Bedside analysis of volatile organic compounds in exhaled human breath as non-invasive diagnostic tool for colon diseases.

BACKGROUND: Measurements of volatile organic compounds (VOCs) in exhaled air represent a promising noninvasive biomarker modality for gastrointestinal diseases and has potential to enable point-of-care diagnostics. AIM: To evaluate the feasibility of differential ion mobility spectrometry (DMS) combined with machine learning to distinguish patients with colon diseases from healthy controls based on VOCs analysis in breath. METHODS: Breath samples were collected from 50 participants, including patients with inflammatory bowel disease (IBD), diverticulosis and healthy controls. Exhaled VOC pro

Time trends in patients with ulcerative colitis and crohn's disease not receiving IBD-specific pharmacological treatment within one and three years of diagnosis - a nationwide cohort study.

BACKGROUND AND AIMS: Most patients with Crohn's disease (CD) or ulcerative colitis (UC) receive medication at diagnosis, but some do not. We examined 25-year trends in the proportion of newly diagnosed IBD patients not receiving any pharmacologic treatment within one or three years of diagnosis. METHODS: Using national health registries, we identified all incident CD and UC patients in Denmark from 1996-2020. IBD-specific medication use was captured from one month before diagnosis through one and three years after diagnosis. Patients with no recorded IBD-specific pharmacological treatment wer

Anemoside B4 from Pulsatilla chinensis Ameliorates Ulcerative Colitis by Inhibiting Inflammatory Responses via the Modulation of the Gut Microbiota-Immune Interplay.

We previously demonstrated that anemoside B4 (AB4) exerts a beneficial effect in a dextran sulfate sodium (DSS)-induced murine model of ulcerative colitis (UC). In this study, we further explored the mechanism of AB4 against UC from the perspective of gut microbiota-immune interplay. The gut microbiota was depleted in two DSS-induced colitis models to investigate the modulatory effects of the gut microbiome on immune responses and the activity of AB4. In parallel, adoptive transfer of CD4[Formula: see text]CD62L[Formula: see text] T cells into Rag1-deficient (Rag1-/-) mice was used to elucidat

Proanthocyanidins in immune-mediated inflammatory diseases: molecular mechanisms and therapeutic potential.

Immune-mediated inflammatory diseases (IMIDs), a heterogeneous group of disorders, are characterized by chronic inflammation and subsequent organ damage, encompassing diverse conditions like atopic dermatitis (AD), inflammatory bowel disease (IBD) - which includes Crohn's disease (CD) and ulcerative colitis (UC), psoriasis, rheumatoid arthritis (RA), and systemic lupus erythematosus (SLE). These chronic diseases mainly stem from inappropriate or excessive immune responses. In recent years, the incidence of IMIDs has been increasing. Considering their frequent association with various complicat

A rare case of lesser omental hernia in a male patient following total colectomy.

INTRODUCTION AND IMPORTANCE: A lesser omental hernia is a rare internal hernia in which intestinal loops become entrapped through a defect in the lesser omentum, potentially causing bowel obstruction. Its diagnosis is challenging because of nonspecific symptoms that resemble those of other gastrointestinal diseases. This report presents a case of lesser omental hernia following total colectomy. CASE PRESENTATION: A 65-year-old male with a history of laparoscopic total colectomy for ulcerative colitis 4 years prior presented with acute abdominal pain and distension. Computed tomography (CT) re

Unraveling the Clinical Puzzle of Cerebral Venous Sinus Thrombosis in Ulcerative Colitis: Insights from a Systematic Review.

Ulcerative colitis (UC) is a chronic inflammatory bowel disease (IBD) that primarily affects the colon. Among its extraintestinal complications, cerebral venous sinus thrombosis (CVST) is a rare yet potentially life-threatening emergency that can manifest as headaches, seizures, and/or neurological deficits. This review aims to characterize the clinical presentation, diagnostic modalities, and management strategies for CVST in UC patients. A comprehensive search of case reports and series yielded 1,156 studies, of which 39 met the inclusion criteria. A total of 47 patients were identified; the

Challenges and opportunities facing inflammatory bowel disease in Sub-Saharan Africa: towards a contextually relevant research and practice agenda.

Inflammatory bowel disease (IBD), comprising Crohn's disease and ulcerative colitis, has traditionally been considered a disease of Western, industrialized countries. Over the past few decades, however, many Sub-Saharan African (SSA) countries have experienced significant increases in cases. Yet, IBD remains highly neglected in SSA with most research conducted in high-income settings and treatment guidelines geared towards these settings. An emerging body of research on the sub-continent is highlighting several unique realities, dilemmas, and priorities facing IBD, and the need for evidence-ba

Infliximab Serum Level Assessment After Switching from Intravenous to Subcutaneous Administration: The ISLASIS Study.

INTRODUCTION: Subcutaneous infliximab (SC-IFX) has emerged as an alternative maintenance therapy for inflammatory bowel disease (IBD), offering absorption and sustained systemic exposure. Although higher and more stable serum concentrations have been reported with SC-IFX, prospective real-world data evaluating pharmacokinetic evolution after transition from intravenous infliximab (IV-IFX) remain limited, particularly in Latin America. We assessed infliximab trough concentrations following transition from IV-IFX to SC-IFX and explored associations with biomarkers and clinical outcomes. METHODS

Current evidence for first-line advanced therapies in pediatric ulcerative colitis: a critical review.

BACKGROUND: The therapeutic landscape of pediatric ulcerative colitis (UC) has evolved substantially with the introduction of multiple advanced therapies, creating new challenges in selecting the most appropriate initial treatment. While infliximab has long served as the reference biologic, biologics with different mechanisms of action and small-molecule therapies have expanded therapeutic options. However, the pediatric evidence supporting first-line advanced therapy remains heterogeneous across currently available agents. METHODS: This review critically summarizes the current pediatric evid

New Perspectives on STAT3 in Ulcerative Colitis: Molecular Mechanisms and Dual Roles.

In recent years, the global incidence of ulcerative colitis has been on the rise. As a core hub in the pathogenesis of UC, the extensive aberrant activation of signal transducer and activator of transcription 3 is a crucial cause of exacerbated intestinal inflammation, immune dysregulation, and imbalanced cell proliferation and differentiation. In this review, we systematically summarize the multidimensional regulatory mechanisms of STAT3 in UC, and further explore its dual roles in intestinal barrier function and mucosal immunity along with the underlying mechanisms, clarifying that its funct

Ulcerative colitis, but not Crohn's disease, displays demographically and clinically driven mucosal immune heterogeneity: implications for patient stratification.

BACKGROUND: Inflammatory bowel disease (IBD) encompasses Crohn's disease (CD) and ulcerative colitis (UC), two conditions with distinct immunopathological profiles despite shared inflammatory pathways. Comparative mucosal and systemic immune characterisation across disease subtypes and clinical variables remains limited. This study aimed to map leukocyte dysregulation in tissue and blood from patients with CD and UC compared to controls. METHODS: Intestinal tissue and blood samples were obtained from 35 controls and endoscopically active IBD patients, including 54 with CD and 74 with UC. Leuk

Plasma lipopolysaccharide-binding protein (LBP), a proxy for intestinal permeability, relates to clinicopathological and immunological disease activity in inflammatory bowel disease patients at diagnosis and during disease course.

BACKGROUND AND AIMS: Intestinal barrier healing is the treatment goal in ulcerative colitis (UC) and Crohn's disease (CD). Despite this, systematic analyses of the relationship between intestinal permeability and clinicopathological and immunological disease activity at diagnosis and during follow-up are lacking. Therefore, we assessed this relationship using plasma lipopolysaccharide-binding protein (LBP)-an indirect biomarker of systemic exposure to bacterial endotoxins-as a proxy for intestinal permeability, in a uniquely well-characterized pediatric inflammatory bowel disease (IBD) cohort.

Pouchitis Incidence Following IPAA: Multicenter comparison of Open and Minimally Invasive Surgical Approaches.

BACKGROUND: Pouchitis is the most common long-term complication following ileal pouch-anal anastomosis (IPAA) for ulcerative colitis (UC) and is a leading cause of chronic morbidity and pouch failure. We sought to examine whether the surgical approach, open versus minimally invasive (MIS), may influence the incidence of pouchitis post-IPAA surgery in patients with UC. METHODS: We conducted a retrospective cohort study using the Epic COSMOS electronic health record database (2016-2025) to identify UC patients who underwent IPAA. The cohort was stratified by open vs MIS. Demographics were compa

Kaempferol-3-O-glucorhamnoside alleviates DSS-induced ulcerative colitis via the arginine-polyamine metabolism pathway.

Ulcerative colitis (UC) is a chronic, relapsing condition with a rising global prevalence. At present, there is a lack of therapeutic strategies that provide both high efficacy and minimal adverse effects. Kaempferol-3-O-glucorhamnoside (KA), a natural compound derived from Thesium chinense Turcz., has demonstrated significant anti-inflammatory properties. Nevertheless, its therapeutic potential and the precise molecular mechanisms underlying its effects in UC remain unexplored. In this study, KA administration effectively ameliorated a dextran sulfate sodium (DSS)-induced murine model of UC,

Contemporary multicentre validation and comparative performance of Day 1 prognostic scores in acute severe ulcerative colitis.

BACKGROUND AND AIMS: Early identification of steroid non-response in acute severe ulcerative colitis (ASUC) is critical to guide timely escalation of therapy. Several Day 1 prognostic scores have been proposed, but their comparative performance in contemporary cohorts remains uncertain, with limited direct head-to-head validation in real-world populations. We aimed to validate and directly compare the performance of previously proposed Day 1 prognostic scores, including the Admission Model for Intensification of Therapy in Acute Severe Colitis (ADMIT-ASC), the Albumin-C reactive protein-Endosc

The burden of inflammatory bowel disease in Asia (1990-2023) and projections to 2050: A comprehensive analysis of the 2023 global burden of disease study.

BACKGROUND: Inflammatory bowel disease (IBD), including ulcerative colitis (UC) and Crohn's disease (CD), is becoming an increasingly important public health concern in Asia, with rising incidence and prevalence reported across the region. Despite progress in global burden studies, data gaps remain in Asia due to variations in healthcare and socio-demographic factors. This study aims to address these gaps by assessing the burden of IBD across Asia from 1990 to 2023, focusing on regional trends in UC and CD. METHOD: We utilized data from the Global Burden of Diseases, Injuries, and Risk Factor

eCONUT-UC for nutritional screening and malnutrition identification in ulcerative colitis.

BACKGROUND: Detecting malnutrition in patients with ulcerative colitis is considered a clinical challenge. The Global Leadership Initiative on Malnutrition (GLIM) criteria constitute a standardized approach for diagnosis. However, the Controlling Nutritional Status (CONUT) score is a simple tool based on laboratory analysis; its performance in early malnutrition detection could be improved. Our aim was to exploratorily derive and assess the apparent diagnostic performance of enhanced CONUT-based scores (eCONUT-UC) for identifying GLIM-defined malnutrition in patients with ulcerative colitis.

Norepinephrine-driven neutrophilic inflammation accelerates ulcerative colitis progression.

OBJECTIVES: The role of the sympathetic neurotransmitter norepinephrine (NE) in ulcerative colitis (UC) remains controversial. We aimed to elucidate NE's precise mechanisms in UC, focusing on colonic metabolism, diagnostic potential, and local immune interactions. METHODS: Colonic biopsies from active UC patients and DSS-colitis mice were profiled. Neurotransmitter levels, including NE, were quantified by targeted metabolomics. An NE-pathway diagnostic model was constructed from bulk RNA-seq data using machine learning. In vivo, NE was administered via enema or intraperitoneal injection at gr
