of Crohn's or other co-existing diseases. Determining the cause of esophagitis in CD is important as it affects management and prognosis. Lymphocytic esophagitis (LE) is an example of a disease that can cause esophagitis on a background of CD. Patients presenting with the two conditions simultaneously are relatively rare.

When Tuberculosis Bleeds: Intestinal Tuberculosis Presenting With Hematochezia.

Intestinal tuberculosis is an uncommon but clinically significant manifestation of tuberculosis that often presents with nonspecific gastrointestinal symptoms. Although the ileocecal region is most frequently involved, overt gastrointestinal bleeding is an unusual presentation, particularly in elderly patients who may lack classic constitutional symptoms. An elderly man presented to a community hospital in Guam with hematochezia one week after the diagnosis of pulmonary tuberculosis and initiation of rifampin, isoniazid, pyrazinamide, and ethambutol. Colonoscopy revealed a large semi-circumfer

Successful Pregnancy Outcome in a Patient With Crohn's Disease and Prior Intestinal Surgery: A Case Report.

Crohn's disease (CD) is a chronic autoimmune disorder of the GI system that primarily affects young individuals of reproductive age and is often associated with pregnancy. This case report describes the successful outcome of a pregnancy in a 32-year-old primigravida woman with CD, who had undergone enterectomy five years prior. At the time of conception and throughout the entire pregnancy, the disease was in remission under pharmacological treatment. The pregnancy, under systematic monitoring, progressed smoothly for the mother, fetus, and neonate. After completion of the 38th week of gestatio

Fecal Microbiota Transplantation in Inflammatory Bowel Disease: A Systematic Review of Efficacy and Safety.

Inflammatory bowel disease (IBD), comprising ulcerative colitis (UC) and Crohn's disease (CD), is a chronic inflammatory condition of the gastrointestinal tract associated with immune dysregulation and alterations in the gut microbiota. Growing evidence suggests that intestinal microbial dysbiosis plays an important role in disease pathogenesis, prompting interest in microbiome-targeted therapies, such as fecal microbiota transplantation (FMT). This systematic review aimed to evaluate the efficacy and safety of FMT in adult patients with IBD. A comprehensive literature search was conducted in

The multifaceted regulation of autophagy protein ATG16L1 and its implications in human diseases.

ATG16L1 (autophagy related 16 like 1) is a core macroautophagy/autophagy protein essential for autophagosome formation. It also functions in non-canonical autophagy pathways such as LC3-associated phagocytosis (LAP) and in other processes including immunity, inflammation, and membrane trafficking. This review synthesizes recent advances and proposes that ATG16L1 functions as a central molecular integrator governed by a multi-layered regulatory code. This framework includes genetic polymorphisms, transcriptional control, and diverse post-transcriptional and post-translational mechanisms. We det

Kernel-smoothed permutation for extreme P-value estimation in genetic association studies.

In genetic association studies, permutation tests serve as a cornerstone to estimate P-values. This is because researchers may design new test statistics without a known closed-form distribution, or the assumption of a well-established test may not hold. However, permutation tests require a vast number of permutations, which is proportional to the magnitude of the actual P-values. When it comes to genome-wide association studies where multiple-test corrections are routinely conducted, the actual P-values are extremely small, requiring a daunting number of permutations that may be beyond the av

Pattern-based histologic approach in colitis with chronic architectural damage: GIPAD recommendations.

Histologic evaluation of colonic biopsies in patients with chronic diarrhea or long-standing colitis is frequently performed in the setting of incomplete clinical, endoscopic, or laboratory information. In this context, overlapping morphologic features across different etiologies often lead to nonspecific or misleading diagnoses, such as "chronic colitis," with potential implications for patient management. To improve diagnostic clarity and clinicopathologic communication, the Italian Group of Digestive Disease Pathology (GIPAD) developed a structured, pattern-based histologic approach for the

WBT-DC pipeline: a cross-cohort and cross-platform disease classification pipeline based on whole-blood transcriptomics.

BACKGROUND: Machine-learning models based on tissue transcriptomic data are powerful tools for disease classification. However, their clinical adoption is limited by the invasive nature of tissue sampling. Furthermore, transcriptomic datasets are often affected by batch effects and gene-level noise, which compromise model generalizability across platforms and clinical cohorts. METHODS: We developed WBT-DC (Whole Blood Transcriptomics-based Disease Classification), a computational pipeline designed to overcome these challenges. WBT-DC integrates rank-based feature extraction to mitigate batch
