Epiploic appendagitis: An overlooked cause of acute abdominal pain.

BACKGROUND: Epiploic appendagitis is a rare, often underrecognized cause of acute abdominal pain. Misdiagnosis can lead to unnecessary hospitalization, antibiotic use, or surgical intervention. Advances in imaging have improved the recognition of this self-limiting condition, but clinical awareness remains critical. AIM: To provide a comprehensive update on the epidemiology, anatomy, pathogenesis, clinical presentation, diagnostic strategies, differential diagnosis, and management of epiploic appendagitis, emphasizing its distinguishing features from other causes of acute abdomen. METHODS: A

Ultrasonic-assisted three-phase partitioning with deep eutectic solvents for extraction of Nostoc commune Vauch polysaccharide and its anti-ulcerative colitis activity.

This study explores the extraction of polysaccharides from Nostoc commune Vauch. using ultrasonic-assisted three-phase partitioning with deep eutectic solvents (UA-TPP-DES). Response surface methodology was used to determine the optimized UA-TPP-DES conditions as follows: a 1: 2 molar ratio of lauric acid to terpineol, 30 min of ultrasonication at 60 °C with 100 W power, 20 % moisture content, 20 % w/w (NH4)2SO4 concentration, and a 2: 1 top-to-bottom phase volume ratio. The UA-TPP-DES method significantly improved the polysaccharide extraction efficiency of N. commune (yielding NCP-DES-3, 7.3

Pharmacokinetic Profiling Using 3H-Labeled Eggshell Membrane and Effects of Eggshell Membrane and Lysozyme Oral Supplementation on DSS-Induced Colitis and Human Gut Microbiota.

Eggshell membrane (ESM) is composed of approximately 90% protein. Our previous studies in healthy adults demonstrated that two months of daily ESM intake improved respiratory function, zigzag walking speed, and skin elasticity. The present study aims to address the knowledge gap regarding the in vivo effects of ESM in the context of inflammatory bowel disease (IBD). Proteomic analysis was performed on powdered ESM used as a dietary supplement. To investigate its pharmacokinetics in mice, tritium (3H)-labeled ESM was prepared using the 6Li(n,α)3H nuclear reaction. The therapeutic potential of E

Functional characterization of novel anti-DEFA5 monoclonal antibody clones 1A8 and 4F5 in inflammatory bowel disease colitis tissues.

BACKGROUND: The aberrant expression of α defensin 5 (DEFA5) protein in colonic inflammatory bowel diseases (IBDs) underlies the distinct pathogenesis of Crohn's colitis (CC). It can serve as a biomarker for differentiating CC from Ulcerative colitis (UC), particularly in Indeterminate colitis (IC) cases into UC and CC. We evaluated the specificity of commercially available anti-DEFA5 antibodies, emphasizing the need to further validate their appropriateness for a given application and highlighting the necessity for novel antibodies. METHODS: We established two mice monoclonal DEFA5 antibody c

Comparative outcomes of laparoscopic and open surgery in inflammatory bowel disease in pediatric and young adult patients: a systematic review and meta-analysis.

BACKGROUND: Pediatric inflammatory bowel disease (IBD), encompassing Crohn's disease, ulcerative colitis, and indeterminate colitis, often necessitates surgical intervention in cases of severe or refractory disease. Although biologic therapies have significantly reduced the need for surgery, operative management remains essential for certain patients. The choice between laparoscopic (laparoscopy group [LG]) and open conventional surgery (open group [OG]) continues to be a subject of debate. This meta-analysis aimed to compare the postoperative outcomes of LG and OG in pediatric patients with I

Patient knowledge and acceptance of the upcoming automatic substitution of biological medicines in community pharmacies: survey of Finnish patients with inflammatory bowel disease and psoriasis.

BACKGROUND: Automatic substitution of biological medicines in community pharmacies is among the policy measures to contain increasing pharmaceutical costs, with gradual implementation starting in Finland in 2024. It is important to study patient knowledge of biological medicines, biosimilars, and substitution, as they have an impact on the success of automatic substitution. However, there is a paucity of studies focusing on patients’ perspectives on automatic substitution in pharmacies, and the existing studies report varying patients’ opinions. Thus, the aim of the study was to (1) describe p

Risk of malnutrition increases in the year prior to surgery among patients with inflammatory bowel disease.

BACKGROUND: In patients with inflammatory bowel disease (IBD) who need intestinal resection, prior data suggest that earlier surgical intervention may be associated with improved outcomes. However, surgery is often deferred for additional trials of advanced therapies, which potentially shifts patients from a fit to a frail preoperative state. OBJECTIVES: This study aimed to evaluate clinical changes that occur in the year prior to intestinal resection in patients with IBD. DESIGN: Retrospective cohort study. METHODS: This was a multi-hospital retrospective study of patients ⩾18 years old wh

Penoscrotal swelling as a presentation of Crohn's disease.

Metastatic Crohn's disease (MCD) is a rare extraintestinal manifestation of Crohn's disease (CD), particularly in pediatric patients. It refers to cutaneous involvement at areas distant and non-contiguous from the bowel. We present a boy with a 1.5-year history of perianal lesions, penoscrotal swelling, and intermittent hematochezia. Examination revealed hepatosplenomegaly, perianal erythematous plaques, and inguinoscrotal swelling. Biopsy of the scrotal skin revealed non-caseating granulomatous inflammation, suggestive of MCD. Stool calprotectin was elevated, however, gastrointestinal biopsie

Immunological Linkages Between Inflammatory Bowel Diseases and Type 2 Diabetes.

Background: Inflammatory bowel disease (IBDs) are chronic, immune-mediated disorders of the gastrointestinal tract, encompassing ulcerative colitis (UC) and Crohn's disease (CD), and primarily affecting individuals with genetic susceptibility. IBDs are characterized by dysregulated mucosal immune responses to intestinal microbiota, leading to sustained inflammation and tissue damage. These conditions not only pose a significant burden on healthcare systems but are also frequently associated with distinct comorbidities. Rationale: Given the immunological nature of both IBDs and type 2 diabetes

Structural and epitope characterization of anti-DEFA5 monoclonal antibodies clones 1A8 and 4F5 for inflammatory bowel disease subtype diagnostics.

Understanding antigen-antibody interactions is key to advancing immunodiagnostics and therapeutic development. Here, we characterize two mouse monoclonal antibodies, 1A8 and 4F5, specific to human DEFA5, an innate immune effector implicated in differentiating colonic inflammatory bowel disease (IBD) subtypes, particularly Ulcerative colitis, Crohn's colitis and Indeterminate colitis. Although both antibodies target similar regions of DEFA5, they exhibit distinct binding affinities. Hydrogen/deuterium exchange mass spectrometry (HDX-MS) revealed diffuse protection patterns with partial protecti

Secure Ileal Pouch-Anal Anastomosis for Histologic Indeterminate Colitis.

Background/Objectives: Indeterminate colitis (IC) is an erroneous diagnosis for predominantly colonic inflammatory bowel disease (IBD) when there is a non-definitive foundation of the benchmark for ulcerative colitis (UC) and Crohn's colitis (CC) after a combined state-of-the-art classification system of clinical, endoscopic, radiologic, and histologic tools are used. This confounds an effective surgical regimen; specifically pouch surgery, "the restorative proctocolectomy with ileal pouch-anal anastomosis (PRC-IPAA)". Transforming the distinction between UC and CC in otherwise IC into authent

Janus kinase 1 inhibitors for treating immune checkpoint inhibitor-induced enterocolitis - report of two filgotinib-treated cases and literature review.

BACKGROUND AND PURPOSE: Treatment of cancer with immune checkpoint inhibitors (ICIs) entails a risk of immune-related adverse events (irAEs). Data on the treatment of immune-related enterocolitis (irEC) in patients with inadequate symptom control after treatment with standard therapy including corticosteroids, infliximab, and vedolizumab are scarce. Based on limited data, recommendations include treatment with the pan-Janus kinase (JAK) inhibitor tofacitinib. Filgotinib is a more recently developed JAK inhibitor with preferential inhibition of JAK1, which might imply a more favorable safety pr
