Hemorrhagic manifestations in primary DENV-3 infection: A case of dengue with warning signs in an adolescent.

Severe dengue is typically linked to secondary infection. We report dengue with warning signs due to primary DENV-3 infection in a healthy Italian adolescent returning from the Philippines. Severe thrombocytopenia and mucosal bleeding, with clinical and laboratory features suggestive of possible plasma leakage, were managed without shock, highlighting the need for multidisciplinary monitoring and care even in primary dengue.

Detection of Dengue Virus and Serological Evidence of Chikungunya and Zika Virus Exposure in Patients with Acute Febrile Syndrome in Córdoba, Colombia.

Background/Objectives: Arboviral diseases transmitted by Aedes mosquitoes, including Dengue (DENV), Zika (ZIKV), and Chikungunya (CHIKV), represent a major public health challenge in tropical regions. Their clinical similarity complicates differential diagnosis, particularly in settings of viral co-circulation, and may lead to underdiagnosis. The objective was to detect acute dengue infection and assess serological evidence of Chikungunya and Zika virus exposure among patients with acute febrile syndrome and clinical suspicion of dengue in the department of Córdoba, Colombia. Methods: A prospe

Hyperferritinemia in hospitalized children with dengue-a prospective observational study.

Dengue is a major cause of pediatric hospitalization in tropical countries and low- and middle-income countries (LMICs), where early recognition of severe disease remains challenging. Severe dengue is characterized by dysregulated immune activation and systemic inflammation, leading to organ dysfunction. Ferritin, an acute-phase reactant, has been increasingly studied as a potential biomarker of dengue severity, although pediatric data from LMIC settings remain limited. The aim of this study was to assess the prevalence of hyperferritinemia among children admitted with dengue with warning sign

Progression of dengue during an outbreak (2024) in Dhaka, Bangladesh: a hospital based longitudinal study.

BACKGROUND: Bangladesh has experienced recurrent dengue outbreaks since 2000, driven by climatic changes and socio-demographic vulnerabilities. Limited evidence on circulating serotypes and immune diversity has hindered preparedness for severe dengue (SD). Identifying clinical and laboratory predictors of progression to severe dengue is key to strengthening outbreak response and clinical management. METHODS: A longitudinal cohort study was conducted in four hospitals in Dhaka between January and November 2024. Patients with confirmed dengue with warning signs were enrolled. Clinical data and

Acute-Phase Dengue Antibody Profiles in Pediatric Patients: Influence on Viremia and Disease Manifestations.

Secondary dengue infections are often linked to more severe clinical outcomes, yet pre-existing antibodies may exert either protective or pathogenic effects. To evaluate the role of acute-phase dengue antibodies in pediatric dengue, we analyzed clinical and laboratory features, viremia, and antibody profiles in children infected with DENV-1. We conducted a retrospective study of patients under 18 years diagnosed with DENV-1 in Salta, Argentina. Viremia was quantified by real-time RT-PCR; acute-phase IgG antibodies (within 7 days from symptom onset) against DENV, DENV-1, and DENV-NS1 were measu

A multicenter prospective observational multi-omics study protocol to identify biomarkers for severe dengue: COMBAT study clinical protocol.

BACKGROUND: Dengue virus (DENV) infection poses a major global public health burden, particularly in endemic regions where repeated exposure increases the risk of severe disease. Despite revisions to the World Health Organization (WHO) dengue classification, early prediction of progression to severe dengue remains challenging due to overlapping clinical and laboratory features. Current management strategies rely primarily on supportive care and reactive monitoring, underscoring the need for predictive biomarkers that enable early risk stratification and timely intervention. METHODS: COMBAT is

Plasma-derived extracellular vesicle microRNA as a potential biomarker associated with disease progression in non-severe dengue infection.

BACKGROUND: Dengue can manifest as a mild febrile illness to life-threatening haemorrhagic fever. The early identification of patients at risk of disease progression remains challenging, particularly in non-severe cases. Therefore, this study aims to investigate the capability of extracellular vesicle plasma microRNA (miRNA) expression as a biomarker in dengue infection. METHODOLOGY: A total of 52 dengue patients (26 dengue with warning signs and 26 dengue without warning signs) were enrolled. Microarray analysis was utilised to profile miRNA derived from plasma extracellular vesicles to iden

Monocyte TLR2/4 as predictive biomarkers for progression from dengue warning signs to severe disease.

BACKGROUND: The World Health Organization (WHO) classification system, comprising dengue fever (DF), dengue with warning signs (DWS), and severe dengue (SD), has been widely adopted globally; however, clinical management remains challenging. Current warning sign criteria lack specificity, frequently failing to distinguish patients progressing to severe disease from those with transient illness, thereby contributing to delayed interventions and missed therapeutic windows. Although Toll-like receptors (TLRs) mediate innate antiviral immunity and are implicated in severe dengue pathogenesis, thei

Evaluation of Hematological Indices, Their Longitudinal Trends, and Association With Stratified Dengue Severity in Pediatric Cohorts: A Prospective Observational Study.

BACKGROUND: Dengue fever is a widely prevalent viral disease, with one of the most common laboratory findings being thrombocytopenia. Although platelet indices like mean platelet volume (MPV), platelet distribution width (PDW), platelet large cell ratio (P-LCR), and plateletcrit (PCT) are routinely measured, their utility in clinical practice has been limited. This study was planned to evaluate platelet indices across the range of disease severity, understand their importance in terms of their trends over the disease course, and correlate them with other hematological parameters in pediatric d

Comparison of clinical and laboratory manifestations of dengue in multicenter cohorts from southern Taiwan during the 2014-2015 and 2023 outbreaks.

BACKGROUND: Dengue is a mosquito-borne disease that caused a large outbreak in Taiwan in 2023. Some published studies about this outbreak suggest higher disease severity while we observed the opposite epidemiological trend in the southern part of Taiwan. Therefore, we conducted comparative analysis for clinical manifestation and disease severity between two outbreak periods. METHODS: We retrospectively analyzed 4009 cases diagnosed at one medical center and two regional hospitals in Taiwan, including 3588 cases from 2014 to 2015 and 421 cases from 2023. Moderate-to-severe dengue was defined a

Role of Cellular Mechanisms in Dengue Pathogenesis: Focus on Immune Cells Interactions.

Dengue is an arbovirus mosquito-borne disease that occurs after an infection with dengue virus. Dengue virus releases E-proteins, which act as binding proteins and enter the host cell after infection. It triggers several cellular reactions and activates the immune system; however, the mechanisms are still poorly understood. Our goal is to find out how these cellular interactions participate in the activation of immune cells and participate in dengue pathogenesis. Once dengue infects the host cell, it follows these steps: (1) dengue virus releases M- protein into the skin of the host, and it in
