Intestinal Ultrasound for Inflammatory Bowel Disease: Expanding the Role of Point-of-Care Ultrasound in the Emergency Department.

Patients with inflammatory bowel disease (IBD) frequently utilize emergency department (ED) care. Intestinal ultrasound (IUS) is a noninvasive, point-of-care, radiation-free imaging modality increasingly used in outpatient IBD management. The purpose of this article is to introduce the use of IUS for the evaluation of IBD to emergency medicine physicians who utilize ultrasound as part of their clinical practice. Studies demonstrate that IUS is highly sensitive and specific for detecting active bowel inflammation. Key sonographic findings include increased bowel wall thickness and bowel wall hy

Ultrasound-assisted extraction of novel ancient tea polysaccharide NATP-60a: Process optimization, structural characterization, and mechanism of alleviating DSS-induced colitis in mice.

Plant-derived polysaccharides are considered potentially effective treatments for colitis due to their safety and anti-inflammatory properties. This study used leaves ofancient tea trees endemic to southwestern China as raw materials, established an optimized ultrasound-assisted extraction (UAE) process, subsequently isolated and purified a novel polysaccharide NATP-60a, and systematically investigated its alleviating effect on colitis induced by oral administration of dextran sulfate sodium (DSS) in mice. The results showed that under the optimal UAE process conditions, the yield of crude pol

Chronic paternal exposure to low-dose OBS reprograms progeny's intestinal cholesterol metabolism and increases IBD susceptibility.

Sodium p-perfluorous nonenoxybenzenesulfonate (OBS) as a novel alternative to perfluorooctane sulfonate (PFOS) has been extensively used in numerous manufacturing processes, contributing to increasingly grim environmental contamination. Abundant evidence has highlighted the endocrine and metabolic-disrupting properties of OBS, establishing it as an unsafe surrogate for PFOS. However, the intergenerational toxicity of OBS, particularly the impact of paternal exposure on offspring, remains unexplored. Using a murine model, we demonstrated that chronic paternal exposure to low-dose OBS led to gut

High-Shielding and Colon-Targeted Insect Resin Microgels for Enhanced Probiotic Viability and Colitis Alleviation.

The in vivo functionality of oral probiotics is often compromised by their susceptibility to environmental and gastrointestinal stressors, coupled with limited mucosal retention. To overcome these challenges, we developed a smart, pH-responsive biomaterial platform based on shellac microgels (SCM), derived from a natural, biocompatible insect resin. Combining experimental characterization with finite element analysis, we verified that the densified molecular network of SCM functions as a potent physical barrier that effectively hinders the diffusion of O2, moisture, and UV radiation, as well a

Design, synthesis, and in vitro application of a two-photon fluorescent probe for fluorescent imaging and in vitro biological evaluation of ulcerative colitis.

The clinical diagnosis and management of ulcerative colitis (UC) face considerable challenges, including invasiveness, insufficient sensitivity for early detection, and significant side effects associated with current therapeutics. Based on the altered expression of the TGR5 receptor during UC progression and the anti-inflammatory potential of its endogenous agonists (bile acids), we designed and synthesized a bile acid-derived two-photon fluorescent probe, CATPF-1. Through molecular docking-based screening, CATPF-1 was obtained and exhibited excellent photophysical properties, including a flu

[Transabdominal ultrasound - a method of assessing IBD].

Intestinal ultrasound (IUS), which is performed as transabdominal ultrasound, is a method of assessing inflammatory bowel disease (IBD) that has gained worldwide recognition in recent years. It is widely adopted by gastroenterologists in large parts of Europe and North America, and its implementation continues to spread to new countries. IUS is non-invasive, complication-free, does not entail radiation, is cost-effective, does not have contraindications and is appreciated by patients. It does not require any patient preparation and can be used as a point-of-care examination. Energy and materia

Orally Administered Nanoparticle Coacervate for Therapeutic Coating of Full Gastrointestinal Tract.

Oral delivery of therapeutic nanoparticles offers a non-invasive approach for treating gastrointestinal diseases, but achieving localized and sustained mucosal exposure while avoiding systemic toxicity remains a major challenge. Here, a Sucralfate-based oral formulation is developed that incorporates therapeutic nanoparticles and transforms into a paste-like coacervate upon contact with intestinal fluid, uniformly coating the entire gastrointestinal tract within 6 h, and clearing through fecal transit by 12 h. Systematic investigation shows that nanoparticle hydrophobicity is essential for sta

Nitrate-responsive genetically engineered probiotics locally release TNF-α nanobodies against inflammatory bowel disease.

The clinical potential of oral genetically engineered probiotics is widely recognized, which is usually distinguished by the release of expressed active molecules. However, the impact of the release mode of bacterial payloads remains unknown. Here, we propose a novel release mode, "producing and storing first, then responding and releasing", in a genetically engineered probiotic. Inflammatory bowel disease is a typical chronic intestinal disease, where nitrate in the intestinal route is highly produced. Escherichia coli Nissle 1917 (EcN) was genetically engineered to highly express TNF-α nanob

Near-Infrared-Responsive Poly(lactic-co-glycolic acid)-Coated Prussian Blue Nanocubes Promote Alleviation of Colitis in an Inflammatory Bowel Disease Model via Multiplexed Real-Time Photoacoustic Imaging.

Ulcerative colitis (UC), a chronic inflammatory bowel disease, is characterized by persistent gastrointestinal inflammation and epithelial damage in the colon and rectum, often resulting in rectal bleeding and an increased risk of colorectal cancer. Existing therapies are still insufficient to fully reverse mucosal injury or restore normal physiological homeostasis. This study aimed to develop a nanoparticle-based therapeutic platform as a potential alternative treatment for UC. Near-infrared-active Prussian blue nanocubes (PBNCs) coated with poly lactic-co-glycolic acid (PLGA) were synthesize

Optimization of deep eutectic solvents-based ultrasonic-assisted three-phase partitioning extraction of polysaccharides from Pleurotus tuber-regium and its anti-ulcerative colitis activity.

Based on the various anti-inflammatory and antioxidant bioactivities of Pleurotus tuber-regium and its polysaccharides (PTPs), they represent promising candidates for complementary or alternative therapeutic strategies against Ulcerative Colitis (UC), but conventional extraction methods are often inefficient or environmentally unfriendly. This study presents a novel integrated deep eutectic solvents (DESs)-based ultrasonic-assisted three-phase partitioning (TPP) extraction technique of PTPs to solve the dual challenges of inefficient traditional extraction methods and the unexplored therapeuti

Manipulating metabolism-reprogrammed monocytic-MDSCs prevents colitis-associated dysplasia by IL-10/HIF-1α/DLL4 signaling.

Dysplasia has been described in various inflammatory environments. However, the mechanisms underlying the dysplastic transformation of the intestinal epithelium and the increased risk of colorectal cancer in colitis patients are not yet fully understood. In this study, we observed that IL-10 was negatively correlated with aberrant proliferation and differentiation of colonic epithelium in colitis patients. Deficiency of myeloid IL-10 resulted in a marked accumulation of intestinal myeloid-derived suppressor cells (MDSCs) and colitis-associated dysplasia, which could be mitigated by intra-bone

Proline-Mediated Inhibition of ATPIF1-mTOR Signaling Alleviates Radiation-Induced Macrophage Polarization and Colon Inflammation.

Radiation therapy often triggers inflammatory complications such as radiation colitis, which is driven by persistent M1 macrophage polarization. While metabolic reprogramming is pivotal in macrophage activation, the role of amino acid metabolism-particularly proline-in regulating radiation-induced inflammation remains unexplored. In this study, bone marrow-derived macrophages (BMDMs) and RAW264.7 cells exposed to 4 Gy radiation exhibited robust M1 polarization (increasing from 13.5% in controls to 23.7% in F4/80+CD86+ cells), elevated pro-inflammatory cytokines (TNF-α, IL-1β, IL-6; 3- to 6-fol

In Vivo Microendoscopy in the Near-Infrared II Window.

Non-invasive fluorescence endoscopy of deep biological tissues beyond the mucosa, or trans-intestinal imaging in live mammals with high spatiotemporal resolution, remains challenging due to light scattering. Here, we developed a near-infrared II (NIR-II) microendoscopy with imaging wavelength extended to 1700 nm, enabling deep intestinal imaging beyond the mucosa and transrectal imaging at sub-10-µm resolution without any invasive surgery. It facilitated real-time visualization of the relative motion of vascular networks across different layers of the mouse rectum. Featuring a specially design

Compound Kushen injection improves M1 macrophage polarisation and radiation-induced colitis by regulating Piezo1-mediated NF-κB/NLRP3 pathway.

BACKGROUND: Radiation-induced colitis (RIC) is a common complication following radiotherapy for pelvic and abdominal malignancies. Dysregulated polarisation of macrophages towards the pro-inflammatory classically activated M1 phenotype is a key driver of RIC progression. PURPOSE: This study sought to examine the protective influence of compound Kushen injection (CKI) against RIC and clarify the mechanisms underlying these protective effects. METHODS AND RESULTS: Mice were subjected to whole-abdominal irradiation (14 Gy) to model the clinical characteristics of RIC. The mice in the treatment

Okra Flower Polysaccharides Ameliorate Dextran Sulfate Sodium-Induced Ulcerative Colitis by Modulating ABC Transporters Via the Gut Microbiota.

Current treatments for ulcerative colitis (UC) are limited by insufficient efficacy and potential side effects. This study aimed to investigate the prophylactic efficacy of Abelmoschus esculentus (L.) Moench flower polysaccharide extract (APE) against dextran sulfate sodium (DSS)-induced UC in mice. APE substantially ameliorated DSS-induced UC symptoms, intestinal barrier disruption, and concurrent microbiota dysbiosis with associated metabolite shifts. Gut microbiota depletion using antibiotic treatment removed the alleviative effect of APE against UC. Kyoto Encyclopedia of Genes and Genomes

Red-light-activated living bacterial electron generator for on-demand drug release in colonic inflammation.

Inflammatory bowel disease demands spatiotemporally precise drug delivery, yet the variable gut redox environment limits stimuli-responsive nanocarriers. Here we report a living biohybrid platform in which optogenetically engineered Shewanella oneidensis MR-1 is electrostatically conjugated with azo-bond covalent organic frameworks (TA-COFs) loaded with anti-inflammatory drugs magnolol or 4-iodobenzoic acid. Under intestinal conditions and non-invasive red-light irradiation (660 nm), light-induced restoration of the metal-reducing pathway promotes extracellular electron transfer, thereby cleav
