BACKGROUND/AIM: Ulcerative colitis (UC) and Crohn's disease (CD) are chronic immune-mediated conditions associated with a substantial clinical and economic burden. Italian epidemiological evidence is limited. Mesalazine remains a cornerstone of UC management. This study aimed to provide updated epidemiological estimates of inflammatory bowel disease (IBD) in Italy, and to evaluate real-world treatment patterns, adherence to oral mesalazine, and healthcare costs among patients with UC initiating mesalazine therapy.
METHODS: A retrospective observational analysis was conducted using Italian adm

Ulcerative colitis (UC) remission does not abolish susceptibility to metabolic dysfunction-associated steatotic liver disease (MASLD), but the mechanism linking prior intestinal inflammation to delayed liver injury remains unclear. We establish gut-liver axis memory transduction as an epigenetic-cellular-organ cascade in which inflammatory history is molecularly encoded and redeployed across organs. LCN2+ colonic macrophages persist after colitis resolution and retain heightened glycolysis together with durable histone H3K9 L-lactylation (H3K9la). These macrophages maintain a permissive memory

UNLABELLED: CD28 costimulation contributes to pathogenic T cell responses in inflammatory bowel disease (IBD), but current B7-directed blockade also limits CTLA-4 signaling. Using a sensitive NanoBiT split-luciferase screening platform, we identified and optimized CA-23, a small molecule antagonist that directly binds human and mouse CD28 without measurable binding to CD80, CD86, or CTLA-4. CA-23 inhibited CD28-B7 engagement and CD28-dependent T cell activation without agonist activity in human whole blood and peripheral blood mononuclear cells. CA-23 achieved exposure in the colon and mesente

Magnetic resonance enterography (MRE) is a key noninvasive imaging modality for the diagnosis and longitudinal assessment of inflammatory bowel disease (IBD), particularly Crohn's disease. The global burden of IBD is increasing, with millions affected in North America and Europe and rapidly rising incidence in Asia and Africa. Standardized acquisition protocols improve consistency and diagnostic accuracy. As therapeutic options expand and patient outcomes improve, MRE interpretation has evolved beyond detection toward disease phenotype characterization and prognostic indicator assessment. Inte

Purpose To characterize the clinical presentation, disease course, and outcomes of patients with concurrent primary sclerosing cholangitis (PSC) and celiac disease (CeD), including those with concomitant inflammatory bowel disease (IBD). Methods Retrospective EHR analysis (MGB RPDR, BIDMC Celiac Center, All of Us) identified patients with confirmed PSC and CeD (ICD-10, encounter threshold > 1, validated clinically/serologically/histologically). Primary outcomes were hepatic complications and time to liver transplantation, compared against two 3:1 age-matched cohorts (PSC + IBD, n = 54; PSC-onl

BACKGROUND AND AIMS: Fatigue is common in inflammatory bowel disease (IBD), but prior work has largely focused on symptomatic disease activity rather than progressively objective remission states. We evaluated the relationship between fatigue and hierarchical remission depth, repeated changes in remission status, and patients' attribution of fatigue to IBD.
METHODS: Adults with confirmed IBD completed PROMIS Fatigue surveys at 2 tertiary-care centers. PROMIS Fatigue was analyzed continuously and as clinically significant fatigue (T-score ≥55). Remission was classified hierarchically as no rem

BACKGROUND: Sphingosine-1-phosphate (S1P) receptor modulators are used for immune-mediated diseases, and macular oedema is an established ocular adverse effect. However, ocular-focused, class-wide postmarketing reporting across all five approved agents remains incompletely characterized.
METHODS: We analyzed U.S. Food and Drug Administration Adverse Event Reporting System (FAERS) data from 2004 Q1 through 2026 Q1 using report-level deduplication, approval-aligned windows, and primary-suspect exposure. Ocular events were assessed using a curated set of 52 Preferred Term (PT) strings. Sensitivi

BACKGROUND: Ulcerative colitis (UC) is a chronic mucosal immune disorder in which 30-50% of patients fail biologic therapy, yet the full-thickness immune architecture of medically refractory disease and reliable tissue-level markers of treatment response remain undefined.
METHODS: Using 10x Visium HD spatial transcriptomics (16-µm bins; 615,274 bins), we generated a full-thickness spatial atlas of surgically resected colon from patients with medically refractory UC (n=3) and controls (n=3), integrating niche-stratified differential expression, spatial cell-cell communication, SCENIC regulon i

INTRODUCTION: Atopic dermatitis (AD) and inflammatory bowel diseases (IBD), including Crohn's disease (CD) and ulcerative colitis (UC), are chronic immune-mediated disorders with overlapping immunopathogenic mechanisms. Limited research has investigated clinical features of AD and concomitant IBD.
OBJECTIVES: This study aimed to evaluate the clinical presentation of adult patients with concomitant moderate-to-severe AD and IBD.
METHODS: Adult patients with moderate-to-severe AD and confirmed IBD were identified across 12 hospital-based dermatology centers between 2022 and 2024 by using elect

Xanthogranulomatous pyelonephritis (XGPN) is a rare, chronic granulomatous inflammation that leads to diffuse destruction of the renal parenchyma. This destructive process may be so profound as to infiltrate surrounding tissues and viscera. Although nephrectomy remains the most common option for managing XGPN in adults, we report a case of XGPN in a horseshoe kidney of a 27-year-old woman on active immunosuppressive therapy for inflammatory bowel disease, namely ulcerative colitis. This case was successfully treated conservatively with antimicrobials, with complete resolution of the renal mass

Biotherapeutics are required to achieve high remission rates in patients with severe ulcerative colitis (UC); however, adverse effects, complex dosing regimens, administration routes, and low patient compliance limit their widespread clinical use. Given the localized nature of UC, this study aimed to develop localized delivery strategies for infliximab (IFX), an anti-tumor necrosis factor-α (TNF-α) monoclonal antibody recommended for moderately-to-severely active UC. Exploiting the intrinsic biocompatibility, mucoadhesion, and protein-entrapment capacity of lipid mesophases (LMPs), IFX was enc

The role of eicosapentaenoic acid (EPA) in improving ulcerative colitis (UC) has been extensively studied. EPA-acylated astaxanthin (EA), a novel fish oil, has unclear effects on UC. This study constructed a DSS-induced chronic-like relapsing colitis model in mice induced by 2% dextran sulfate sodium (DSS) and primarily concentrated on the changes in gut microbiota and intestinal bile acids (BA) to investigate the anti-UC mechanisms of EA. The results indicated that EA effectively upregulated the expressions of tight junction proteins, ZO-1 and Occludin, while restraining the triggering of the

BACKGROUND/AIMS: Inflammatory bowel diseases (IBDs), including Crohn's disease (CD) and ulcerative colitis (UC), show variable treatment responses. Data on the prevalence and clinical predictors of difficult-to-treat IBD (DTT-IBD) among Asians receiving advanced therapies (ATs) remain limited. This study evaluated the proportion of DTT-IBD and its risk factors in a Korean IBD cohort.
METHODS: We conducted a retrospective analysis of a prospectively maintained tertiary-care cohort including patients with UC or CD who received at least one AT. Kaplan-Meier analysis, log-rank tests, and Cox prop

BACKGROUND: Quantitative assessment of intestinal inflammation among patients with inflammatory bowel disease (IBD) remains a clinical challenge. Conventional histopathology lacks robust quantification, and a real-time, noninvasive alternative is needed. This study aimed to establish a quantitative histopathological system for IBD and to develop a corresponding optical coherence tomography (OCT) system for real-time detection.
METHODS: Murine models for both ulcerative colitis (UC) and Crohn disease (CD) were developed, and corresponding intestinal histopathological samples were obtained. Art

A patient with ulcerative colitis presented with progressive memory impairment and alexia, with brain imaging showing a mass-like lesion mimicking glioma. Histopathology confirmed central nervous system (CNS) vasculitis. The patient's memory and reading ability improved and finally recovered following corticosteroid treatment, with no recurrence during the long-term follow-up. The case report and literature review highlight that clinicians should consider CNS vasculitis in patients with ulcerative colitis and tumor-like brain lesions.

The gut microbiota is increasingly recognized as a key mediator of the anticolitis effects of curcumin (CUR). However, the clinical translation of this promising activity is severely hampered by CUR's low solubility and poor bioavailability. Although nanoencapsulation represents a promising strategy to address these limitations, the preventive effect of nanocurcumin (nano-CUR) against colitis remains largely unexplored. The present study aimed to elucidate the preventive effects of nano-CUR on intestinal barrier function, gut microbiota composition, and metabolic profiles in a murine model of
