Design and Optimization of Acridine-Based p65 G-Quadruplex Stabilizers for the Treatment of Inflammation-Induced Lung Injury and Ulcerative Colitis.

Aberrant NF-κB activation drives the pathogenesis of inflammatory diseases. Stabilizing the p65 promoter G-quadruplex (p65 G4) with small molecules to suppress p65 transcription represents a promising anti-inflammatory strategy. Herein, guided by the classical G4 stabilizer BRACO-19 as a hit scaffold, we designed and synthesized 39 new acridine derivatives. Among them, C38 exhibited superior p65 G4 stabilization and anti-inflammatory activity compared to BRACO-19. Mechanistic studies demonstrated that C38 selectively induced and stabilized the p65 G4 structure, thereby inhibiting p65 transcrip

An innovative colon inflammation-targeting platform for ulcerative colitis treatment.

The therapeutic options for patients with inflammatory bowel disease (IBD) have recently expanded substantially. Tofacitinib, an oral small-molecule inhibitor of Janus kinase (JAK), recently FDA approved for ulcerative colitis, represented a paradigm shift in a field previously dominated by biologics. Tofacitinib, however, carries a black box warning for major adverse cardiovascular events, an elevated risk of malignancies, risk of blood clots, and pulmonary embolism. In addition, tofacitinib is avoided in pregnancy and used with caution in women of child bearing age due to potential risks to

Multi-cohort meta-analysis, single-cell and Mendelian randomization analyses with experimental validation identify RHBDF2 as a shared mediator of ulcerative colitis and blood-brain barrier disruption.

BACKGROUND: Ulcerative colitis (UC) is increasingly associated with central nervous system comorbidities, including cognitive decline and dementia, yet the molecular mechanisms linking intestinal inflammation to brain dysfunction remain unclear. Blood-brain barrier (BBB) disruption is a plausible convergence point, but the mediators involved are largely unknown. METHODS: We performed random-effects meta-analyses of five human UC bulk-transcriptome cohorts (1,130 UC and 508 control samples) and three mouse BBB-disruption (BBBD) cohorts, followed by cross-species orthologue mapping to identify

Biologic and small-molecule therapies for inflammatory bowel disease: mechanisms, comparative evidence and the emerging pipeline.

Inflammatory bowel disease (IBD), comprising ulcerative colitis (UC) and Crohn's disease (CD), is a chronic immune-mediated disorder of rising global prevalence, with newly industrialised nations now entering the phase of most rapid growth. Its therapeutic landscape has transformed since 2020. Three selective interleukin (IL)-23p19 antagonists have been approved for both UC and CD; two sphingosine-1-phosphate receptor modulators and a second-generation Janus kinase (JAK) inhibitor have entered routine practice; subcutaneous formulations have altered delivery; and the first formal head-to-head

Engineering extracellular vesicles for inflammatory bowel disease and colitis-associated colorectal cancer: from therapeutic innovation to clinical translation.

Inflammatory bowel disease (IBD), encompassing Crohn's disease and ulcerative colitis, is a chronic relapsing inflammatory disorder that increases the risk of colitis-associated colorectal cancer (CAC). Despite major advances in biologics and small-molecule therapies, many patients experience primary or secondary treatment failure, adverse effects, and disease recurrence, underscoring the need for more effective, targeted therapeutic strategies. Extracellular vesicles (EVs), including exosomes, microvesicles, and apoptotic bodies, have emerged as promising nanoscale therapeutic platforms owing

KCTD10 Modulates DSS-induced Ulcerative Colitis via Regulating Intestinal Stem Cell Function.

The incidence of inflammatory bowel disease (IBD) continues to rise annually, which severely threatens public health. While estimates indicate that current therapies cause a 30% IBD clinical treatment failure, the precise pathogenesis remains unclear. Here, we found that a key embryonic development regulator, KCTD10, is correlated with the severity of IBD. We observed enhanced KCTD10 expression during enterocolitis. Intestinal epithelial cell (IEC)-specific knockout of KCTD10 in mice impaired intestinal epithelial barrier function, activated intestinal adaptive immune response, and disrupted i

Anti-Tumor Necrosis Factor-Alpha Associated Kaposi Sarcoma in a Patient with Ulcerative Colitis: A Case Report and Literature Review.

Kaposi sarcoma (KS) is a rare vascular malignancy associated with human herpesvirus-8 (HHV-8). Tumor necrosis factor-alpha inhibitors (anti-TNF-α agents) are widely used in inflammatory bowel diseases such as ulcerative colitis (UC). While these biologics are effective, they carry a risk of opportunistic infections and, rarely, malignancies. We report a rare case of KS developing in a patient with UC following infliximab treatment. A 67-year-old man with a 3-year history of moderate to severe UC, maintained on azathioprine and mesalazine, initiated induction therapy with infliximab (four 100 m

Blood Neutrophil to Lymphocyte Ratio as a Marker of Inflammatory Bowel Disease Severity in Iranian Adults: a Cross-Sectional Survey.

INTRODUCTION: Given the increase in the prevalence of inflammatory bowel diseases (IBD) in developing countries like Iran, studies to find new markers for disease monitoring seem necessary. Thus, we aimed to evaluate the value of a novel non-invasive marker, the neutrophil-to-lymphocyte ratio (NLR), in assessing the severity and activity of IBD in adults. METHODS: We collected retrospective data from adult patients with a history of ulcerative colitis (UC) or Crohn's disease (CD) who attended the clinic, gathering information from one month prior to their enrollment. The data included demogra

Intestinal epithelial memory: from inflammatory imprinting to metabolite-shaped immune tolerance.

Intestinal epithelial cells are rapidly renewing barrier cells that maintain compartmentalization between the luminal environment and the mucosal immune system through epithelial transport, antimicrobial defense, secretory programs, and cytokine-mediated communication. Emerging evidence suggests that the intestinal epithelium can also retain durable imprints of prior inflammatory, microbial, or metabolic exposures. We refer to this phenomenon as intestinal epithelial memory. This form of memory is likely encoded within stem and progenitor compartments through persistent changes in chromatin ac

Assessing inflammation and predicting relapse in ulcerative colitis is more accurate when performed by IBD specialized gastroenterologists.

BACKGROUND: Ulcerative colitis is monitored through clinical, laboratory, endoscopic, and histologic measures. Persistence of histologic inflammation despite clinical and endoscopic remission is associated with disease flare. METHODS: We compared the identification of clinically significant histologic inflammation by general and IBD-specialized gastroenterologists (GI) using a simplified binary scoring system and looked at associations with disease relapse. RESULTS: Both general and IBD-specialized GIs identified significant inflammation associated with relapse, though IBD-specialized GIs sc

Response surface-optimized Cycas revoluta leaf extract attenuates experimental ulcerative colitis through anti-inflammatory and oxidative stress-related mechanisms.

Cycas revoluta Thunb. has traditionally been used for the management of gastrointestinal and inflammatory disorders, but its bioactive constituents and anti-inflammatory potential remain insufficiently characterized. This study aimed to optimize the extraction of phenolic-rich compounds from C. revoluta leaves and evaluate the anti-inflammatory and coloprotective effects of the optimized extract against experimental ulcerative colitis (UC). Four extraction techniques, namely Soxhlet extraction, maceration, ultrasound-assisted extraction (UAE), and microwave-assisted extraction (MAE), were comp

Ebastine attenuates DSS-induced ulcerative colitis via modulation of NF-κB, oxidative stress and pro-inflammatory cytokines: evidence for antihistamine repurposing.

Inflammatory bowel disease (IBD) is a chronic inflammatory disorder of the gastrointestinal tract, driven by immune dysregulation, oxidative stress, and the release of pro-inflammatory cytokines. Histamine, an important mediator in gut inflammation, has immunomodulatory properties. Ebastine, a second-generation histamine H1-receptor antagonist, is primarily used for allergic conditions, but its potential role in managing IBD has not been fully explored. This study aimed to evaluate whether ebastine could prevent colitis induced by dextran sodium sulfate (DSS) in mice. The severity of the condi

A macrophage-targeted TIMD4 Peptide-Nanoparticle attenuates colitis symptoms and monocyte recruitment in a mouse model.

Macrophages play a pivotal role in the pathogenesis of inflammatory bowel disease (IBD). T-cell immunoglobulin and mucin domain-containing molecule 4 (TIMD4) is involved in the regulation of tissue-resident macrophages. TIMD4 expression is elevated in colonic tissues from patients with ulcerative colitis (UC) and mice with dextran sulfate sodium (DSS)-induced colitis, but its role in IBD pathogenesis remains unclear. In this study, we found that Timd4 deficiency aggravated DSS-induced colitis, accompanied by increased infiltration of monocytes into the colonic tissues. Treatment with a CCR2 an

Bidirectional associations between depression and inflammatory bowel disease in a nationwide Korean cohort.

Inflammatory bowel disease (IBD) and depression frequently co-occur, and previous studies have suggested temporal associations in both directions. We conducted two complementary matched cohort studies using the Korean National Health Insurance Service database to further characterize these associations in a nationwide Korean population. In Study I, 11,725 patients with ulcerative colitis (UC) and 2918 patients with Crohn's disease (CD) were matched with 58,572 controls without IBD to assess incident depression. In Study II, 622,131 individuals with depression were matched 1:1 with 622,131 cont

Microbiota-Taurine Pathway Links Dietary Fiber to Mucosal Healing.

BACKGROUND AND AIMS: Dietary fibers modulate host-microbiota interactions. Whether fiber promotes epithelial repair through microbiota-dependent mechanisms remains unclear. We determined how alginate influences colitis recovery and whether the pathway is relevant to ulcerative colitis (UC). METHODS: We screened fibers in dextran sodium sulfate (DSS)-induced murine colitis and assessed survival, inflammation, barrier function, and epithelial regeneration. Microbiota dependence was evaluated using antibiotics and bacterial supplementation. Metabolites were profiled using capillary electrophores

Comparative efficacy and safety of subcutaneous vedolizumab versus other targeted inflammatory bowel therapies in patients with moderate to severe ulcerative colitis: a network meta-analysis update.

BACKGROUND: The growing number of targeted inflammatory bowel disease (IBD) therapies available for the treatment of moderate to severe ulcerative colitis has increased the complexity of treatment decisions for clinicians. Clinical decision-making is further hindered by a paucity of head-to-head clinical trials comparing the relative efficacy and safety of available targeted IBD therapies; however, network meta-analyses (NMAs) can indirectly compare treatments. METHODS: Networks of evidence were formed from 29 randomized controlled trials of targeted IBD therapies. To address differences in s
