Inflammatory bowel disease in the older patient.

This paper presents a review of 64 patients in whom the first symptoms of inflammatory bowel disease occurred after the age of 60 years. Inflammatory bowel disease in older patients affects the large bowel much more commonly than the small bowel. The commonest variant is disease localized in the rectum and distal colon, and many of these patients have Crohn's disease (14 out of 28 patients). In 19 patients with localized left-sided colitis it was difficult to distinguish ischaemic colitis, acute diverticulitis and Crohn's disease. Patients with left-sided colitis or total colitis (15 patients)

HLA antigens in inflammatory bowel disease.

HLA antigens were studied in 60 patients with ulcerative colitis, 10 with Crohn's disease and 120 healthy Japanese controls. HLA typing was performed by the microdroplet lymphocyte cytotoxicity test.B5 was recognized in 62% of the patients with ulcerative colitis, whereas in 40% of controls (p smaller than 0.01). Furthermore, in these patients B7 was decreased in incidence (p smaller than 0.05) and there was the highest relative risk value for A3 (R.R = 6.3). When the patients were classified into 2 groups according to the extent of the lesions, left-sided or total colitis group and proctitis

Studies on blood coagulation in ulcerative colitis and Crohn's disease.

An investigation was made on 23 aspects of coagulation in 14 cases of ulcerative colitis, 5 cases of Crohn's disease and 3 cases of related diseases using a classification of increased and decreased coagulability. Of the 14 cases of ulcerative colitis, 10 were total colitis and 4 were left-sided colitis; 12 were on the active stage and 2 were in a state of remission. Treatment at the time of the investigation included prednisolone and/or Salazopyrin. In these cases, increased fibrinogen content, increased Factor VIII and Factor IX activity, increased platelet count, accelerated platelet aggreg

Radiographic appearances in Crohn's disease. II. The course as reflected at repeat radiography.

The course and distribution of the lesions in 174 patients with Crohn's disease were evaluated from preoperative films. In the small intestine the course was locally progressive. Colonic disease involved either the right hemicolon or the entire colon. Fluctuations of mucosal lesions were common and could explain the radiographic finding of segmental and left-sided colitis. Variations in the anatomy of the lymphatic system could explain the varying distribution of the lesions.

Cancer in universal and left-sided ulcerative colitis: factors determining risk.

A retrospective study of 267 patients with ulcerative colitis admitted to The Mount Sinai Hospital during the period 1960--1976 revealed 26 (9.7%) with adenocarcinoma of the colon. Twenty-one cases of colorectal cancer were observed among 158 patients with universal colitis (13%), and 5 occurred among 109 patients with left-sided disease (5%). Patients with left-sided disease tended to develop cancer at least a decade later than patients with universal disease. The median duration from onset of colitis to diagnosis of cancer was 20 yr for those with universal colitis, and 32 yr for those with

Serial carcinoembryonic antigen (CEA) blood levels in patients with ulcerative colitis.

Fifty-seven patients with ulcerative colitis were folloued 1-49 months (mean, 18 months) with serial CEA determinations during periods of remission, mild relapses, and severe relapses. Elevated CEA titers correlated with activity and possibly extent of disease: 12% of patients with proctitis, 47% of patients with left-sided colitis, and 60% of patients with transverse or universal colitis had elevated CEA titers during a flare. Moreover, 24% of patients with mild flares and 86% of patients with severe flares had elevated CEA titers. Ninety-two percent of patients with extensive disease and sev
